INTRODUCTION: Despite available prophylactic therapies for haemophilia A, breakthrough bleeding and consequent pain and joint damage still occur.
AIM: To provide insights into the unmet treatment needs of people with moderate-to-severe haemophilia A in the US.
METHODS: For this descriptive, observational cohort study, eligible participants diagnosed with moderate-to-severe haemophilia A were enrolled in the PicnicHealth database on or before 3 October 2022. Escherichia coli is a leading cause of neonatal sepsis, with infection occurring in approximately one in every 1,000 live births. However, with E. coli colonization beginning soon after birth and defects in neonatal host defence maturation, an alternative consideration is why infection does not occur even more frequently. Here we show that newborn babies with E. coli sepsis have selectively reduced vertically transferred natural antibodies that recognize E. coli, mechanistically explaining their susceptibility to infection. Complementary preclinical studies show that preconceptual intestinal colonization with probiotic E. coli Nissle 1917 (EcN) primes anti-E. coli immunoglobulin G (IgG) antibodies with broad cross-reactivity …

BACKGROUND: Transforming growth factor-β1 (TGF-β1) contributes to more severe pulmonary disease in people with cystic fibrosis (pwCF). Our preclinical in vitro and in vivo studies demonstrated that losartan could reverse TGF-β1-mediated mucociliary dysfunction, albeit at doses difficult to achieve by oral administration. This study therefore examined airway losartan concentrations after oral and inhaled dosing to evaluate if oral losartan could be even expected to improve mucociliary clearance.

IMPORTANCE: Since its enactment in 2003, the Pediatric Research Equity Act (PREA) has significantly increased the number of pediatric drug studies performed and expanded pediatric drug labeling. Despite these advancements, many drugs used in children still lack pediatric-specific US Food and Drug Administration (FDA) labeling, even when pediatric studies are required by law. Recent legislative changes strengthened the FDA's enforcement authority over PREA. As these changes are implemented, persistent gaps in pediatric drug development warrant examination. BACKGROUND: Escherichia coli is a leading cause of neonatal sepsis. This study's objective was to investigate the clinical characteristics of newborns with E. coli bacteremia, and recent antibiotic resistance trends and molecular epidemiologic features of these invasive strains.
METHODS: Clinical data and blood E. coli isolates collected from 54 neonates with bacteremia were studied. E. coli antibiotic susceptibility trends over time, and whole-genome sequencing data relevant to the genomic relatedness and capsule genotype of the isolates were analyzed.
RESULTS: Thirty percent of bacteremic newborns were preterm. Mortality was 19% in preterm versus 5% in term infants. Cerebrospinal fluid was collected …

Recurrent pregnancy loss (RPL), defined as two or more consecutive miscarriages, affects 1-5% of couples attempting to conceive and most frequently occurs between 6 and 8 weeks of gestation, coinciding with critical stages of placental development. Despite this temporal association, placental contributions to RPL pathogenesis remain poorly understood. We hypothesized that trophoblast lineage development, including extravillous trophoblast (EVT) and syncytiotrophoblast (STB) differentiation, are disrupted in RPL. Sodium imbalance is a common concern in children with kidney diseases, presenting as either sodium excess or sodium deficit, each with significant clinical implications. Sodium excess contributes to fluid overload and hypertension, while increased sodium losses, particularly via urine or peritoneal fluid, can predispose patients to hypotension and growth failure. Effective sodium management is thus a critical component of care in pediatric kidney diseases, with dietary sodium intake playing a pivotal role in maintaining homeostasis. BACKGROUND: There is a scarcity of information regarding the distribution of the diseases leading to kidney failure (KF) in children living in the emerging world. We used registry data to provide a global overview of the underlying disease spectrum in children commencing kidney replacement therapy (KRT).

