This study aims to characterize the erosional behaviour of the Ganges riverbanks by investigating the geotechnical properties and seasonal hydrological fluctuation in Bangladesh. Multi-temporal satellite images (1973–2020) were analysed using Geographic Information System (GIS) to select study locations for detailed analysis. The GIS-based analysis identified erosion-accretion patterns and highlighted erosion-prone and stable riverbank locations. Further, field investigation, geotechnical laboratory analyses, and slope stability model using SLIDE software were used to assess the targeted study factors from the selected locations. Field investigation and geotechnical assessments revealed that bank material cohesion and composition strongly influence erosion. River flow at the outer bends …

This commentary examines the growing need for a comprehensive biodeterrence strategy in response to the convergence of biotechnology, artificial intelligence, and cross-domain warfare capabilities. Dr. Giordano argues that advances in life sciences, combined with the activities of peer competitors, are transforming the biological domain into a contested strategic space where biological effects intersect with other forms of warfare. Traditional biodefense approaches are no longer sufficient. Background: Emergence coughing after general anesthesia with an endotracheal tube (ETT) is a frequent occurrence related to airway irritation, and it can result in the spread of infectious respiratory droplets and hemodynamic instability. This is of particular concern when caring for patients with suspected or confirmed airborne infections. The irritation of the tracheal mucosa by the endotracheal tube (ETT) stimulates the cough reflex as anesthesia lightens. For this reason, preventing coughing at emergence is important to protect both patients and providers.

BACKGROUND: Transitions of care are high-risk periods for patient safety in the emergency department (ED), particularly for patients who are still in the diagnostic process and are discharged with diagnostic uncertainty. Care transitions must be improved for these patients, as one third of discharged ED patients have diagnostic uncertainty. Yet there are no validated measures that assess the quality of care transitions from the ED, limiting the ability to assess the impact of interventions. This essay examines the paradoxical relationship between pilgrimage and inner peace, focusing on the Camino de Santiago and referencing the Shingon Buddhist Goma fire ritual in Japan and the holy mountain of Kata Tjuṯa in Australia. While pilgrimage is often associated with physical hardship and discomfort, the paper argues that adopting a mindset of acceptance toward inevitable suffering can foster psychological and spiritual contentment. This article examines fishery management along and across the Canadian-US border through the comparison of collaborative transboundary networks in four regions: the Salish Sea, the Great Lakes, the Gulf of Maine, and the northern region including the Gulf of Alaska and the Hecate Strait. Transnational fishery management is an inter-organizational and multi-jurisdictional enterprise constituted by shared understandings of a suite of tasks and by communications among the participants. We report results from a multi-year, worldwide survey of artificial intelligence (AI)-producing startups to describe the types of technologies, frameworks, data protection, and partnerships nascent firms use to develop their AI products. Analyses of our survey findings also inform how startups gather the resources to commercialize their AI applications in alignment with their entrepreneurial strategies, address competitive barriers, and respond to regulations. These data provide initial evidence of the perceived impact of these products on their customers’ tasks and industries, suggesting that these firms’ AI products not only reduce labor costs but augment human capabilities.

BACKGROUND: Supraventricular tachycardia affects many critically ill patients. Dexmedetomidine, a commonly used sedative, has sympatholytic and vagotonic effects that may alter atrioventricular nodal conduction and contribute to SVT. We aim to determine the association between dexmedetomidine exposure and clinically significant supraventricular tachycardia in a heterogeneous cohort of mechanically ventilated, critically ill adults. METHODS: This retrospective cohort study included critically ill patients from 11 hospitals admitted between July 2018 and October 2022, who were intubated within 48 h of admission. Transformation of polycythemia vera (PV) into post-polycythemic myelofibrosis (MF) and subsequently acute myeloid leukemia (AML) represents one of the most aggressive trajectories among myeloproliferative neoplasms (MPNs). Post-MPN AML carries a median survival of approximately six months, particularly among older adults with adverse cytogenetics. We report the case of a 73-year-old man with JAK2-positive PV diagnosed in 2020 who progressed to MF in 2024 and developed AML later that year. Cytogenetic analysis revealed a KMT2A (11q23) rearrangement with KMT2A::ELL fusion, a finding rarely described in secondary AML arising from MPNs. Prior structural magnetic resonance imaging (MRI) studies that investigated structural alterations in focal dystonia have reported inconsistent findings, potentially due to methodological limitations or sample heterogeneity. This work investigates gray and white matter changes in patients with upper limb dystonia using T1-weighted images and diffusion tensor imaging (DTI). Imaging data collected with MRI at 3T from 28 right-handed individuals with right upper limb dystonia and 29 healthy controls were analyzed. T1-weighted images were analyzed using FreeSurfer to examine cortical thickness, volume and area. This study presents a high-performance piezoelectric nanogenerator (PENG) based on aluminum-doped zinc oxide (Al:ZnO) nanorods synthesized via a novel sonochemical method. This rapid, cost-effective, and reproducible approach enables the synthesis of ZnO nanorods (ZnO NRs) under ambient conditions. The PENG, fabricated on a 177 µm flexible Indium Tin Oxide (ITO) coated polyethylene terephthalate (PET) substrate, benefits from optimized aluminum doping, enhancing the output voltage. Structural analyses conducted using atomic force microscopy (AFM), scanning electron microscopy (SEM), and X-ray diffraction (XRD) demonstrated that the synthesized ZnO nanorods possessed a high degree of crystallinity. For all drugs, effective target engagement requires sufficient intracellular concentrations of drug to be reached, but whether tumour heterogeneity impacts drug distribution and efficacy is poorly studied. Poly (ADP-ribose) polymerase (PARP) inhibitors have transformed treatment opportunities for women with high-grade serous ovarian carcinoma, but resistance remains a clinical hurdle in this highly heterogeneous tumour type. Here, we present a patient-derived explant multi-modal imaging pipeline, which demonstrates that cell-intrinsic PARP inhibitor accumulation is highly variable, both between patients and within tumours. Spatial transcriptomics reveals enrichment of apoptotic and lysosomal signatures in high-drug regions. Rucaparib, an intrinsically fluorescent PARP inhibitor, accumulates heterogeneously …

