BACKGROUND: Holistic review promotes diversity, but widespread implementation remains limited.

OBJECTIVE: We aimed to develop a practical approach to incorporate holistic review principles in screening applicants in the Electronic Residency Application Service (ERAS) and to assess the impact on diversity.

METHODS: Three residency programs (internal medicine [IM], pediatrics, and surgery) at McGovern Medical School developed filters to identify applicants with experiences/attributes aligned with the institutional mission. These filters were retroactively applied to each program's 2019-2020 applicant pool using built-in ERAS capabilities to group applicants by user-defined features. BACKGROUND: Patients and families report experiencing a multitude of harms from medical errors resulting in physical, emotional, and financial hardships. Little is known about the duration and nature of these harms and the type of support needed to promote patient and family healing after such events. We sought to describe the long-term impacts (LTIs) reported by patients and family members who experienced harmful medical events 5 or more years ago.

METHODS: We performed a content analysis on 32 interviews originally conducted with 72 patients or family members about their views of the factors contributing to their self-reported harmful event. Interviews …

Rothmund-Thomson syndrome (RTS) is an autosomal recessive genetic disorder characterized by poikiloderma, small stature, skeletal anomalies, sparse brows/lashes, cataracts, and predisposition to cancer. Type 2 RTS patients with biallelic RECQL4 pathogenic variants have multiple skeletal anomalies and a significantly increased incidence of osteosarcoma. Here, we generated RTS patient-derived induced pluripotent stem cells (iPSCs) to dissect the pathological signaling leading to RTS patient-associated osteosarcoma. RTS iPSC-derived osteoblasts showed defective osteogenic differentiation and gain of in vitro tumorigenic ability. Transcriptome analysis of RTS osteoblasts validated decreased bone morphogenesis while revealing aberrantly upregulated mitochondrial respiratory complex I gene expression. Attribute based encryption is suitable for data protection in data outsourcing systems such as cloud computing. However, the leveraging of encryption technique may retrain some routine operations over the encrypted data, particularly in the field of data retrieval. This paper presents an attribute based date retrieval with proxy re-encryption (ABDR-PRE) to provide both fine-grained access control and retrieval over the ciphertexts. The proposed scheme achieves fine-grained data access management by adopting KP-ABE mechanism, a delegator can generate the re-encryption key and search indexes for the ciphertexts to be shared over the target delegatee’s attributes. Several studies have established specific relationships between White Matter (WM) and behaviour. However, these studies have typically focussed on fractional anisotropy (FA), a neuroimaging metric that is sensitive to multiple tissue properties, making it difficult to identify what biological aspects of WM may drive such relationships. Here, we carry out a pre-registered assessment of WM-behaviour relationships in 50 healthy individuals across multiple behavioural and anatomical domains, and complementing FA with myelin-sensitive quantitative MR modalities (MT, R1, R2∗).

In genetics, aggregation of many loci with small effect sizes into a single score improved prediction. Nevertheless, studies applying easily replicable weighted scores to neuroimaging data are lacking. Our aim was to assess the reliability and validity of the Neuroimaging Association Score (NAS), which combines information from structural brain features previously linked to mental disorders. Participants were 726 youth (aged 6–14) from two cities in Brazil who underwent MRI and psychopathology assessment at baseline and 387 at 3-year follow-up. This project seeks to engage political theory surrounding processes of creolization and solidarity, as well as their usefulness towards political movements. Current socialization within the United States promotes individualism and, as a result, an inability to see how various sociopolitical issues interact with one another and affect large portions of the population in ways that are not always obvious. Through a commitment to open engagement and practical empathy, creolized solidarity could provide a baseline from which to evaluate prevalent issues as well as our relationships with one another, the natural connection that comes from simply being human.

This study has emerged through a combination of art and science, visually and contextually analyzing anatomy lesson paintings as commemorations of physicians, artists, and medical and anatomical advancements. This study focuses on the works of five major artists through the Dutch Baroque and American Realist periods: Michiel Janszoon van Mierevelt, Rembrandt Harmenszoon van Rijn, Thomas Cowperthwait Eakins, Albert Sands Southworth, and Josiah Johnson Hawes. CASE: Biologic therapy has revolutionized the treatment of inflammatory bowel disease (IBD). Nevertheless, it is well known that biologics are associated with an increased risk of opportunistic infections such as tuberculosis (TB). Screening tests indicated before biologic initiation, namely interferon gamma release assay (IGRA) or tuberculin skin tests (TST), may be falsely negative. We present the case of a male with uncontrolled Crohn’s disease (CD) with a negative screen for TB and how a high index of suspicion led to the discovery of latent tuberculosis infection (LTBI) and the appropriate deferment of biologic therapy. High grade neuroendocrine carcinoma of the breast (HGNCB) is an aggressive and rare primary breast malignancy. No standard management recommendations exist for this entity and they are managed similarly as traditional breast cancers. Here we present the first case of a 54 year old female with high grade neuroendocrine carcinoma of the breast managed non-surgically with concurrent chemoradiation. At the time of this publication, she is more than 8 years from diagnosis without evidence of recurrence.

This study uses the recently released Cataloguing Code of Ethics to evaluate a project which explored how to ethically, efficiently, and accurately add demographic terms for African American authors to catalog records. By reviewing the project through the lens of these principles the authors were able to examine how their practice was ethical in some ways but could have been improved in others. This helped them identify areas of potential improvement in their current and future research and practice and explore ethical difficulties in cataloging resources with records that are used globally, especially in a linked data environment.

