BACKGROUND: Treatment strategies for pulmonary arterial hypertension (PAH) depend on patients' 1-year mortality risk; however, real-world practices and outcomes remain inadequately described.
OBJECTIVE: We aimed to investigate real-world experience with selexipag, an oral selective prostacyclin I2 receptor agonist used to treat PAH.
METHODS: This analysis of two large, prospective, observational studies: SPHERE (NCT03278002; USA; completed) and EXPOSURE (EUPAS19085; Europe and Canada; ongoing, data cutoff November 2021) included adults newly initiating selexipag with follow-up available.

PURPOSE: To report our findings in a patient with an atypical case of supertemporal iris and retinal coloboma, and to assess its systemic associations and potential genetic causes.
OBSERVATIONS: A premature female infant born at 31 weeks gestation presented a left eye superotemporal coloboma affecting both the iris and retina. Systemic abnormalities included a left multicystic kidney, hypoplastic left kidney, atrial septal defect, ventricular septal defect, and mild pulmonary valve stenosis. Whole genome sequencing and mitochondrial deoxyribonucleic acid (DNA) sequencing were performed, and both returned negative results.

Zonular fibers are essential for suspending the lens within the eye and, in primates, for transmitting forces generated by the ciliary body to drive ocular accommodation. Despite their biomechanical importance, the response of these fibers to sudden strains, and the molecular basis of their exceptional durability remain poorly understood. This is due, in part, to the absence of tools for studying them in isolation. Here, we present a custom-built, cost-effective apparatus designed to isolate, manipulate, and characterize individual zonular fibers under controlled physical and chemical conditions. OBJECTIVE: The objective of this study was to assess trends in Molecular Breast Imaging (MBI) utilization among privately insured women in the United States (U.S.) from 2017 to 2022.
METHODS: The utilization of MBI among women aged 25-64 years from January 1, 2017, to December 31, 2022, was obtained using the Merative MarketScan Commercial Database. MBI utilization was captured from the outpatient services file using Current Procedural Terminology (CPT) or Healthcare Common Procedure Coding System (HCPCS) codes for MBI.

Advanced-stage lung squamous-cell carcinoma (LUSC) remains a therapeutic challenge. Although immune checkpoint inhibitors (ICIs) have revolutionized LUSC treatment, only a small subset of LUSC patients can achieve durable clinical benefits, underscoring the need for novel strategies. Emerging immunotherapy strategies have demonstrated early evidence of promise for LUSC patients, particularly multi-specific antibodies and fusion proteins, antibody-drug conjugates, adoptive cell therapies, and cancer vaccines. Additionally, advances in multi-omics and artificial intelligence offer potential for precise immunotherapy by deciphering the complex tumor-immune microenvironment and predicting therapeutic responses. Volume-regulated anion channels (VRACs) are central to cell volume homeostasis. They mediate swelling-induced efflux of chloride and organic osmolytes to drive regulatory volume decrease. In the brain, VRACs have been proposed to play a key role in astrocytic volume regulation. Genetic defects in astrocytic VRAC modulating proteins (MLC1, GlialCAM, Aquaporin-4, GPRC5B) cause the leukodystrophy Megalencephalic leukoencephalopathy with subcortical cysts (MLC), characterized by chronic white matter edema and myelin vacuolization. Disrupted VRAC activity in MLC-patient-derived lymphoblasts and primary astrocytes from MLC mice further supports a pathogenic link between defective VRAC activity and MLC. BACKGROUND: The Hokusai Venous Thromboembolism (VTE) Cancer trial demonstrated that edoxaban was noninferior to dalteparin for the treatment of cancer-associated venous VTE.
OBJECTIVES: We reanalyzed the trial using the win ratio, an approach that evaluates a composite of outcomes in a hierarchical order.
METHODS: Forty-nine thrombosis experts ranked 10 outcomes in order of clinical importance from all-cause death (most important) to clinically relevant nonmajor bleeding (least important). We performed unmatched pairwise comparisons between participants on edoxaban and those on dalteparin at 6- and 12-month follow-up. OBJECTIVE: Single-cell RNA-sequencing (scRNA-seq) has advanced our understanding of the heterogeneity of synovial fibroblasts and their roles in tissue repair and osteoarthritis. Here, we compared the fibroblast responses to different types and duration of joint injury in mice.
DESIGN: Published scRNA-seq data of synovial fibroblasts from a model of joint surface injury (JSI, day 6) and two post-traumatic osteoarthritis models, destabilisation of the medial meniscus (DMM, day 7 and 2 months) and anterior cruciate ligament rupture (ACLR, day 7 and day 28), were integrated and analysed using clustering, DEG, gene-set enrichment, regulon, pseudotime trajectory and cell-cycle analyses.

