OBJECTIVE: To investigate the impact of clinical surveillance, primary radiotherapy, and primary surgery on overall survival (OS) in laryngeal carcinoma in situ (Cis).

METHODS: The 2006-2020 National Cancer Database was queried for adults with a biopsy-proven diagnosis of laryngeal Cis. Multivariable binary logistic and Cox proportional hazards regression models were implemented.

RESULTS: Of 3567 unique patients satisfying inclusion criteria, 514 (14.4%) underwent clinical surveillance, 1074 (30.1%) underwent primary radiotherapy, and 1979 (55.5%) underwent primary surgery. Receiving treatment at an academic/research facility was associated with higher odds of undergoing primary surgery compared to primary radiotherapy. Among 646 patients undergoing primary surgery …

A novel method of measuring endocochlear potential and cochlear function in a patient is presented. The method uses nuclear imaging, such as PET or SPECT, with a radiotracer such as 201Tl to visualize any potassium ion loss in the cochlea of the patient which can contribute to auditory disorders such as hearing loss. This non-invasive technique represents a new method for auditory diagnoses by measurement of cochlear properties which can aid in the development of new therapeutics for auditory disorders.

Calving icebergs at tidewater glaciers release large amounts of potential energy. This energy—in principle—could be a source for submarine melting, which scales with near‐terminus water temperature and velocity. Because near‐terminus currents are challenging to observe or predict, submarine melt remains a key uncertainty in projecting tidewater glacier retreat and sea level rise. Here, we study one submarine calving event at Xeitl Sít’ (LeConte Glacier), Alaska, to explore the effect of calving on ice melt, using a suite of autonomously deployed instruments beneath, around, and downstream of the calving iceberg. Putting An End To Protective Privilege: Georgia Should Recognize The Psychotherapist’S Duty To Warn, Jan M. Levine

This article addresses several anomalies and problems with Georgia’s law concerning the duty a psychotherapist2 has towards third parties (i.e., non-patients): the lack of a duty to warn third parties about dangerous patients; flaws in the mental health commitment statute; and inconsistencies in the way the state treats medical confidentiality and evidentiary privilege. Georgia is the only state that has failed to definitively address the psychotherapist’s duty to warn via caselaw or statute; it is time to finally and definitively fix a problem that has existed at least since 1982.

Methemoglobinemia is a rare congenital or acquired cause of functional anemia resulting from oxidation of hemoglobin. Prompt recognition and treatment are critical to prevent tissue hypoxia and end-organ injury. While other complications are well described, cardiac dysfunction is rarely reported—especially in children. We present what, to our knowledge, is the first known case of methemoglobinemia-induced transient cardiac dysfunction—or “myocardial stunning— in a pediatric patient. This case underscores the importance of maintaining a high index of suspicion and assessing cardiac involvement in such patients. Older adults who are grappling with food insecurity may enhance their food supply through community-based programs that provide free and subsidized food. This complicated system, categorized by group meal programs, home-delivered meals, and food provided to be eaten off-site, is complex, expensive, and may be difficult for older adults to use. Based on the book, Food for Thought: Understanding Older Adults Food Insecurity, this brief describes three key limits of community-based food programs for older adults. While several factors that influence the properties of ferroelectric devices have been extensively studied in bulk ceramics and polycrystalline thin films, limited is known about the same on single crystalline thin films which have great potential in memory and energy applications. This knowledge gap is largely due to the limited availability of substrates with lattice parameters suitable for supporting high-quality epitaxial growth. In this study, undoped, Fe-doped, and Nb-doped (001)-oriented Pb(Zr₀.₅Ti₀.₅)O₃ thin films were deposited using pulsed laser deposition, each with a SrRuO₃ bottom electrode and a PrScO₃ substrate. In this paper, a blind symbol synchronization algorithm is presented for orthogonal frequency-division multiplexing (OFDM) systems, and a timing function based on the redundancy of the cyclic prefix (CP) is introduced. The existing algorithms rely on the prior knowledge of the channel energy distribution i.e. channel power profile. In practical environment the channel power profile is unknown to the receiver and its statistics are expected to be highly changing. The present work provides suitable conditions for the synthesis of mesoporous materials with improved porosity using an optimization technique. The proposed preparation method involves forming final products by varying the amount of castor oil, media, and template removal. The obtained samples were examined by TGA, XRD, DRIFT and nitrogen physisorption studies. It was observed that the porosity of the obtained samples was dependent on the conditions under which the materials were synthesized. Remote, rural, and off-grid communities, particularly in Sub-Saharan Africa (SSA), face significant challenges in securing reliable and affordable electricity. While solutions such as solar home systems (SHSs) and diesel generators (DGs) have been adopted by some individuals, their deployment is often not optimized to meet the full load demand. This study addresses this challenge by proposing a hybrid swarm grid (HSG) design that integrates photovoltaic (PV), wind energy, and diesel generators (DGs). This paper explores the feasibility of implementing a flywheel energy storage system designed to generate voltage for the purpose of mitigating current flow through the transformer neutral path to ground, which is induced by a high-altitude electromagnetic pulse (HEMP) event. The active flywheel system presents the advantage of employing custom optimal control laws, in contrast to the conventional approach of utilizing passive blocking capacitors. A Hamiltonian-based optimal control law for energy storage is derived and integrated into models of both the transformer and the flywheel energy storage system. OBJECTIVE: Medical professionalism is fundamental to the delivery of high-quality patient care. There is a paucity of data to describe behaviors associated with negative professional and employment outcomes. This study examines common factors leading to corrective actions, remediation, and termination of emergency physicians (EPs).

METHODS: This was a cross-sectional survey sent to academic chairs in emergency medicine to identify factors for corrective actions, remediation, or termination of EPs. Survey items were piloted and response process validity gathered prior to administration. Data are reported as descriptive statistics.

Every social situation that people encounter in their daily lives comes with a set of unwritten rules about what behavior is considered appropriate or inappropriate. These everyday norms can vary across societies: some societies may have more permissive norms in general or for certain behaviors, or for certain behaviors in specific situations. In a preregistered survey of 25,422 participants across 90 societies, we map societal differences in 150 everyday norms and show that they can be explained by how societies prioritize individualizing moral foundations such as care and liberty versus binding moral foundations such as purity.
