Principles of long-term medical management in individuals with urea cycle disorders (UCDs) encompass (1) a low protein diet, (2) supplementation of arginine and/or citrulline along with essential amino acids, nutrients, vitamins and trace elements, and (3) use of nitrogen scavenging agents to reduce recurrent hyperammonemic events (HAEs). These principles aim at providing metabolic stability, elimimation of chronic complications, and achievement of normal development as well as growth. Background: Pulse oximetry is widely used to estimate arterial oxygen saturation, yet accuracy may vary for a number of reasons. Data on children with functionally univentricular circulation are limited. The primary aim of this study was to evaluate the agreement between arterial oxygen saturation measured by blood gas and pulse oximetry in children with functionally univentricular circulations. Methods: A retrospective analysis was performed of paired arterial blood gas and pulse oximetry oxygen saturation measurements following Norwood, Glenn, or Fontan procedures. Background: A valid definition of severe heart failure (HF) is essential for earlier identification, timely referral for advanced therapies, and to optimize clinical trial design.

Objectives: To assess the prevalence, prognostic performance, and 1-year outcomes associated with different definitions of severe and advanced HF (AdvHF) in patients with heart failure with reduced ejection fraction (HFrEF).

Methods: We included 15,153 patients with EF < 40%, HF duration >6 months, and no prior left ventricular assist device or heart transplant, from the Swedish Heart Failure Registry. Several definitions of severe/AdvHF were evaluated: a simplified Heart Failure Association (HFA) definition (NYHA III-IV, EF < 30%, ≥2 HF hospitalizations in 12 months), the GALACTIC-Severe-definition (NYHA III-IV, EF < 30%, ≥1 hospitalization in 6 months), and variations adding/removing criteria such as NT-proBNP levels and diuretic dose to the HFA-AdvHF definition. Predictive performance and 1-year outcomes were analyzed.

Introduction:  Dyspepsia affects 6.9% of people globally and significantly impairs quality of life, yet it often remains undiagnosed. Undiagnosed dyspepsia (UD) affects up to one-third of patients and is prevalent among students due to stress, irregular eating, and self-medication. Interpersonal violence on college campuses disrupts academic experiences and impacts outcomes for students. This study sought to understand the experiences of students in the United States engaging with Title IX offices, within the context of additional state mandates. Interviews were conducted with 24 students and analyzed through inductive content analysis. Students described negative impacts such as challenges with the organizational processes, impacts to institutional trust, frustration with ineffective protections, and high personal and academic costs. Positive outcomes expressed included feeling more empowered to engage in self-advocacy and finding validation of their experience. Upper gastrointestinal endoscopic procedures such as esophagogastroduodenoscopy (EGD) are common procedures most often utilizing monitored anesthesia care (MAC), primarily using propofol as the sedative. Propofol is regularly used due to its rapid onset and recovery profile, however, its narrow therapeutic window and dose dependent responses can cause adverse respiratory events. In patients with respiratory pathophysiology, obesity, or obstructive sleep apnea, these adverse events can be catastrophic. Dexmedetomidine (DEX), a selective a2-adrenergic receptor agonist with anxiolytic, sedative, and modest analgesic properties, produces sedation with minimal respiratory depression. Traditionally, vasopressors are avoided in microvascular surgery. This tradition is largely based on animal studies, which demonstrated that ephedrine and phenylephrine may lead to decreased graft perfusion and pedicle thrombosis. Modern clinical evidence is largely unable to replicate these results, as no consistent correlation is observed between flap compromise and vasopressor use. Although some isolated studies show potential flap compromise with vasopressor exposure, the vast majority of modern clinical evidence suggests that vasopressor exposure poses no increased risk to flap compromise.

Postoperative nausea and vomiting (PONV) remains a major challenge in anesthesia for high-risk surgical patients. Its complications extend beyond discomfort as increased dehydration, fluid shifts, aspiration risk, and hospital stay duration all reduce patient satisfaction. Research and clinical experience highlight the need for anesthesia providers to move away from single-drug prophylaxis and implement individualized, multimodal strategies for high-risk patients.

