Post-traumatic stress disorder (PTSD) is a common and debilitating psychiatric disorder that may occur in individuals exposed to traumatic events such as accidents, interpersonal violence, war, combat, or natural disasters. Additionally, PTSD has been implicated in the development of a variety of chronic conditions including cardiovascular and metabolic diseases, suggesting that the biological alterations associated with the disorder can manifest themselves as chronic diseases in those suffering from PTSD. The biological underpinnings of the disorder are not well understood. Centromere binding factor 1 (Cbf1) is a transcription factor that controls the transcription of many genes involved in cellular respiration and lipid biogenesis and, as such, has been associated with hypolipidemia in humans. It is a known substrate for PAS kinase, which phosphorylates Cbf1 and alters its activity. Our hypothesis is that this phosphorylation affects the genes it regulates and the DNA motifs it binds to, perhaps due to different transcription complexes being formed. Background: The novel Coronavirus 2019 (COVID-19) pandemic had a large impact on education in the healthcare setting. At Henry Ford Jackson Hospital (HFJH), due to staffing constraints and emergent clinical needs, nurses hired during the COVID-19 pandemic did not complete standard critical care orientation after March 2020. During yearly learning needs assessments, nurses identified opportunities for enhanced education regarding the management of critically ill patients. Introduction: Endophthalmitis, a potentially devastating eye infection, can arise from various sources, including trauma, surgery, or intraocular injections. We present a case report of a patient with systemic signs of infection and bilateral eye erythema, whose condition rapidly deteriorated. Swift recognition of the symptoms and timely intervention by the medical team led to a conclusive diagnosis of endophthalmitis. The patient underwent anterior chamber (AC) washout and intravitreal injections, resulting in partial vision recovery. This case emphasizes the critical importance of early diagnosis and intervention to prevent irreversible vision loss and underscores the necessity of proactive management strategies.

In 420-bed Midwest hospital, early 2023 HCAHPS results revealed that only 53% of patients always experienced a quiet nighttime environment. The state average where this hospital resides was 58%, while the national average was 62%.  Further break down revealed a single unit-based score of 16.67% in October 2023, ranking the unit in the 1st percentile nationwide, making this the primary unit of focus for this quality improvement project.

Abstract: Commonly reported adverse effects of vaccine administration include localized erythema, pain, lymphadenopathy, myalgias, fatigue, nausea, vomiting, fever, and in extreme cases, anaphylaxis. Myocarditis, pericarditis, and thrombosis have been reported as rare side-effects of COVID vaccine administration.3 Here, we present the case of a 74 year old male, who reported the onset of persistent dysgeusia and hyposmia shortly after receiving the Moderna COVID mRNA vaccine.

Background: There is documented evidence associating hyponatremia with mortality in hospitalized patients, including patients admitted with coronavirus disease-2019 (COVID-19) pneumonia [1] One of the proposed mechanisms of hyponatremia in COVID-19 is the syndrome of inappropriate antidiuretic hormone (SIADH) due to circulating cytokines such as IL-6 [2] These cytokines are proposed to be associated with lung inflammation and correlate with respiratory failure and in-hospital mortality [3] This study was undertaken to associate the severity of hyponatremia with in-hospital mortality in patients admitted with COVID-19 in multiple hospitals over a single health system network.

Introduction: Bronchoscopy with associated mediastinal lymph node sampling and biopsy of pulmonary lesions is critical in the diagnosis of pulmonary disease, such as cancer or inflammatory conditions, as well as in oncologic staging. Biopsies via traditional flexible bronchoscopy have been limited to lesions easily accessible by the mainstem bronchi or larger segmental branches. Endobronchial ultrasound with fine needle aspiration (EBUS-FNA) of mediastinal lymph nodes or extraluminal lesions in central locations may also be performed. However, peripheral pulmonary lesions are often unable to be accessed easily or safely through flexible bronchoscopy. Trauma bays are a place in which the importance of teamwork and communication play an integral role in patient outcomes. Effective teamwork ensures that each team member plays their designated role with precision and contributes to the overall effort of stabilizing and treating trauma patients. Due to the large draw of resources to a small area (the trauma bay) the area can become crowded, and roles of each person become blurred or redundant. Mothers of children with autism have a higher rate of stress than mothers of neurotypical children and mothers of children with other disabilities. This impacts their family quality of life. This study aimed to show that by teaching mothers trauma-informed mindfulness-based stress reduction techniques they were able to switch their perspectives and feel that they had increased the relationships with their child with autism and increased the rating they give their family quality of life. Most symptom measures either implicitly or explicitly distinguish between symptom presence and symptom severity. For example, item 2 on the PHQ-9, a commonly used measure of depressive symptoms, asks respondents to rate how much they have been 'feeling down, depressed, or hopeless.' The response options are 0 (Not at all), 1 (Several Days), 2 (More than half the days), and 3 (Nearly every day). Answering 0 indicates that the symptom is not present, and any response greater than 0 suggests the symptom is present. Higher values indicate higher severity of the symptom. In this review, we cover the current understanding of BRAF mutations and associated clinical characteristics in patients with metastatic NSCLC, approved and emerging treatment options, BRAF sequencing approaches, and unmet needs. The BRAFV600E mutation confers constitutive activity of the MAPK pathway, leading to enhanced growth, proliferation, and survival of tumor cells. Testing for BRAF mutations enables patients to be treated with therapies that directly target BRAFV600E and the MAPK pathway, but BRAF testing lags behind other oncogene testing in metastatic NSCLC. Additional therapies targeting BRAFV600E mutations provide options for patients with metastatic NSCLC. Emerging therapies and combinations under investigation could …