We present results from Euler–Lagrange simulations of turbulent flow over an erodible monodisperse bed of particles at a shear Reynolds number of R e τ = 180 . The Galileo number G a and the ratio of Shields to the critical Shields number Θ / θ c r were varied in the range 11.4 to 29.8 and 1.32 to 5.98, respectively. Two drag force models were investigated along with the influence of lift, particle rotation, and tangential collision forces for each model. Background: Progress in precision psychiatry is predicated on identifying reliable individual-level diagnostic biomarkers. For psychosis, measures of structural and functional connectivity could be promising biomarkers given consistent reports of dysconnectivity across psychotic disorders using magnetic resonance imaging.

Methods: We leveraged data from four independent cohorts of patients with psychosis and control subjects with observations from approximately 800 individuals. We used group-level analyses and two supervised machine learning algorithms (support vector machines and ridge regression) to test within-, between-, and across-sample classification performance of white matter and resting-state connectivity metrics.

Background: The hippocampus (HP) is affected across psychoses, including schizophrenia (SZ), bipolar type 1 (BDP) and schizoaffective (SAD) disorders. We examined HP subfield volumetric abnormalities along the anterior-posterior (ventral-dorsal) axis of the HP in psychosis probands, defined by traditional (DSM) diagnoses and biologically defined subtypes (biotypes, based on cognition and electrophysiology). We hypothesized that biotypes would be better discriminated by HP longitudinal axis subfields abnormalities than DSM. Somatic mutations arise randomly or are induced by environmental factors, which may increase the risk of Alzheimer's disease (AD). Identifying somatic mutations in sporadic AD (SAD) may provide new insight of the disease. To evaluate the potential contribution of somatic single nucleotide variations (SNVs), particularly that of well-known AD-candidate genes, we investigated sequencing data sets from four platforms: whole-genome sequencing (WGS), deep whole-exome sequencing (WES) on paired brain and liver samples, RNA sequencing (RNA-seq), and single-cell whole-genome sequencing (scWGS) of brain samples from 16 AD patients and 16 non-AD individuals. Background: Lithium is regarded as a first-line treatment for bipolar disorder (BD), but partial response and non-response commonly occurs. There exists a need to identify lithium non-responders prior to initiating treatment. The Pharmacogenomics of Bipolar Disorder (PGBD) Study was designed to identify predictors of lithium response.

Methods: The PGBD Study was an eleven site prospective trial of lithium treatment in bipolar I disorder. Subjects were stabilized on lithium monotherapy over 4 months and gradually discontinued from all other psychotropic medications. A loop is a rather general algebraic structure that has an identity element and division, but is not necessarily associative. Smooth loops are a direct generalization of Lie groups. A key example of a non-Lie smooth loop is the loop of unit octonions. In this paper, we study properties of smooth loops and their associated tangent algebras, including a loop analog of the Maurer-Cartan equation. BACKGROUND: The heterogeneity of tinnitus perception and its impact necessitates a tailor-made management approach in everyone. The current study examined the effects of residual inhibition in combined amplification and sound therapy in individuals with tinnitus and coexisting hearing loss.


METHODS: A retrospective analysis was performed on patients with tinnitus and coexisting hearing loss between 2016 and 2019. Cold Spray Additive Manufacturing is a technology capable of mass manufacturing components with complicated geometry and coating substrates in hard-to-reach areas. In addition, cold spray also has the ability of conducting a green manufacturing process, with zero waste of renewable feed material, and zero gas and chemical emission. This paper investigates solely cold spray as an additive manufacturing technology with cork as the natural material. CFD results were used to predict the physical behavior of air and the cork particles. After unsuccessful coatings, final results showed that when moisture is added, cork is successfully cold sprayed, and agglomeration is experienced. …

Introduction: The multifaceted clinical presentation of fibromyalgia (FM) supports the modern understanding of the disorder as a more global condition than one simply affecting pain sensation. The main pharmacologic therapies used clinically include anti-epileptics and anti-depressants. Conservative treatment options include exercise, myofascial release, psychotherapy, and nutrient supplementation.

Methods: Narrative review.

Results: Nutrient supplementation is a broadly investigated treatment modality as numerous deficiencies have been linked to FM. Additionally, a proposed link between gut microbiome patterns and chronic pain syndromes has led to studies investigating probiotics as a possible treatment. This commentary in response to a case considers how merit and features of medical school applicants’ dossiers should be drawn upon in admissions processes to promote equity and inclusion in medicine. It is argued that medical schools should incentivize inclusion by redefining merit in their admissions goals and processes, promote meaningful inclusion, and show institutional leadership in addressing social justice.

Communities of Practice (CoPs) are naturally forming groups of individuals who come together through a shared passion or goal and learn collectively by reciprocating knowledge and experiences. With competing demands of faculty, universities are increasingly challenged to provide professional development opportunities that actively encourage innovative pedagogical practices to enhance student success and quality of the learner experience. We report on four embryonic CoPs, based on collaborative processes, supporting institutional transformation and the formation of a new education model at Ireland’s first Technological University (TU). The IMPACT project at TU Dublin has been informed in part by the outcomes of the Co-CREATE project in 2019-20. Co-CREATE was a Team Teaching Fellowship project to support and underpin the building of a quality curriculum framework for the new technological university. A number of IMPACT projects (for example, NorthStar) have continued the work started within the Co-CREATE project to make it sustainable and embedded within the university. This paper explores the literature related to educator engagement in teaching, learning, assessment and feedback innovation (further referred to here as ‘teaching innovation’) in higher education institutions, describing the main enablers and barriers to innovation. These include consideration of formal and informal learning opportunities for educators, the role of management and leadership, as well as recognition and reward systems, teaching-research conflict, workload and time demands, and policies and procedures.
