Lip Canting Correction In Patients With Facial Asymmetry After Orthognathic Surgery, Pr Wu, Sc Chen, Tf Chen, Ty Wu

Patients with facial asymmetry usually accompany with lip canting. Most of these patients came for orthognathic surgery combine orthodontic treatment for chin deviation and asymmetric facial problem. However, the improvement of lip canting after surgery is not as predictable as correction of chin deviation. From our literature review, the correlated factors of lip canting were discussed, and the relationship between improvement of lip canting with two jaw/ one jaw orthognathic surgery were reviewed.

Correction of facial asymmetry is one of the most difficult challenges in orthognathic surgery. Bardinet classified facial asymmetries based on three clinical entities: mandibular lateral deviations, dental asymmetries without skeletal involvement, and skeletal asymmetries. In some cases, combinations of soft tissue deformities and skeletal discrepancies complicate treatment outcome and management. This report presents a case of a 29-year-old male with skeletal Class I malocclusion, high mandibular plane angle, facial asymmetry, and dental Class III malocclusion. Facial asymmetry affects more frequently on the lower face than the upper face. Significant facial asymmetry compromises both occlusal function and facial esthetics. The etiology of facial asymmetry may be skeletal, dental, and soft tissue components of dentofacial structures. Correct diagnosis and appropriate treatment planning are very important in the management of the orthodontic patients with the chief complaint of facial asymmetry. Facial asymmetry is a common feature in orthodontic patients. Mild skeletal facial asymmetry can be treated effectively by camouflaged orthodontic technique. Orthodontic treatment combined with orthognathic surgery would be suggested in some extreme cases of facial asymmetry. In treating patients with facial asymmetry, orthognathic surgery is often needed in combination with orthodontic treatment. Conventional surgical treatment planning uses dental casts and two-dimensional radiography to predict the amount of movement needed. However, the real movement modeled on the mounted casts on the articulator cannot be simulated accurately based on the two-dimensional radiographs. A case of mandibular prognathism with facial asymmetry was presented. The missing left central incisor was previously restored with a compromising cantilever bridge. Interdisciplinary approach including orthodontics, orthognathic surgery, and prosthetic dentistry was performed and gave patient a better esthetic and functional result. Ways to achieve better esthetics, evaluation and possibility of surgery-first orthognathic approach, and other possible treatment options were discussed.

Facial asymmetry may originate from mandibular deviation combined with secondary maxillary compensation in vertical dimension, which can lead to occlusal plan canting. Two-jaw surgery would be an ideal choice for these cases in the past. Nowadays, using mini-implants to intrude elongated maxillary teeth to correct the occlusal plane canting could simplify the surgical plan into one-jaw surgery. A case of mandibular prognathism with facial asymmetry was presented. Surgery-first orthognathic approach was performed that no pre-surgical orthodontic treatment was done before surgery. Within this treatment modality, patient had major profile improvement in the beginning of the treatment and total treatment time was shortened. The etiology of mandibular deviation, treatment principles of facial asymmetry, and the advantages of surgery-first orthognathic approach were discussed.

A case of anterior open bite with facial asymmetry is presented. The un-expected anterior open bite and retrusive mandible occurred shortly following orthognathic surgery (LeFort I - 3 pieces + Bilateral sagittal split osteotomy + genioplasty). The anterior open bite was then successfully corrected with miniscrew orthodontics.Possible causes of post-surgical anterior open bite and the treatment options were discussed comprehensively.
