Surgery Articles

The Bier block for intravenous regional anesthesia is generally well tolerated, and few complications have been reported. It provides a bloodless surgical field and adequate analgesia when performed correctly. A diluted short-acting local anesthetic such as 0.5% lidocaine is typically used. Our case is a 41-year-old female who presented for endoscopic carpal tunnel release of her right wrist. An accidental injection of 30 mL of 0.5% ropivacaine was given instead of the normal 30 mL of 0.5% lidocaine, with a total tourniquet time of 46 min. In this study, a new hybrid nanoparticle composed of magnesium hydroxide and copper oxide (Mg(OH)2/CuO) with an optimized ratio of magnesium (Mg) to copper (Cu) was designed and incorporated into a 3D-printed scaffold made of polycaprolactone (PCL) and gelatin. These hybrid nanostructures (MCNs) were prepared using a green, solvent-free method. Their topography, surface morphology, and structural properties were characterized using scanning electron microscopy (SEM), transmission electron microscopy (TEM), X-ray diffraction (XRD), and X-ray photoelectron spectroscopy (XPS). Hybrid quantum–classical (HQC) computing refers to the approach of executing algorithms coherently on both quantum and classical resources. This approach makes the best use of current or near-term quantum computers by sharing the workload with classical high-performance computing. However, HQC algorithms often require a back-and-forth exchange of data between quantum and classical processors, causing system bottlenecks and leading to high latency in applications. The objective of this study is to investigate novel frameworks that unify quantum and reconfigurable resources for HQC and mitigate system bottleneck and latency issues. The introduction of 6G networks is expected to significantly change the threat landscape related to Distributed Denial of Service (DDoS) attacks, requiring robust detection and mitigation measures. This paper methodologically classifies and evaluates the growing collection of research efforts focusing on detection and protection mechanisms against DDoS attacks in 5G and beyond networks. The survey covers various methods used in the scientific literature and categorizes DDoS mitigation efforts into two main areas: DDoS detection and DDoS defense. We have developed a robust method for efficiently tracking multiple co-occurring mutations in a sequence database. Evolution often hinges on the interaction of several mutations to produce significant phenotypic changes that lead to the proliferation of a variant. However, identifying numerous simultaneous mutations across a vast database of sequences poses a significant computational challenge. Our approach leverages a matrix factorization technique to automatically and efficiently pinpoint subsets of positions where co-mutations occur, appearing in a substantial number of sequences within the database. While the original Waltz filter only handles adjacent line segments meeting in a single point, we show it is relevant to handle the case of relations between disjoint but collinear segments in positions suggesting hidden continuity. Waltz filtering is a seminal work being the first well known example of polynomial time artificial intelligence reasoning for improving the consistency of a set of constraints. Here we propose and prove the usefulness of one such constraint, opening the way for more involved logical solutions to image understanding. Highly Scalable Solid-Core Inhibited-Coupling Fiber-Based Plasmonic Refractive Index Sensor, Mohammad Al Mahfuz, Md Selim Habib

Advancements in plasmonic sensing require simultaneous detection capability that ensures large-scale detection with reduced losses. In this work, we propose a new solid-core fiber-based refractive index (RI) sensor with an ultra-broad detection range. The proposed fiber consists of a relatively simple single-ring cladding with six circular tubes in which the light is guided in the core based on the inhibited-coupling (IC) mechanism. The sensing performance is investigated using extensive finite-element modeling (FEM) through the combination of IC and surface plasmon resonance (SPR) sensing technology. Our results show a low loss of <3 dB/cm across the RI detection range from 1 to 1.60, peaking the wavelength sensitivity (WS) of 3000 nm/RIU and figure of merit (FOM) of 120 RIU−1. Our study also includes the investigation of the fabrication tolerance, fiber bending, and the use of alternative plasmonic materials, providing insights into the practical implementation capability of the proposed sensor. Our findings highlight the potential of the proposed sensor in emerging applications such as detecting air pollutants, biochemical substances, and DNA, paving the way towards bio-sensing within a lab-on-a-chip platform. © 2025 Optica Publishing Group under the terms of the Optica Open Access Publishing Agreement.

