Improved Efficiencies With Virtual Wound Nurse Consults: An Evidence-Based Practice, Quality Improvement Project, Brooke M. Mock Contreras

Abstract

Background: Completing wound care consultations using digital technology is an uncommon practice in the acute care setting. Evidence suggests that virtual wound care consultations may decrease consultation completion times and improve departmental efficiency; however, evidence specific to the acute care setting remains limited.

Purpose: The purpose of this evidence-based practice, quality improvement project was to evaluate the feasibility and impact of implementing virtual wound care consultation in the acute care setting to improve departmental efficiency. Efficiency was measured by calculating the total time elapsed between the consultation order and consultation completion.

This paper is part of a larger clinical practice guideline on the diagnosis and management of parapneumonic effusion and empyema in children, developed by the Infectious Diseases Society of America. In this paper, the panel provides recommendations for pleural fluid drainage in children (3 months to 18 years) with parapneumonic effusion and empyema. The panel's recommendations are based upon evidence derived from systematic literature reviews and adhere to a standardized methodology for rating the certainty of evidence and strength of recommendation according to the GRADE (Grading of Recommendations Assessment, Development, and Evaluation) approach.

As the first part of an update to the clinical practice guideline on the management of community-acquired pneumonia in infants and children older than 3 months of age, we present six updated recommendations. The updated recommendations span the characterization and management of pneumonia with parapneumonic effusion. The panel's recommendations are based on evidence derived from systematic literature reviews and adhere to a standardized methodology for rating the certainty of evidence and strength of recommendation according to the GRADE (Grading of Recommendations Assessment, Development, and Evaluation) approach.

INTRODUCTION: FDA-approved chimeric antigen receptor (CAR)-expressing T cell therapies (CARTs) have revolutionized the treatment of blood cancers. Yet none have been successful for "solid" tumors, such as colorectal cancer (CRC), the 2nd leading cause of cancer deaths. Guanylyl cyclase C (GUCY2C) has emerged as a clinical-stage target for CART and bispecific T-cell engager (BiTE) therapies in CRC. IFNγ has been canonically recognized as beneficial for the effector functions of T cells by enhancing antigen processing and HLA presentation and is essential for CART targeting of solid malignancies by inducing adhesion molecule expression for synapse stabilization.

Background:  Type 2 diabetes mellitus (T2DM) is the most common type of endocrine disorder among adults, accounting for 90%-95% of diabetes mellitus globally. In Uganda, non-adherence to anti-diabetic medication remains high at 38.1%. This results in complications like diabetic ketoacidosis, hyperglycemia and microvascular complications.

Artificial intelligence (AI) offers opportunities to meaningfully narrow the access to justice gap by improving the ability of Legal Services Organizations (LSOs) to connect individuals with pro se resources and more effectively connect with individuals who could not otherwise advocate for themselves. Some LSO early adopters have already developed or are in the process of developing such tools. Others are unsure where or how to start.

This 37-y record (1989–2025) of field experiments tested seasonal and annual variation in predation on juvenile blue crabs (Callinectes sapidus) in a representative mesohaline site of Chesapeake Bay, USA, where the species is a dominant epibenthic predator and major fishery. We used replicated sets of individually tethered crabs to provide a relative index of lethal and sublethal predation rates on intermolt individuals as a function of size and habitats. Cannibalism by large adult crabs accounted for>97% of predation of juvenile crabs; predation by fish was not observed; and mortality from physiological stress was rare (< 1%). Sampling by depth showed that juveniles occurred in nearshore shallows, while adults were in deeper water. Cannibalism was also depth-dependent: Rates were high at 75 cm depth; intermediate at 40cm; and lowest at 15 cm shallows, which provided partial refuge. Cannibalism was size-dependent: 30 to 50 mm juveniles experienced high rates of predation; 90 to 110mm preadults had low rates; and >120 mm adults …

