背景
髓鞘少突膠質細胞糖蛋白抗體相關疾病(MOGAD)是一種罕見的自體免疫發炎性疾病,主要涉及中樞神經系統(CNS),其主要特徵是視神經、腦部和脊髓脫髓鞘。我們報告一例最初疑似復發性中風的病例,該病例在住院中風復健期間最終確診為MOGAD。
病例概要
49歲女性,左側大腦半球梗塞,在中風後復健期間出現多次右側肢體肌力下降伴隨失語症。這些神經功能波動偏離了典型卒中的預期復健軌跡。卒中發作兩個月後行腦部磁振造影(MRI)檢查,結果顯示右側內囊後肢、腦橋及C1脊髓出現多處新發病灶。後續血清學檢查顯示抗髓鞘少突膠質細胞糖蛋白(MOG)IgG抗體陽性,而水通道蛋白4(AQP4)抗體陰性,確診為MOGAD。患者接受了甲潑尼龍治療及充分的復健治療,追蹤期間肌力顯著改善,未見脫髓鞘病灶復發。
結論
本案例凸顯了復健科醫師在早期發現類似中風的神經發炎性疾病的關鍵作用。復健科醫師處於獨特的領先地位,能夠透過持續的功能監測識別「危險信號」;任何偏離預期復健路徑或無法解釋的神經功能波動都應促使醫生進行進一步檢查。早期識別此類非典型模式,並及時進行免疫治療和多學科康復,可以預防永久性功能衰退,並優化長期預後。

Background: Competency-Based Medical Education (CBME) emphasizes formative, learner-centred assessment. Blood pressure (BP) measurement, a core clinical skill, integrates knowledge, skills, communication, ethics, and attitude. While rubrics are widely used to standardize evaluation, their utility in triangulating faculty, peer, and self-assessment in BP measurement remains underexplored.
Aim: To evaluate the reliability and educational value of rubric-based faculty, peer, and self-assessment in BP determination among Phase 1 MBBS students using a mixed-methods approach.

Language barriers significantly hinder academic performance, psychological well-being, and clinical communication skills of Arabic-speaking healthcare students educated through English-medium instruction. Although existing instruments focus on clinical communication—often among physicians—critical dimensions such as academic challenges and psychological impacts remain underassessed, and tools rarely address disciplines beyond medicine (e.g., nursing, pharmacy, radiology). Studies in Arabic-speaking contexts typically employ author-developed or adapted questionnaires lacking rigorous validation. The development of Artificial Intelligence (AI) in healthcare is largely dependent on the quality of medical datasets. However, these datasets often fail to accurately represent women (and those seen as women) due to historic, implicit and biological biases. This under-representation can lead to biased, inequitable and even harmful models. Building upon the findings of previously conducted qualitative semi-structured semantic interviews with clinicians on their perceptions of women’s health, this paper presents a framework for translating qualitative findings to dataset characteristics via operationalisation. Iron electrowinning and electrodeposition sit at the intersection of classical metallurgy and developing sustainable manufacturing. With the iron and steel industry responsible for 7 - 9% of global CO₂ emissions, low-temperature electrochemical pathways for iron production have garnered significant research interest. This review examines the fundamental electrochemical principles governing iron deposition, the thermodynamic constraints imposed by the iron-water system, and the kinetic challenges posed by the parasitic hydrogen evolution reaction (HER). The three key electrolyte systems are objectively evaluated: acidic sulfate and chloride electrolytes, alkaline suspension electrolytes and water-in-salt electrolytes. Artificial intelligence is increasingly embedded in safety-critical systems, including transportation, where people must make fast decisions under uncertainty and high risk. As AI takes on a larger role in supporting human judgment, researchers and designers must evaluate more than just its technical performance, they must ask whether people can understand the system and use it effectively in high-risk settings.
This dissertation presents two distinct evaluation methodologies. First, the Human-Centered Evaluation, examines how intelligent in-vehicle alert systems influence driver decision behavior across two experiments. Electrochemical sensing is widely used for chemical and biological detection due to its high sensitivity, label-free operation, and compatibility with miniaturized electronic systems. However, conventional microelectrode platforms operate in an ensemble-averaged regime in which the measured current represents the collective response of many molecules interacting with the electrode surface. This ensemble averaging masks localized nanoscale electrochemical events and limits the ability to detect rare interactions, such as single molecules or nanoparticles. Achieving single-entity electrochemical detection therefore requires strategies that confine electrochemical reactions to nanoscale regions while maintaining compatibility with scalable planar microfabrication.

