Objective: This study investigates the barriers to and facilitators of sustaining a decision aid (DA) tool for patients with SLE in routine rheumatology outpatient care across the USA. The DA was initially developed for assisting patients with SLE in making informed medication choices by providing personalized information on their treatment process.

Methods: We conducted semi-structured interviews with clinicians, administrators, and information technology (IT) professionals from 15 geographically diverse rheumatology clinics that participated in a large-scale DA implementation trial. Interviews were transcribed and analysed using NVivo, with thematic coding to identify key factors influencing DA sustainability.

Direct pacing of the mid myocardium where re-entry originates can be used to prevent ventricular arrhythmias and circumvent the need for painful defibrillation or cardiac ablation. However, there are no pacing electrodes small enough to navigate the coronary veins that cross these culprit scar regions. To address this need, we have developed an injectable ionically conductive hydrogel electrode that can fill the epicardial coronary veins and transform them into flexible electrodes. A new hydrogel chemistry based on a polyether urethane diacrylamide macromer was developed that matches myocardial stiffness and is resistant to hydrolysis. Background

Multiple myeloma (MM) is virtually always preceded by monoclonal gammopathy of undetermined significance (MGUS). Elevated serum markers are used to classify MGUS patients into clinical risk categories. Previous research has indicated that the absence of a normal plasma cell signature in MGUS is linked to early progression.

Objective

To confirm that the presence of a “polyclonal plasma cell (PolyPC) signature” serves as a robust negative predictor of MGUS progression.

Methods

Purpose: AMH inhibits hormone production in luteinized granulosa cells (GCs) and stalls ovarian follicle development in vitro and in vivo. We sought to confirm AMH's mechanism of action through SMAD activation and investigate AMH inhibition of follicle development and function, in vitro and in vivo.

Methods: A primary culture of GCs isolated from follicular fluid was used, and cells were treated with recombinant AMH (rAMH) or placebo for 24 h. Comparing Self-Construal Scales: Cultural Implications For The United States, East Asia, Southeast Asia, Latin America, And The Middle East, Jon Lim, Christine Ma-Kellams

Contemporary self-construal research indicates that people define themselves either as independent or interdependent. We review self-construal scales across the United States, East Asia, Southeast Asia, Latin America, and the Middle East to compare/contrast their operationalizations of interdependence (or independence). Taken together, this body of research suggests that, consistent with findings on American and East Asian selves, Southeast Asian cultures also exhibit interdependent self-construal. Latin Americans, however, display independent self-construal despite their long-standing characterization as a collectivist culture, while Arabs display both independence and interdependence. As urbanization reduces species’ habitats and population sizes, managers need information on whether within-population processes, such as changes in mate and nest-site fidelity and dispersal distances, may be contributing to declines. Few avian studies have examined changes in these behaviors in declining populations or in urban settings. We investigated whether mate fidelity, nest-site fidelity or breeding dispersal distance changed over time in a population of burrowing owls (Athene cunicularia), a short-lived, socially-monogamous species. During the 18-year period of the study, the population declined by 69% in urban Santa Clara County, California, USA-a region of rapid urbanization. We assessed whether these …

Periodontal diseases, a group of complex conditions marked by an excessive immune response and periodontal tissue destruction, are a global health concern. Since 1990, the incidence of these diseases has doubled, with Western sub-Saharan Africa experiencing the highest burden. Accurate diagnosis and case identification are crucial for understanding the etiology, features of disease, research, treatment and prevention. Modern perspectives on periodontal disease classification are based on commonality among those affected. Recently, there has been a growing interest in automatically collecting distributed solar photovoltaic (PV) installation information in smart grid systems, including the quantity and locations of solar PV deployments, as well as their profiling information across a given geospatial region. Students come to medical school with diverse backgrounds and experiences. Accordingly, many schools have developed pre-matriculation programs to support students' transition into medical education, especially for those whose educational path may not have offered the same breath of opportunities as their incoming counterparts. Baylor College of Medicine established a pre-matriculation program for students from early assurance programs and non-traditional pathways to address gaps in student support and normalize help-seeking behavior. This program took place over 2 weeks and was developed with leadership and student partners. Participants included 21 students who completed baseline and post-program surveys. Background: Existing studies on osteoradionecrosis of the jaw (ORNJ) have primarily used cross-sectional data, assessing risk factors at a single time point. Determining the time-to-event profile of ORNJ has important implications to monitor oral health in head and neck cancer (HNC) long-term survivors.

Methods: Data were retrospectively obtained for a clinical observational cohort of 1129 patients (198 ORNJ cases) with HNC treated with radiotherapy (RT) at The University of Texas MD Anderson Cancer Center. Background: World Health Organization Grade 2 meningiomas (G2Ms) often recur and resist therapies. Grade 2 meningiomas with histopathological necrosis have been associated with worse local control (LC) after radiation therapy, but the drivers and biomarkers of radiation resistance in G2Ms remain unknown.

Methods: We performed genetic sequencing and histopathological analysis of 113 G2Ms and investigated the role of genetic and microenvironmental factors on clonogenic survival after ionizing radiation. We performed transcriptional profiling of our in vitro model and 18 human G2M tumors by bulk RNA sequencing as well as 8 G2Ms by single nuclei RNA sequencing.

