BACKGROUND: Non-trauma center (NTC) emergency departments (EDs) play regular but underappreciated roles in the management of severely injured patients, yet many lack formal protocols to guide care. We describe the implementation and outcomes of a structured trauma evaluation and transfer protocol at an urban NTC emergency department.

METHODS: A multidisciplinary trauma protocol was implemented on April 1, 2023, at an urban NTC, supported by a Level I trauma center, 2.5 miles apart. OBJECTIVE: The Tetanus, Diphtheria, Pertussis (Tdap, "tetanus") vaccine is recommended for adults every 10 years. In a national sample of adult emergency department (ED) patients, we sought to determine (1) knowledge and up-to-date status of the tetanus vaccine; (2) reasons for nonreceipt of the vaccine; (3) vaccine acceptability if offered in the ED; and (4) reasons for nonacceptance in the ED if offered.

Despite the lack of evidence of a mechanism-based drug interaction, some professional organizations have advised caution coadministering apixaban and levetiracetam. Eligible patients taking apixaban with or without levetiracetam for any indication for at least 72 h were screened from the electronic medical records. Plasma samples from 77 patients were retrieved from salvaged clinical blood collections. Apixaban minimum and maximum concentrations (Cmin and Cmax) were compared using descriptive statistics in R. While the median Cmin and Cmax appear to be marginally elevated, there were no apparent differences relative to apixaban alone. DHS Science & Technology Directorate (S&T) tasked the NCITE Center, in support of Customs and Border Protection (CBP), to develop a framework for understanding the forms and underlying processes through which Mexico-based drug cartels innovate and adapt tactically. This rapid review report identifies the processes, drivers, and constraints that shape tactical innovation within cartel ecosystems. The framework adapts and extends multilevel frameworks of tactical innovation established in the terrorism literature, most of which were developed through the study of traditional terrorist organizations whose ideological constraints, resource profiles, and internal structures often differ substantially from those of Mexican drug cartels.

This report provides a framework for how analysts can leverage observable activities of foreign terrorist organization-designated transnational criminal organizations (FDTCOs) as indicators for underlying decision-making processes around organizational stress, adaptation, and other strategic tradeoffs. Building on our understanding of FDTCOs as operating within entrepreneurial ecosystems, we apply a decision domain framework to three groups through case studies: Sinaloa Cartel, Jalisco New Generation Cartel, and MS-13. These groups pose a persistent as well as growing threat to American communities and the broader Western Hemisphere.

As the U.S. State Department increasingly designates drug cartels as Foreign Terrorist Organizations (FTOs), scholars and policymakers face a pressing need to better understand the structure and behavior of these groups. This report argues that drug cartels, particularly the thirteen currently designated as FTOs, are more accurately conceptualized as criminal entrepreneurial ecosystems than as traditional terrorist organizations.

BACKGROUND & AIMS: The genetic basis of impaired intestinal barrier function in preterm infants is poorly understood. Variants in single immunoglobulin interleukin1-related receptor, a negative regulator of Toll-like receptor signaling, have been identified in preterm infants with necrotizing enterocolitis. We hypothesized that single immunoglobulin interleukin1-related receptor variants associated with necrotizing enterocolitis impair gut barrier function to pathobionts. The aim of this study was to determine how single immunoglobulin interleukin1-related receptor genetic variants disrupt neonatal intestinal barrier integrity and promote susceptibility to Gram-negative bacteria implicated in sepsis and necrotizing enterocolitis pathogenesis.

BACKGROUND: The 2023 US Anaphylaxis Practice Parameter indicated that some patients receiving epinephrine in the community might avoid emergency department (ED) visits; however, the criteria and safety of this approach remain unproven.

This paper presents a model of the activity of patent intermediaries and potential infringers in a system of costly patent litigation.  Because the intermediary, in comparison to the inventor, has a more credible threat to sue, both the revenue from infringement litigation and the intrinsic value of the patent are greater in the intermediary’s hands.  The central finding is that intermediaries license patents to potential infringers when infringement litigation is inefficient. Intermediaries tend to acquire high-quality patents from inventors and assign low-quality patents to potential infringers. Skin carotenoid score (SCS) as measured using pressure-mediated reflection spectroscopy has been validated among adults with normal and overweight body mass index, with little validation research among adults with obesity. Therefore, the purpose of this exploratory study was to examine the validity of SCS as an approximation of fruit and vegetable intake among adults with obesity. We used samples from adults with obesity from three prior studies: The first study examined the validity of SCS among 213 adults, 34 with obesity; the second, examined sensitivity of SCS to a β-carotene-rich juice intervention among 162 adults, 14 with obesity; the third …

Background: Cannabis use and cannabis-related disorders have increased significantly worldwide. The aim of this study was to assess the prevalence of acute periapical abscesses (PAs) in cannabis users.

Methods: Data from 1,799,122 patients were analyzed and the odd ratio (OR) of acute PAs and their association with cannabis use were statistically compared.

Results: Patients using cannabis showed significantly more PAs than nonusers. The difference was statistically significant. Following adjustment for diabetes and smoking comorbidities, the statistical difference remained significant.

Conclusion: The risk for acute PAs is significantly higher in cannabis users as compared to nonusers.

On July 22, the New York State Department of Health finally adopted an entire set of new regulations covering all aspects of ionizing radiation. Contained in Article 16 of Title 10 of the Official Compilation of Codes, Rules and Regulations of the State of New York (10 NYCRR), the new regulations repealed and replaced the old Article 16. It is important to note that the new regulations are in effect only in counties outside of New York City.
