BACKGROUND: HIV services and innovations are delivered through implementation strategy bundles that are often complex, comprising numerous discrete strategies. Systematically characterizing the use patterns of discrete strategies may inform strategy prioritization and selection to optimize service delivery. We used the Living Database of HIV Implementation Science (LIVE) to describe the commonality and co-occurrence of discrete strategies within multi-component strategy bundles in published HIV implementation studies from low- and middle-income countries.

The February 2026 IVMF employment report shows veteran unemployment improving despite a slight national increase. Overall veteran unemployment fell from 4.5% to 4.1%, and Post‑9/11 veteran unemployment dropped sharply from 5.9% to 4.8. Younger veterans (18–24) saw the largest gains, while rates for older veterans rose slightly. Male veteran unemployment decreased to 3.9%, and female veteran unemployment fell to 5.2%. Black veteran unemployment declined significantly, while Hispanic veteran unemployment rose. Compared with nonveterans, veterans generally experienced better or comparable outcomes, though differences vary by age, gender, and race.

Mycobacterium tuberculosis (Mtb) causes the disease tuberculosis (TB) and is one of the leading causes of death worldwide by an infectious agent. Furthermore, the emergence and evolution of drug resistance has significantly hindered global health efforts to combat this deadly epidemic. Thus, there is a critical need for development of new antibiotics with unique mechanisms of action and design of novel strategies for optimizing the efficacy of existing therapeutic regimens. It is estimated that 10.7% of new infected and 27.2% of previously treated TB cases are resistant to the frontline antibiotic isoniazid (INH), a central component of TB chemotherapeutic regimens. …

Background: Serratus anterior plane block (SAPB) is a widely used fascial block that reduces postoperative pain and perioperative opioid consumption. This meta-analysis assessed the efficacy and safety of SAPB in cardiac surgery patients.
Methods: We systematically searched PubMed, Web of Science, Cochrane Library, Embase, China National Knowledge Infrastructure, and Wanfang databases on May 10, 2024. Pooled relative risks and mean differences (MD) with 95% confidence intervals (CI) were calculated for dichotomous and continuous outcomes, respectively. Background: Telemedicine expanded rapidly during the COVID-19 pandemic, but its role in early-phase oncology trials remains uncertain. We evaluated its impact on recruitment, enrollment, equity, and access in a large phase I program.
Patients and methods: We retrospectively analyzed patients seen for first consultation in the MD Anderson Phase I Program (2016-2023). Recruitment (consent) and enrollment (treatment initiation) were compared by visit type, race/ethnicity, and geography across pre-pandemic (2016-2019), pandemic (2020-March 11, 2023), and post-public health emergency (PHE; March-December 2023) periods. A chart review of 400 patients validated findings.

Cancer is an evolutionary process characterized by profound intratumor heterogeneity (ITH), which can be quantified using in silico estimates of cancer cell fractions (CCFs) of tumor-specific somatic mutations. We demonstrate a data-driven approach based on CCF distributions to identify 4 robust pan-cancer evolutionary signatures from 4,146 tumors across 17 cancer types in The Cancer Genome Atlas (TCGA). These signatures define a continuum of cancer cell fractions reflecting neutral evolution, clonal expansion, and clonal fixation. Correlating evolutionary signatures with mutational and biological programs reveals that tumors enriched for clonal expansion and fixation are associated with immune evasion and distinct changes in …