Obsessive-compulsive disorder (OCD) affects ~1% of children and adults and is partly caused by genetic factors. We conducted a genome-wide association study (GWAS) meta-analysis combining 53,660 OCD cases and 2,044,417 controls and identified 30 independent genome-wide significant loci. Gene-based approaches identified 249 potential effector genes for OCD, with 25 of these classified as the most likely causal candidates, including WDR6, DALRD3 and CTNND1 and multiple genes in the major histocompatibility complex (MHC) region. The global population is ageing and the gap between lifespan (total years lived) and healthspan (years lived free of diseases) is increasing. Healthy Longevity Medicine (HLM) is an approach to optimise health and healthspan, and it has substantial public health implications. Despite those implications, the understanding of public perspectives on this field is lacking. The HEalthy LOngevity (HELO) framework was developed through a literature review guided by expert discussions across disciplines to include evidence-based concepts of health-related decision-making, ageing, and HLM. Public electric vehicle charging stations (EVCSs) are vital for boosting EV adoption. This study investigates Seoul’s public EV charging patterns, taking into account the surrounding urban built environment. We collected built-environment data from land-use maps, Point of Interest (POI) data, and panorama images near public EVCS. The computer-vision technique was used to extract scene features from panorama images. We conducted a spatiotemporal analysis of public EVCS usage. The built-environment factors underwent dimensionality reduction and were assessed for outliers. Descriptive analysis revealed afternoon peak charging times and variations between chargers. This paper explores how ecological responsibility becomes distributed across the surfaces and volumes of the city. Our focus is urban mosquito management – specifically, the management of the Aedes aegypti mosquito, the vector by which dengue and several other diseases are transmitted to humans – and the governance strategies deployed in Singapore to manage breeding sites and mosquito outbreaks. This article considers the ways in which the material infrastructures of China’s Belt and Road Initiative (BRI) intersect with other infrastructural formations, and how the resulting overlaps can trigger processes of what I call “infrastructural splintering”. These processes cause infrastructure to be experienced in differentiating ways, creating divisive politics where there might once have been unity. Embracing these politics as an analytical starting point undermines the techno-material stability of the BRI, and reveals its more-than-material affects. This essay examines how Washington’s deep preoccupation with the British-Malayan counterinsurgency tactics–tactics that routed the guerilla fighters of the Malayan Communist Party–shaped US commitment to Vietnam. This US preoccupation, stoked by Britain’s (and Malaya’s) promotion of their counterinsurgency knowledge to US leaders, also influenced the lessons that US analysts and thinkers absorbed from the United States’ defeat in Indochina. Most studies of British-Malayan counterinsurgency and its connection to the Vietnam War fixate upon debating the relevance of the former to the latter and how this bears on US failures in Indochina. Hemophilia-A (HA) is the X-linked bleeding disorder caused by heterogeneous factor (F)VIII gene (F8)-mutations and deficiencies in plasma-FVIII-activity that prevent intrinsic-pathway mediated coagulation-amplification. Severe-HA patients (HAPs) require life-long infusions of therapeutic-FVIII-proteins (tFVIIIs) but ~30% develop neutralizing-tFVIII-antibodies called “FVIII-inhibitors (FEIs)”. Bloom's cognitive learning taxonomy is widely used in medical education. The revised taxonomy published by Kathwohl and colleagues describes the levels in action forms to support learner thinking: Remember, Understand, Apply, Analyse, Evaluate and Create. Genome-wide association has identified regions of the genome that mediate risk for psychosis. It is possible that variants in these regions confer risk by altering gene expression. This work has predominantly been conducted in individuals of European descent and has focused narrowly on schizophrenia rather than psychosis as a syndrome. As far as researchers know, there's no way to prevent BBTs from forming. Although these tumors generally do not metastasize, they can still be dangerous as they may grow and compress healthy tissues and lead to poor outcomes. The principal objective was to evaluate the demographic and clinical characteristics linked to BBTs in individuals from the Rio Grande Valley. Background

Obesity paradox in post-percutaneous coronary intervention (PCI) outcomes among acute myocardial infarction (AMI) patients is a known controversy. However, these studies included patients who had diabetes, hypertension, or hyperlipidemia. We studied relationship between metabolically healthy obesity (MHO—without diabetes, hypertension, or hyperlipidemia) and in-hospital post-PCI outcomes among AMI patients.

Methods

We extracted data from National Inpatient Sample 2020 using International Classification of Diseases, Tenth Revision, Clinical Modification (ICD-10-CM) codes for admissions of AMI patients who underwent PCI. Background

Deep brain stimulation of the dentate nucleus (DN-DBS) is an emerging therapy to improve upper extremity (UE) motor function after stroke. This study sought to investigate the physiologic mechanisms of acuteDN-DBS in chronic stroke survivors enrolled in a phase I trial for DN-DBS.

Methods

Twelve chronic stroke participants with moderate-to-severe UE impairment received (acute) single sessions (≥45 min) of active DBS and sham DBS in a sham-controlled, double-blind, cross-over experiment (order randomized). The primary question of interest involves the relationship between a set of factors and a set of response variables under studies. This dissertation explores the phenomenon of adult women who self-diagnosed Attention Deficit Hyperactivity Disorder (ADHD) through content encountered on social media platforms. Drawing on interpretive phenomenological analysis of ten in-depth interviews, the study investigates the emotional, cognitive, and institutional dimensions of this self-diagnosis process. Participants, all over the age of 30, reported that social media played a transformative role in helping them recognize ADHD traits in themselves. Rather than being misinformed or seeking shortcuts, participants engaged thoughtfully with ADHD-related content, valuing lived experience itself as a meaningful source of recognition and reflection.
