Endothelin-1 In The Failing Fontan: Pathobiology, Precision Therapeutics, And Future Trial Design, Iman Maiza

The Fontan circulation, devised as definitive palliation for single-ventricle congenital heart disease, imposes systemic venous hypertension, loss of pulmonary arterial pulsatility, and restricted preload reserve. These hemodynamic trade-offs progressively injure the pulmonary vasculature, liver, and lymphatic system, producing late morbidities including elevated pulmonary vascular resistance, Fontan-associated liver disease (FALD), protein-losing enteropathy, and arrhythmias. Endothelin-1 (ET-1), a potent vasoconstrictor and profibrotic mediator, plausibly unifies these complications. Mechanistic studies demonstrate ET-1 upregulation in failed Fontan lungs, activating PLC–Ca²⁺, RhoA/ROCK, and MAPK/ERK cascades to drive vasoconstriction and remodeling. The current study is a discourse analysis investigating anti-idyllic cinematic representations, specifically in the horror genre, of rural people, places, and cultures. Findings reveal that anti-idyllic images and discourses are prominent within recently released horror films, which rely heavily on framing rural people as the deviant “other.” Special attention is given to the cultural work of these stereotypical depictions and the associated social harms for rural communities. Specifically, we argue that (1) corporations, particularly film companies, contribute to social harm in rural areas by disseminating negative and anti-idyllic representations; (2) these portrayals have tangible and detrimental effects on both people …

Intimate partner violence (IPV) is a recurring social problem that has individual, interpersonal, community, and societal level consequences necessitating interdisciplinary knowledge and a holistic, systemic approach for prevention. Recent research on IPV indicates the need to identify the implications of the COVID-19 pandemic for IPV risk and subsequent consequences to inform mitigation solutions. The ebb and flow of social control efforts aimed at LGBTQ people necessitate a thorough investigation into their experiences. This review of the literature is organized around four key themes that characterize LGBTQ people's experiences with crime, criminalization, victimization, and the criminal legal system—including police, courts, and carceral facilities. LGBTQ people experience excessive rates of arrest and incarceration and have higher rates of victimization, which illustrate disproportionality. Their experiences differ from those of their cisgender and heterosexual counterparts and are conditioned by age, race, class, gender, and sexual orientation, illustrating intersectionality. This study investigates the effects of electron beam (e-beam) irradiation on the mechanical and structural properties of eight bulk metallic samples, comprising both polycrystalline (PC) and single-crystalline (SC) forms of Ni, Cr, V, and Ti. These metals were evaluated as potential candidates for beam exit windows in high-power (MW-class) particle accelerators. The primary objective is to identify metals capable of withstanding the conditions of high-power/MW-class e-beam accelerators and serve effectively as exit windows. As the urgency to address climate change and modernize energy infrastructure grows, the building sector plays a key role in improving energy efficiency and reducing carbon emissions. This study evaluates five energy retrofit strategies for Building 101 at The Navy Yard in Philadelphia, comparing two real-world proposals from energy service companies with three simulation-based packages derived from Building Energy Simulation (BES) tools. The study examined whether advanced BES tools provide greater accuracy and decision-making value compared to simpler alternatives. The global energy transition toward decarbonization and digitalization requires advanced methods to manage decentralized, data-intensive cyber-physical energy systems. This systematic review analyzes 106 research studies on Generative Artificial Intelligence (GenAI) and Large Language Models (LLMs) in renewable energy and smart grids, organized into seven application clusters covering forecasting, system design, operation, reliability, data and cybersecurity, and energy markets. The review situates these applications within a Cyber-Physical-Social Systems (CPSS) framework. Results show that GANs dominate current applications (47.2%), followed by LLMs (10.4%) and VAEs (9.4%), with growing adoption of diffusion and score-based models (7.5% each). [Introduction] Smirnova’s The Prescription-to-Prison Pipeline offers a critical examination of the impacts of prescription drugs as well as the over-policing and policies created to push individuals into the carceral system rather than providing other options for assistance. Smirnova further captures the social inequalities of race and class of those encased in this pipeline. Within this understanding of the detriment that the system, medical providers, and society place on those who use substances, Smirnova’s use of interviews allows for a clear understanding of how these systems work in conjunction to uphold the pipeline.