Artificial intelligence (AI) is rapidly emerging as a transformative force in pediatric nephrology, enabling improvements in diagnostic accuracy, therapeutic precision, and operational workflows. By integrating diverse datasets-including patient histories, genomics, imaging, and longitudinal clinical records-AI-driven tools can detect subtle kidney anomalies, predict acute kidney injury, and forecast disease progression. Deep learning models, for instance, have demonstrated the potential to enhance ultrasound interpretations, refine kidney biopsy assessments, and streamline pathology evaluations. Sexual and gender-based violence (SGBV) remains a pervasive yet under-addressed issue in humanitarian settings. Refugee and internally displaced person (IDP) camps along the Thai– Myanmar border represent a complex nexus of protracted displacement, ethnic conflict, and limited access to medical and psychosocial care. Although the prevalence of sexual assault in these contexts is well-documented, there is a critical gap in the literature regarding effective, context-specific treatment and intervention strategies. The 2025 Community Health Needs Assessment (CHNA) shows that over 11% of adults in Finney County, Kansas, have diabetes, which surpasses both state and national averages. Census data from 2016-2020 indicate that 42.6% of residents ages five and older speak a language other than English at home, and 36.1% speak Spanish (United States Census Bureau, n.d.). The CHNA further highlights concerns about language and cultural barriers to care, especially among the Spanish-speaking population. Notably, 21.74% of residents have limited English proficiency—much higher than the state’s 12.1% (CHNA, 2025).

Gestational Diabetes Mellitus (GDM) poses significant health risks, specifically increasing the risk of developing Type 2 Diabetes post-delivery, as well as increasing the risk of adverse neonatal and maternal outcomes. Introduction: Real-world effectiveness data for ursodeoxycholic acid (UDCA), commonly prescribed after sleeve gastrectomy to reduce the risk of biliary events, are limited. We evaluated whether UDCA within 6 months of sleeve gastrectomy reduces 1-year biliary complications across a multi-institution network. Methods: We performed a retrospective cohort study on TriNetX (Global Collaborative Network; 17 health care organizations). Adults (18–89 years) undergoing sleeve gastrectomy (CPT 43775) from 9/21/2015 onward were included. Exposure was UDCA (RxNorm 62427) within 6 months post-surgery vs no UDCA in the same window. With the increasing number of research priority setting (RPS) exercises, systematic reviews synthesising their findings have also grown in prevalence. While these reviews offer a structured way to compare methodologies, identify underrepresented stakeholder groups, and guide funding decisions, conventional systematic review methodologies, designed primarily for clinical and health research, often fail to capture the complexity, contextual nuance, and participatory nature of RPS. In this commentary, we critically examine these limitations and propose methodological adaptations to enhance the relevance and utility of systematic reviews of RPS. We investigate the contents of web-scraped data for training AI systems, at sizes where human dataset curators and compilers no longer manually annotate every sample. Building off of prior privacy concerns in machine learning models, we ask: What are the legal privacy implications of web-scraped machine learning datasets? In an empirical study of a popular training dataset, we find significant presence of personally identifiable information despite sanitization efforts. Our audit provides concrete evidence to support the concern that any large-scale web-scraped dataset may contain personal data. BACKGROUND: The modified Blalock-Taussig-Thomas shunt (mBTTS) is a critical palliative procedure for infants with single-ventricle physiology, but thrombosis-related occlusion affects 8% to 12% of infants and carries nearly 50% mortality. Meanwhile, existing antithrombotic strategies fail to address the hemodynamic factors driving thrombosis, highlighting the need for a deeper understanding of flow dynamics in shunt failure.

Patients with high-risk acute myeloid leukemia (AML) and myelodysplastic syndrome (MDS) are likely to relapse despite allogenic hematopoietic cell transplantation (HCT). Post-HCT preventative maintenance can be limited by toxicity toward the normal donor cells. BACKGROUND: Preterm birth increases the long-term risk of diabetes and chronic kidney disease (CKD), yet its impact on diabetic kidney disease (DKD) is unclear. We previously showed that male preterm mice with diabetes develop early features of DKD, including reduced podocyte density, decreased renin expression, activation of angiogenesis pathways, and impaired endothelial-podocyte signaling. Here, we examine whether preterm birth similarly accelerates DKD progression in female mice and compare structural and transcriptomic outcomes in the females to the prior male cohort to assess sex-specific differences.