Early risk identification is vital. The Apfel score—based on female gender, non-smoking status, history of PONV, and likely opioid use—effectively forecasts risk. An Apfel score of 4 indicates over 80% chance of PONV, so timely prevention is crucial. More than tallying …

Between 2012 and 2015, elective TKA and THA increased by nearly 50%, and there is a substantial increase in these procedures projected over the next decade. This increase reflects both an aging population and a higher number of younger patients electing for TKA or THA earlier in life. Background: Venous thromboembolism prophylaxis (VTE) is a major consideration in orthopaedic trauma surgery. In recent years, there has been an increasing debate about whether patients should be given enoxaparin or aspirin as VTE prophylaxis medication. The objective of this systematic review is to compare enoxaparin versus aspirin for VTE prophylaxis in orthopaedic trauma patients.

Knowledge of arsenic inorganic speciation in groundwaters is critical for optimising water treatment approaches. Here, we characterise the on-site analytical performance of the As(V)-selective ImpAs ion-exchange resin to separate As(V) from As(III) in groundwater with contrasting redox conditions. Field speciation results obtained by ImpAs in the oxic, Fe-poor groundwaters of Guanajuato (Mexico) and in the reducing, Fe-rich groundwaters of West Bengal (India) agreed with voltammetry (slope of 0.91 ± 0.03, r = 0.99, p <  0.001, n = 21). Like for arsenic, oxyanions of Mo, V, U and Cr are retained by ImpAs but their reduced hydroxy forms are not. High As(V) extraction by ImpAs (>95%) was obtained in synthetic groundwater for hundreds of samples without any regeneration, highlighting negligible interactions of major anions such as sulfate, a major advantage over …

Epithelioid haemangioendothelioma (EHE) is a rare vascular sarcoma of intermediate malignancy. In 2023, a 31-year-old female presented to a tertiary care hospital in Muscat, Oman, with symptoms later diagnosed as primary vaginal EHE. The patient had primary, locally invasive vaginal EHE that subsequently metastasised to the lungs. The patient experienced progression of systemic disease despite chemotherapy and was therefore transitioned to palliative chemotherapy. The clinical, radiological and histopathological findings as well as the diagnostic features that distinguish EHE from its aggressive mimic, epithelioid angiosarcoma, are described in this report. There is a gap at the heart of the law governing the use of nuclear weapons. Repeated threats by Russian leaders to use nuclear weapons against Ukraine highlight the urgency of closing this gap. This article reviews the central holding of the 1996 International Court of Justice advisory opinion on the Legality of the Threat or Use of Nuclear Weapons. The article focuses on the controversial “extreme circumstances” clause, in which the ICJ stated that it could not determine the legality of using nuclear weapons when the survival of a State was in question. Background: Metabolic dysfunction-associated steatotic liver disease (MASLD) and depression frequently occur together. Identifying the genes that influence both MASLD and depression may facilitate the discovery of biological pathways associated with disease risk.

Methods: We recruited 525 participants from Mexican American families living in the Rio Grande Valley of south Texas. We collected clinical data, biometric measurements, hepatic health assessments using Vibration-Controlled Transient Elastography (VCTE), and depression evaluations determined with the Beck Depression Inventory-II. Historical research documents feminism's evolution from 19th-century suffrage movements to modern intersectional approaches. Though there are gaps in understanding how personal experiences and contexts, such as cultural and socio-economic, influence current feminist practice. This study investigates how lived experiences shape contemporary feminist engagement in Baguio City, Philippines. Using a descriptive phenomenological analysis of face-to-face interviews with a total of nine participants, the study identifies the following: results reveal generational divides, with younger participants developing progressive, digitally-informed feminist consciousness, while economic realities constrain activist participation for marginalized groups. To address this gap, the transit agency for Indianapolis, IndyGo, integrated near miss conflict data into a transit-oriented safety project at 33 bus stops and intersection across Indianapolis.