Oxidized lipids, once viewed as byproducts of oxidative stress, are now recognized as critical mediators linking metabolism, redox imbalance, and immunity. Generated through enzymatic and nonenzymatic oxidation of polyunsaturated fatty acids, oxidized phospholipids, lipid aldehydes, and oxidized lipoproteins act as context-dependent signals regulating immune activation, resolution, and metabolic adaptation. In metabolic inflammation, these lipids engage scavenger and Toll-like receptors to influence macrophage polarization, dendritic-cell function, and T-cell differentiation. Their context and concentration determine whether oxidized lipids amplify inflammation or promote immune tolerance. Age-associated reduced motivation is a hallmark of neuropsychiatric disorders in the elderly. In our rapidly aging societies, it is critical to keep motivation levels high enough to promote healthspan and lifespan. However, how motivation is reduced during aging remains unknown. Here, we used multiple mouse models to evaluate motivation and related affective states in young and old mice. We also compared the effect of social isolation, a common stressor in aged populations, to those of aging. Therapy-related myeloid neoplasm (tMN) is a fatal consequence of exposure to cytotoxic therapy administered in the treatment of cancer. Individuals with pre-existing TP53 clonal hematopoiesis (CH) are at high risk of tMN, with avoidance of therapy being the only strategy to reduce tMN risk. Here, in four randomized clinical trials, we show that the CDK4/6 inhibitor trilaciclib, given in conjunction with a variety of chemotherapeutic regimens and across diverse populations of patients with cancer, mitigates chemotherapy-related expansion of CH clones with mutations in DNA damage response genes, including TP53. BACKGROUND: Previous studies have investigated the dose-response relationship between external beam radiotherapy (EBRT) and leukemia by reconstructing the mean active bone marrow (ABM) dose as part of the exposure assessment. However, no prior study has leveraged electronic medical records (EMR) to reconstruct ABM dose and dose-volume in an EBRT patient population.
PURPOSE: To support future studies on the relationship between radiation exposure and adverse health effects among EBRT patients, we demonstrate methods to retrospectively calculate ABM dose and dose-volume metrics in a large patient cohort using EMR.

Triggering Receptor Expressed on Myeloid Cells 2 (TREM2) is one of the strongest genetic risk factors for late-onset Alzheimer's disease (AD). Several TREM2 ligands are known, including charged lipids and AD-associated proteins like apolipoprotein E and amyloid-β, but the full range of endogenous ligands for TREM2 remains unknown. Here we combined virtual screening of the Human Metabolome Database with molecular dynamics simulations, binding free energy estimation, and biolayer interferometry to identify novel TREM2 ligands and map their binding sites. Intrinsically disordered proteins (IDPs) are often rich in charged residues, and electrostatic interactions have a pronounced effect on their conformational distributions, interactions and functions. However, attaining quantitative understanding of electrostatics is challenging because of the sequence-specific arrangement of charges in the chain, the long-range nature of electrostatic interactions, charge screening, and the condensation of counterions-effects that all need to be taken into account self-consistently. Here, analytically tractable quantitative models are developed to predict ensemble average distances between any pair of residues in IDPs as a function of sequence and salt concentration, explicitly considering charge patterning. BACKGROUND: Valoctocogene roxaparvovec, a gene therapy for severe hemophilia A, enables endogenous factor VIII (FVIII) expression and confers bleed control.

本研究以質性訪談方式,探討香港「多重照顧者」在日常生活中面對的主要困難及採用的應對策略,並就未來政策及服務提出建議。在本研究中,多重照顧者是指同時需要照顧兩名或以上家庭成員(例如長者、配偶、子女或其他親屬),而其中一名為殘疾或長期病患者。本研究共訪問了45位多重照顧者,結果顯示他們長期承受多方面的壓力,包括時間貧乏、情緒困擾、財政壓力及使用服務困難,影響他們的身心健康、人際關係及職業發展。
就照顧歷程而言,多數受訪者已持續提供照顧,需在工作、家庭與多位照顧對象之間不斷拉鋸,出現「永遠做不完」的感受。在主觀感受上,多重照顧者普遍視照顧為理所當然的責任,但同時感到沉重壓力。他們主要依靠家人分工、嚴密的時間管理、降低對家務與個人生活質素的期望,以及有限度使用社會服務來勉強維持照顧安排。然而,服務分散、申請程序繁複、輪候時間長及資訊不足,使他們難以獲得及時和合適的支援服務。
研究建議有多個方面,包括(1)推行個案經理制度、(2)建立常規化和科學化的照顧者意見收集機制、(3)調整照顧者津貼申請門檻與金額、(4)建立照顧者友善工作環境、(5)優先發展具策略性的「一站式支援」及喘息資源,以及(6)將「多重照顧者」納入長期規劃與制度設計。