As electric vehicles (EVs) gain popularity, efficient routing and charging solutions remain challenging due to time-dependent travel variability, sparse charging infrastructure, and heterogeneous user preferences. To address these challenges, this paper introduces a decision-support system that integrates three complementary methods: Temporal Multimodal Multivariate Learning (TMML) for real-time characterization of travel time uncertainty, Time-Dependent Shortest Path (TDSP) for reliability-aware route choice, and Deep Q-Network (DQN) reinforcement learning for adaptive charging decisions in sparse infrastructure environments. TMML updates link-level travel time distributions in real-time through Bayesian inference with cluster-based propagation, reducing uncertainties across the network. "When I Think Of The Public Library, I Think Of White": Black Families Counter-Narratives Of Library Irrelevance And Community Repair, Amber Matthews

While public libraries espouse equitable access, few studies examine Black community disengagement. Drawing on Critical Race Theory and the theme "Information in Repair," this paper analyzes semi-structured interviews with seven Black parents of youth aged 13-24 in London, Ontario. Findings reveal that their lack of library engagement stems not from access barriers but from libraries functioning as racialized spaces irrelevant to Black lived experiences in Canada. Where libraries fall short, Black-led organizations perform continuous, invisible reparative labour through culturally responsive programming and trusted information networks. Students often struggle to employ effective learning and study strategies in postsecondary settings that challenge their academic success. These struggles can be amplified depending on student characteristics like gender and ethnicity. One way to improve their success is an awareness they may need help and how to most productively use this help. We sought to understand students’ patterns of help-seeking behaviors as well as whether these patterns differed by gender and ethnicity. Objective

Prior research on confessions of physical abuse (PA) has explored mechanisms of injury. Little is known about conditions supporting a confession of abuse or how confessions may influence case outcomes.

Methods

This cross-sectional study of suspected PA in children age < 10 years at 10 centers participating in CAPNET, a multicenter child PA research network, included children with in-person Child Abuse Pediatrics (CAP) consultations between Feb 2021-Dec 2022 and excluded children without injury. Our focus was a confession of inflicted injury known to the CAP during clinical involvement. We compared child and clinical characteristics, out-of-home (OOH) placements, and arrests between cases with and without confessions. Multivariable models using generalized estimating equations (GEE) produced adjusted predicted probabilities (APP) clustering by CAPNET site.

Results

Confessions were known to CAPs in 115/4297 (2.7%) cases, with significant site variability. In a multivariable GEE model, confessions were more likely in cases with injuries with high-specificity for abuse (APP 6.0% vs 1.5%, p< 0.001), near-fatality (APP 5.5% vs 2.3%, p< 0.001), and older children (APP 4.8% vs 2.5%, p = 0.025). OOH placements and arrests were more common in cases with confessions. CAP awareness of confessions, OOH placements, and arrests did not follow racial or ethnic patterns seen in the US child welfare system. Lower socioeconomic status was associated with OOH placements and arrests but not CAP awareness of confessions.

Conclusions

Our findings highlight differences in case characteristics and outcomes where a confession is known to the CAP and raise questions about agency …

[Introduction] Prurigo nodularis (PN) is a chronic neuroinflammatory dermatosis of unknown etiology characterized by severely pruritic, symmetrically-distributed nodules with a negative impact on quality of life [1]. While some consider PN a primary idiopathic disorder, it often arises as a secondary manifestation of underlying conditions, including atopic dermatitis (AD), chronic kidney disease (CKD), human-immunodeficiency-virus (HIV), and thyroid disorders [2]. [Introduction] Central centrifugal cicatricial alopecia (CCCA) is a scarring alopecia that predominantly affects middle-aged women of African descent.¹ It begins at the vertex scalp and expands centrifugally, and given that symptoms may be absent, many patients have scarring at presentation. CCCA is primarily described in adult populations, with limited reports of CCCA in children.²-⁴ We aimed to characterize demographics, clinical findings, and histological features among pediatric patients with CCCA to facilitate earlier diagnoses, prior to irreversible alopecia.

On-demand topical inserts for HIV prevention may be a good option for people who have a low frequency of sexual activity. We recently demonstrated in a preclinical macaque model that rectal application of inserts containing tenofovir alafenamide fumarate (TAF) and elvitegravir (EVG) conferred protection against SHIV infection (93% efficacy) when applied as pre-exposure prophylaxis 4 hours before SHIV exposure. Here, we show that the same inserts provide significant postexposure protection (82.9% efficacy) when applied 4 hours after SHIV exposure. Our findings further support the ongoing clinical development of TAF/EVG inserts for on-demand HIV prevention.

This article combines methods from existing techniques to identify multiple changepoints in non‐Gaussian autocorrelated time series. A transformation is used to convert a Gaussian series into a non‐Gaussian series, enabling penalized likelihood methods to handle non‐Gaussian scenarios. When the marginal distribution of the data is continuous, the methods essentially reduce to the change of variables formula for probability densities. When the marginal distribution is count‐oriented, Hermite expansions and particle filtering techniques are used to quantify the scenario. This paper studies the use of Multi-Grade Deep Learning (MGDL) for solving highly oscillatory Fredholm integral equations of the second kind. We provide rigorous error analyses of continuous and discrete MGDL models, showing that the discrete model retains the convergence and stability of its continuous counterpart under sufficiently small quadrature error. We identify the DNN training error as the primary source of approximation error, motivating a novel adaptive MGDL algorithm that selects the network grade based on training performance. Numerical experiments with highly oscillatory (including wavenumber 500) and singular solutions confirm the accuracy, effectiveness and robustness of the proposed approach.

