In 2019, the Colorado General Assembly passed the Minor Conversion Therapy Law (MCTL), a statute which prohibits mental health professionals from engaging in conversion therapy for minors. Three years later, Kaley Chiles, a licensed professional counselor and addiction counselor, filed a pre-enforcement challenge and sought a preliminary injunction, alleging that the MCTL improperly abridges her First Amendment right to free speech. Colorado challenged her claim, arguing that the MCTL merely regulates professional conduct and only incidentally (and permissibly) burdens speech. Chiles v. Salazar will decide whether the MCTL is a speech restriction or professional regulation, and, if it is a …

Trial courts play an essential role in our judicial system. At the trial level, lived realities meet legal theories, and generalist judges must resolve constitutional disputes, develop the factual record, and administer justice. However, institutions, especially courts, face increasing pressure. This Keynote Address draws on cases over which I have presided to illustrate how careful process in trial courts helps maintain public trust in our institutions. These experiences reveal three lessons that trial courts teach us. First, the people bringing claims matter; they are real individuals seeking relief by telling authentic stories. This study aimed to analyse the symptomatic differences of hip dysplasia before or after separate surgical interventions, including pain location and characteristics, severity, patient-reported outcomes, and a body pain diagram. A REDCap-based survey was distributed through social media from October 2023 to May 2024 and targeted individuals with current or past hip pain. The survey gathered data on demographics, comprehensive hip-specific history, and various patient-reported outcome measures. Of 509 completed surveys, 287 respondents reported a diagnosis of hip dysplasia and were divided into cohorts based on treatment history: no surgery (n = 65), post-periacetabular osteotomy (PAO) (n = 162), post-THA …

Background/Objectives: Sport-related concussion (SRC) elicits multi-systemic symptoms, including nausea, fatigue, and cognitive changes. Gastrointestinal (GI) symptoms are not well captured in current concussion assessments and may be under-recognised in clinical follow-up. GI disturbances may influence intake tolerance and day-to-day fuelling during post-SRC recovery. This study investigated the prevalence and severity of self-reported GI symptoms in Irish athletes after their most recent SRC, examined sex-based patterns, and evaluated the rationale for integrating GI symptom checks into standard concussion tools (e.g., SCAT6) and post-injury monitoring. Clinical discharge summaries contain rich patient information but remain difficult to convert into structured representations for downstream analysis. Recent advances in large language models (LLMs) have introduced new approaches for clinical text extraction, yet their relative strengths compared with supervised methods remain unclear. This study presents a controlled evaluation of three dominant strategies for structured clinical information extraction from electronic health records: prompting-based extraction using LLMs, retrieval-augmented generation for terminology canonicalization, and supervised fine-tuning of domain-specific transformer models. Using discharge summaries from the MIMIC-IV dataset, we compare zero-shot, few-shot, and verification-based prompting across closed-source and open-source LLMs, evaluate retrieval-augmented canonicalization …

Lung cancer is the second most commonly diagnosed cancer among men and the third among women in Italy, representing a major public health concern. This ecological, cross-sectional study examined and analyzed regional patterns of smoking prevalence, Mediterranean diet adherence, and age-adjusted lung cancer incidence across Italy, stratified by sex, using publicly available regional and macro-regional data and accounting for age structure and air quality. Smoking prevalence and lung cancer incidence were higher among males than females nationwide, although regional lung cancer patterns did not consistently mirror smoking prevalence, particularly among females. Human immunodeficiency virus type 1 (HIV-1) is a retrovirus that integrates into the host cell's DNA as a provirus. Transcription from the provirus is regulated in large part by cellular proteins and epigenetic factors. These may be repressive or permissive to productive infection. The host factors that regulate this balance are therefore attractive targets for HIV-1 therapeutics. Indeed, proviral chromatin is the focus of two of the current HIV-1 cure strategies. "Shock and Kill" uses latency reversal agents to open the provirus's chromatin, promoting high levels of gene expression that induce the killing of infected cells. "Block and Lock" uses …

BACKGROUND AND AIMS: Metabolic dysfunction-associated steatotic liver disease (MASLD) remains a global public health threat. With emerging effective pharmacologic therapies, early identification of significant fibrosis in primary care is critical. We evaluated the cost-effectiveness of blood-based noninvasive tests incorporating real-world diagnostic performance in the contemporary MASLD treatment era.

METHODS: We developed a decision-analytic model for MASLD natural history and treatment to compare 6 noninvasive test strategies using Fibrosis-4 (FIB-4) and/or enhanced liver fibrosis (ELF) tests at varying cutoffs. This work investigated the relationship between crevice chloride concentration ([Cl−]) and the potential necessary for initiation in stainless steel 316L (SS 316L). Real-time fluorescence measurements were used to measure [Cl−] inside SS 316L crevice assemblies as a function of bulk [Cl−] and potential. Crevice initiation at potentials slightly less than or equal to the bulk repassivation potential was associated with chloride transport, owing to passive dissolution, and an increase in the crevice [Cl−]. Once the [Cl−] within the crevice increased to the concentration necessary to initiate SS 316L pitting at the applied potential, crevice corrosion was initiated. For example, in …

The pygmy rabbit (Sylvilagus idahoensis) is a species of conservation concern that has declined due to fragmentation and loss of essential habitat, primarily sagebrush-steppe. Pygmy rabbits are currently under review for listing under the U.S. Endangered Species Act (ESA) and consequently require a clearer understanding of current distribution and patterns of occurrence in big sagebrush (Artemisia tridentata) habitats. From 2024 to 2025, we surveyed for pygmy rabbits using remote cameras at several historically occupied sites in Utah, USA, to understand patterns of activity and occupancy. The efficacy of hydrophobic ocular drugs is significantly hindered by their low bioavailability, for which poor water solubility is a major contributing factor. In this work, we synthesized pilocarpine-derived ionic liquid [Pilo-OEG] Cl (PO) and evaluated its cosolvent properties for the codelivery of the hydrophobic antiglaucoma drug brimonidine (BM) for glaucoma therapy. Pilocarpine was quaternized with 2-[2-(2-chloroethoxy) ethoxy] ethanol in a one-pot reaction to yield PO, and its structure was confirmed using ¹H NMR and FT-IR spectroscopy methods and further characterized for its rheological property and thermal stability. Acute Respiratory Distress Syndrome (ARDS) is a life-threatening lung disease that results from direct or indirect insults to the lung, leading to air-blood barrier damage, pulmonary edema, hypoxemia, and multi-organ failure. Hyperoxia, or exposure to high fractions of oxygen, is used as a life-saving therapy to improve blood oxygenation in patients with ARDS. However, it can also worsen lung injury by inducing excessive production of reactive oxygen species (ROS), primarily from mitochondria, and by contributing to the progression of ARDS. Six cases of tutoring Chinese-speaking students and one case with English-speaking American student are presented to demonstrate students’ perceived hierarchy in tutoring sessions, as well as their different preferences and comfort levels with a hierarchical or peer-tutorial dynamic. Some students often addressed the tutor with the formal “you” in Mandarin Chinese, preferred to call tutors “teachers,” and expressed admiration for tutors' educational background, while English-speaking students occasionally signal hierarchical expectations, suggesting this phenomenon may transcend a single cultural group.
