Design And Implementation Of Error Estimators For Finite Element Eigenvalue Problems, Gabriel Esteban Pinochet Soto

Dissertations and Theses

We present three publications, all encompassed under the umbrella of a posteriori error estimation theory for eigenvalue problems for finite element discretizations. The central objective of the research is the development of a general framework for the study of reliable estimation of eigenvalues and eigenspaces. We introduce applications to problems of theoretical interest as well as problems arising in real-life scenarios, such as optical fibers. The first paper focuses on the implementation of a dual-weighted residual error estimator for a nonselfadjoint eigenvalue problems arising from the study of leaky modes in optical fibers---Maxwell's equations, Perfectly Matched Layers, and a conforming …

All Decisions

In this no-grounds holdover, the tenant moved to dismiss, arguing the apartment was subject to VAWA, requiring specific notices. The court denied this, finding an issue of fact regarding whether the premises was a covered housing accommodation. The tenant also claimed non-receipt of a GCEL notice with the petition, but the court found it was attached electronically and the petition restated the small landlord exemption. Finally, the tenant sought dismissal for insufficient small landlord information, but the landlord's cross-motion to amend the petition to include co-owner details was granted, as no prejudice was shown. All tenant's motions to dismiss were …

All Decisions

The Civil Court dismissed this no-grounds holdover because the lease lacked a conditional limitation permitting the landlord to terminate the tenancy early. The court found the relevant lease provisions did not "provide that the term of the lease will automatically expire upon the lapse of time fixed in the notice," as required. Without a valid conditional limitation, the landlord lacked a cause of action, citing Fourth Hous. Co., Inc v Bowers and 1900 Albermarle, LLC v Solon.

All Decisions

In this holdover, the court grants respondent's motion to dismiss where the petition pleaded alternate theories of occupancy: either the tenant had no rights, or was a licensee and therefore failed to comply with RPAPL 741(4). This case is useful to distinguish the holding in Kew Gardens v. Bucheli, 69 Misc 3d 129(A) (App Term, 2d Dept 2020), which allows pleading inconsistent theories under certain circumstances.

Advances in Clinical Medical Research and Healthcare Delivery

Introduction: Spontaneous isolated celiac artery dissection (SICAD) is a rare condition, with an unpredictable natural history. It typically affects middle aged men with risk factors including hypertension and smoking. It can remain asymptomatic, and for many patients is detected as an incidental finding. The increasing number of reported cases over the past decade has been attributed to advances in imaging modalities. Management strategies vary depending on presentation. Case presentation: We describe a case of SICAD with atypical demographics. A 77-year-old female with no smoking history presented with progressively worsening abdominal pain. Contrast-enhanced CT revealed an isolated celiac artery dissection. She …

Toxoplasmosis-Associated Immune Reconstitution Inflammatory Syndrome In A Patient With Newly Diagnosed Hiv/Aids, Shivam Singh, Divya Samat Md, Viraj Shah, Kelly Brenan-Rothschild, Anthony Donato

Advances in Clinical Medical Research and Healthcare Delivery

Background: Cerebral toxoplasmosis is a common cause of central nervous system (CNS) lesions in advanced HIV, and initiation of antiretroviral therapy (ART) can precipitate immune reconstitution inflammatory syndrome (IRIS). Toxoplasma-related IRIS is rare but poses significant diagnostic and therapeutic challenges. Case Presentation: We report a 40-year-old male with advanced HIV (CD4 <35 cells/mm3, viral load 540,000 copies/mL) who initially presented with seizures and multiple ring-enhancing supratentorial lesions. Serology and cerebrospinal fluid (CSF) polymerase chain reaction (PCR) confirmedToxoplasma gondii, and he was treated with high-dose sulfamethoxazole-trimethoprim, dexamethasone, and ART. A lymph node biopsy revealed granulomatous inflammation with intracellularHistoplasmaspecies. One month later, following brief interruption of anti-toxoplasma therapy, he developed recurrent seizures and radiographic progression despite ART adherence. Repeat evaluation excluded other infections or malignancy. Brain biopsy confirmedT. gondii, and IRIS was diagnosed. The patient improved with re-initiation of high-dose anti-toxoplasma therapy, corticosteroids, and seizure prophylaxis. Discussion: This case illustrates paradoxical toxoplasmosis-associated IRIS in the context of advanced HIV and concurrent opportunistic infection. Early recognition, exclusion of alternative diagnoses, and continuation of ART with targeted antimicrobial therapy are essential. Conclusion: Clinicians should maintain a high index of suspicion for IRIS in HIV patients with CNS …

