Objectives: Using a multicenter child abuse pediatrics research network (CAPNET), the study objectives were to characterize the adherence to American Academy of Pediatrics (AAP) recommendations for bleeding disorder testing and the frequency of bleeding disorder identification.
Methods: We performed a descriptive study of bleeding disorder evaluations among children younger than 10 years of age who presented with bruising and/or intracranial hemorrhage (ICH) concerning for abuse from February 2021-May 2022 utilizing CAPNET. Cases were classified as bruising without ICH or ICH with or without bruising. Based on AAP guidance, testing was not recommended in cases with concomitant suspicious injuries, inflicted injury …

Nearly a decade after the National Academy of Medicine released the "Improving Diagnosis in Health Care" report, diagnostic errors remain common, often leading to physical, psychological, emotional, and financial harm. Despite a robust body of research on potential solutions and next steps, the translation of these efforts to patient care has been limited. Improvement initiatives are still narrowly focused on selective themes such as diagnostic stewardship, preventing overdiagnosis, and enhancing clinical reasoning without comprehensively addressing vulnerable systems and processes surrounding diagnosis. BACKGROUND: To investigate the pathologic differences in patients with pediatric-onset multiple sclerosis (POMS) and acute disseminated encephalomyelitis (ADEM) using diffusion tensor imaging (DTI) and diffusion basis spectrum imaging (DBSI).
METHODS: Fifteen children with POMS and eight children with ADEM underwent DTI and DBSI. The comparison of DTI and DBSI diffusivity measures of POMS (31 scans) and ADEM (17 scans) was performed as group comparison and association over time.
RESULTS: In univariate analysis of average measures of DBSI and DTI over time, DBSI fractional anisotropy is lower in POMS than ADEM (P = 0.002), indicative of axonal injury of POMS. Higher …

PURPOSE: Volar locking plate (VLP) of distal radius fractures (DRF) carries a number of surgical risks including flexor pollicis longus (FPL) rupture secondary to irritation from prominent hardware. This study reviews FPL reconstruction (with interposition graft or tendon transfer) after VLP-related rupture.
METHODS: This retrospective case series reports the experience at one tertiary center with FPL reconstruction after DRF. Patients treated between January 1, 2013, and June 12, 2023, were eligible for inclusion. Preoperative radiographs were analyzed for Soong grading and fracture reduction parameters. Accessible patients (n = 7) were examined to evaluate long-term success of reconstruction.

OBJECTIVES: The prognostic characteristics of lung point-of-care ultrasound (L-POCUS) to predict respiratory decompensation in patients with emerging infections remains unstudied. Our objective was to examine whether scored lung ultrasounds predict hypoxia among a nonhypoxic, ambulatory population of patients with COVID-19.
METHODS: This was a diagnostic case-control study. Three academic emergency departments across the United States collected a convenience sample of nonhypoxic subjects with COVID-19, scored subjects' hemithorax at 7 locations using lung ultrasound, and followed outcomes for 40 days. The AAA protease FtsH associates with HflK/C subunits to form a megadalton-size complex that spans the inner membrane and extends into the periplasm of E. coli. How this bacterial complex and homologous assemblies in eukaryotic organelles recruit, extract, and degrade membrane-embedded substrates is unclear. Following the overproduction of protein components, recent cryo-EM structures showed symmetric HflK/C cages surrounding FtsH in a manner proposed to inhibit the degradation of membrane-embedded substrates. Here, we present structures of native protein complexes, in which HflK/C instead forms an asymmetric nautilus-shaped assembly with an entryway for membrane-embedded substrates to reach and be engaged by FtsH. …

