HCA Healthcare Journal of Medicine
Porokeratosis was first described in 1893. It is a relatively rare disorder with over 9 subtypes. Lesions are clinically characterized as well-demarcated, erythematous papules (raised, <1 cm) or plaques (raised, >1 cm), with an atrophic center, and raised scaly border. Porokeratosis is an important diagnosis to identify because it may undergo malignant transformation and mimics many commonly encountered diagnoses. These commonly mimicked diagnoses include squamous cell carcinoma, tinea corporis, nummular dermatitis, and psoriasis vulgaris, to name a few. The clinical images in this review focus on identifying porokeratosis along the full spectrum of skin tones.

Atopic dermatitis is a chronic inflammatory skin disorder classically affecting flexural areas of the body. It is present in children and adults, including those with darker skin pigmentation. Chronic lesions are hyperpigmented plaques that are dry, cracked, and/or scaly often with lichenification. Differential diagnoses include psoriasis, seborrheic dermatitis, ichthyosis, and pityriasis rosea. This article will showcase clinical images with varying presentations of chronic atopic dermatitis in a range of age groups and skin colors according to the Fitzpatrick scale.

Seborrheic dermatitis is a common dermatologic disease affecting patients of all ages, ethnicities, and skin pigmentations. The rash often affects the scalp, ears, and central face. The underlying skin pigmentation of the individual may affect how this disease presents. We present several cases of seborrheic dermatitis in individuals of varying ages, genders, and skin pigmentations.

Opioid-Related Emergency Department Visits Before And During Covid-19: Association With Community-Level Factors, Timothy F. Page, Weiwei Chen, François Sainfort, Julie A. Jacko
HCA Healthcare Journal of Medicine
Background
The purpose of the study was to investigate the relationship between community-level variables and emergency department (ED) visit rates before and during COVID-19. The focus was on opioid-related ED visits. Despite large declines in overall ED visits during COVID-19, opioid-related visits increased. While visits for avoidable conditions decreased, the opposite was true for opioid-related visits.
Methods
We combined data from Florida EDs with community-level variables from the 2020 American Community Survey. The outcome measures of the study were quarterly ZIP code tabulation-area-level ED visit rates for opioid-related ED visits as well as visit rates for all other causes. Associations …

Drivers Of Being Unhoused And The Prevalence Of Health Conditions Among Unhoused Individuals In Asheville, Nc, Andrea K. Yontz, Amber Beane, Tessa Frank, Amy Upham, Dustin V. Patil, Dan Pizzo, Steve Buie, Jacqueline R. Halladay
HCA Healthcare Journal of Medicine
Background
During the COVID-19 pandemic, there was an increase in the number of unhoused individuals in Asheville, North Carolina resulting in more tent encampments.
Understanding the physical, mental, and socially determined health characteristics associated with being unhoused can help guide stakeholders with policy development, healthcare program planning, and funding decisions to support unhoused individuals.
Methods
In this study, we used an observational cross-section methodology. Using a convenience sample approach, we interviewed 101 participants who were receiving services from 2 emergency hotel shelters, a day center, and a resource center. Data were analyzed using descriptive statistics, and open-ended responses were collected …

Evaluation Of The Efficacy Of Remdesivir For The Treatment Of Coronavirus Disease 2019, Alexandra Flannery, Anastasia Knecht, Koravangala Sundaresh
HCA Healthcare Journal of Medicine
Background
Coronavirus disease 2019 (COVID-19), caused by a novel coronavirus, SARS-CoV-2, has accounted for more than 1 000 000 deaths in the United States alone. In May 2020, the Food and Drug Administration issued an Emergency Use Authorization to allow the investigational use of intravenous remdesivir for the treatment of suspected or confirmed COVID-19 in hospitalized children and adults. Several other agents, such as hydroxychloroquine, dexamethasone, and tocilizumab have been investigated as potential treatment options; however, dexamethasone is currently the only agent that has been proven to reduce mortality in patients who require supplemental oxygen. The purpose of this study …

