Newly licensed nurses require professional confidence to deliver quality patient care within today’s complex healthcare settings. While nursing programs provide a foundational knowledge base, many newly graduated nurses report challenges in clinical judgment and translating learned knowledge into practice, contributing to a lack of professional confidence. Extended mentorship during the transition to practice may offer a solution to address this gap.

This study aimed to measure the impact of mentorship on the confidence levels of newly graduated Registered Nurses (RNs) during their first six months of practice. A cohort of nine newly licensed RNs completed a standard twelve-week orientation program. …

The law creating the new Trump Accounts explicitly authorizes employers to provide deposits matching contributions that their employees make to the accounts and also authorizes contributions by philanthropic organizations. Both features are essential for build­ing substantial lifetime assets for America’s children. Implementing them successfully – and ensuring that the funds reach every eligible child efficiently – will require an effective policy platform for routing, verifying, and reporting on third-party contributions. This brief discusses why traditional retail IRAs cannot support matching contributions and how to build a policy platform capable of accommodating multiple contribution streams.

This literature review explores the multifaceted relationship between gut microbiome (GM) dysbiosis and multiple sclerosis (MS), highlighting key themes that advance understanding of disease pathogenesis and therapeutic potential for treatments targeting the gut. The review underscores the immune system dysregulation in MS, emphasizing the roles of T cells, B cells, and microglia in neuroinflammation and demyelination. Historically, MS pathogenesis was thought to be driven mainly by T-cell dysregulation, but current research highlights the additional roles of B-cells and microglia, the resident immune cells of the central nervous system (CNS). Confidential Virtual Machines (CVMs) are increasingly adopted to protect sensitive workloads from privileged adversaries such as the hypervisor. While they provide strong isolation guarantees, existing CVM architectures lack first-class mechanisms for inter-CVM data sharing due to their disjoint memory model, making inter-CVM data exchange a performance bottleneck in compartmentalized or collaborative multi-CVM systems. Under this model, a CVM's accessible memory is either shared with the hypervisor or protected from both the hypervisor and all other CVMs. This design simplifies reasoning about memory ownership; however, it fundamentally precludes plaintext data sharing between CVMs because all inter-CVM communication must pass through hypervisor-accessible …

This quantitative study analyzed 47 survey responses to gauge in-service, pre-licensed teacher, and paraprofessional perceptions of cultural competency. Participants responded to the Central Vancouver Island Multicultural Society’s Cultural Competence Self-Assessment Checklist (Alpha = 0.7) to explore their awareness, knowledge, and skills in cultural competency. Moreover, through the application of Culturally Responsive Teaching (CRT) as a theoretical framework, this study highlighted how these perceptions related to culturally responsive teaching and provided several implications for their future preparation to further develop their cultural competency through the tenets of CRT. This paper presents a research project with a group of STEM majoring undergraduates attending a Hispanic Serving Institution participating in the CRESCENT program intended to support undergraduates through their basic math and science courses. We analyze 10,986 participants (mean age 77; 63% women; 54% non-White) across seven U.S. cohorts to study the relationship between mitochondrial DNA (mtDNA) heteroplasmy and nuclear DNA methylation. We identify 597 CpGs associated with heteroplasmy burden, generally showing lower methylation. These CpGs are enriched in dynamically regulated island shores and depleted in CpG islands, indicating involvement in context-specific rather than constitutive gene regulation. In HEK293T cells, we introduce a truncating mtDNA mutation (MT-COX3, mt.9979) and observe a positive correlation between variant allele fraction and methylation at cg04569152, supporting a direct mtDNA-nDNA epigenetic link. This research highlights the long-term value of compost in dryland wheat systems, where water scarcity and declining soil fertility limit productivity. A single, high-rate compost application (22 tons per acre) can improve soil moisture content, organic carbon content, aggregate stability, and microbial activity. All these benefits contributed to higher and more stable wheat yields, even more than two decades after application. These changes help to make the compost-treated wheat more resilient during dry years. While composting incurs a high upfront cost, its long-term benefits can significantly reduce the need for repeated fertilizer or soil amendments, offering cost and labor savings …

