Showing a collective interest in the preservation and restoration trades workforce, four statewide preservation nonprofits (representing NY, VT, NH, and ME) joined forces in 2021 to create the Northeast Regional Initiative for the Preservation Trades. The initiative’s aim is to better understand this issue and develop courses of action to foster a more robust and sustainable preservation trades workforce. In 2022, specialists from UNH Cooperative Extension’s Community and Economic Development team partnered with this initiative to lead a multi-methods research project consisting of a survey, interviews, and focus groups with trades professionals, preservation specialists, workforce development professionals, educators, and other …

An increasing number of farms in NH and beyond are incorporating agritourism activities and/or event hosting into their business models. While these can be valuable strategies in terms of community and economic benefits, neighbor/abutter disputes, zoning regulations, and state laws may pose challenges. METHODS: A single-center retrospective cohort study was performed from 2011 to 2020 of 80 fetuses with left-sided congenital diaphragmatic hernia that were delivered and repaired at our institution. Perinatal, perioperative, and postoperative data were collected. OBJECTIVES: We compared the outcomes of patients with or without systemic lupus erythematosus (SLE) who were diagnosed with coronavirus disease 19 (COVID-19) and evaluated factors within patients with SLE associated with severe outcomes.

METHODS: This retrospective cohort study used the deidentified Optum COVID-19 electronic health record dataset to identify patients with COVID-19 from 1/1/2020 to 31/12/2020. Cases with SLE were matched with general controls at a ratio of 1:10 by age, sex, race and ethnicity and COVID-19 diagnosis date. OBJECTIVE: Radiotherapy for pediatric brain tumor has been associated with late cognitive effects. Compared to conventional photon radiotherapy (XRT), proton radiotherapy (PRT) delivers lower doses of radiation to healthy brain tissue. PRT has been associated with improved long-term cognitive outcomes compared to XRT. However, there is limited research comparing the effects of XRT and PRT on verbal memory.

Objective: People living with HIV have high rates of obesity and obesity-related comorbidities. Our study sought to evaluate weight trajectory in a retrospective cohort of people living with HIV and matched HIV-negative veterans (controls) and to evaluate risk factors for weight gain.

Methods: This was a retrospective database analysis of data extracted from the VA Corporate Data Warehouse that included people living with HIV (n = 22 421) and age-matched HIV-negative controls (n = 63 072). Purpose: We report the results of a phase II, randomized, window-of-opportunity trial of neoadjuvant durvalumab versus durvalumab plus tremelimumab followed by surgery in patients with resectable malignant pleural mesothelioma (MPM; NCT02592551).

Patients and methods: The primary objective was alteration of the intratumoral CD8/regulatory T cell (Treg) ratio after combination immune checkpoint blockade (ICB) therapy. Secondary and exploratory objectives included other changes in the tumor microenvironment, survival, safety, tumor pathologic response (PR), and systemic immune responses.

Results: Nine patients received monotherapy and 11 received combination therapy. Seventeen of the 20 patients (85%) receiving ICB underwent planned thoracotomy. Both ICB regimens …

Rabbit meat could play an important role in health, the rural economy, and sustainable development. Rabbit meat has excellent nutritional features, such as high protein content, low-fat content, and a high percentage of unsaturated fatty acids, low cholesterol and sodium levels. In addition, rabbit meat production contributes to maintaining economic activities in rural marginal areas. Featured Application: The work described here aims to design and characterize a more efficient bicycle braking system. In 2019, the Centers for Disease Control and Prevention estimated that 329,000 Americans were injured in cycling-related incidents. Since the first bicycle brake in 1817, there has been an individual brake lever for decelerating each wheel, while on cars, there has been a single control lever for decelerating multiple wheels since 1921. Exploring A Cooperative Learning Model With A Mixed Methods Design, Zhidong Zhang, Liza Ramos

The exploratory sequential design was used in this study. The mixed methods design consisted of two modules: a qualitative module and a quantitative module. The qualitative module was used to code the data into categories and further into the structure in terms of Saldana’s model. Based on the analysis of the interview transcripts, three themes were recognized to support the cooperative learning models. These three themes were Knowing by Undergone Events and Contexts (KUEC), Implementation and Action (I&A), and Opinions and Perceptions (OP). These three themes consisted of a Collaborative Learning Model describing and evaluating different topic patterns.

