Based on the innovation value chain, this study establishes the "three horizontal and two vertical dimensions" measurement framework of National Innovation Force, which has a science-technology-industry horizontal dimension and a strength-effectiveness vertical dimension. The framework realizes a comprehensive analysis of National Innovation System from different perspectives and supports the categorized research and differentiated implementation of innovation policies and strategies. Problem: Surgical site infections (SSIs) occur in 3-15% of cesarean sections, making SSIs one of the most common hospital-acquired infections (HAIs). Surgical site infections (SSIs) have increased within a Labor & Delivery (L&D) unit in a large California teaching hospital for the past three years, affecting reimbursement and creating a range of adverse outcomes for patients. Performance of consistent and thorough hand hygiene practices is key to preventing infection, yet the unit’s hand hygiene compliance of 66.7% is well below the facility’s target goal of 90%. Human trafficking is a growing problem globally. Recent data indicates that airports are one of the few locations that victims can be identified and removed from a human trafficking situation. As a result, US airports have been working to raise awareness through public campaigns and employee trainings. Problem: This quality improvement initiative aims to implement AIDET, Commit to Sit, and culturally competent care in a pediatric outpatient surgery clinic to increase patient satisfaction. The microsystem falls beneath the 85th percentile of patient satisfaction scores on their unit. They have a pre-intervention Net Promoter Score (NPS) of 80.7, indicating about 80% of patients were "promoters" and would likely return to the clinic, and 20% of patients were "detractors" and unlikely to return (Appendix F). A detailed and rigorous exercise was undertaken to examine each SOE based on the ownership rationale and its financial performance as explained earlier, and all the commercial SOEs were categorized into two broad categories: (a) to be retained under state ownership; and (b) to be privatized or liquidated. Alternative splicing is critical for development; however, its role in the specification of the three embryonic germ layers is poorly understood. By performing RNA-Seq on human embryonic stem cells (hESCs) and derived definitive endoderm, cardiac mesoderm, and ectoderm cell lineages, we detect distinct alternative splicing programs associated with each lineage. The most prominent splicing program differences are observed between definitive endoderm and cardiac mesoderm. Integrative multi-omics analyses link each program with lineage-enriched RNA binding protein regulators, and further suggest a widespread role for Quaking (QKI) in the specification of cardiac mesoderm. Background: Pre-operative margin planning for the segmental resection of affected bone in mandibular osteoradionecrosis (ORN) is difficult. The aim of this study was to identify a possible relation between the received RT dose, exposed bone volume and the progression of ORN after segmental mandibular resection.
Method: Patients diagnosed with grade 3-4 ORN for which a segmental resection was performed were included in the study. Three-dimensional reconstructions of RT isodose volumes were fused with postoperative imaging. The primary outcome was the recurrence of ORN after segmental resection. In the era of immunotherapies and targeted therapies, the focus of early phase clinical trials has shifted from finding the maximum tolerated dose to identifying the optimal biological dose (OBD), which maximizes the toxicity-efficacy trade-off. One major impediment to using adaptive designs to find OBD is that efficacy or/and toxicity are often late-onset, hampering the designs’ real-time decision rules for treating new patients. To address this issue, we propose the model-assisted TITE-BOIN12 design to find OBD with late-onset toxicity and efficacy. Bone marrow (BM) fibrosis was thought to be induced exclusively by mesenchymal stromal cells (MSCs). However, we and others found that neoplastic fibrocytes induce BM fibrosis in myelofibrosis (MF). Because glioma-associated oncogene-1 (GLI1), an effector of the Hedgehog pathway, plays a role in the induction of BM fibrosis, we wondered whether GLI1 affects fibrocyte-induced BM fibrosis in MF. Multiplexed fluorescence immunohistochemistry analysis of MF patients' BM detected high levels of GLI1 in MF fibrocytes compared to MSCs or normal fibrocytes. Pirtobrutinib (LOXO-305), a reversible inhibitor of Bruton's tyrosine kinase (BTK), was designed as an alternative strategy to treat ibrutinib-resistant disease that develops due to C481 kinase domain mutations. The clinical activity of pirtobrutinib has been demonstrated in CLL, but the mechanism of action has not been investigated. We evaluated pirtobrutinib in 4 model systems: first, MEC-1, a CLL cell line overexpressing BTKWT, BTKC481S, or BTKC481R; second, murine models driven by MEC-1 overexpressing BTKWT or BTKC481S; third, in vitro incubations of primary CLL cells; and finally, CLL patients during pirtobrutinib therapy (NCT03740529, ClinicalTrials.gov). Objective: The authors previously evaluated risk and time course of adverse radiation effects (AREs) following stereotactic radiosurgery (SRS) for brain metastases, excluding lesions treated after prior SRS. In the present analysis they focus specifically on single-fraction salvage SRS to brain metastases previously treated with SRS or hypofractionated SRS (HFSRS), evaluating freedom from progression (FFP) and the risk and time course of AREs.