Prophylactic Mesh Reinforcement For Non-Midline Incisions: A Systematic Review, Rainna Coelho, Oluwatunmininu Anwoju, Ali Siddiqui, Andrew Youssef, Oscar A. Olavarria, Nalia H. Dhanani, Karla Bernardi, Zuhair Ali, Mike Liang
HCA Healthcare Journal of Medicine
Background
Many abdominal-pelvic surgeries utilize incisions not along the linea alba, such as transverse, laparoscopic, ostomy reversal, or ostomy formation incisions. The prevalence of ventral incisional hernias (VIH) at these sites and the efficacy of prophylactic mesh in preventing VIH remains unclear.
Methods
PubMed, Embase, Scopus, and Cochrane databases were systematically reviewed from inception to September 2022. We included published randomized controlled trials (RCTs) that compared prophylactic mesh reinforcement versus no mesh. The primary outcome was the incidence of VIH at postoperative follow-up equal to or greater than 24 months. Secondary outcomes included surgical site infection (SSI) and surgical site …

A Continuous Local Ancestry Measure For Efficient Local-Ancestry-Aware Association Tests, Hanxiao Sun
Dissertations and Theses (Open Access)
Genetic association tests have enabled people to identify susceptible loci and broadened our understanding of complex diseases. However, the GWAS (Genome-wide Association Studies) results are probably confounded by population stratification thus leading to potential false-positive findings. This problem is pronounced particularly in admixed populations, a group of populations with multiple ancestries whose local genetic ancestry may drastically vary at local genomic positions (local ancestry) compared to the overall genetic ancestral composition (global ancestry). It is insufficient to only account for global population structure in admixed populations. Methods have been developed to account for local population stratification but followed by subsequent …

Karbala International Journal of Modern Science
In this study, two different methods are used to investigate carbon black as a cold field electron emitter. The first method is to incorporate carbon black into a specially designed insulated copper hollowed wire. The wire has a cup-shaped structure created by electrochemical etching. The second method involves the incorporation of carbon black into narrow glass tubes. A Comparative analyses is carried out to evaluate the effectiveness of each method. To evaluate the performance of the samples, the current-voltage characteristics will be examined using field electron microscopes. This analysis will provide an understanding of the emission of the carbon black …

Enhanced Observability And Analytics Platform For Troubleshooting Business Process Workflows, John Solomon Jebamani Asirvatham, Matthew P. Schuetze, Anna E. Min, Jaideep Padhye
Defensive Publications Series
Conventional observability and analytics platforms allow customers to track workflow performance and examine exceptions, but do not provide mechanisms through which users can conduct further analysis and investigation of potential workflow performance issues. Moreover, since conventional platforms do not distinguish between incomplete and slow journeys, users are not able to accurately identify the root cause of a performance issue. In order to address such issues, techniques are proposed herein to provide an enhanced digital observability and analytics platform that generates detailed analysis and visualization of business journeys and allows users to investigate potential reasons why a set of journeys did …

Philosophy In Science: Can Philosophers Of Science Permeate Through Science And Produce Scientific Knowledge?, Thomas Pradeu, Maël Lemoine, Mahdi Khelfaoui, Yves Gingras
Presidential Fellows Articles and Research
Most philosophers of science do philosophy ‘on’ science. By contrast, others do philosophy ‘in’ science (PinS), that is, they use philosophical tools to address scientific problems and to provide scientifically useful proposals. Here, we consider the evidence in favour of a trend of this nature. We proceed in two stages. First, we identify relevant authors and articles empirically with bibliometric tools, given that PinS would be likely to infiltrate science and thus to be published in scientific journals (‘intervention’), cited in scientific journals (‘visibility’), and sometimes recognized as a scientific result by scientists (‘contribution’). We show that many central figures …