Introduction: Nellore cattle (Bos taurus indicus) is the main beef cattle breed raised in Brazil. This breed is well adapted to tropical conditions and, more recently, has experienced intensive genetic selection for multiple performance traits. Age-Of-Information (AoI) is a metric that focuses directly on the application-layer objectives, and a canonical AoI minimization problem is the update-through-queues models. Existing results in this direction fall into two categories: The open-loop setting for which the sender is oblivious of the packet departure time, versus the closed-loop setting for which the decision is based on instantaneous Acknowledgment (ACK). Neither setting perfectly reflects modern networked systems, which almost always rely on feedback that experiences some delay. Objectives Brazil has high rates of COVID-19 and tuberculosis among healthcare workers (HCWs). Personal protective equipment (PPE) is essential for their protection. We aimed to evaluate PPE use, training, and preparedness among HCWs in the early months of the SARS-CoV-2 pandemic in Brazil.

Methods A cross-sectional study was performed using questionnaires available to HCWs through a website created to provide PPE guidelines. χ2 test and robust Poisson regression identified factors associated with HCWs treating COVID-19 patients (TCOVID-19), lack of training on PPE use and N95 respirator reuse. The speech content of open-ended questions was analysed.

Relief In Gastrointestinal Symptoms With Medical Marijuana Over 1 Year, Matthew P. Wallingford, Erin L. Kelly, Allison Herens, Daniel Hanna, Emily Hajjar, Brooke Worster

INTRODUCTION: Subjective improvement in gastrointestinal (GI) symptoms was assessed among patients using medical marijuana (MMJ).

METHODS: Participants completed surveys at 0 days, 30 days, 6 months, and 12 months with questions about the severity of their GI symptoms on a scale from 1 (mild) to 3 (severe).

RESULTS: In each survey, participants reported a significant decrease in GI symptom severity when using MMJ versus when not using MMJ (p < 0.05). The most common self-reported side effects from using MMJ were increased appetite (12-21.4%), fatigue (6-16.7%), anxiety (4-11.9%), cough (4-11.9%), headache (6-7.9%), and dry mouth (4-7.1%).

CONCLUSION: In patients with chronic GI symptoms, MMJ may provide persistent symptom severity improvement. Limited product availability and mild to moderate side effects are factors to consider before trialing MMJ.

The alternative splicing of PML precursor mRNA gives rise to various PML isoforms, yet their expression profile in breast cancer cells remains uncharted. We discovered that PML1 is the most abundant isoform in all breast cancer subtypes, and its expression is associated with unfavorable prognosis in estrogen receptor-positive (ER+) breast cancers. PML depletion reduces cell proliferation, invasion, and stemness, while heterologous PML1 expression augments these processes and fuels tumor growth and resistance to fulvestrant, an FDA-approved drug for ER+ breast cancer, in a mouse model. The inherently weak chiroptical responses of natural materials limit their usage for controlling and enhancing chiral light-matter interactions. Recently, several nanostructures with subwavelength scale dimensions were demonstrated, mainly due to the advent of nanofabrication technologies, as a potential alternative to efficiently enhance chirality. However, the intrinsic lossy nature of metals and the inherent narrowband response of dielectric planar thin films or metasurface structures pose severe limitations toward the practical realization of broadband and tailorable chiral systems. This study compared marginal and conditional modeling approaches for identifying individual, park and neighborhood park use predictors. Data were derived from the ParkIndex study, which occurred in 128 block groups in Brooklyn (New York), Seattle (Washington), Raleigh (North Carolina), and Greenville (South Carolina). Survey respondents (n = 320) indicated parks within one half-mile of their block group used within the past month. Parks (n = 263) were audited using the Community Park Audit Tool. Background: Athletes who play outdoor sports have multiple risk factors for the development of skin cancer. Literature highlights the need for the use of evidence-based strategies to improve the knowledge and utilization of sun protection practices in this high-risk population. Methods: Playing it Sun Safe was a multifaceted skin cancer prevention initiative implemented in a population of collegiate soccer athletes in collaboration with a university athletic department and campus health clinic. Evidence-based interventions were designed through application of the Health Belief Model (HBM) and were implemented using the Institute for Healthcare Improvement (IHI) quality improvement framework.