Therefore, we performed a study to determine how the adequacy of initial antibiotic therapy affects in-hospital mortality for patients with COBP.We carried out a retrospective cohort study among the 11 BJC Healthcare community and academic hospitals in Missouri and Illinois. BACKGROUND: Identifying individual and neighborhood-level factors associated with worsening cardiometabolic risks despite clinic-based care coordination may help identify candidates for supplementary team-based care.
METHODS: Secondary data analysis of data from a two-year nurse-led care coordination program cohort of Medicare, Medicaid, dual-eligible adults,
RESULTS: 6378 participants had pre-and post-intervention levels reported for at least one outcome measure. In adjusted models, higher pre-intervention cardiometabolic measures were associated with worsening of all cardiometabolic measures. Women had worsening LDL-cholesterol compared with men. Women with pre-intervention HbA1c > 6.8% and systolic blood pressure > 131 mm of Hg had worse post-intervention HbA1c and systolic blood pressure compared …

By complicating viewers’ relationships to my painted figures through the application of the gaze, my work analyzes how America’s colonial past affects our current landscape to find ways to break the cycle, and to make space for Black autonomy. Blackness should be free to exist without being tethered in a position of inferiority to Whiteness. Radical defiance, resiliency, and expressions of agency have been used by Black people for centuries, and their employment must continue to combat systems of oppression. Inferences about executive functions are commonly drawn through serial administration of various individual assessments that often take a long time to complete and cannot capture complex trends across multiple variables. In an attempt to improve upon current methods used to estimate latent brain constructs, this thesis makes two primary contributions to the field of behavioral modeling. First, it brings attention to sequential designs for more efficient diagnostic testing of fluctuations in executive functions with respect to a baseline level. Alzheimer’s disease is the most common cause of dementia. None of the available drugs can cure the disease. Chimeric Antigen Receptor (CAR) macrophages, because of their phagocytic activity, have potential as a cellular treatment for amyloid aggregation. In this study, we generated an anti-amyloid CAR hematopoietic progenitor cell line. By inducing the progenitor cell line to differentiate into macrophages, we show that the anti-amyloid CAR-Macrophage has enhanced specific phagocytic activity towards amyloid in in vitro experiments. In addition, in ex vivo experiments, anti-amyloid CAR significantly reduces the plaque load on brain slice from APP/PS1 mice when compared to a non-targeted …

Spectrometers have been an integral part of space exploration in the late 20th and 21st centuries and will continue to provide quantitative measurements to answer exciting questions like, “Is or was there life on other planets?” PERISCOPE, Probe for Exploring Regolith and Ice by Subsurface Classification of Organics, Polycyclic aromatic hydrocarbons, and Elements, is a next generation spectrometer designed to explore icy worlds like Europa. It uses time-resolved ultraviolet (UV) fluorescence spectroscopy, a technique that identifies organic molecules, polycyclic aromatic hydrocarbons (PAH), and rare earth elements (REE). The thesis presents a re-reading of Kafka's novel "The Trial" against the background of Kafka's negotiation of Freudian psychoanalysis. Rather than merely employing the Freudian framework as a hermeneutic tool, the thesis argues that Kafka constructs his novel along the lines of psychoanalytic theories while simultaneously attempting to deconstruct them and question the effectiveness of the so-called talking cure by means of ironically subverting a therapeutic configuration. This thesis focuses on transcribing and analyzing a repertory of underground rap songs, examining how the quality of undergroundness manifests within beats and flows. I define underground as a subgenre of hip-hop that is unified by a culture of meditative listening, and I opt for it over related terms because it is style agnostic. Therefore, I am able to investigate how a small set of techniques manifest across a variety of approaches to making underground hip-hop. I argue that these are a way of sounding the underground, communicating meaning to the listener at the level of the song. After an …

This essay imagines fictional storytelling and urban practices of maintenance, repair, and care as interrelated to one another. They are both practices engaged in building a better world. Urban scholars like Gautam Bhan, Shannon Mattern, and David Harvey propose the city as a space for envisioning the kind of community we want to be—a space for working towards our collective future. By engaging in fictional storytelling, I argue, we are doing the same thing: the dual work of taking from fact and representing it to an audience while also engaging in imagining and world-building. This thesis essay accompanies a built installation of twenty-six densely drawn illustrations, each consisting of numerous creatures and objects whose names all begin with the same letter. Thus, each illustration represents a different letter of the alphabet. Printed on cloud-shaped substrates and suspended as a group in a 10”x10” ring, the array of illustrations form a small space where viewers can immerse themselves in the act of free and careful looking. This essay elaborates on the ideas that propelled the design and construction of this illustrated installation.
