Introduction:

Streptococcus dysgalactiae causes pharyngitis, cellulitis, necrotizing soft tissue infections, sepsis, and streptococcal toxic shock syndrome. It can lead to both toxin-mediated and immune-mediated diseases. Mostly seen in adults, rare in pediatric patients. Here we present a 15-year-old with S. dysgalactiae- associated septicemia, and epiglottitis with extensive neck infection.

Case:

A previously healthy 15-year-old male, recently immigrated from Egypt

Presented with fever and sore throat

Initial tests for influenza and Group A streptococcal infection were negative

Rapid overnight decompensation - CT scan revealed extensive neck inflammation, cellulitis, epiglottitis, and cervical lymphadenopathy

Blood cultures confirmed S. dysgalactiae septicemia

Background: Pediatric Feeding Disorder affects 1 in 37 young children, with some using feeding tubes for survival. Due to the financial, social, and medical burden of feeding tubes, the goal is to transition to all oral feedings when medically ready; however, children are referred to the multidisciplinary feeding clinic with wide variability in age, nutritional needs, and tube feeding duration. Slipped capital femoral epiphysis (SCFE) is strongly associated with pediatric obesity, yet post-operative care often prioritizes surgical recovery without addressing broader lifestyle and metabolic risk factors. SCFE may represent a critical “wakeup call” for families to initiate health behavior change, but little is known about how families perceive and respond to this opportunity.

Background: Robin Sequence (RS) is a congenital anomaly characterized by glossoptosis, micrognathia, and airway obstruction. The goal in treating airway obstruction secondary to RS is to maximize developmental potential; however, identifying the best interventions for individual patients remains a challenge.

Background: The GOLDILOKs® (Genomic and Ontogeny-Linked Dose Individualization and Clinical Optimization for Kids) Clinic provides personalized care for children receiving pharmacologic therapy.1 Since opening in July 2010, clinic providers have offered genotype informed clinical guidance to pediatric patients and their healthcare providers. However, no comprehensive retrospective study has specifically examined psychiatric conditions to evaluate how frequently and effectively this clinical guidance is utilized by providers.

Newborns, especially preterm infants, are highly susceptible to sepsis, a major cause of neonatal morbidity and mortality. Known risk factors include prematurity, low birth weight, prolonged Neonatal Intensive Care Unit (NICU) stay, and use of central venous catheters. The link between early nasal colonization (within the first week of life) and later invasive infections/sepsis is unclear. Background:

Neonatal early‑onset sepsis (EOS) remains a significant cause of morbidity and mortality in the immediate postnatal period. While Group B Streptococcus and Escherichia coli account for most EOS cases, Streptococcus pneumoniae is a rare but highly aggressive pathogen associated with mortality rates as high as 20–60%. In this case, a term newborn experienced fulminant multi‑organ failure within hours of birth despite minimal perinatal risk factors and negative maternal GBS screening.

Eosinophilic esophagitis (EoE) is an increasingly prevalent chronic antigen-mediated TH2-type immune disorder characterized by symptoms of esophageal dysfunction resulting in difficulty swallowing, food impaction, nausea, vomiting, abdominal pain, and eventually esophageal strictures if not controlled. Currently, there are no minimally invasive tests that can screen, diagnose, or monitor patients with EoE. Patients with suspected EoE usually undergo general anesthesia for an upper endoscopy to obtain esophageal biopsies. Diagnosis is established when histological evaluation shows intraepithelial eosinophilia. Background: Vitamin K deficiency can present as bleeding in infants due to the inability to produce clotting factors needed for sufficient hemostasis. Newborns have sterile gut microbiomes and do not absorb large amounts of vitamin K. Additionally, vitamin K is transferred poorly via the placenta, leading to limited vitamin K stores.

Background: Maternal antenatal selective serotonin reuptake inhibitor (SSRI) use has been suggested as a risk factor for neonatal complications, though evidence remains inconclusive. Mental health conditions increased during the COVID-19 pandemic, raising questions about SSRI prescription trends. This study examines associations between maternal SSRI use and neonatal outcomes across three time periods: before, during, and after the pandemic.

Problem Statement/Question: Will a standardized nebulized TXA protocol reduce the need for surgical control of post-tonsillectomy hemorrhage without increasing adverse clinical outcomes?

Background/Project Intent (Aim Statement): Emerging evidence suggests that tranexamic acid (TXA), a widely used antifibrinolytic agent, may be effective in the management of post-tonsillectomy hemorrhage (PTH). We hypothesized that initiation of a standardized nebulized TXA protocol would reduce the need for surgical control of bleeding without increasing adverse clinical outcomes.

Methods (include PDSA cycles): This quality improvement (QI) project included patients < 18 who underwent extracapsular tonsillectomy and subsequently presented with PTH. Three cohorts were analyzed: a pre-intervention group (January 2020-June 2022), a non-standardized TXA phase following departmental review of emerging evidence (PDSA cycle 1; July 2022- December 2023), and a standardized nebulized TXA protocol phase (PDSA cycle 2; January 2024-July 2025). The primary outcome was surgical intervention for PTH. Balancing measures included hospital length of stay, blood product transfusions, 30-day readmissions, and TXA-attributable complications. Data was analyzed using statistical process control methods.
