Campylobacter jejuni remains a major cause of foodborne illness worldwide, with poultry serving as the primary reservoir. In the absence of commercial vaccines or effective feed additives, probiotics and their byproducts (postbiotics) represent a promising and sustainable approach to reducing Campylobacter colonization in poultry. This study compared the efficacy of oral and cloacal administration of probiotic lactobacilli and their postbiotics in reducing Campylobacter colonization and modulating the cecal microbiome in broiler chickens. Introduction: Children with high-intensity neurologic impairment (HINI) account for a sizeable proportion of inpatient resources. As survival into adulthood for children with medical complexity increases, children’s hospitals will likely admit adults with HINI. Understanding adult HINI hospitalization patterns is necessary to develop effective care strategies for this population. Objective: Compare characteristics of adults and adolescents with HINI hospitalized at children’s hospitals and describe hospital-level variation in hospitalizations for adults with HINI. Statement of Purpose: Gunshot wounds to the foot pose a unique surgical dilemma—whether to pursue aggressive foreign body removal or to preserve soft tissue integrity. This case highlights the management of a birdshot injury to the rearfoot where selective non-removal of pellets near the neurovascular bundle led to complete healing and full functional recovery. Purpose: Complex Regional Pain Syndrome (CRPS) is a pain disorder that typically follows several forms of trauma, including surgery or fractures. The diagnostic gold standard for CRPS is the Budapest diagnostic criteria. In the general population, CRPS is 3-4 times more common in middle-aged women. Neighborhood-scale environmental factors, including disinvestment in infrastructure, may impact cardiometabolic disease risk. To our knowledge, no prior studies have investigated the association between neighborhood disinvestment and incident diabetes. We used a virtual street audit of Google Street View Imagery to generate a neighborhood disinvestment score for participants' residential addresses in the Hispanic Community Health Study/Study of Latinos (HCHS/SOL). An item response theory model was fit to indicators (litter, graffiti, under-maintained buildings, bars on windows, and abandoned buildings) to form a scale measuring a latent level of disinvestment. OBJECTIVE: Prior research on confessions of physical abuse (PA) has explored mechanisms of injury. Little is known about conditions supporting a confession of abuse or how confessions may influence case outcomes.

METHODS: This cross-sectional study of suspected PA in children aged < 10 years at 10 centers participating in CAPNET, a multicenter child PA research network, included children with in-person Child Abuse Pediatrics (CAP) consultations between February 2021 and December 2022 and excluded children without injury. Our focus was a confession of inflicted injury known to the CAP during clinical involvement. We compared child and clinical characteristics, out-of-home (OOH) placements, and arrests between cases with and without confessions. Multivariable models using generalized estimating equations (GEE) produced adjusted predicted probabilities (APP) clustering by CAPNET site.

RESULTS: Confessions were known to CAPs in 115/4297 (2.7%) cases, with significant site variability. In a multivariable GEE model, confessions were more likely in cases with injuries with high specificity for abuse (APP 6.0% vs 1.6%, P < .001), near-fatality (APP 5.6% vs 2.3%, P < .001), and older children (APP 4.8% vs 2.5%, P = .025). OOH placements and arrests were more common in cases with confessions. CAP awareness of confessions, OOH placements, and arrests did not follow racial or ethnic patterns seen in the US child welfare system. Lower socioeconomic status was associated with OOH placements and arrests but not with CAP awareness of confessions.

CONCLUSIONS: Our findings highlight differences in case characteristics and outcomes where a confession is known to the CAP and raise questions about …

Glycoprotein sialylation represents a critical posttranslational modification with diverse biological roles in animals. This review explores its multifaceted functions in the nervous system, with particular emphasis on neurophysiology, homeostasis, and associated neurological disorders. The sialylation pathway modulates key neural processes through effects on glycoprotein stability, localization, activity, and molecular interactions. Examples include its crucial role in the regulation of neuronal excitability by modulating the functions of voltage-gated ion channels. Recent studies have uncovered remarkably rapid, activity-dependent changes in synaptic sialylation, suggesting dynamic sialylation-mediated regulation of neural transmission and highlighting the importance of neuraminidases in these processes. Atomoxetine, a selective norepinephrine reuptake inhibitor, is a nonstimulant alternative to treat attention-deficit/hyperactivity disorder in children and adolescents. Atomoxetine exposure is highly variable due to cytochrome P450 polymorphism; however, the impact of these genetic variations has not been adequately characterized in the pediatric population. A population pharmacokinetic (PK) model was developed to evaluate steady-state atomoxetine exposures after oral administration in a large, heterogeneous population of children and adolescents with different metabolic phenotypes. The investigatory, or reporting, power of grand juries refers to the body’s ability to issue statements on wide-ranging matters of public policy, generally aimed at exposing “inefficiency, neglect, or criminal or quasi-criminal conduct” by government officials. Grand jurors may propose an investigation themselves or respond to a request from a citizen. The reports produced by these investigations need not be tied to a specific indictment to be released.

