Background: The volume of Electronic Medical Records (EMR) messages from patients to providers continues to rise, yet clinic workflows have not been adequately restructured to accommodate. Increased EMR, especially the inbasket, is associated with provider burnout and decreased job satisfaction, contributing to provider turnover.
Purpose: To improve the triage and distribution of the EMR messaging workload
Method: A Nurse Triage Flowchart was implemented in the Gastroenterology Clinic at the University of Kentucky to help RNs/MAs triage patient messages for improved workflow. Surveys were completed by RNs/MAs and providers before and three months post-implementation. Data from EPIC over six …

Background: Stigma towards substance use has been identified as a contributor to poor care and health outcomes amongst people who use drugs (PWUD), a vulnerable population. Healthcare worker stigma negatively affects the outcomes of individual patients and may have greater public health implications. Methods of addressing drug-use stigma have previously been implemented in a variety of clinical settings. Presenting the lived experience of PWUD to healthcare workers via emotionally compelling narratives has been found to be an effective means of stigma reduction.

Dysbiosis and impaired gut barrier integrity contribute to chronic immune activation associated with both obesity and HIV infection. Given the increased incidence of obesity in people living with HIV, we explored the impact of obesity on the gut microbiome and microbial translocation (MT) biomarkers during HIV infection and antiretroviral therapy (ART). Lean and obese rhesus macaques were infected with simian immunodeficiency virus (SIV) and subsequently treated with ART. Obese animals exhibited higher initial MT and inflammation biomarkers that remained constant throughout the study, while lean animals exhibited significant increases in these biomarkers that approached levels observed in obese animals. In Bourbon County, Kentucky, where rural health challenges and chronic disease burdens persist, a coalition of public health partners set out to improve population-level physical activity through an innovative, community-centered initiative. The Bourbon County Physical Activity Program emerged through collaboration between the Bourbon County Health Department, the University of Kentucky College of Public Health, and UK HealthCare. Together, these partners designed a six-month intervention that combined discounted YMCA memberships, partnerships with employers, a physical activity tracking app with behavioral incentives, and community-wide events to promote accessible and sustained engagement.

European and Canadian regulators enforce strict limits on the ethyl carbamate content of alcoholic beverages. North American distillers’ malt have high glycosidic nitrile levels which amplify the ethyl carbamate level of whiskey. Guidelines are required to produce low ethyl carbamate whiskey from North American distillers’ malts. This work challenges several strategies for reducing ethyl carbamate levels in bourbon and American whiskey products. New make spirit was produced using a bourbon mash bill on a continuous beer still with doubler. This article investigates the concept of tenure security within international law, emphasizing the global legal architectures that influence and shape land tenure governance at the intersections of international human rights law and development. By tracing the evolution of tenure security from colonial practices to modern development paradigms, the article contends that international development and human rights frameworks often perpetuate dispossession and inequality. It critiques the convergence of human rights and development narratives around the formalization of land tenure, demonstrating how this practice reinforces Western legal frameworks and ontologies of land. Bilateral labor agreements (BLAs) aim to facilitate the movement of temporary migrant workers between countries. So far, studies of BLAs have focused on whether they have effects on migration flows. Despite countries entering hundreds of BLAs, evidence for their effects on migration flows remains limited. Yet, even if BLAs have limited material effects, they may still have important symbolic effects. On this topic, this Comment highlights BLAs’ potential to change rhetoric about international migration among heads of state. Introduction: Several factors influence individuals’ confidence to perform diabetes-related self-care activities, including perceived patient-provider communication, diabetes duration and age at diagnosis. It has been well-documented that patient-provider communication is essential when managing chronic diseases such as diabetes; however, the impact of this communication with diabetes duration and age at diabetes diagnosis on confidence in performing self-care behaviors is obscure.