This paper examines and critiques English Learner (EL) testing accommodation policies. They have been ineffective at increasing test scores and accessibility. The policies have mandated that EL students participate in high-stakes assessments with accommodations, and sometimes without them. The purpose of the accommodation is to create an even playing field for all test-takers, so that no subgroup is put at a disadvantage, which would compromise their scores and lead to misclassification of students. Differentiated instruction (DI) is widely promoted as a means of supporting diverse learners, yet its enactment for multilingual learners (MLs) remains uneven and under-theorized in mainstream classrooms. This study examines how DI is conceptualized and implemented to support MLs by attending to learners’ linguistic, cultural, and academic profiles. Using reflexive thematic analysis, this qualitative synthesis examines 20 peer-reviewed empirical studies published between 2010 and 2024 in English-medium K–12 contexts in the United States, with particular attention to elementary settings when specified. The purpose of this quasi-experimental pretest-posttest study was to determine if a significant difference existed in the comprehension scores of secondary English Language Learners with learning disabilities after treatment. The problem addressed is that a significant correlation exists between English language learners and learning-disabled students’ low reading proficiency scores and student achievement, leading to decreased graduation rates. Multilingual learners with disabilities (MLWDs) face substantial barriers to reading comprehension given the intersecting challenges of language acquisition and disability (Artzi et al., 2022; Frates et al., 2024). Students with disabilities often need explicit, strategy-based instruction with modeling, guided practice, and scaffolding (Frates et al., 2024; Wawire et al., 2024; Moura, 2024). App-based rideshare platforms’ rapid expansion has transformed gig work from a temporary, flexible labor model into a sustained form of employment that serves as the primary source of income for millions of workers. Despite this evolution, leaders in the rideshare industry continue to classify drivers as independent contractors, thereby avoiding statutory obligations related to minimum wage, overtime, unemployment insurance, workers’ compensation, and healthcare benefits. This Comment argues that as platform-based labor becomes more permanent and economically dependent, the continued reliance on independent contractor classification becomes increasingly inconsistent with the governing legal standards designed to distinguish employees from independent contractors.

For decades, the U.S. Food and Drug Administration (FDA) has allowed synthetic food dyes to remain in the American food supply, despite evidence linking them to cancer and behavioral issues in children. These additives offer no nutritional value yet remain common in everyday products. This essay scrutinizes the FDA’s failure to enforce its own legal obligations, particularly its continued approval of synthetic dyes, which were later found to be harmful to human health. This Article chronicles my experimental adoption of Problem-Based Learning (PBL) in first-year Contracts. After three decades of conventional teaching, I observed that Gen Z students struggled to transition from undergraduate memorization strategies to law school-level analytical reasoning, prompting my desire to engage in a fundamental pedagogical change. Drawing on successful PBL implementations in medical and mathematics education, I restructured my Contracts course around collaborative problem-solving in small groups, transforming my role from lecturer to facilitator. Students worked through authentic legal scenarios at whiteboards, developing rule-based reasoning through active engagement rather than passive reception.

This article traces the historical development of the “arising out of” and “relate to” nexus requirements, examines the evolution of state long- arm statutes following International Shoe Co., and analyzes the origins and judicial interpretation of C.P.L.R. § 302(a). It argues that New York’s statutory scheme no longer fully reflects the constitutional scope of specific jurisdiction as articulated by the Supreme Court. This study offers a sociocritical analysis of female figures in the contemporary Moroccan novel, confronted with the pressures of patriarchy and radicalization. Drawing on the theories of Duchet, Zima, and Goldmann, alongside gender studies, the article examines how narrative devices—such as silence, fragmentation, and temporality—stage power dynamics. The analysis reveals a duality: ranging from the exposure of rigid structures of domination to the reclamation of the self through art and writing. Ultimately, the novel asserts itself as a space of symbolic resistance, capable of questioning social norms and outlining horizons for transformation.
