Objective: This study aimed to assess the feasibility and acceptance of integrating geriatric house call practice clinical exposure into paramedic education and to explore the impact on attitudes towards older adults among paramedic students.
Methods: Paramedic students from a fire department training program participated in a geriatric house call practice in collaboration with an academic health system. Pre- and post-program surveys were conducted to evaluate changes in attitudes towards older adults using the Ageing Semantic Differential (ASD) scale. Patient feedback on paramedic involvement was collected through post-visit surveys.
Results: Six paramedic students completed 25 house call visits over eight weeks. …

Few aerobic hyperthermophilic microorganisms degrade polysaccharides. Here, we describe the genome-enabled enrichment and optical tweezer-based isolation of an aerobic polysaccharide-degrading hyperthermophile, Fervidibacter sacchari, previously ascribed to candidate phylum Fervidibacteria. F. sacchari uses polysaccharides and monosaccharides for growth at 65–87.5 °C and expresses 191 carbohydrate-active enzymes (CAZymes) according to RNA-Seq and proteomics, including 31 with unusual glycoside hydrolase domains (GH109, GH177, GH179). Fluorescence in-situ hybridization and nanoscale secondary ion mass spectrometry confirmed rapid assimilation of 13C-starch in spring sediments. Purified GHs were optimally active at 80–100 °C on ten different polysaccharides. Finally, we propose reassigning Fervidibacteria as a class …

Modern calcifying marine organisms face numerous environmental stressors, including overfishing, deoxygenation, increasing ocean temperatures, and ocean acidification (OA). Coastal marine settings are predicted to become warmer and more acidic in coming decades, heightening the risks of extreme events such as marine heat waves. Given these threats, it is important to understand the vulnerabilities of marine organisms that construct their shells from calcium carbonate, which are particularly susceptible to warming and decreasing pH levels. Background: Lifestyle plays an important role in shaping the gut microbiome. However, its contributions to the oral microbiome remain less clear, due to the confounding effects of geography and methodology in investigations of populations studied to date. Furthermore, while the oral microbiome seems to differ between foraging and industrialized populations, we lack insight into whether transitions to and away from agrarian lifestyles shape the oral microbiota. IMPORTANCE: Federally qualified health centers (FQHCs) have increasingly participated in the Medicare Shared Savings Program (MSSP) accountable care organizations (ACOs), one of the most widespread value-based programs. Although FQHCs may strengthen ACOs' ability to provide affordable care to diverse Medicare beneficiaries, evidence on ACOs' performance by FQHC participation is limited.
OBJECTIVES: To compare beneficiary characteristics, utilization, expenditure, and quality between ACOs with and without FQHC participation and assess changes in ACO performance after including first FQHCs.

Introduction: As the US continues to battle the opioid epidemic, recovery residences remain valuable services for people in recovery. While there is a growing body of literature describing positive outcomes experienced by people who live in recovery residences, little is known about the experience of people who live in these residences while taking medications for an opioid use disorder (MOUD) as part of their recovery. White matter hyperintensities index structural abnormalities in the cerebral white matter, including axonal damage. The latter may promote atrophy of the cerebral cortex, a key feature of dementia. Here, we report a study of 51,065 individuals from 10 cohorts demonstrating that higher white matter hyperintensity volume associates with lower cortical thickness. The meta-GWAS of white matter hyperintensities-associated cortical 'atrophy' identifies 20 genome-wide significant loci, and enrichment in genes specific to vascular cell types, astrocytes, and oligodendrocytes. White matter hyperintensities-associated cortical 'atrophy' showed positive genetic correlations with vascular-risk traits and plasma biomarkers of neurodegeneration, and negative genetic correlations with cognitive functioning. …

The problem addressed was that academic achievement gaps existed for low socioeconomic status (SES) family special education students among special education students in United States schools. This problem was important because it negatively affected equity in education as low SES community special education students lagged behind more affluent community special education students. The purpose of this qualitative study was to investigate secondary teachers' perspectives on how low SES influenced learning for special education students from such communities, thereby creating an achievement gap for the students. The problem addressed in this study is that novice early childhood educators (ECEs, Grades kindergarten through 3) underestimate the challenges of teaching. Researchers have shown that although over three million teachers are employed in public schools yearly, on average 23% and up to 50% of newly inducted teachers leave the profession within 5 years on a national level. Many novice ECEs feel unprepared and underestimate the challenges of working in a school. In this doctoral project, I focused on educating healthcare staff on using the situation-background-assessment-recommendation (SBAR) communication tool to improve documentation practices and reimbursement rates from Medicare, Medicaid, and private insurance companies. My goal for the project was to determine if training on the SBAR tool would improve healthcare providers' knowledge of how to use it for documentation. Person-environment (P-E) fit, particularly in recruitment, has gained significant attention, expanding beyond the traditional focus on job-applicant compatibility to include alignment with team and organizational dynamics. Despite its importance, there is no consensus on how to effectively measure this fit, which limits its predictive power for employee outcomes. This quantitative correlational study addressed this gap by examining the predictive validity of congruence between candidates’ psychological characteristics and various facets of the work environment among software engineers. Drawing on Schneider’s attraction-selection-attrition model, the study examined congruence at multiple levels (candidate-team, candidate-leader, and candidate-role) and its influence on hiring desirability. Malaria is a major public health problem in Africa. Traditional methods of controlling malaria no longer provide adequate protection against transmission, and future approaches likely require a combination of technical solutions and behavioral change. We use a cluster randomized controlled trial to study the impacts of an intervention that combines house screening with a behavioral intervention based on health messaging. While house screening provides modest positive benefits, these benefits can be leveraged if it is combined with health messaging. Families are often the forgotten partners in education and in educational research. It is important to consider input from families regarding their children’s education. Their voices are crucial ones that should be heard more often. The research is currently lacking information about families’ perceptions about the education of their children with significant disabilities. This qualitative study explored mothers’ perspectives on the literacy education of their children and the role they play in the process. This is the second in a series of papers in which we use JWST Mid Infrared Instrument multiband imaging to measure the warm dust emission in a sample of 31 multiply imaged quasars, to be used as a probe of the particle nature of dark matter. We present measurements of the relative magnifications of the strongly lensed warm dust emission in a sample of nine systems. The warm dust region is compact and sensitive to perturbations by populations of haloes down to masses ~ 10⁶ M☉. This study explores the use of the Deep Inspiration Breath Hold (DIBH) technique in radiotherapy for right-sided breast cancer, comparing the dosimetric outcomes of 3D conformal radiation therapy (3DCRT) and volumetric modulated arc therapy (VMAT). Using a retrospective design, ten patients with prior left breast DIBH scans were analyzed to assess right breast radiation dose distributions to critical organs at risk (OARs), including the heart, lungs, liver, and contralateral breast. Each patient’s treatment plan was developed for both VMAT and 3DCRT using Varian Eclipse software.

Objective: To describe sociodemographic and racial disparities in receipt of poly ADP-ribose polymerase inhibitors (PARPi) and bevacizumab among insured patients with ovarian cancer.
Methods: This retrospective study used the Surveillance, Epidemiology, and End Results (SEER)-Medicare database to identify patients with advanced stage, high grade serous ovarian cancer diagnosed between 2010 and 2019. The primary outcome of interest was receipt of PARPi or bevacizumab at any time after diagnosis. χ2 tests were used to compare categorical variables. Factors independently associated with the receipt of PARPi and/or bevacizumab were identified using a multivariable logistic regression.
