Molecular carbapenem-resistance testing, such as for the presence of carbapenemases genes, is commonly implemented for the detection of carbapenemase-producing Enterobacterales. Carbapenemase-producing P. aeruginosa is also associated with significant morbidity and mortality, although; prevalence may be underappreciated in the United States due to a lack of carbapenemase testing. The present study sought to compare hands-on time, cost and workflow implementation of carbapenemase gene testing in Enterobacterales and P. aeruginosa isolates versus sending out isolates to a public health laboratory (PHL) for testing to assess if in-house can provide actionable results. Recent developments in stakeholder theory have refined our understanding of value creation via bilateral reciprocity. Generalized exchange is another important microfoundational mechanism in value creation, but because of the potential for free-riding it is surprising that some stakeholders contribute more resources to a firm’s value-creating nexus than would be expected based on contractual obligations, and even beyond what bilateral reciprocity would predict. This paper aims to identify the minimum conditions that promote generalized exchange in a firm’s value-creating nexus. We report a minimal auxiliary basis model for time-dependent density functional theory (TDDFT) with hybrid density functionals that can accurately reproduce excitation energies and absorption spectra from TDDFT while reducing cost by about \change{two} orders of magnitude. Our method, dubbed TDDFT-ris, employs the resolution-of-the-identity technique with just one $s$-type auxiliary basis function per atom for the linear response operator, where the Gaussian exponents are parametrized across the periodic table using %using tabulated atomic radii with a single global scaling factor. By tuning on a small test set, we determine a single functional-independent scale factor that balances errors in excitation energies …

Background and Purpose: Chronic inflammation is pathogenic and contributes to human diseases, causing a significant threat to public health. The NLR family pyrin domain‐containing protein 3 (NLRP3) is the best‐characterized factor regulating inflammation. Therefore, targeting NLRP3 has the potential to treat inflammatory diseases and improve human health. Experimental Approach: Lipopolysaccharide was used to induce inflammation in cell cultures. Lipopolysaccharide/d‐galactosamine and dextran sulfate sodium salt were used to induce acute liver inflammation and ulcerative colitis respectively in C57BL/6J mice. Existing viscosity prediction methods and relevant literature are reviewed. An exhaustive review of group contribution, corresponding states, and interpolative prediction methods finds that even the best of these models produces large prediction errors and often require significant experimental data. Molecular dynamics simulation techniques for viscosity prediction are evaluated and compared to one another to determine the best choice for this work. A thorough investigation finds that Equilibrium Molecular Dynamics (EMD) simulations are the best option for reproducible and reliable liquid viscosity predictions. Breast implant-associated infections (BIAIs) are the primary complication following placement of breast prostheses in breast cancer reconstruction. Given the prevalence of breast cancer, reconstructive failure due to infection results in significant patient distress and health care expenditures. Thus, effective BIAI prevention strategies are urgently needed. This study tests the efficacy of one infection prevention strategy: the use of a triple antibiotic pocket irrigant (TAPI) against Staphylococcus aureus, the most common cause of BIAIs. TAPI, which consists of 50,000 U bacitracin, 1 g cefazolin, and 80 mg gentamicin diluted in 500 mL of saline, is used to irrigate the breast implant …

Colloidal Ag particles decorated with Fe3O4 islands can be electrochemically or photochemically activated as inverse catalysts for C(sp2)–H heteroarylation. The silver–iron oxide (SIO) particles are reduced into redox-active forms by cathodic charging at mild potentials or by short-term light exposure, and can be reused multiple times by magnetic cycling without further activation. A negative shift in the reduction peak is attributed to an overpotential produced by surface Fe3O4 which separates residual Ag ions or clusters from bulk silver. The catalytic efficiency of SIO is maintained even with acid degradation, which can be countered simply by adding water to the reaction …

The environmental impact of anesthetics is a growing consideration in operating rooms (ORs) worldwide. In the United States, healthcare is responsible for an estimated 10% of greenhouse gas emissions (GHGs) with a considerable portion of these originating in perioperative services. Anesthetic gases contribute to GHG emissions with varying degrees of severity. In the case study examined, a 58-year-old female presented for bilateral mastectomy, axillary node dissection, and breast reconstruction. Total time under anesthesia - maintained with fresh gas flows (FGF) of 2 L/min and end-tidal sevoflurane of 1.8-2.2% - was 7 hours. There are currently no universal guidelines to monitor …

Opioids are a traditional anesthesia mainstay but are associated with adverse effects. Recent data suggests that perioperative exposure to opioids may predispose patients to opioid dependence. In response to the American Society of Anesthesiologists' statement encouraging the use of multimodal analgesia without opioids when possible, the anesthesia community shifted toward enhanced recovery after surgery (ERAS) protocols and opioid-sparing techniques using regional anesthesia, nonopioid analgesics, and adjuncts.5 The efficacy of these techniques makes opioid-free anesthesia (OFA) a feasible option.5 When compared to traditional opioid-based anesthesia (OBA), is OFA capable of providing equivalent postoperative pain control?

Emergence delirium (ED) is a common problem in the pediatric population following general anesthesia. The condition is defined as a state of mental disturbance characterized by non-purposeful movements, agitation, and possible dissociation. Concern for ED is even greater in children with disorders such as autism spectrum disorder (ASD) and attention deficit hyperactivity disorder (ADHD). Dexmedetomidine is a selective alpha-2 agonist with sedative, analgesic, and anxiolytic effects that promote optimal emergence from anesthesia in pediatric patients.

Emergence from general anesthesia (GA) can lead to a broad spectrum of cognitive and behavioral dysregulation in the young adult population. This includes emergence delirium (ED), an acute alteration in mental sttus or behavior after surgery that is typically transitory. Emergence delirium is a common application that can present in the immediate post-operative period. In patients undergoing maxillofacial surgery it can often be intensified by pain, anxiety, and fear. The use of VLs for routine orotracheal OR intubation has gained popularity due to excellent glottic visualization, minimal cervical manipulation, and a high first-pass intubation success rate. However, despite clear benefits, reports of soft palate and tonsillar pillar injuries with VL use have increased, particularly with hyperangulated VL blades such as the GlideScope. These injuries may be associated with seemingly smooth intubations and are often unrecognized until after successful intubation or surgery completion.

Background: Live dietary microbes have been hypothesized to contribute to human health but direct evidence is lacking.

Objectives: This study aimed to determine whether the dietary consumption of live microbes is linked to improved health outcomes.

Methods: Data from the NHANES 2001–2018 were used to assess microbial intake and their adjusted associations with selected physiological parameters (e.g., blood pressure, anthropometric measures, and biomarkers) among adults aged 19 y and older.
