Acute febrile illness (AFI) surveillance networks can play a key role in identifying emerging pathogens and promoting global health security, especially for vulnerable populations in low- and middle-income countries. In January 2020, the Belize AFI Surveillance network was formed, and 5,643 participants were enrolled over the first 3-year period across 11 participating public hospitals. Using real-time polymerase chain reaction testing for vector-borne pathogen detection and BioFire® diagnostic testing for respiratory and gastrointestinal pathogens, the causes of illness were examined in relation to 54 pathogens in these participants. Current literature does not adequately address the factors that lead to and prevent burnout and poor work performance among inexperienced clinicians and clinical supervisors working in group private practice. Exploring supervision experiences, specifically leadership style, employee engagement, and internal communication, may provide insight into the experiences of novice mental health professionals and their supervisors in addressing burnout and enhancing work performance. Through a qualitative single-case study design informed by interpretative phenomenological principles, data were collected in semistructured interviews with five behavioral health leaders from a two-location group private practice who hold dual roles as clinicians and supervisors. Hypertension is one of the most common chronic conditions managed in primary care. The identified practice problem for this staff education project was the lack of structured staff training and inconsistent provider knowledge regarding evidence-based strategies to improve medication adherence among patients with hypertension. In this DNP staff education project, I aimed to solve two main issues which disrupted patient treatment continuity: better documentation and follow-up care practices. The existing gaps in knowledge have led to missing scheduled appointments, missing medical information, and weak communication between hospital staff and clinic staff resulting in disorganized patient care. The main goal of the project was to improve staff understanding of post treatment procedures, maintenance systems, and medical recordkeeping requirements. Social determinants of health (SDOH) training is a critical component of medical education, equipping physicians to address factors such as economic stability, education, and health care access. Despite its importance, the evaluation of SDOH training often focuses on lower-level outcomes, such as learner satisfaction and knowledge assessment, with limited emphasis on behavioral change and patient outcomes. Kirkpatrick’s model offers a validated framework for addressing this gap by categorizing training outcomes into four levels: Reaction, Learning, Behavior, and Results.

Tranexamic acid (TXA) is a synthetic antifibrinolytic agent that inhibits fibrinolysis without promoting new clot formation. Although FDA-approved for limited indications, TXA is frequently used off-label to reduce perioperative blood loss. Its use is well established in adult populations; however, pediatric data remain limited. Available evidence demonstrates that TXA effectively decreases intraoperative blood loss and transfusion requirements. This case involves an 8-year-old female with Lennox-Gastaut syndrome undergoing a right hip triple osteotomy with spica casting. The patient had a history of significant surgical blood loss resulting in hypotension and transfusion. Given the history, TXA was incorporated into the anesthetic plan, …

My goal is to prevent depression among single mothers ages 20–30 in low-income areas of Southeast Missouri by expanding access to community-based emotional support, parenting resources, and skills training that strengthen coping and resilience.

Significant Findings

Young single mothers in Southeast Missouri, especially those in their twenties, often carry the emotional and financial weight of raising children on their own, which puts them at higher risk for long-term stress and depression when support is limited (Tafet & Nemeroff, 2016; NAMI Missouri, 2025). The clinical success of chimeric antigen receptor (CAR) T-cell therapies has revolutionized oncology, yet the high costs and logistical complexities of ex vivo manufacturing remain significant barriers to global patient access. In vivo cell therapy, which involves the direct injection of lentiviral vectors (LVVs) to engineer cells within the patient’s body, offers a promising, cost-effective alternative. To understand the relative importance of cis and trans effects on regulation, we crossed multi-parent recombinant-inbred lines (RILs) to a common tester and measured allele-specific gene expression in the offspring. Testing the difference of allelic imbalance between two RIL × Tester crosses is a test of cis or trans, depending on the RIL alleles compared. The study design also enables to separation of two sources of trans variation, genetic and environmental, detected via interactions with cis effects. We demonstrate the effectiveness of this approach in a long-read RNA-seq experiment in female abdominal tissue at two time points in Drosophila melanogaster. …

