A transdiaphragmatic intercostal hernia (TDIH) describes the herniation of abdominal contents through diaphragm and chest wall defects. It has been documented following traumatic injuries as well as after episodes of increased thoracoabdominal pressure. However, those resulting from iatrogenic cause remain uncommon and are less often recorded. We report an iatrogenic TDIH in a patient presenting 39 days after a complicated thoracic procedure, highlighting the surgical technique used for successful hernia reduction, diaphragmatic closure, and rib re-approximation. The case reinforces the possibility of a TDIH occurring as a post-surgical complication. Surgeons should anticipate the potential development of TDIH, particularly after thoracic …

Propofol Related Infusion Syndrome (PRIS) was first described in 1998. It has a strange collection of symptoms, including marked bradycardia, persistent, recalcitrant metabolic acidosis, liver enlargement, rhabdomyolysis, and lipemic blood. We present two recent patients who appeared to have had PRIS. Creatinine kinase seems to be an early detector of PRIS. If PRIS is recognized early, this complex metabolic process seems to be completely reversible.

Introduction

The purpose of this report is to describe a technique for performing a robotic total laparoscopic hysterectomy (rTLH) with clinical outcomes on safety and efficiency. The rationale for our approach is based on a critical evaluation of the literature.

Methods

Data from all rTLH procedures performed on our gynecologic oncology service between January 2017 and December 2019 were retrospectively reviewed. Using this database, perioperative data including surgical times, intra- and postoperative complications, and length of hospital stays were evaluated. The steps used to perform the procedure were outlined and illustrated.

Based on the notion that applied research can be theoretically grounded or not theoretically grounded, I suggest that there are roughly four categories within which quality work in this area can fall: basic research, auxiliary assumptions, theory combined with auxiliary assumptions, and research without theory. Although the importance of auxiliary assumptions in traversing the distance between theories and their applications cannot validly be denied, applied researchers, including those in the aviation area, often have ignored them. But auxiliary assumptions play an important role in understanding the four categories that I propose. Finally, I argue that much of the applied research …

An experiment was conducted in an austere desert location (entirely lacking artificial light pollution) to evaluate visual observers’ ability to maintain line of sight with a light-sport manned aircraft and a small unmanned aircraft system (sUAS; Raven RQ-11B or Wasp III) and predict imminent collisions between them. We investigated the impact of night and dusk operational settings on observers’ performance as compared to daytime and manipulated the placement of the critical visual observer in relation to the sUAS pilot. A general analytical solution of the navigational wind-triangle problem and the calculation of the critical tailwind angle are presented in this study among other findings. Any crosswind component will effectively create a headwind component on fixed course tracks. The meaning of a route track is lost with excessive crosswinds representing the bifurcation point between the possible and the impossible navigational solutions. Any wind of constant direction and speed will effectively reduce groundspeed and increase time-of-flight on closed-loop multi-segment flights. 1. Water availability for irrigation is threatened in many regions by rapidly increasing demand for nonagricultural water uses in industry, households, and the environment. The scarcity of irrigation water will not only impact crop production, but also meat production, as much of the pasture used to feed livestock is irrigated.
