The focus of this study was to identify risk factors for severe and critical COVID-19, evaluate local respiratory immune responses to SARS-CoV-2 infection, and develop a prognostic tool for COVID-19 severity using accessible early indicators. Using nasopharyngeal swab samples from hospitalized patients with COVID-19 of varying severity during the first wave of the pandemic from March to May 2020 in Louisiana, we evaluated the association between COVID-19 severity and viral load, respiratory immune mediators, and demographic/clinical factors. We found that the SpO2/FiO2 ratio at triage, total comorbidity burden (represented by Charlson Comorbidity Index), and gender were significantly associated with COVID-19 …

The Politics Of Constitutional Dignity Jurisprudence, Daniel Haefke

This Article traces the politics of (human) dignity in US constitutional law. It reveals that the notion has undergone a conservative shift, which has resulted from Supreme Court Justices increasingly unleashing dignity’s dormant conservative potential. Legal scholars arguing for prominently including the notion in constitutional jurisprudence reflect the belief that adopting the language of dignity would push the Supreme Court to be more sensitive to progressive political demands. This progressive constitutional dignity optimism is historically plausible yet conceptually misguided. This paper presents an experimental investigation on the quasi-static indentation performance of polyurethane (PU) foam-filled carbon fibre-reinforced polymer (CFRP) and glass fibre-reinforced polymer (GFRP) corrugated sandwich composite structures (SCSs) under different indenter tip geometries (conical, trapezoidal, hemispherical, pyramidal, and cylindrical shapes). The SCSs were produced using hybrid manufacturing methods: (1) the facesheets were fabricated using the vacuum-assisted resin infusion (VARI) process, (2) the CFRP and GFRP corrugated cores were additively manufactured using the fused filament fabrication (FFF) process, and (3) the self-expanding PU foam-filled the open spaces in the core. This qualitative study, grounded in hermeneutic phenomenology, explores the experiences of Black mothers who have lost their only child, addressing a topic often overlooked in counseling training programs. Many counselor educators and supervisors are unprepared to train counselors on how best to support these mothers through their unique grief journeys. This study outcomes fill that gap by providing a deeper understanding of their experiences, intending to shape better counseling practices. Summary Poor treatment retention rates are significant problems in mental health, with adverse outcomes being more profound for vulnerable populations, such as individuals experiencing homelessness. The current project was vital because the lack of engaging strategies in working with individuals experiencing homelessness contributes to high rates of treatment dropout. Although the Covid-19 pandemic emerged in 2020, limited information about how the virus affects adolescents regarding long-term health risks is available. This quantitative study focused on the effect of U.S. adolescents' daily diet, race/ethnicity, levels of income, access to health care, age, and comorbidities on the risks of long Covid-19 syndrome. The constructs of the health belief model were used to understand the impact of those variables. Caribbean immigrants are vital to the U.S. health care workforce, yet they face barriers that often hinder their career satisfaction and well-being. This qualitative study aimed to explore how intrinsic motivations, specifically competence, autonomy, and relatedness, shape the career experiences of Caribbean immigrants in health care. Using self-determination theory as the theoretical framework, this study addressed the research question of how these intrinsic motivations shape immigrant experiences in the U.S. health care sector. Data were collected from 10 Caribbean immigrants who were naturalized U.S. citizens using semistructured, audio-recorded interviews. The data were thematically analyzed to detect patterns and themes. This systematic review examines how Social Network Analysis (SNA) is used in the study of Organizational Behavior (OB). Industrial-Organizational Psychology Literature suggests SNA tends to be applied using either a structural or relational approach (Erikson, 2013). Some researchers claim that this methodology is applied inconsistently due to the intermingling of two distinct underlying frameworks and the exclusion of individual differences and interpersonal factors limits the relevance of SNA (Erikson, 2013; Kilduff & Lee, 2020). Psychological research on climate change often focuses on driving adaptive individual behaviors in nonspecific contexts. Focusing on contexts where group-level behavior emerges (e.g., workplaces) is likely to yield insights that affect more significant numbers of people, in turn, having a greater impact on the natural environment. This study, therefore, explored the implications of a new operationalization of pro-environmental work climate, Organizational Climate for Environmental Action (OCEA). Participants (N = 393) responded to an online survey. To protect consumers from unfair contracts, legislatures have implemented measures requiring consumers to signal specific agreement to crucial clauses. One such common practice is requiring consumers to initial specific provisions within contracts. This dissertation investigates the efficacy of initialing and other secondary tasks in promoting consumer understanding and memory of contractual information. Through a series of three experimental studies, participants engaged in tasks involving initialing, semantic processing, or irrelevant letter-classification tasks while processing contract information. Desmoplastic melanoma (DM) is an uncommon subtype of cutaneous melanoma that presents distinct diagnostic and treatment challenges. This review aims to explore the role of adjuvant radiation therapy (RT) in managing DM. To evaluate this question, we reviewed relevant published reports on DM and its treatment and synthesized these findings. It was found that the clinical behavior of DM varies significantly based on its classification as either "pure" DM (pDM, ≥90% desmoplastic features) or mixed DM (mDM, ≤90% desmoplastic features). Patients with pDM have a uniquely high risk of local recurrence but a relatively lower likelihood of nodal disease. Recent …

