Introduction: Shewanella algae is a gram-negative, facultatively anaerobic bacterium most commonly found in marine environments. It is an emerging human pathogen, most often associated with hepatobiliary and skin and soft tissue infections (SSTI) following water exposure. Known risk factors for infection include those participating in recreational water activities, flood victims, and those with underlying liver disease, immunosuppression, or IV drug use. This case involves a 70-year-old male who was involved in a motor vehicle accident in 2008 leading to multiple orthopedic surgeries complicated by recurrent prosthesis infections who presented with concern for sepsis. This case report of a veteran with a history of multiple traumatic brain injuries, posttraumatic stress, and chronic pain highlights the MossRehab Institute for Brain Health (MRIBH) model of care: a collaborative, interdisciplinary, intensive outpatient treatment program. Unique to this case was the severity of symptomatology and the concerted engagement by the clinicians, who fully tailored and adjusted their approaches based on team rounds and daily exchanges. The client's main concerns of headaches, difficulty sleeping, difficulty with memory and cognition, depression, anxiety, posttraumatic stress, anger, and chronic musculoskeletal pain were addressed and significantly improved within three weeks. Medicaid fraud and abuse in health care billing is committed by licensed practitioners, patients, or criminal organizations and can pose a serious business problem. Health care managers are particularly concerned because fraudulent claims can result in lost revenue, legal consequences, and diminished resources for legitimate patient care. Grounded in the Complex Adaptive Systems Model, the purpose of this qualitative pragmatic inquiry was to explore the strategies some health care managers use to reduce fraud and abuse in health care billing. This Doctor of Nursing Practice education project focused on improving staff knowledge of medication reconciliation through an evidence-based educational intervention in an outpatient clinic. Medication reconciliation is crucial for ensuring accurate and consistent medication lists across care transitions, helping to prevent errors, such as omissions, duplications, dosing mistakes, and drug interactions. The project’s central question was whether staff education on an evidence-based approach to medication reconciliation would improve staff knowledge. A pre- and post-education survey was used to measure changes in staff knowledge after a PowerPoint presentation. Between 2001 and 2018, China’s outward foreign direct investment increased by 66.3 times, with a particular focus on resource-rich countries in Africa. While the economic impact of these investments has been widely studied, their potential impacts on democratic freedoms have been neglected. This thesis investigates how Chinese foreign direct investment affects African countries’ free speech, free press, and internet freedom. Based on case studies of Zambia, Kenya, and Ghana, this thesis finds a causal relationship between increased foreign direct investment from China and decreased freedom of expression and internet freedom in host countries. A Single Center Qualitative Study To Improve Coordination Of Follow-Up Care For Patients Diagnosed With Abusive Head Trauma, Ruairi Smith-Dewey, Elise Wright, Daniella Donofrio, Hannah Goebel, Sara Kilbride