Over the last several decades, inpatient psychiatric length of stay (LOS) has been greatly reduced to the detriment of patients. Latent variable mixture modeling, can be used to improve the quality of care for patients by identifying unobserved subgroups and optimize treatment variables, including LOS. This study had three objectives (1) to replicate the findings made by Oh et al. in a distinct sample, (2) to examine demographic differences related to inpatient treatment trajectories, and (3) to relate additional variables to each trajectory. Altered white matter (WM) microstructure likely occurs in children with bipolar disorder (BD) with impulsivity representing one of the core features. However, altered WM microstructures and their age-related trendlines in children with BD and those at high-risk of developing BD, as well as correlations of WM microstructures with impulsivity, have been poorly investigated. In this study, diffusion MRI, cognitive, and impulsivity assessments were obtained from children/adolescents diagnosed with BD, offspring of individuals with BD (high-risk BD) and age-matched healthy controls. The transcription factor TFEB is a master regulator of lysosomal biogenesis and autophagy1. The phosphorylation of TFEB by the mechanistic target of rapamycin complex 1 (mTORC1)2–5 is unique in its mTORC1 substrate recruitment mechanism, which is strictly dependent on the amino acid-mediated activation of the RagC GTPase activating protein FLCN6,7. TFEB lacks the TOR signalling motif responsible for the recruitment of other mTORC1 substrates. The present study investigated the interactive effect of reactive (negative emotionality) and regulatory (effortful control) aspects of temperament in the prediction of child anxiety and depressive symptoms. Clinically anxious children and their mothers completed a battery of questionnaires that included self- and mother-ratings of child effortful control, negative emotionality, anxiety, and depressive symptoms. Multiple regression analyses were conducted to examine the moderating effect of effortful control on the relation between negative emotionality and child anxiety and depressive symptom severity. OBJECTIVES: Preterm infants are born functionally pancreatic insufficient with decreased pancreatic production of lipase and proteases. Developmental pancreatic insufficiency (PI) may contribute to reduced nutrient absorption and growth failure. We sought to determine longitudinal fecal elastase (ELA1) levels in a cohort of preterm infants and whether levels are associated with growth outcomes.

Missense and truncating variants in the X-chromosome-linked CLCN4 gene, resulting in reduced or complete loss-of-function (LOF) of the encoded chloride/proton exchanger ClC-4, were recently demonstrated to cause a neurocognitive phenotype in both males and females. Through international clinical matchmaking and interrogation of public variant databases we assembled a database of 90 rare CLCN4 missense variants in 90 families: 41 unique and 18 recurrent variants in 49 families. For 43 families, including 22 males and 33 females, we collated detailed clinical and segregation data. Racial and socioeconomic disparities have become apparent in acute myeloid leukemia (AML) outcomes. We conducted a retrospective cohort study of hospitalizations for adults with a diagnosis of AML from 2009 to 2018 in the Nationwide Inpatient Sample (NIS). We categorized patients' ages in groups of≥60 years and stratified them by reported race/ethnicity. Exposures of interest were patient sociodemographics, hospital characteristics, and Elixhauser-comorbidity Index. Outcome of interest was in-hospital death. Statistical analyses included survey logistic regression to generate adjusted odds ratios (aORs) and 95% confidence intervals (CIs) to quantify the independent associations between patient characteristics and mortality. Brain tumors are among the 10 leading causes of cancer-related death and present unique treatment challenges due to their critical location, genetic heterogeneity, and the blood-brain barrier. Recent advances in targeted immunotherapy and immune checkpoint blocking therapy provide alternative therapeutic strategies for brain tumors. Fibrinogen-like protein 2 (FGL2), which induces transformation from low-grade glioma to high-grade glioblastoma, is a type II membrane protein that is highly expressed in both host immune cells and tumor cells. Studies have uncovered multiple forms of FGL2 proteins with a broad range of roles in inducing immune tolerance and avoiding immune surveillance in tumor cells. …

BACKGROUND: Viral respiratory infections (VRIs) are common and are occupational risks for healthcare personnel (HCP). VRIs can also be acquired at home and other settings among HCPs. We sought to determine if preschool-aged household contacts are a risk factor for VRIs among HCPs working in outpatient settings.

METHODS: We conducted a secondary analysis of data from a cluster randomized trial at 7 medical centers in the United States over 4 influenza seasons from 2011-2012 to 2014-2015. Culturally responsive content, accessible and inclusive tools, and meaningful interdisciplinary tasks can aid in developing equitable and creative learning environments. Music and mathematics are ideal disciplines for interdisciplinary creative learning. In this article, we reflect on our experiences engaging in interdisciplinary music and mathematics tasks with preservice teachers. In particular, we highlight specific efforts taken to design and implement a creative music and mathematics workshop for use in a mathematics methods course. In the wake of the fall of Kabul in August 2021, hundreds of thousands of Afghan refugees found temporary shelter at American airbases, prompting non-profit organizations and faith communities to assist in their resettlement. The Rev. Dr. William J. Danaher Jr. details the efforts of Christ Church Cranbrook and the Muslim Unity Center in Michigan to address the healthcare needs of Afghan refugees, particularly in dental care. Facing language and cultural barriers, they established an Interfaith Refugee Dental Program that successfully provided critical dental services to these refugees. Over 145 MDA members and legislator contacts at 23 events showcased the program's effectiveness. Connecting members with their legislators fosters personal connections, reinforces the MDA as a dental resource, and offers a unique local perspective on key issues. With many new legislators taking office in 2023, the grassroots program will remain vital in the MDA's advocacy efforts. This month’s Dentistry and the Law column addresses the ethical termination of the dentist-patient relationship, discussing how to terminate a challenging patient-dentist relationship. Dentists have an ethical and legal obligation to end the relationship appropriately, and abrupt termination can lead to claims of "abandonment." To safeguard against such claims, dentists should provide reasonable notice, offer emergency services, facilitate referrals, and ensure the completion of ongoing treatment. While claims of patient abandonment are rare, proactive termination before critical treatment needs arise is advised.