In the fourth primer of the Granite Guide to Early Childhood series, authors Tyrus Parker and Jess Carson report that, in 2023, the average price of full-time, center-based care for an infant and a four-year-old in New Hampshire was nearly $32,000 a year. Equivalent to 28 percent of median family income for New Hampshire households with children under 5, this is four times the 7 percent threshold that the U.S. Department of Health and Human Services has deemed affordable.

Cardiovascular disease is the leading cause of death for women. Women with cardiovascular diseases are less likely to receive the available advanced therapies such as defibrillators, pacemakers, and left ventricular assist devices as part of their cardiovascular care plan when compared to their male counterparts. Biobased plastics are sometimes promoted as “environmentally friendly” compared to their conventional petrochemical-based counterparts, but their ecotoxicity is only partially understood. Biobased fibers are widely used in clothing and wet wipes and can accumulate in soils through the application of biosolid fertilizers. This study examined the lethal thresholds and sublethal toxicity of chemically characterized, additive-free, biobased (viscose and lyocell) compared to petrochemical-based (polyester) fibers on the key ecosystem engineer, Esenia fetida. Viscose and lyocell had LC20 values of 14.00 and 22.66 mg·L-1, respectively, and no observed effect concentrations (NOEC) of 0-2.8 mg·L-1 (72 h, OECD TG207 …

Delta-like Ligand 3 (DLL3) targeting therapies are promising in small cell lung cancer (SCLC) treatment. However, DLL3 expression in SCLC and other neuroendocrine neoplasms (NEN) is heterogeneous and not well characterized. We describe the landscape of DLL3 at the mRNA and protein levels across SCLC, large cell neuroendocrine carcinoma (LCNEC), and non-small cell lung cancer. Additionally, we explore its expression in extra-pulmonary NEN (EP-NEN) using a standardized DLL3 IHC assay. DLL3 expression is enriched in SCLC, LCNEC along with combined histology lung cancers. This narrative review seeks to summarize chemotherapeutic regimens commonly used for patients with newly diagnosed Philadelphia (Ph) chromosome-negative B-cell precursor acute lymphoblastic leukemia (BCP-ALL) in the frontline setting and to describe the latest clinical research using the bispecific T-cell-engaging immunotherapy blinatumomab in the first-line treatment setting. Current standard-of-care chemotherapeutic backbones for newly diagnosed Ph-negative BCP-ALL are based on the same overarching treatment principle: to reduce disease burden to undetectable levels and maintain lasting remission. The adult treatment landscape has progressively evolved following the adoption of pediatric-inspired regimens. However, these intense regimens are not tolerated by all, and high-risk patients still …