Colorectal cancer (CRC) remains a leading cause of cancer mortality globally, yet current histopathological diagnostics capture only limited features. This study aimed to discover subtle, prognostically significant histomorphological patterns in CRC tissues using unsupervised deep learning. We developed a framework integrating convolutional neural networks with deep clustering, trained on 23,341 image patches from 493 patients. We identified 30 distinct histomorphological clusters from CRC tissue images. Through univariate and multivariate survival analyses, three clusters (Cluster13, Cluster19, and Cluster24) were consistently associated with patient prognosis. This work presents SENTISEC, a hybrid LLM-based threat detection framework designed to classify security logs by integrating keyword heuristics, domain-adapted sentiment scoring, and Retrieval-Augmented Generation (RAG). The system achieves an overall accuracy of 93.67%, with 91.46% macro recall, 89.07% macro F1, and 95.15% threat recall, while maintaining a low false-positive rate of 1.68%. Its methodology incorporates strict keyword and IOC matching, a domain-tuned DistilBERT sentiment module, hybrid BM25–MiniLM retrieval enhanced with BGE reranking, adaptive quantile-based threshold calibration, and SHAP-based explainability. Genomic data sharing has become an essential component of biomedical research, enabling large-scale collaborations and accelerating discoveries in human genetics. To balance the need for accessibility with privacy concerns, several controlled-access mechanisms have been proposed, including genomic beacons. Genomic beacons answer simple presence/absence queries about specific genetic variants. However, prior work has demonstrated that beacons remain vulnerable to genome reconstruction attacks, where an adversary can recover large portions of participants’ genomes using summary statistics. The escalating demand for high-resolution multimedia content has necessitated more efficient video compression solutions. The Versatile Video Coding (VVC) standard, despite achieving remarkable compression gains, introduces significant computational complexity, primarily due to its exhaustive Rate-Distortion Optimization (RDO) process. To address this, we propose an intelligent approach leveraging supervised machine learning techniques to streamline the VVC encoding process. Tsunamis pose severe and time-critical risks to densely populated coastal cities, where limited warning times and infrastructure constraints demand carefully coordinated evacuation strategies. This study develops an integrated, risk-aware optimization framework that jointly considers vertical and horizontal sheltering options together with mixed pedestrian-vehicular evacuation dynamics. The proposed mixed-integer second-order cone programming (MISOCP) model simultaneously determines vertical shelter location, evacuee assignment, road-use designation for pedestrians and vehicles, and route selection under congestion, capacity, and budget constraints. Vehicle travel times incorporate congestion effects through a convex flow-dependent function, while pedestrian routing ensures convergent and conflict free evacuation paths. Power distribution systems play a crucial role in transmitting electrical power from generation sources to end users. During transmission, significant power losses occur in the form of heat as the current flowing along the lines/cables has resistance. To minimize power losses, distribution network reconfiguration (DNR) has been widely adopted. This paper proposes optimal DNR based on metaheuristic techniques with discrete mutation feature targeting active power loss reduction, which subsequently lowers carbon emissions and operational costs. Aedes aegypti (Diptera: Culicidae), the primary vector of dengue, poses a significant public health threat in tropical and subtropical regions. Botanical insecticides and phytochemicals such as β- asarone, derived from essential oils,offer a promising alternative to synthetic larvicides. The rhizome of Acorus calamus L. Sandfly-borne phleboviruses cause febrile illness and neuroinvasive disease in humans. While infections are reported in the Mediterranean region, the discovery of previously unknown phleboviruses in sandflies from Kenya suggests a wider geographic distribution. Detection and characterization of novel phleboviruses are often hindered by low-quality and low-viral-load samples. We developed a capture-based target enrichment next-generation sequencing approach that showed a 99%-100% fold enrichment of viral genomes from primary material and provides a robust tool for generating complete genomes of both known and previously unknown viruses. BACKGROUND: Healthcare transition (HCT) refers to the shift from a child-centered to an adult-centered model of care, a process that, when well-functioning, can reduce disengagement and improve satisfaction with care. The Six Core Elements (6CE) framework structures HCT services; however, its implementation process remains understudied, limiting its broader applicability. Learning how to effectively implement the 6CE strategies can enhance their application in real clinical contexts. This study investigates the fundamental limits of UAV propeller fault discrimination using an interpretable XGBoost–SHAP framework applied to the high-resolution DronePropA dataset. Three fault types—edge-cut, structural crack, and surface erosion—were evaluated across three severity levels under constrained flight conditions to isolate the physical signatures of degradation. A curated subset of 1 kHz IMU and ESC telemetry from Drone 1 (trajectory t1, speed SP1) enabled analysis of fault separability independent of kinematic variability. Despite deploying a moderately complex XGBoost classifier (300 estimators, depth = 6) and a comprehensive multi-domain feature set, the model achieved a performance ceiling of 38.04% accuracy and …

This study examines the fragmentation of the self and the appropriation of things in contemporary thought and Saudi literature through an interpretive reading of Seham al-Aboudi’s Secret Migration to Things. It argues that external pressures fragment the self while objects act as semantic agents shaping characters’ consciousness and behavior, making narrative a means to explore human interaction with the material world. Using a hermeneutical approach inspired by Paul Ricoeur and combining semantic, stylistic, and phenomenological analysis, the study addresses three axes: narrative hermeneutics and meaning-making, representations of objects and identity disintegration, and the formation of narrative space and consciousness transformations. …