OBJECTIVES: Evaluate the association of plating strategies with hardware removal and operative time, after adjusting for relevant clinical variables.
STUDY DESIGN: Retrospective Review.
SETTING: Multiple Academic Medical Centers.
METHODS: Patients undergoing osseous free flap reconstruction stratified based on the plating strategy [conventional plating with a hand-bent plate, precontoured plate, and virtual surgical planning (VSP) with patient-specific hardware and cutting guides]. Variables associated with hardware removal were analyzed using multivariable logistic regression. To evaluate the effect of plating strategy on operative time, a multivariable linear regression model was created. Adjusted odds ratios (aOR) with 95% confidence intervals (CI) were calculated.

Three-dimensional reconstruction of cortical surfaces from MRI for subsequent morphometric analysis is fundamental for understanding brain structure. While high-field Magnetic Resonance Imaging (HF-MRI) is the standard in research and clinical settings, its relatively limited availability hinders widespread use. Low-field MRI (LF-MRI), particularly portable systems, offers a cost-effective and accessible alternative. Objective: Amyotrophic lateral sclerosis (ALS) is characterised by progressive degeneration of upper and lower motor neurons and their motor units (MUs). MU loss is compensated by collateral sprouting and reinnervation of muscle fibres. There is limited information about the properties of these surviving MUs as these processes take place. High-density surface electromyography (HD-sEMG) decomposition enables non-invasive analysis of individual MU firing behaviour during maximal voluntary contractions and assess their changes with ALS progression. Introduction We aimed to explore the potential incremental cost-effectiveness of the PRODEMOS coach-supported mobile health intervention for primary prevention of dementia versus standard of care provided to people aged 55–75 years with low socio-economic status (SES) in the United Kingdom (UK), and any SES in China. Methods 12–18-month PRODEMOS trial (ISRCTN15986016) efficacy outcomes on hypertension, obesity, hypercholesterolemia, physical inactivity and smoking were extrapolated to lifetime impact on dementia onset, myocardial infarction, stroke and death using a health-economic open-source simulation model. Thomas Jefferson was a deeply paradoxical figure—both a founding champion of liberty and a lifelong slaveholder—whose legacy must be understood in its full complexity. While he owned more than 600 enslaved people and participated in a system he publicly condemned, Jefferson consistently articulated and promoted ideals of natural rights, equality, and freedom that reshaped the United States and influenced democratic movements worldwide. He made repeated (though often unsuccessful) efforts to curb or end slavery through legislation, legal cases, and policy, including opposing the slave trade, limiting slavery’s expansion into western territories, and helping abolish the transatlantic slave trade. Anthropogenic inputs to the environment—including microplastics and microfibres—are global stressors known to impair multiple facets of organismal biology. However, the effects of these pollutants on sensory structures remain critically understudied. Sensory systems mediate organism-environment interactions, and pollution-derived impairment at these interfaces may cascade through bioprocessing, from information acquisition to behaviour and ultimately fitness. Microfibres, in particular, pose an additional threat, potentially physically obstructing sensory organs and chemically interfering with sensory processes. This dataset provides a comprehensive set of ecosystem service (ES) and natural capital (NC) indicators for the Plymouth Sound National Marine Park (UK), developed through a co-evaluation of monitoring systems for transformative ocean governance. A total of 109 indicators are identified and mapped across the natural capital logic chain (asset state, pressures, ecosystem service flows, and benefits), drawing on existing datasets, literature, and stakeholder workshop input. Each indicator includes metadata, evaluation against standardised quality criteria, and assessment of strengths and limitations. Background: Neonates requiring extracorporeal membrane oxygenation (ECMO) are at high risk for thrombotic complications, which contribute substantially to morbidity and mortality. Current circuit surveillance methods rely on intermittent visual inspection and indirect laboratory markers, both of which often detect thrombosis only after clinically significant progression. Optical approaches offer a non-invasive alternative for earlier clot visualization, yet the influence of illumination wavelength on detection performance under realistic ECMO conditions remains incompletely characterized.

This report shows that so-called “irregular” Latinos are an essential part of the
workforce. Beyond community spending, consumer activity, and taxes paid
(including tax benefits that many will never benefit from), it is important to
remember that we are talking about people— human beings with stories, families,
difficult choices, and dreams. I hope that in the near future, when we are able to
dream big again, this analysis will sustain our advocacy for systems that truly reflect
our (Latino) contributions to the Commonwealth and beyond.