BACKGROUND: Expanding human papillomavirus (HPV) vaccination for girls and HPV self-collection for women can reduce the global burden of cervical cancer. However, HPV vaccination and self-collection services are rarely implemented simultaneously in mother-daughter dyads, leaving a critical gap in cervical cancer prevention. From 2023 to 2024, a community-engaged model for combined HPV vaccination and screening was co-designed using crowdsourcing open calls and designathons with mother-daughter teams and pilot-tested by trained research facilitators. The ability to detect and quantify microbiota over time from shotgun metagenomic data has a plethora of clinical, basic science and public health applications. Given these applications, and the observation that pathogens and other taxa of interest can reside at low relative abundance, there is a critical need for algorithms that accurately profile low-abundance microbial taxa with strain-level resolution. Here we present ChronoStrain: a sequence quality- and time-aware Bayesian model for profiling strains in longitudinal samples. Objectives: Evaluate structural validity, hypothesis testing and measurement invariance of the Victorian Institute of Sport Assessment–Achilles (VISA-A) using Rasch analysis to determine if it can be recommended to measure Achilles tendinopathy severity. Methods: We performed an evaluation of the VISA-A measurement properties using Rasch Measurement Theory by pooling individual participant data from previously conducted studies (n = 217). Structural validity was assessed via threshold/category mean order; adequate item fit; unidimensionality; local dependence; internal consistency. Background: The homebirth setting provides opportunities for midwifery students to witness autonomous midwifery practice, but these experiences are limited. Factors that limit practicum placement in the home birth setting are either logistical, with less than one percent of women having a homebirth in Australia or related to university governance, whereby approval is not granted for midwifery students attending homebirth. Background/purpose Excess weight has been identified as a potential risk factor for post-COVID-19 condition (PCC). This systematic review and meta-analysis aimed to investigate whether excess weight is associated with the development or experience of neurological and neuropsychiatric symptoms in PCC. Methods We conducted a comprehensive search of eight databases (PubMed, Embase, Scopus, Web of Science, VHL, Google Scholar, ProQuest, and medRxiv) for studies published up to July 2023. Aim: The role of ecological specialisation in shaping biogeographic and evolutionary patterns remains unresolved. To date, few studies have quantitatively examined consumer niche breadth at a global scale. We describe global biogeographic and diversification patterns of specialisation, measured using trophic and thermal niches, for a highly diverse assemblage of consumers: reef fishes. First, we investigated the confluence of specialisation with global biogeographic patterns in species richness and reef area. BACKGROUND: Low quality of life (QoL) in university students is often associated with unhealthy lifestyle and negative impacts on students' ability to learn and succeed. To date, no research has examined the QoL of medical students in Jordan or the demographic factors that may associate with lower QoL. Early detection of online radical content is important for intelligence services to combat radicalisation and terrorism. The motivation for this research was the lack of language tools in the detection of radicalisation in the Maldivian language, Dhivehi. This research applied Machine Learning and Natural Language Processing (NLP) to detect online radicalisation content in Dhivehi, with the incorporation of domain-specific knowledge. Product recalls, particularly when perceived as opportunistic, can trigger severe consumer backlash, undermining brand loyalty and escalating protest behaviors that threaten long-term consumer-brand relationships. This study integrates expectancy violation theory and situational crisis communication theory to understand the implications of opportunistic recalls for brand loyalty and repurchase intention. A survey with 425 responses from car owners in the United States was analyzed using partial least squares structural equation modelling (PLS-SEM). It found that opportunistic recalls significantly exacerbate negative brand personality and directly influence protest behavior and brand loyalty. Myopia is the most common eye disease and a leading cause of blindness. A contemporary, large, and ethnically and geographically diverse study of ocular biometry and refractive error in children in the United States is needed to help better describe differences between myopic and nonmyopic eyes.
This is an initial cross-sectional report from the Refractive Error and Axial Length Growth Curve Study that includes ocular component data as a function of age, gender, and geographic location. CpG island hypermethylation, a hallmark of cancer, exhibits substantial heterogeneity across tumors, presenting both opportunities and challenges for cancer diagnostics and therapeutics. While this heterogeneity offers potential for patient stratification to predict clinical outcomes and personalize treatments, it complicates the development of robust biomarkers for early detection. Understanding the mechanisms driving this heterogeneity is essential for advancing biomarker design. Here, simulation-based analyses demonstrate that tumor purity and the high prevalence of low epi-mutation samples significantly obscure the identification of negative, rather than positive, regulators of CpG island hypermethylation, limiting a comprehensive understanding of heterogeneity sources. Vascular rings represent a heterogeneous set of aberrant great vessel anatomic configurations which can cause respiratory symptoms or dysphagia due to tracheal or oesophageal compression. These symptoms can be subtle and may present at varied ages. More recently, many have been identified in patients without symptoms, including fetal echocardiogram, resulting in a conundrum for practitioners when attempting to determine who will benefit from surgical correction. Here, we provide a review of vascular rings and a guide to the practitioner on when to consider additional imaging or referral. Additionally, we discuss the changing landscape regarding asymptomatic patients and fetal echocardiogram.

OBJECTIVE: To provide a comprehensive overview of the American Urological Association Medical Student Surveys and includes data 2022, 2023, and 2024.
METHODS: Data collected through surveys distributed prior to match day and were distributed to all medical students who submitted a rank list for urology residency. Questions consisted of various topics, including applicant demographics, application metrics, and experiences with preference signals and virtual interviews.
RESULTS: 440 (79%), 428 (84%), and 387 (77%) applicants participated in surveys for the years 2022, 2023, and 2024, respectively. Applicants continue to participate in research at a high level. Additionally, United States Medical Licensing Exam …

The demand for sustainable food and energy production continues to rapidly rise. Utilizing aquaculture wastewater as microalgae growth medium is a potential opportunity for nutrient recycling and industrial symbiosis, but more research is required to understand how best to design and scale-up a dual-purpose facility. This study investigates the growth of Coelastrella sp. in aquaculture wastewater compared to industry-standard growth medium BG-11 and modified tap water as controls. Though cell size and pigment density varied throughout media, aquaculture wastewater performed very comparably to BG-11 as a microalgae culture broth.

Spinal cord injury (SCI) disrupts white matter, causing periaxonal swelling, axonal degeneration, and impaired function. To examine vascular edema’s role, we developed a 3Dprinted dual-compartment imaging chamber to mimic and manipulate interstitial and vascular fluid dynamics in real time. We hypothesized that hypertonic saline (HTS) in the "vascular" chamber would osmotically shift fluid from the periaxonal space and preserve myelinated fibers after SCI. Using two-photon excitation microscopy, we imaged myelin and axons ex vivo in Thy1YFP+ transgenic mice following C5 contusive SCI and assessed three clinically relevant HTS concentrations versus normal saline for their effectiveness in mitigating periaxonal swelling …

A group ring, R[G], is the set of all elements of G together with a coefficient from R. Elements of group rings are classified similarly to rings. For example, an element, x, is classified as a unit if there exists another element, y, such that xy produces the multiplicative identity. In the integers, 1 is the trivial unit. Due to the variably large sizes of group rings, using excel sheets often allows for easier computation of group elements and multiplication.

Substance use disorders (SUDs) pose a significant public health concern. Preliminary research suggests benefits of physical activity (PA) for individuals receiving SUD treatment. However, current PA interventions have limitations, particularly regarding PA maintenance, due to motivational issues among people with SUDs. Our co-designed intervention is the first to target PA motivation for sustaining PA in individuals with SUDs. We employed the Behaviour Change Wheel (BCW) to co-design an intervention to increase PA motivation and support PA maintenance among people in residential SUD treatment.