The adoption of large language models (LLMs) has facilitated significant advancements in natural language processing. In a short space of time, LLMs have permeated a wide array of disciplines including healthcare, finance, education, etc. However, in their native form, LLMs retain information in their parameters, which sometimes causes the underlying models to produce inaccurate results or hallucinations. Almost a decade ago, the state of Iowa used the Next Generation Science Standards’ (NGSS) performance expectations as the basis for its state science standards for K-12 students. Several key changes were implemented within these standards, including an increase in the amount of Earth and space science content at the high school level and a three-dimensional approach interweaving science and engineering practices and crosscutting concepts along with disciplinary core ideas. In Harrington v. Purdue Pharma, the United States Supreme Court rejected the bankruptcy plan of OxyContin producer Purdue Pharma, which purported to settle claims against both Purdue (the bankruptcy debtor) and Purdue’s individual owners and managers, all members of the Sackler family who had not themselves filed for bankruptcy (the Sackler Release). Concluding that the United States Bankruptcy Code does not authorize settlement of claims against non-debtors without claimant consent, the Harrington Court appeared set to stem the tide of “bankruptcy without distress,” a flourishing practice in which solvent non-debtors (like the Sacklers) would settle their liabilities within the …

This study examined the effects of image-based assignment of agency (IAA) on COVID-19 and flu fear-appeals by employing eye-tracking technology. Volunteers participated in a 2x2x2 between-groups experiment. Fixation-count and fixation-duration were collected by a Tobii Pro X3-120 Screen-Based Eye-Tracker. After reviewing a fact sheet, participants answered a 29-item survey that included items aligning with the extended parallel process model. The flu/agentic-image/virus-agentic-language message produced significantly higher fixation-frequency to the image and a higher perception of severity than the COVID-19/real-image/human-agentic-language message. Fixation-frequency on the image produced significantly higher fear perceptions, but significantly lower message-quality and message-credibility perceptions. Flu messages led to significantly …

Charitable nonprofit organizations have a responsibility not only to the communities they serve but also to the individuals who support them. When nonprofits fail to act as good stewards of raised monies, they may find themselves caught in a scandal that not only holds legal ramifications but financial and ethical implications as well. Stigma surrounding non-native American accents in the United States adversely influences the health-related decisions and behaviors of those who speak with such accents. Grounded in stigma management communication theory (SMCT), this study explored how individuals with non-native American accents communicatively manage accent stigma experiences in healthcare situations in the United States through ten semi-structured interviews. SMCT’s strategies of accepting, avoiding, ignoring, evading responsibility, and denying were identified by participants as the management strategies employed when they face stigmatization. The findings suggest that stereotype threat and patients’ stigma management strategies are important phenomena that can impact the healthcare outcome of patients …

Hybrid warfare is a conflict form that combines traditional military offensive measures with nontraditional methods, including the harnessing of legal systems to achieve strategic goals. Lawyers have always been at the frontline of typical legal risk assessment, thinking about a client's risk of liability or regulation costs generally. My purpose in this essay is to comment on national health policy in the United States over the past sixty years.  My conclusions are less optimistic than what I had hoped when I began my teaching and research career in the 1990s to be able to say in 2025.  In particular, the successful expansion of publicly supported health coverage via the passage of the Affordable Care Act (“ACA” or “Obamacare”) in 2010 seems to have triggered -- though I hesitate to say that it caused – a backsliding from broader commitments to clinical performance, population health, and social solidarity that …

Protective Effects Of Calligonum Comosum As A Natural Remedy To Counteract Pregabalin-Induced Toxicity: Insights From Chemical Profiling, In Vivo, And In Silico Analyses, Smail Mehda, Ibtissam Laib, Feriel Diab, Raounek Attia, Yousef Benaissa, Attia Hanane, Khiari Rayhana, Meriem Bellabidi, Huda Alsaeedi, David Croun, Mikhael Bechelany, Ahmed Barhoum

This study explores the therapeutic potential of Calligonum comosum extract in alleviating pregabalin (PGB)-induced toxicity in male Wistar rats, with a focus on hepatic, renal, and reproductive health. PGB exposure led to significant biochemical disturbances, including elevated liver enzymes (AST, ALT, LDH), impaired kidney markers (urea, creatinine, uric acid), reduced reproductive hormones (testosterone, FSH, LH), and notable histopathological damage in liver, kidney, and testicular tissues. Treatment with C. comosum extract effectively restored liver and kidney functions and partially corrected hormonal imbalances. The extract reduced AST, ALT, and LDH levels by 18.5%, 25.2%, and 13.7%, respectively. Similarly, urea, creatinine, and uric …