The present study investigated associations between Big Five personality traits, their facets, and the frequency and variety of sexual fantasies in a sample of 5,225 adults (M = 58.30 years old; 56.5% men). People high in conscientiousness and agreeableness report fewer sexual fantasies across exploratory, intimate, impersonal, and sadomasochistic domains; these effects were primarily driven by respectfulness and responsibility facets. Negative emotionality, particularly the depression facet, was associated with more frequent sexual fantasizing. Extraversion and open-mindedness showed minimal relationships with sexual fantasy frequency. OBJECTIVES: The impact of the genetic risk for RA on resultant disease phenotype in RA is incompletely understood. Using individual genetic variants associated with RA and a polygenic score (PGS), we hypothesized that those with higher genetic risk for RA would demonstrate a more severe disease course.

METHODS: We genotyped participants from the Veterans Affairs RA registry (VARA), a prospective cohort of RA. Soft tissue sarcomas (STSs) are rare malignant tumors of mesenchymal origin with heterogeneous histologic subtypes and variable clinical behavior. Although STSs most commonly arise in the extremities, rapidly progressive sarcomas involving the distal lower extremity in adolescents have not been well documented. We report a case of an STS in a previously healthy adolescent female whose initial presentation was presumed to be a muscle strain. Over a three-month period, her symptoms worsened with progressive enlargement of a calf mass and development of widespread metastatic disease. Drug repurposing is the process of reusing existing pharmaceuticals for novel clinical purposes, which offers advantages such as streamlined clinical trial access and reduced drug development costs. Clarithromycin (CAM), a member of the macrolide antibiotics family, is a promising candidate for repurposing in cancer therapy due to its known preclinical and clinical immunomodulatory and anticancer properties. Foundation models have emerged as powerful feature extractors in computational pathology. However, they typically omit mechanisms for leveraging the global spatial structure of tissues and the local contextual relationships among diagnostically relevant regions-key elements for understanding the tumor microenvironment. Multiple instance learning (MIL) remains an essential next step following the foundation model, designing a framework to aggregate patch-level features into slide-level predictions. We present EAGLE-Net, a structure-preserving, attention-guided MIL architecture designed to augment prediction and interpretability. Objectives This process evaluation explores patient and healthcare professional acceptability of community-based monitoring versus hospital-based care for patients with quiescent neovascular age-related macular degeneration (QnAMD). Design Qualitative process evaluation was conducted as part of a randomised controlled trial. Setting Six hospitals and six community-based practices. Participants 25 patients and 16 healthcare professionals (ophthalmologists and optometrists). This approach helped differentiate between common issues and those specific to community-based monitoring. Dysregulated ribosome biogenesis and p53 mutations are known to play oncogenic roles in various cancers, including pancreatic cancer. In this study, we demonstrated the therapeutic potential of BMH-21, a pharmacologic inhibitor of RNA polymerase I, against pancreatic cancer by uncovering a novel molecular mechanism involving RPA194-mediated ubiquitination of mutant p53 without affecting the ubiquitination of wild-type p53. Our key findings are that (i) BMH-21 selectively induces apoptosis and cell growth inhibition of pancreatic cancer cells with no effect on normal human pancreatic ductal epithelial cells; (ii) BMH-21 degrades RPA194; (iii) BMH-21 inhibits recruitment of both RPA194 and RPA135 on rDNA …

This paper examines the intriguing contrast between the ambitious goals of antitrust laws in the Gulf countries and the reality of their enforcement. While this region seeks to protect competition and curb monopolistic practices, only Kuwait and the Kingdom of Saudi Arabia (KSA) have made significant strides in achieving these ideals.

Through a comparative legal analysis, this paper examines how the statutory objectives of competition law in Qatar, Bahrain, Oman, and the UAE often remain more rhetorical than substantive, with enforcement primarily symbolic.