Illegal, unreported, and unregulated (IUU) fishing poses a significant threat to fisheries sustainability, global food security, and biodiversity conservation. Many regions rely on patrol-based enforcement to promote compliance, yet the effectiveness of such strategies in deterring IUU activity remains underexplored. In this study, we employed a mixed-methods approach to assess the impact of management and policy decisions on illegal fishing within the Chagos Archipelago marine protected area (MPA). This MPA has maintained near-continuous patrols since its implementation in 2010, providing a unique opportunity to evaluate the influence of patrolling on illegal fishing deterrence. Although research findings have indicated that obesity is associated with suicide behavior, especially in females, scant research has examined mediators in this relationship. Moreover, although suicides have increased in young Latina adults in the United States, no published studies have examined obesity and suicide risk in young Latina adults. The present study thus examined the relationship of obesity to suicide behavior in young Latina adults and assessed sleep problems, health problems, and dissatisfaction with personal appearance as possible mediators. Social media increasingly serves as important spaces for mental health information seeking and peer support, especially for individuals facing barriers to traditional care. Although nonsuicidal self-injury is frequently discussed on social media, there is limited understanding of the factors that influence the visibility of this discourse. We analyzed 973 videos related to nonsuicidal self-injury on TikTok to examine the prevalence and characteristics of posts that become unavailable (i.e., ephemeral) over time, whether due to user deletion or platform removal. Videos that used addiction-related language, included trigger warnings, or depicted scars were significantly associated with ephemerality. These findings highlight how anticipated …

T cells, genetically modified to express chimeric antigen receptors (CAR T), successfully used in hemato-oncologic malignancies, are showing promising and sustained benefits in refractory autoimmune neurological diseases, including Myasthenia Gravis, stiff-person syndrome, neuromyelitis optica, myositis, autoimmune neuropathies, and multiple sclerosis. Several reported patients with a steadily progressive disease and evolving disability unresponsive to available therapies, including rituximab and new biologics, after 2-3 months of treatment with CARs targeting CD19-positive, antibody-secreting, long-lived plasma cells, and plasmablasts exhibit impressive, long-lasting, and drug-free clinical improvements with the potential for immune reset shifting immunity to a healthy state without the need for continuing more …

Effective communication is vital in healthcare, especially across language barriers, where non-verbal cues and gestures are critical. This paper presents a privacy-preserving vision-language framework for medical interpreter robots that detects specific speech acts (consent and instruction) and generates corresponding robotic gestures. Built on locally deployed open-source models, the system utilizes a Large Language Model (LLM) with few-shot prompting for intent detection. We also introduce a novel dataset of clinical conversations annotated for speech acts and paired with gesture clips. Our identification module achieved 0.90 accuracy, 0.93 weighted precision, and a 0.91 weighted F1-Score. Our approach significantly improves computational efficiency and, …

The rise of Generative Artificial Intelligence (GenAI) in higher education has altered the conditions under which doctoral students learn, research, and develop as scholars. Although doctoral student use of GenAI has accelerated rapidly, institutional frameworks for responsible and developmentally appropriate use have not kept pace. This paper examines a central paradox in doctoral education: the same tools that enhance research productivity may also weaken intellectual independence when used without guidance. Using a critical review methodology, the study synthesizes 47 sources on doctoral education, GenAI use, policy, and epistemic development. Aluminum-based high-entropy alloys have been of interest owing to their potential for improved mechanical strength and corrosion resistance. In this research, TiCuNiSi_xAl high-entropy alloys (x = 0.5–1.7 at.%) were synthesized using spark plasma sintering at a temperature of 600 °C and a pressure of 40 MPa to examine the effects of incremental aluminum addition on the densification behavior, microstructure, microhardness, and corrosion resistance in 0.5 M H₂SO₄ solution. The current work extracted the chitosan from the waste of mushroom crops for the first time in Iraq. The chemical process was adopted for demineralization with 2M HCl, deproteinization with 2M NaOH, and decolorization. Also, the refluxing process with 60% NaOH for chitin was applied. The XRD result shows two peaks around 2θ =10° and 20°, indicating the ordered crystalline structure of chitosan. The FT-IR spectrum showed specific bands of chitosan at 3327.97, 2918.71, and 1645.95 cm^-1. Also, the FE-SEM results show a smooth, dense, and non-porous surface.