In-situ resource utilization (ISRU) is the practice of finding, extracting, and using the local resources found on extraterrestrial bodies, such as the Moon and Mars. ISRU enables in-situ production of mission consumables—such as rocket fuel and potable water—which will be critical for establishing a sustainable human presence on the Moon, a goal NASA intends to accomplish by 2028 with the Artemis missions. This dissertation starts from the position that the stability of liberal-democratic political schemes relies on its citizens mutually accepting a basic conception of political justice. While these conceptions of justice may vary, most incorporate the inter-connected concepts of autonomy, liberty, liberalism, and the rule of law. Within this conception of a liberal democracy, a potentially destabilizing issue arises when a legislatively passed law is put into effect, but a portion of the citizens perceive it as unjust. Wheat middlings are grain byproducts that are underused for bioenergy production despite having a high lignocellulose content. Its use as a feedstock for catalytic pyrolysis depends on effective and sustainable fractionation to enhance efficiency and product yield. This study tested mechanical pre-fractionation (sieving and air classification) to reduce ash content of certain fractions to < 1% for catalytic pyrolysis and enrich nutrient levels of other fractions for feed applications. Wheat middlings were separated into 11 fractions using screen sizes from 106 µm to 1651 µm, and the composition of each fraction was characterized (ash, protein, structural carbohydrates, lignin, starch, and simple sugars). For cyclone separation, a full factorial design was used to study the effect of air velocity (15,18, 20 m/s) and feed rate (33, 35, 38 kg/h) on the amount of material collected in the underflow and overflow, and the composition of these fractions was characterized. Sieving showed a pronounced segregation of ash content, from 4.5% to 6.5% in wheat middlings fractions, with 5.7% being the lowest for coarse fractions which has higher lignocellulose content, and 4.5% in the fine fraction (150-212 µm). The same defined enrichment was observed in all nutrients except for protein. Starch enrichment zone were defined in the fine fractions (212 µm and below), for structural carbohydrates and lignin in the coarse fractions (600 µm and above), and for ash in coarse fractions (1180-1651 µm). In contrast, protein content was not significantly different between various fractions (p value=0.787). The fiber rich fraction (> 600 µm), suitable for pyrolysis, which have a composite ash value of 6.0% can be sorted and mixed with intermediate fractions to reduce ash to 5.3% and the remaining fractions rich in digestible nutrients used for animal feed formulation. The concept of neighborhood has been long studied with little agreement beyond the notion that neighborhoods are somewhat self-contained areas within a wider urban environment. Despite the lack of a finite definition, there is wide agreement of the importance of studying population dynamics at the neighborhood scale. Many attempts have been made to spatially define, measure, and quantify contemporary neighborhoods, but less attention has been given to methods to spatially define historical neighborhoods. Electromagnetic invisibility cloaks guide waves around an object so that the transmitted wavefront remains undisturbed. Most experimental microwave cloaking studies have focused on transverse electric (TE) polarization due to established measurement techniques. In this thesis, the experimental realization and characterization of a dielectric photonic crystal cloak operating under transverse magnetic (TM) polarization are presented. A measurement system operating in the X-band was developed to map the electric field distribution of the transmitted waves. Cloaking performance was first evaluated qualitatively by comparing numerical and experimental field distributions, where restoration of a flat wavefront indicated effective cloaking. This article conceptualizes classical guitar left-hand technique through a physiomechanical framework that places coordinated movement—particularly shifting—at the center of technical practice. Challenging traditional positional doctrines that privilege static hand forms and digital independence, the author argues that such approaches often disregard the functional anatomy of the limb, resulting in inefficiency, unnecessary tension, and potential injury. Advancing the principle that form must follow function, the study proposes shifting as the fundamental technical category governing all left-hand procedures. BACKGROUND: Stroke leads to complex chronic structural and functional brain changes that specifically affect motor outcomes. The brain predicted age difference (PAD) has emerged as a sensitive biomarker of both sensorimotor and cognitive function after stroke. Our previous study showed a higher global brain PAD associated with poorer motor function after stroke. However, the association between local stroke lesion load, regional brain age, and motor impairment is unclear. This study aimed to investigate the associations between focal lesion damage, regional brain PAD in both hemispheres, and motor outcomes in chronic stroke, and to identify key predictors of motor impairment.