Through a critical analysis of interactive horror, this paper argues that video games can function as ethical diagnostic narratives that uniquely expose systemic failures surrounding women’s bodies and medical authority. Focusing on Silent Hill 3 (2003) and Alice: Madness Returns (2011), I examine how these games render harm legible through player participation. By requiring players to move characters through hostile environments and make decisions within systems that simultaneously grant and restrict control, these games foster identification and complicity by positioning players directly within the structures they critique.

Termed Vergangenheitsbewältigung in the post-war period and envisioned as a project extending beyond the academy into public consciousness, “dealing with the past” was first envisioned in response to the Nazi regime and its horrors. While already a disjointed project in the Nazi context, the question of how reunified Germany reckons with the former German Democratic Republic’s past remains unresolved.

Arguing that the rich symbolic associations of trees become a medium to synthesize shared identity during a country’s formative moments, this paper theorizes the image of the forest as a site of national memory. By tracking representations of the natural landscape during nation-building to representations that attempt to recover and reclaim dignity postwar, this paper lays bare the potential of the national symbol to transform as history progresses. Acral lentiginous melanoma (ALM) classically presents in glabrous skin. Some studies suggest an association between mechanical stress and ALM development. People working jobs with long periods of standing/walking may be exposed to near-daily mechanical stress. To investigate the relationship between occupation type and ALM development, we performed a retrospective cohort study using Optum’s Clinformatics Database (2007—2023). We compared the risk of ALM in adults with occupational data (n = 9,270,935). Retired patients had the highest prevalence (0.081) and incidence per 1,000,000 person-years (0.514, 95% confidence interval = 0.487—0.542) of ALM. We present the case of a 35-year-old male with no known comorbidities who presented with chronic gastrointestinal (GI) and constitutional symptoms and was empirically treated with several antibiotics during multiple clinic visits at different health facilities. The laboratory test sent included a stool GeneXpert, which was positive for Mycobacterium tuberculosis (MTB). CT imaging revealed multiple necrotic abdominal and retroperitoneal lymph nodes, hypoenhancing splenic lesions, a nodo-colonic fistula between the necrotic retroperitoneal lymph nodes and the adjacent colon, and a mild left-sided pleural effusion. Colonoscopy demonstrated caseating discharge from the fistulous tract 40 cm from the anal verge, and histology showed …

Current weakly supervised video anomaly detection (WSVAD) task aims to achieve frame-level anomalous event detection with only coarse video-level annotations available. Existing works typically involve extracting global features from full-resolution video frames and training frame-level classifiers to detect anomalies in the temporal dimension. We introduce the Self-Exemplar Illumination Equalization Network, designed specifically for effective portrait shadow removal. The core idea of our method is that partially shadowed portraits can find ideal exemplars within their non-shadowed facial regions. Rather than directly fusing two distinct classes of facial features, our approach utilizes non-shadowed regions as an illumination indicator to equalize the shadowed regions, generating deshadowed results without boundary-merging artifacts. Our network comprises cascaded Self-Exemplar Illumination Equalization Blocks (SExmBlock), each containing two modules: a self-exemplar feature matching module and a feature-level illumination rectification module. Quantization is widely used to enable local deployment of large language models (LLMs) on resource-constrained devices. Recent work (e.g., QuRA) shows quantization can be exploited via rounding manipulation to implant backdoors. However, such an attack has been evaluated only on small models and does not directly apply to LLMs due to three key constraints: (1) limited poisoning data from small, task-agnostic calibration sets; (2) layer-wise quantization restricting adversarial access to global representations; and (3) lack of gradient access in quantization pipelines, blocking gradient-based attacks.We propose LLMQuA, a practical quantization-phase backdoor attack tailored to the LLM setting.
