Echoes Of Malignancy: Triple-Hit Lymphoma Presenting As An Isolated Pericardial Effusion, Nathan Fletcher, Anne Marie Bonaguro, Yanxia Li

Background: Malignancy is the second most common cause of pericardial effusion, typically occurring in advanced stages. Lymphomas account for a minority of malignant effusions, with symptomatic pericardial disease as their initial presentation being exceptionally rare. Case: A 64-year-old man with chronic obstructive pulmonary disease presented with two weeks of progressive dyspnea, orthopnea, and productive cough. Chest radiograph showed an enlarged cardiac silhouette and bibasilar opacities. Computed tomography of the chest revealed a complex circumferential pericardial effusion up to 3.6 cm, confirmed on transthoracic echocardiography (TTE) with diastolic underfilling but no tamponade physiology. Introduction: Acute chest syndrome (ACS) is a severe complication of sickle cell disease. Rarely, it may progress to acute respiratory distress syndrome (ARDS). Inability to oxygenate despite mechanical ventilation necessitates consideration of extracorporeal membrane oxygenation (ECMO). Case Summary: A 31-year-old man with sickle cell disease presented with low back pain similar to prior pain crises. Vital signs were normal, and his hemoglobin level was 10.2 g/dL, with a reticulocyte index of 0.05. He was admitted for pain control and rehydration. That night, he developed fever, tachypnea, and hypoxemia with imaging showing a right lower lobe infiltrate, consistent with ACS. He …

Abstract: Neonates are an extremely sensitive population often subject to painful interventions. This pain can have numerous adverse effects including physiologic instability, altered brain development, and abnormal stress responses, which can persist into childhood. Effective post-operative pain management for neonates is crucial for promoting positive developmental outcomes. No specific post-operative pain protocol for neonatal patients has shown to be most effective, so the American Academy of Pediatrics recommends that healthcare institutions establish a neonatal pain protocol to provide more standardized care. Background: Delirium affects one-third of general medical patients aged 70 or older. Patients with older age, preexisting dementia, or cognitive impairment are at increased risk of developing delirium. Delirium is associated with increased length of stay, mortality, transfer to skilled nursing facilities, and costs. Although the Confusion Assessment Method (CAM) is a validated tool for delirium, its sensitivity can be low when administered by nurses because of its subjective nature and the need for formal training. The Ultra Brief Confusion Assessment Method (UB-CAM) is an objective, two-question test administered in 30-35 seconds compared to 5-10 minutes for CAM. The objective …

Separate But Equally Funded: Reforming Education Savings Accounts For Homeschoolers, Matthew Mcguire

Public commodities require public access. I review twenty-one state-funded education savings accounts (ESAs) funded by general fund revenues to argue that limiting home-education access violates principles of public access—The General Availability Doctrine. To the dismay of many home-educated students, nine state-funded ESAs require parents to terminate their home-education status as a condition for public funds. At least five state-funded ESAs require public school attendance as a prerequisite to receive funds, thereby precluding homeschooling students. Three state-funded ESAs treat home-education negatively by reducing funding based on a student's home-education status. The sustainability of global palm oil production faces significant threats from a range of diseases and infections affecting palm oil crops. This study presents a comprehensive bibliometric analysis to map the scientific landscape and evolution of research related to palm oil diseases over the past decade. Using data from the Scopus database, 366 relevant documents were analyzed using R and VOSviewer to uncover trends in publication volume, citation impact, prolific authors, country contributions, and keyword co-occurrence networks. This study employs the fuzzy analytic hierarchy process (FAHP) to identify the critical factors and their degree of relevance to the vehicle emission process. Its innovation lies in the potential to blend ambiguity and uncertainty with the established AHP. FAHP transforms information into a defuzzification state through signal-to-noise ratios, normalization, and pairwise comparison. The principal parameters considered are revenue, sold packing units, CAGR, packing materials, consumption, and CO2 emissions (A, B, C, D, E, and F, respectively). This quality improvement (QI) project aimed to reduce uncaptured INR results by 5% over 12 weeks in an outpatient cardiovascular clinic by implementing a standardized tracking workflow.

Suboptimal anticoagulation management in outpatient settings can lead to uncaptured International Normalized Ratio (INR) results, delayed therapy adjustments, and increased risk of bleeding or thromboembolic events.

Guided by the Plan-Do-Study-Act (PDSA) framework, a pre–post intervention design was used. Intention to leave the full-time faculty role for non-retirement reasons, including poor effort-reward balance or burnout, may be a underacknowledged contributor to the faculty shortage (Leiberg et al., 2025; Malin et al., 2024). Background: Trauma is ubiquitous within society; it is experienced by people who access healthcare, and their care providers, including nursing students who experience high rates of adverse childhood experiences (Gilroy et al., 2024). Although trauma-informed care (TIC) is an established approach that addresses the widespread health effects of trauma (Wathen et al., 2021), its systematic integration in nursing education curricula is lacking (Pfeiffer & Grabbe, 2022). In January 2025, over 230 employees were rapidly deployed from a Federally Qualified Health Center (FQHC) system to support the medical and social needs of Eaton wildfire evacuees at the Pasadena Convention Center (PCC). Of the thousands of Pasadena and Altadena residents displaced, many of the employees were directly impacted. As environmental disasters are becoming more frequent, organizations can implement measures to reduce the psychological impact by promoting coping strategies to improve mental wellbeing for nursing professionals.^1,2