Appendiceal Neuroendocrine Tumor: A Case Report, Daniel Zipfel, Thomas Russo, Mathew Devine

Advances in Clinical Medical Research and Healthcare Delivery

Neuroendocrine tumors (NETs) are rare neoplasms that most commonly occur in the gastrointestinal tract, lungs, and pancreas but may arise anywhere where there are neuroendocrine cells within the body. When neuroendocrine tumors metastasize to the liver, patients present with carcinoid syndrome, a group of symptoms characterized by flushing, diarrhea, and right-sided heart disease. However, many NETs do not cause carcinoid syndrome, sometimes leading to a unique clinical presentation. Patients may present with symptoms of appendicitis, ovarian torsion, or be completely asymptomatic. We report a case of a 25-year-old male with a Grade 1 neuroendocrine tumor of the appendix, who underwent …

Checkpoint Inhibitor–Associated Autoimmune Diabetes Following Pd-1 Blockade: Severe Diabetic Ketoacidosis After Pembrolizumab, Divya Samat, Osadebamwen Osaghae, Shivam Singh, Mohammad Abu Tineh, Anthony Donato

Advances in Clinical Medical Research and Healthcare Delivery

Immune checkpoint inhibitors (ICIs) have transformed cancer therapy by enhancing antitumor T-cell activity through blockade of inhibitory pathways including programmed cell death protein 1 (PD-1) and its ligand (PD-L1). Loss of immune tolerance can lead to immune-related adverse events (irAEs), including endocrine toxicities. Checkpoint inhibitor–associated autoimmune diabetes (CIADM) is rare but clinically significant because it often presents abruptly with diabetic ketoacidosis (DKA) and profound insulin deficiency. We describe an 81-year-old male with stage III clear-cell renal cell carcinoma who developed abrupt-onset autoimmune diabetes 8 weeks after initiating adjuvant pembrolizumab. The patient presented with severe hyperglycemia and high anion-gap metabolic acidosis …

Addressing Doscrimination: Examining Bias Against Osteopathic Physicians, Sophia M. Valerino

Advances in Clinical Medical Research and Healthcare Delivery

Osteopathic medicine arose as a response to the limitations of conventional medical practices of the time. It emphasizes the body's innate ability to heal and the importance of holistic care and has since evolved into a rigorous and evidence-based system that parallels allopathic education in scope, standards, and licensure. Nevertheless, osteopathic physicians (DOs) continue to face systemic bias across multiple areas. This article examines the historical context and current manifestations of discrimination against DOs, highlighting disparities in residency selection, research participation, academic leadership, and media portrayal. Despite clear evidence showing comparable performance between DOs and MDs in clinical care and …

Vitamin D Deficiency And Risk Of Cognitive Decline And Dementia: Implications And Prevention Strategies, Auda Fares

Advances in Clinical Medical Research and Healthcare Delivery

Emerging evidence suggests a link between vitamin D deficiency and an increased risk of cognitive impairment and dementia, including Alzheimer's disease (AD). This paper reviews the epidemiological evidence, explores potential mechanisms underlying this association, and discusses implications for prevention strategies. Current supplementation strategies may not be fully effective due to individual variability. Personalized interventions based on genetic factors and routine vitamin D status testing in older adults are proposed as future directions for optimizing the impact of vitamin D supplementation in dementia prevention.