Acute myeloid leukemia (AML) care underwent a paradigm shift in 2017 with the development of a number of targeted therapies and less-intensive treatments. This changed expectations for survival and treatment course, particularly for those who are inappropriate candidates for intensive therapy due to frailty, comorbidities or disease-specific features. Tuberculosis (TB) is a leading cause of infection-related mortality worldwide, causing an estimated 1.13 million deaths in 2022. The majority of infections can be successfully treated with antibiotics, but the prevalence of drug-resistant TB and a lack of rapid and cost-effective drug susceptibility tests for many key antibiotics is a significant barrier to treatment. Whole-genome sequencing of Mycobacterium tuberculosis can be used for drug susceptibility testing and can provide valuable insight into transmission patterns. Over the past decade, the number of IV hydration spas in the United States has grown. These facilities offer IV infusions of hydration solutions and other supplements but are exempted from most routine FDA oversight as compounding pharmacies. Instead, state governments are predominantly responsible for regulation and oversight. Descriptive data was collected on state-level policies governing IV hydration spas and through website review and secret shopper calls performed for facilities in each state and DC. Staff within the microsystem strive to provide quality care to patients experiencing acute mental health crises in hopes of stabilizing their condition, allowing them to function safely and appropriately among others. This will hopefully lead to eventual rehabilitation back into society. The main technique used in this microsystem for the rehabilitation of patients is providing group therapy (GT) sessions. In the United States, there are approximately 7.2 million people living with Alzheimer’s dementia and these numbers are expected to rise. This work presents three experimental investigations analyzing the microstructural evolution, void nucleation and growth, and failure in titanium and magnesium. The first study compares the void nucleation and growth of Grade 2 and Grade 4 commercially pure titanium during monotonic tensile testing and cyclic-bending-under-tension. Voids were characterized by X-ray computed tomography (XCT). It was shown that CBT (cyclic bending under tension) did not suppress the growth in void volume but allowed for further plastic deformation despite it. During the 16th century, prisons as we know them did not exist in Spain nor in most of Western Europe. Jails, as a temporary place to hold criminals before their trial, did exist but were quickly filling up with a wave of new criminals and debtors. This system of jails was poorly organized and poorly understood until Tomás Cerdán de Tallada wrote his treatise Visita de la carcel, which was the first in Spain to analyze the system of jails as a whole, and one of the first to offer criticisms of the system. In the early 1980s Diadema antillarum, or the long-spined urchin, was hit by a catastrophic mass mortality event across the western Atlantic and Gulf of Mexico, inclusive of the Florida Keys. The cause of this die-off event was likely triggered by a pathogen or environmental stressor that decimated native populations by up to 98%. The species has not yet fully recovered and is struggling with recruitment success which is why it may benefit from an ex-situ recovery program. This paper explores the intersection of digital inequity and biomedical healthcare access in the United States, positioning the digital divide as a structural determinant of health. As telehealth and digital platforms become central to care delivery, communities lacking broadband infrastructure, digital literacy, and affordable internet access face compounded barriers to equitable healthcare. The study introduces five critical dimensions—access, availability, adequacy, acceptability, and affordability—as a framework for addressing these disparities. Background: Moral distress, burnout, and compassion fatigue in acute care nurses has increased significantly since the Covid-19 pandemic and has been associated with high levels of nursing turnover and increased costs to healthcare systems. Nurse leaders at the study organization had never received education on recognizing and responding to moral distress. Nursing engagement scores were low at the study organization in the areas of leadership support and well-being investment in nurses.

Background: Celiac disease (CD) is projected to be the most common genetic autoimmune disease in the world with 1% or 3 million people effected in the United States. It is estimated that as many as 70% of those effected are not aware. Diagnosis routinely takes 10-12 years from the initial onset of symptoms, exposing patients and the healthcare system to significant cost and stress through excess testing and misdiagnosis. Background: A critical incident is a sudden, unexpected, emotionally distressing event commonly experienced by oncology nurses. Debriefing is an evidence-based best practice that facilitates communication, reflection, and learning to identify quality improvement opportunities to enhance clinical practice, patient safety, and quality of care. However, debriefing within oncology care settings is inconsistent despite the demonstrated benefit and availability of frameworks. Infusion-related reaction management follows an algorithm that directs care delivery, aligning with a care bundle approach. Background: Peer review has been in use in healthcare for many years, primary by physicians. Recently, however, nursing has adopted a framework for peer review within the profession. According to the American Nurses Association (admin, 2011), organizations are required to maintain a systematic peer review process aimed at evaluating nursing care and nursing care providers to obtain/maintain Magnet designation. Background: Anaphylaxis is a life-threatening allergic reaction that requires early recognition and administration of intramuscular epinephrine. In the elementary school setting, non-licensed personnel are often the first staff to identify this, making training and preparedness essential. BACKGROUND: The American Cancer Society estimates 2,041.90 new cases of cancer in the U.S. (1 in 18 Americans) with a global projection of thirty-five million new cases annually by 2050. With this alarming increase, early recognition of oncological emergencies in the Emergency Departments is essential. Life-threatening conditions require rapid assessment, accurate diagnosis, and immediate interventions. Qualified emergency nurses must understand the effects that cancer and treatment have on the immune, metabolic, and hematologic systems. This quality improvement (QI) project focuses on assessing emergency nurses’ oncological knowledge of acute emergencies and providing a resource for rapid identification.

This Improvement Science in Practice dissertation employs a mixed-methods, critical participatory action research approach to examine strategies educators can use to disrupt anti-Black practices contributing to racial discipline disparities affecting Black students. The Iceberg Model (Monat & Gann, 2015), a framework for understanding the systemic structures, patterns, and mental models perpetuating anti-Blackness in educational spaces guided the analysis. This study focused on evaluating the performance of concrete bridge barriers reinforced with Glass Fiber Reinforced Polymer (GFRP) compared to those reinforced with conventional mild steel. Four full-scale barriers were constructed, three using GFRP bars as the reinforcement and one with mild steel reinforcement. These were cast on two concrete slabs to ensure consistent support conditions. The use of GFRP aims to address durability concerns associated with steel reinforced concrete (RC), particularly corrosion of these bars in harsh environments.