BACKGROUND: During the COVID-19 pandemic, the Centers for Medicare and Medicaid Services created a waiver to reimburse telemedicine services. It is important to understand factors that facilitate incorporation of telemedicine into ongoing cardiovascular practice.

METHODS: This was a retrospective cohort study of telemedicine and office visits delivered by cardiologists between January 1, 2022, and December 31, 2023, for Medicare beneficiaries. We calculated the adjusted incidence rate ratio (aIRR) of telemedicine visits, representing the proportion of a physician's visits delivered by telemedicine, to identify factors associated with telemedicine use.

This cross-sectional study evaluated the knowledge, attitudes, and practices (KAP) related to avian influenza among residents of the Nsawam-Adoagyiri Municipality; an area characterized by intensive poultry production with the aim of delineating deficits in community-level preparedness and guiding evidence-based public health interventions. Aortitis is a rare but serious complication of infection that results in high morbidity and mortality. The most common location is in preexisting aneurysmal disease in a patient with multiple chronic comorbidities. The pathogens often reflect gastrointestinal bacteria, the classic example being Salmonella spp., but also include other enteric flora, Staphylococcus aureus, Streptococcus spp., and more. Herein, we describe 2 cases of aortitis caused by gastrointestinal bacteria and review management of this complex syndrome.

Background: Global dementia policies advocate for early or timely diagnosis, yet evidence on benefits or harms remains limited. This systematic review evaluates quantitative evidence comparing outcomes of early versus late and timely versus untimely dementia diagnoses. Methods: Following PRISMA guidelines, the protocol was registered on PROSPERO. Comprehensive searches of PsycINFO, Medline, Embase, SCOPUS, and CINAHL were conducted without date restrictions. Eligible studies defined diagnostic timing and examined associations with outcomes for people with dementia and/or carers. This paper presents optimisation studies for the combination of high efficiency reversible solid oxide cells (rSOC) with offshore wind farms in the Celtic Sea. Reversible systems can be used both in electrolyser and fuel cell mode, allowing potential benefits from switching between modes depending on the instantaneous condition of the electricity and hydrogen markets. This paper examines the shifting contours of criminal law in relation to political speech, with a focus on Section 152 of the Bharatiya Nyaya Sanhita (BNS). It advances three arguments. First, the decolonising narrative behind deleting sedition glosses over substantive continuities between colonial and post-colonial periods, with Section 152 BNS refashioning the colonial-era bifurcation between ‘sedition’ and ‘sedition-like’ activity. The BNS provision represents a backslide in the way it converges the various legal and political connotations of sedition from colonial to contemporary times, creating a new criminal category constructed through popular political-extra-legal discourses. Ignazio Visco served as deputy director general (the equivalent of deputy governor) of the Bank of Italy during the Global Financial Crisis of 2007–09 (GFC) and as governor during the European Sovereign Debt Crisis in 2011 (SDC). During his multidecade tenure at the bank, he held several positions and served under nine prime ministers before stepping down in 2023. Although Italy had the third-largest economy in the eurozone, its banking sector had little exposure to mortgages, which resulted in its being somewhat less impacted by the GFC than other countries.
