The Importance Of Pedal Acceleration Time (Pat): A Case Study, Kelly N. Bui

A case study manuscript about the importance of pedal acceleration time (PAT) exams in assessing lower extremity perfusion at the level of the foot and its role in clinical decision making. The diagnostic value of PAT exams is compared with other methods such as ankle brachial indices (ABI), toe-brachial indices (TBI), and lower extremity arterial duplex. This case presents a patient with a complex medical history and the ultrasound exams listed above displayed its strengths and limitations as to how it contributed to the patient's overall vascular assessment.

Regulatory non-compliance among public water systems poses a direct threat to public health, so we develop and apply a compliance assessment framework for state-level data from the Mississippi State Department of Health in order to aid public water systems in updating infrastructure to improve water quality. The framework evaluates public water system compliance across financial, managerial, and technical indicators, producing a composite score to support targeted interventions and data-driven resource allocation. Analyzing 1,107 systems, we identify a tiered structure of deficiencies, ranging from critically non-compliant rural utilities and mobile home parks to mid-range deficient municipal systems, and highlight the 15 …

The management of forest-based land uses and water resources largely complement one another. Formal research where vegetation versus water-focused resources intersect has been limited primarily to assessing short-term impacts of intensive forest harvesting operations on water quality or watershed discharge. Outside of harvesting, foresters and watershed managers are generally aware that changes in forest structure or evolving management protocols that play out on multi-decadal scales also impact water resources. Background: Fluoxetine is a commonly prescribed selective serotonin reuptake inhibitor in children and adolescents. It is administered as a racemate where the R- and S-enantiomers differ in pharmacological activity and metabolism. Additionally, fluoxetine and its primary metabolite, norfluoxetine, contribute to overall therapeutic effects. S-fluoxetine is primarily metabolized by CYP2D6 to form S-norfluoxetine, while R-fluoxetine is metabolized by CYP2D6, CYP2C9, and CYP2C19 to form R-norfluoxetine. These enzymes are highly polymorphic, and the combination of alleles an individual carries corresponds to a metabolizer phenotype, such as CYP2D6 poor (PM), intermediate (IM), normal (NM), and ultrarapid metabolizers (UM). Patients with short bowel syndrome (SBS) experience nutrient malabsorption. The clinical manifestations of malabsorption are heterogeneous due to the variety of congenital and acquired anatomic abnormalities that result in SBS. Because of various gastrointestinal pathophysiologic alterations, fat malabsorption is an important nutritional factor to consider. The Intestinal Rehabilitation Program at Children’s Mercy has been providing a comprehensive approach to caring for children with SBS, and has incorporated RELiZORB (Alcresta Therapeutics, Inc.) immobilized lipase cartridge (ILC) into their treatment armamentarium. ILC is a single-use digestive enzyme cartridge that connects in-line with enteral feeding circuits to hydrolyze triglycerides in enteral formulas. Although …

Background: Anemia is a common disorder of prematurity secondary to limited blood volume, excessive blood draws, inadequate nutrition, poor iron reserves, and insufficient endogenous erythropoietin. While many NICUs use recombinant erythropoietin (rEpo) to treat anemia of prematurity, the absence of universal guidelines has

led to variability in dosing and schedules. Here we present our experience on the use of rEpo in a level 3 NICU. Our unit protocol uses 400 IU/kg three times a week for a total of 9 doses if hematocrit < 30%.

Enteric duplication cysts (EDCs) are rare congenital anomalies of the gastrointestinal (GI) tract that may present with abdominal pain, vomiting, palpable mass, or bleeding, depending on their location. We present a case of a 9-year-old male with recurrent rectal bleeding who was ultimately diagnosed with an EDC via small bowel capsule endoscopy and laparoscopy. The patient had a history of iron deficiency anemia and recurrent admissions for hematochezia and fatigue. Previous imaging and endoscopy procedures were unrevealing. Background: Refractory benign esophageal strictures (RBES) are persistent esophageal narrowings resistant to repeated surgical or endoscopic treatment. RBES may result from various causes, including chemical injury, gastroesophageal reflux disease, tracheoesophageal fistula, achalasia, or congenital esophageal stenosis (CES). CES is a rare congenital anomaly caused by tracheobronchial remnants, segmental fibrosis, or membranous diaphragm. Management varies based on severity and includes endoscopic dilation, stent placement, and surgical esophagoplasty.

Dexmedetomidine is a highly hydrophilic alpha-2 agonist commonly used for sedation in critically ill patients to maintain devices and support.  However, critical illness can prolong its elimination in the critically ill child. A problem that is likely due to reduced metabolic metabolism and increased volume of distribution.

Extracorporeal life support (ECLS), including extracorporeal membrane oxygenation (ECMO), further alters drug pharmacokinetics through increased distribution volume, reduced clearance, and drug sequestration with the circuit. Introduction

JAK2 rearrangements are recurrently observed (and correlate with some of the lowest survival rates) within Ph-like B-cell acute lymphoblastic leukemia but are very rare in T-cell Lymphoblastic Leukemia (T-ALL). Here we present a patient with pediatric T-ALL who was shown to have a TBL1XR1::JAK2 fusion by Optical Genome Mapping (OGM), which was further supported by long-read sequencing. This is only the second report of JAK2 fusing with TBL1XR1 in pediatric T-ALL.

Background:

Methotrexate (MTX) is a slow-acting disease modifying anti-rheumatic drug and one of the first drugs of choice for the treatment of juvenile idiopathic arthritis (JIA). Efficacy is observed within 6 months, and approximately one third of patients are nonresponders.  Delays in obtaining early disease control place nonresponders at risk for lifelong disability. Therefore, predicting responsiveness to MTX treatment can prevent long-term consequences in these patients. Methotrexate is a folate agonist and undergoes intracellular processing by addition of up to seven glutamate residues by the cytosolic enzyme folylpolyglutamate synthetase encode by the gene FPGS.
