Sickle cell disease, a genetic disorder affecting a sizeable global demographic, manifests in sickle red blood cells (sRBCs) with altered shape and biomechanics. sRBCs show heightened adhesive interactions with inflamed endothelium, triggering painful vascular occlusion events. Numerous studies employ microfluidic-assay-based monitoring tools to quantify characteristics of adhered sRBCs from high resolution channel images. The current image analysis workflow relies on detailed morphological characterization and cell counting by a specially trained worker. This is time and labor intensive, and prone to user bias artifacts. Recent studies in cognitively unimpaired elderly individuals suggest that the APOE ε4 allele exerts a dosage-dependent effect on brain tau deposition. The aim of this study was to investigate sex differences in APOE ε4 gene dosage effects on brain tau deposition in cognitively impaired individuals using quantitative 18F-flortaucipir PET. Preprocessed 18F-flortaucipir tau PET images, T1-weighted structural MRI, demographic information, global cortical amyloid-β burden measured by 18F-florbetapir PET, CSF total tau and phosphorylated tau measurements were obtained from the Alzheimer's Disease Neuroimaging Initiative database. Introduction: The COVID-19 pandemic has taken a significant toll on societal, physical, and psychological health. Emergency physicians (EPs) are susceptible to burnout under ordinary circumstances and may be particularly vulnerable during the pandemic. To reduce pandemic-related burnout, we implemented a residency-led physical wellness initiative and evaluated the effect on burnout among EPs.

Methods: In the spring of 2020, we invited all resident and attending EPs in our department to participate in a four-week physical wellness initiative as part of a prospective study. After completing or opting out of this wellness initiative, EPs responded to an online survey comprised …

Persons living with HIV (PLWH) are at higher risk of developing secondary illnesses than their uninfected counterparts, suggestive of a dysfunctional immune system in these individuals. Upon exposure to pathogens, monocytes undergo epigenetic remodeling that results in either a trained or a tolerant phenotype, characterized by hyper-responsiveness or hypo-responsiveness to secondary stimuli, respectively. We utilized CD14+ monocytes from virally suppressed PLWH and healthy controls for in vitro analysis following polarization of these cells toward a pro-inflammatory monocyte-derived macrophage (MDM) phenotype. We found that in PLWH-derived MDMs, pro-inflammatory signals (TNFA, IL6, IL1B, miR-155-5p, and IDO1) dominate over negative feedback signals (NCOR2, …

Up until the Supreme Court’s 2005 ruling in Roper v. Simmons, juveniles could constitutionally be executed for qualifying criminal offenses. The Roper Court raised the minimum age for execution to eighteen, citing both a national consensus against executing minors, as well as recent research (at the time) showing that juveniles are more vulnerable to negative influences and outside pressures. Since Roper, the Supreme Court has remained silent regarding the requisite minimum age for execution and has left the decision up to individual states. This thesis charts the course of the JeffVanderLou (JVL) organization between the pivotal years of 1966 to 1976, using the life of a man named Macler Shepard as the primary lens of exploration. Born in Marvell Arkansas, Macler Shepard followed in the footsteps of tens of thousands of other Southern migrants to cities like St. Louis, hoping to find a new life in the industrial North. However, no sooner had he settled in, he was displaced by the construction of Pruitt-Igoe, one of St. Louis’ first large-scale urban renewal programs. Atmospheric moisture transportation associated with the occurrence of relatively wet and dry southwest monsoon (SWM) years over Sri Lanka is still not fully understood. This study focused on investigating the role of moisture transport in contrast SWM years. We selected seven wet (SWM_Wet) and nine dry (SWM_Dry) years for 1985-2015 and found that the whole country experiences above-average (below average) rainfall in SWM_Wet (SWM_Dry) years. In SWM_Wet years, strengthening moisture-laden low-level jets (LLJ) from the Arabian Sea bring a large amount of moisture towards Sri Lanka. In contrast, the weakening of the …

Background and objectives: Describe the unique functions of immunoglobulin G4 (IgG4) in IgG4-neurologic disorders (IgG4-ND) and explain why, in contrast to their IgG1-counterparts, they respond poorly to intravenous immune globulin (IVIg) but effectively to anti-B cell therapies.

Methods: The IgG4 structure and isotype switch, B cells and plasmablasts relevant to IgG4 production, and IgG4-induced disruption of the targeted antigens are reviewed and compared with IgG1-mediated autoimmune ND, where IVIg inhibits IgG1-triggered inflammatory effects.

Urbanization has appropriated millions of hectares of cropland, and this trend will persist as cities continue to expand. We estimate the impact of this conversion as the amount of land needed elsewhere to give the same yield potential as determined by differences in climate and soil properties. Robust spatial upscaling techniques, well-validated crop simulation models, and soil, climate, and cropping system databases are employed with a focus on populous countries with high rates of land conversion. We find that converted cropland is 30–40% more productive than new cropland, which means that projection of food production potential must account for expected …

Diarrhea in an immunocompromised patient has a broad infectious differential. Diagnosis is difficult despite advances in diagnostic modalities. We report a case of a 45-year-old Nigerian woman who immigrated to the United States 2 years ago. She presented to the hospital with gastrointestinal bleeding, newly diagnosed HIV, and disseminated Kaposi sarcoma. During hospitalization, the patient had an onset of watery diarrhea and high eosinophilia. Subsequent stool analysis using multi-parallel real-time quantitative polymerase chain reaction for 13 parasites was positive for Cystoisospora belli. The patient was treated with trimethoprim-sulfamethoxazole, but had relapsed disease when her antibiotics were stopped prematurely. After restarting …

Lengenbachite is a naturally occurring layered mineral formed with alternating stacks of two constituent PbS-like and M₂S₃-like two-dimensional (2D) material layers due to the phase segregation process during the formation. Here, we demonstrate to achieve van der Waals (vdW) heterostructures of lengenbachite down to a few layer-pair thickness by mechanical exfoliation of bulk lengenbachite mineral. The incommensurability between the constituent isotropic 2D material layers makes the formed vdW heterostructure exhibit strong in-plane structural anisotropy, which leads to highly anisotropic optical responses in lengenbachite thin flakes, including anisotropic Raman scattering, linear dichroism, and anisotropic third-harmonic generation. Over the past two decades, policymakers have expressed considerable optimism about the capacity of international development to curb transnational migration, yet there is a dearth of research examining how and under what conditions development interventions impact migration decisions. Enlisting a case study approach in the Maya-K’iche’ community of Almolonga, this article examines divergent meanings and practices of “development” and its impact on the migratory aspirations and outcomes of Indigenous families in Guatemala. Government authorities and international development experts exalt Almolonga as a prosperous example of a globalized, agrarian-based economy. This study reveals trends in the choice of Kaqchikel Maya, Spanish, or both languages for household activities and by the generations, that is, the children, parents, and grandparents, in a village and a town in the central highlands of Guatemala. Similarities and differences between the village and town are described in both cases. The study also highlights the direct relationship between the language choice of parents and the acquisition of Kaqchikel by children. A total of 171 indigenous residents in the town and the village participated in the study. Findings are based on responses to a sociolinguistic questionnaire that also …

After the Spanish conquest, the IxilMayainitially remained less affected than more centrally located groups. However, dramatic changes occurred due to Guatemalan national government laws in the late 1800s which allowed claims on “vacant land”. Maya on those lands quickly adapted, claiming the areas where they had lived for over a millennium. The government favored large landowners who contributed to the national economy with export crops, particularly coffee. In the 1950s era of land reform, small farmers briefly obtained land, something quicklyreversed with the overthrow of the national government, accused of communism for these reforms.