Background: Accurate CYP2D6 genotyping is essential for pharmacogenetic-guided prescribing of many clinically used medications. While digital PCR is increasingly used to determine copy number (CN) status for accurate phenotype prediction, there are few convenient methods to discriminate duplicated alleles and diplotypes such as CYP2D6*2/*4x2 and *2x2/*4, which predict intermediate and normal metabolizer phenotypes, respectively. We developed a novel method, StarTRAC-CYP2D6, "Targeted Reporting of Allele-specific CNV", for determining the number of specific CYP2D6 star allele copies. UNLABELLED: In the current landscape of clinical studies, the concept of statistically significant association is often mixed up with the expectation of improved prediction performance. We discuss the two concepts, association and prediction, and present the epidemiologic principles and statistical constructs that underlie the discrepancy between statistically significant associations and the rationale for their lack of impact on improving prediction in terms of discrimination. This issue is illustrated using an existing breast cancer dataset. It is human nature to seek a sense of belonging, acceptance, and community. College students also share this desire for connection in higher education, but some face more barriers and challenges. This study examined the experiences of first-generation college students who are also immigrants. The goal was to understand how to best support students in college, including through academic advising, at a four-year research university in the Pacific Northwest. Thogotoviruses are arthropod-borne viruses belonging to the genus Thogotovirus within the family Orthomyxoviridae. Like other orthomyxoviruses, such as the influenza A viruses (IAV), thogotoviruses replicate in the nucleus. As a result, progeny viral ribonucleoprotein complexes (vRNPs) must be exported to the cytoplasm prior to virion assembly and budding at the plasma membrane. In IAV, this export depends on binding of the viral nuclear export protein (NEP) to the cellular exportin chromosomal maintenance 1 (CRM1). In contrast, the mechanism of vRNP nuclear export, including identification of a protein with NEP functionality, has not been characterized for any thogotoviruses. Here, we …

Netherton syndrome is an autosomal recessive genodermatosis caused by biallelic mutations in the SPINK5 gene, a gene that codes for lymphoepithelial Kazal-type-related inhibitor 1 (LEKT1) protein. This disorder is characterized by erythroderma, ichthyosis, hair shaft abnormalities, and immune dysregulation. While there is increasing research and recognition of Netherton syndrome, much remains unknown, with limited knowledge of the history, manifestations, and responses to therapies. Background: Greater understanding of differential therapeutic sensitivity, specifically to immunotherapy, in small-cell lung cancer (SCLC) is required.

Methods: We explored SCLC heterogeneity through integrated molecular characterization of tumor tissue samples from 159 treatment-naive patients, utilizing genetic, epigenetic, transcriptional, and proteomic profiling, immunohistochemistry staining for multiple biologically relevant markers including transcriptional subtype-defining proteins, and spatial immune profiling using multiplex immunofluorescence.

Results: Multi-omics analysis confirmed high heterogeneity across/within neuroendocrine and non-neuroendocrine subtypes. Methylomics analysis identified four methylome clusters that may enhance subtype prediction, prognosis, and longitudinal monitoring of subtype evolution. Epigenetically targeted therapies, especially those inhibiting S-adenosyl-l-methionine (SAM)-dependent methylations of DNA, mRNA, and histones, have advanced rapidly in cancer treatment. However, these therapies remain underexplored for antibiotic development, despite the growing threat of antimicrobial resistance. Here, we screened a focused library of SAM analogs against the DNA adenine methyltransferase CamA specific to the enteric pathogen Clostridioides difficile. At the same time, we examined six other adenine methyltransferases, including two bacterial DNA methyltransferases and four human RNA methyltransferases having distinct RNA substrates. Importance: Intraoperative hypotension is associated with organ injury. However, it remains unknown if targeted blood pressure management during surgery can improve clinical outcomes.

Objective: To evaluate whether individualized vs routine perioperative blood pressure management during major abdominal surgery improves clinical outcomes in patients considered at high risk of postoperative complications.

Design, setting, and participants: This randomized single-blind clinical trial enrolled patients 45 years or older undergoing elective major abdominal surgery with general anesthesia expected to last 90 minutes or longer who had at least 1 additional high-risk criterion between February 26, 2023, and April 25, 2024, at 15 German university …

This paper examines behavior in contests where the prize value is ambiguous. We develop a theoretical model of bidding in a Tullock contest with an ambiguous prize where contestants account for the ambiguity attitude of their rival. Ambiguity affects optimal behavior via two countervailing channels - a direct effect arising from contestants’ ambiguity about the value of the prize and an indirect effect corresponding to the effect of ambiguity on the opponent’s behavior. The Woven EndoBridge (WEB) device treats wide-necked bifurcation aneurysms, but occlusion rates vary. This study aims to identify factors associated with immediate WEB device occlusion. Data from patients treated with WEB devices across 36 sites were analyzed. Machine learning algorithms and ordinal regression models were developed to predict immediate incomplete occlusion for ruptured and unruptured aneurysms. The study included 1565 patients, with 436 ruptured and 1129 unruptured aneurysms. Immediate complete occlusion was achieved in 38.3% of ruptured and 32.8% of unruptured aneurysms. For ruptured aneurysms, the CatBoost classifier achieved an AUROC of 0.69. Key predictors of incomplete occlusion included pretreatment …

Multiple myeloma (MM) arises from abnormal plasma cells (PCs) progressing from precursor states, including monoclonal gammopathy of undetermined significance (MGUS) and smoldering multiple myeloma (SMM). Understanding this transition and progression to overt MM requires improved non-invasive strategies. We employed a liquid biopsy approach to detect and characterize circulating PCs across disease states in 68 patients (MGUS = 11, SMM = 21, NDMM = 19, RRMM = 17) using multi-channel immunofluorescence staining and machine learning-assisted rare event detection.