Introduction: Children diagnosed with abusive head trauma (AHT) have subsequent long-term developmental, behavioral, and medical consequences resulting in increased medical system utilization. There were 16 overall potential needs identified, with a single patient having 12. In 2023 at Children’s Mercy Hospital (CMH), 47% (8/17) of patients with AHT had < 50% of follow-up needs addressed at time of discharge. None had 100% of follow-up needs addressed. Needs not being addressed included need not identified or written documentation for need of follow-up without subsequent referral placed/appointment scheduled. Project goal was all follow-up needs to be addressed for at least 50% of children with AHT by January 2025. Methods (include PDSA cycles): Children admitted to CMH from January 2024-December 2024 and diagnosed with AHT by the Safety, Care & Nurturing (SCAN) team were included. Interventions were designed and implemented by a multidisciplinary team to address barriers including variation in scheduling process per specialty, lack of knowledge, and changing caregivers. • PDSA 1 (1/1/24): SCAN added standardized list of potential follow-up needs to documentation. • PDSA 2 (2/1/24): Trauma NP placed ophthalmology outpatient referrals. • PDSA 3 (3/1/24) Social work placed and documented early intervention referrals. Transitioned to SCAN RN responsibility. • PDSA 4 (4/10/24): AHT caregiver handouts with description of all potential follow-up needs added to discharge instructions. • PDSA 5 (12/11/24): AHT Evidence Based Practice guideline created including follow-up needs. Appointments and referrals were tracked by the author. QI tools used included fault tree, fishbone, and PICK Chart. Results: In 2024, 27 children were diagnosed with AHT by SCAN team at CMH. Five children were excluded from the study as 3 died and 2 were transferred to another hospital. The number of patients with < 50% of follow-up needs addressed was reduced from 47% to 43%. All follow-up needs were addressed in 13% (3/23) of patients. Conclusions: While project aims weren’t met, there was progress. Next steps include educating all discharging teams at CMH as there is no single team discharging these patients. Overall, there are few of these patients so standardization of discharge practices is challenging. SCAN created a working group and point person for appointment tracking to ensure follow-up needs established at discharge.

This quality improvement project aimed to implement Lean strategies to optimize the intake process in a psychiatric emergency department, thereby reducing patient wait times, minimizing left-without-being-seen (LWBS) occurrences, and improving access to timely psychiatric care.

The project was conducted in a hospital-based psychiatric emergency department in Southeast Texas, serving adults and children six years and older.

Typical Electron-Withdrawing Groups Are Ortho, Meta-Directors Rather Than Meta-Directors In Electrophilic Aromatic Substitution, Paul R. Rablen

Electron-withdrawing groups are traditionally considered meta-directing in aromatic substitution reactions. However, when the pre-existing substituent is a π-acceptor, both experiment and calculation indicate that substantial amounts of ortho as well as meta substitution occur, with very little para reactivity. A simple perturbative MO argument rationalizes this finding. It is therefore suggested that these substituents are best understood as ortho, meta-directors, with a preference for meta, just as electron-donating groups are considered ortho, para-directors, with a preference for para.

Transportation’S Influences On Wellbeing: A Literature Review And Scoping Framework, William Bouck, Patrick Singleton, Louis Alloro, Kim Clark

This study investigates the relationship between transportation and wellbeing: the conditions humans need to flourish at all levels. Using the results gleaned from our literature review of 44 articles, we found correlations linkages between transportation elements and components of physical, psychological, economic, and social wellbeing. These transportation factors affected overall wellbeing, not merely wellbeing during travel. We find that active travel, followed by public transit, are the best travel modes for wellbeing, and that congestion, pollution, and transport disadvantage harm wellbeing. Our findings also indicate that multimodal, people-focused infrastructure corresponds to higher wellbeing. Generally, the four areas of wellbeing we …

BACKGROUND: Ovarian high-grade serous carcinomas (HGSC) comprise four distinct molecular subtypes based on mRNA expression patterns, with differential survival. Understanding risk factor associations is important to elucidate the etiology of HGSC. We investigated associations between different epidemiologic risk factors and HGSC molecular subtypes.

METHODS: We pooled data from 11 case-control studies with epidemiologic and tumor gene expression data from custom NanoString CodeSets developed through a collaboration within the Ovarian Tumor Tissue Analysis consortium. The PrOTYPE-validated NanoString-based 55-gene classifier was used to assign HGSC gene expression subtypes. We examined associations between epidemiologic factors and HGSC subtypes in 2,070 cases and 16,633 …

Case Description: A 21-year-old female with a history of asthma presented to the emergency department (ED) with restlessness, anxiety, and loss of appetite shortly after returning from a trip to France, where she and her boyfriend ended their relationship. Her initial evaluation, including unremarkable laboratory results, led to the attribution of her symptoms to anxiety from life stressors. She was treated with supportive measures in the ED and prescribed Xanax by her primary care physician at follow-up.