We propose a new method for parallelization of the first-order backward difference discretization (BDF1) of the first-order time derivative in nonlinear partial differential equations, such as conservation law equations. The time derivative term is discretized by using the method of lines based on the implicit BDF1 scheme, while the inviscid and viscous terms are approximated by conventional 2nd-order central discretizations of the 1st- and 2nd-order derivatives in each spatial direction. We examine the role of major customers in shaping firms' environmental, social and governance (ESG) practices. We find that firms with major customer relationships undertake fewer ESG activities compared to those without such ties. The association is attenuated when institutional ownership is high, firms are less diversified, customers exhibit greater bankruptcy risk and lower switching costs and during periods of elevated equity market sentiment. Taken together, our findings highlight that reliance on a concentrated customer base can weaken firms’ incentives to engage in ESG practices, with important implications for supply chain sustainability.

In the United States, Colorectal Cancer (CRC) is one of the most commonly diagnosed cancers and the third leading cause of death among non-Hispanic African American/Black (henceforth Black) men. Inadequate CRC knowledge leads to missing early warning signs, delaying prevention and increasing risk. Emerging adulthood (18-25) is a key period for building healthy behaviors and addressing barriers to CRC knowledge in addition to understanding attitudes and practices to CRC screening. Introduction

Antipsychotic medication (APM) use in nursing home (NH) patients with dementia is common but carries risks. This study assessed the association between APM use and mortality, stroke, and myocardial infarction (MI) compared to non-use, as well as differences between first- and second-generation APMs.

Methods

A serial cross-sectional study using Medicare data examined outcomes from 2012 to 2015 in a national sample of 328,138 US NH residents aged 50 and older with dementia. Multivariate logistic regressions were used to analyze risk.

Detrital sediments that accumulate downstream and are preserved in sedimentary rocks can allow characterization of geologic formations that are inaccessible for spatial or temporal reasons. However, mixing, sorting, and alteration of sediment during transport may complicate reconstruction of protolith characteristics. We test the preservation of three key provenance signals in coarse fluvial sand at a Mars analog watershed in Iceland to determine whether detrital sediments capture (a) watershed magmatic chemical variation, (b) textural indicators of lava-water interaction during eruption and cooling, and (c) hydrothermal alteration. Chronic respiratory diseases (CRDs) remain 1 of the leading causes of preventable morbidity and disability worldwide, affecting up to one-third of the total Western population in 2025. Background

Tezepelumab (an anti-thymic stromal lymphopoietin monoclonal antibody) treatment resulted in reduced exacerbations and improved sino-nasal symptoms and health-related quality of life in patients with severe uncontrolled asthma (SUA) and chronic rhinosinusitis with nasal polyps (CRSwNP) in the NAVIGATOR study.

Objective

To evaluate the effect of tezepelumab versus placebo on sleep disturbances, activity levels, and physical exertion tolerance in NAVIGATOR patients with SUA overall and in those with past medical history of comorbid CRSwNP.

The marine aquarium trade (MAT) is a significant global industry harvesting millions of wild-caught, live coral reef fishes for public and private aquaria markets in the United States and Europe annually, while supporting fisher livelihoods in the Indo-Pacific. This diverse and species-rich trade is considered data-limited, creating barriers to quantifying the current and future socio-ecological sustainability of the fishery. We present a revised and expanded productivity–susceptibility analysis (PSA) that serves as a holistic risk assessment to estimate the vulnerability of marine aquarium fish to overfishing. Worldwide, groupers (Epinephelidae) are commercially valued fishes, which also play key ecological roles on tropical and subtropical reefs. In 2007 and 2016, the IUCN's Groupers and Wrasses Specialist Group assessed all 160+ grouper species, with 17 of these being identified as threatened in 2016 and the major threat factor being overexploitation. Our present study aimed to identify whether management measures (MMs) for previously assessed groupers were established, whether these measures aligned with IUCN's Red List categories, and whether they effectively protect grouper populations. The IUCN Red List of Threatened Species, the most widely used global system for assessing species' extinction risk, has become a foundational source of information for conservation management, policy and research. Since the adoption of quantitative extinction risk criteria more than three decades ago, the Red List has expanded substantially in scope and influence, informing decisions ranging from species conservation and protected area designation to international agreements, corporate risk assessments and global biodiversity indicators. Background:

F/TAF was shown to be non-inferior to F/TDF as pre-exposure prophylaxis (PrEP) in men, but approval was not extended to cisgender women. We report the results of PrEPVacc, in which a predominantly female population was randomly allocated to receive daily oral F/TDF or F/TAF for ∼6 months within a HIV-1 prophylactic vaccine trial.

Setting:

Five study sites in three African countries (Uganda, Tanzania, South Africa).