Alameda County departments and organizations have continually provided varying services to members in the unhoused community to increase pathways to stabilization for those served, with little success for over 37 years. While there were studies that discussed the issues of high service utilization faced by unhoused behavioral health care recipients, there was little to no literature that discussed these issues in Alameda County, where the numbers of unhoused persons total more than 9,500. Wastewater stabilization ponds, such as facultative and maturation lagoons, have been shown to be effective processes to manage conventional water quality parameters found in domestic wastewater. The main goal of this thesis is to explore whether wastewater stabilization ponds can simultaneously reduce pathogens to safe levels while supporting agricultural activities through nutrient recovery. A large dataset for water quality of wastewater lagoon effluent in the U.S. was obtained at the state level and combined into regional southern and northern locations. The purpose of this dissertation is to emphasize notions of citizenship and accessibility as experienced by Black women Veterans. By expanding our worldview of Veteran identity, the ethnographic data demonstrate issues of displacement, alienation, and longing that contribute to the dissonance Black women Veterans rooted in the U.S. South may experience. To forge belonging through place-making, Black women Veterans may seek sisterhood by building sites of resistance in person or online. Findings are based on in-depth interviews with Black women Veterans and participant observation with community partners who service Veteran needs. Implants used in lumbar spinal fusion and lumbar total disc replacement procedures have improved substantially over the years, however, the opportunity remains to improve either the adjustability, durability, motion, or minimally invasive characteristics which could aid in achieving optimal surgical outcomes. It is hypothesized that the inherent advantages of compliant mechanisms specifically Deployable Euler Spiral Connectors (DESCs) and a new mechanism known as the interior contact aided rolling element (I-CORE) are well suited for each of these applications and will be explored and utilized as the basis of these proposed designs. Previous studies have shown that beta-band transcranial alternating current stimulation (tACS) applied at the M1 hotspot can modulate corticospinal excitability. However, it remains controversial whether tACS can influence motor unit activities at the spinal cord level. This study aims to compare the efficacy of applying tACS over the hotspot versus the conventional C3 site on motor unit activities and subsequent behavioral changes. This study used a randomized crossover trial design, where fifteen healthy participants performed a paced ball-squeezing exercise while receiving high-definition tACS (HD-tACS) at 21 Hz and 2 mA for 20 min. Background: Recent trials confirmed the efficacy and safety of endovascular thrombectomy in patients with low Alberta Stroke Program Early Computed Tomography Score (ASPECTS); however, evidence in tandem lesions is limited. This study evaluates endovascular thrombectomy safety and efficacy in patients with acute large-vessel occlusion with tandem lesions, stratified by baseline ASPECTS.
Methods and results: We conducted a retrospective analysis of data from 16 centers. Inclusion criteria included the following: age ≥18 years, anterior circulation tandem lesions, endovascular thrombectomy < 24 hours of symptom onset, and ≥70% internal carotid artery stenosis/occlusion. Patients were categorized into low (0-5) and high (6-10) ASPECTS. Inverse probability of treatment weighting matching was used to balance the groups. Primary outcomes included the following: 90-day modified Rankin Scale (mRS) score 0 to 2 and symptomatic intracranial hemorrhage. Secondary outcomes included the following: ordinal mRS, mRS 0 to 3, modified Thrombolysis in Cerebral Infarction ≥2b and 2c-3, petechial hemorrhage, parenchymal hematoma (1/2), early neurologic improvement, and mortality. Of 691 patients, 44 had ASPECTS 0 to 5 and 505 had ASPECTS 6 to 10. Patients with low ASPECTS had lower odds of 90-day mRS 0 to 2 (adjusted odds ratio [OR], 0.48; P=0.036) and higher odds of symptomatic intracranial hemorrhage (adjusted OR, 3.78; P=0.014).
