Introduction: Use of telemedicine in abortion care is safe and effective. Patient satisfaction with telemedically supported abortion is high, but as use expands in Britain, little is known about patients’ or health professionals’ views on how it is best used. We sought the views of both groups on telemedicine's role in abortion provision and how its use might be optimised. This working paper examines the nuanced relationship between narcissism and psychopathy, with a specific focus on how these traits manifest among females in both community and corporate contexts. Drawing on a 12-year research programme into dark-triad traits, the paper highlights key differences between grandiose and vulnerable narcissism, distinctions between narcissistic emotional reactivity and psychopathic affective detachment, and gendered variations in psychopathic expression—including relational aggression, emotional instability, and covert manipulation. Findings from corporate samples reveal unexpected patterns, such as elevated rebellious nonconformity and coldheartedness among female executives. Gastric Linitis plastica is characterized by extensive infiltration of gastric wall by poorly differentiated tumor cells, creating a "leather-bottle stomach" appearance. We describe a case involving a 71-year-old male presenting with globus sensation, early satiety and weight loss. Recent EGD had revealed chronic gastritis with polypoid mucosa at the GE junction, and subsequent FDG-PET indicated asymmetric FDG localization. On admission, repeat EGD with deep biopsies confirmed invasive poorly differentiated gastric adenocarcinoma with signet ring features. Despite palliative radiation, the patient died within a month, highlighting the disease’s aggressiveness and importance of advanced diagnostic techniques in suspected cases of Linitis plastica.

Most cases of Myxedema Coma are associated with primary hypothyroidism characterized by significantly elevated TSH levels. However, this case presents an atypical manifestation of myxedema coma with low TSH levels despite severe hypothyroidism. The rarity of this presentation lies in the absence of central lesions typically responsible for such TSH suppression. This highlights a critical consideration: the profound impact of severe sepsis on thyroid hormone regulation and the hypothalamus-pituitary-thyroid axis.

Lymphocytic gastritis (LG) is a rare form of gastritis characterized by lymphocytosis in the gastric mucosa, while microscopic colitis (MC) is the chronic inflammatory disease of the large intestine with lymphocytic or collagenous colitis as two distinct histologic forms. These lymphocytic disorders of the gastrointestinal tract (GIT) have various associations, commonly gluten-sensitive enteropathy, Helicobacter pylori infection and while others are less commonly associated. Neurosyphilis is a condition characterized by insidious onset of encephalopathy and delirium. The infrequency with which it is encountered makes neurosyphilis a formidable diagnostic challenge. We present a rare case of a 71-year-old male with ischemic cardiomyopathy, chronic obstructive pulmonary disease (COPD), undifferentiated arthritis and alcohol use disorder who was brought to the emergency department after he was found altered, confused, and paranoid. His hospital stay was eventful for multiple episodes of agitation that were difficult to control despite benzodiazepines and high doses of antipsychotics. Intracardiac masses are rare and potentially life-threatening entities with diverse clinical presentations. The prompt identification of cardiac masses is critical. However, even with the advancement we have in imaging modalities, diagnosing cardiac masses remains a formidable challenge. Herein, we present the case of a 62-year-old female who presented with a three-week history of dyspnea and chest pain. Further workup revealed elevated troponins, thrombocytosis, and ST-segment elevation in the anterolateral leads. Chest CT revealed a concerning lung mass, along with vertebral lesions and a large pleural effusion. The patient underwent treatment for suspected myocardial infarction. Paraneoplastic limbic encephalitis (PLE) is a poorly understood condition, thought to be caused by the cross-reacting of tumor antibodies with neurons in the brain, resulting in neuropsychiatric sequelae, such as personality and behavioral changes, psychosis, memory loss, and seizures. Anti-contactin-associated protein-like 2 (CASPR2) antibodies can cause PLE in patients with particular tumors, which in most cases can be identified as thymoma, lung cancer, or endometrial cancer. Some case reports show rare instances with other tumors, such as throat or sigmoid carcinoma. We present the first reported case of CASPR2-antibody encephalitis secondary to a testicular mixed germ cell tumor.
