Hypernatremia is defined as a serum sodium greater than 145 mEq/L and is due to either a deficit of free water or excess sodium intake. Extreme hypernatremia (sodium level > 190 mEq/L) is immediately life–threatening with mortality rates > 60%. It is especially challenging to avoid complications, such as permanent neurological injury, respiratory failure, and profound electrolyte derangement, when children are in kidney failure. Established guidelines are lacking for correcting such extreme levels due to a paucity of information in the literature, although general recommendations are to decrease levels by no more than 10-12 mEq/L per day to prevent neurological complications. Recombinant Erythropoietin Use For Anemia Of Prematurity: A Three Year Since Center Experience, Hannah Darland, Krishna Dummula, Chaitali Mahajan, Vishal Pandey

Background: Anemia is a common disorder of prematurity secondary to limited blood volume, excessive blood draws, inadequate nutrition, poor iron reserves, and insufficient endogenous erythropoietin. While many NICUs use recombinant erythropoietin (rEpo) to treat anemia of prematurity, the absence of universal guidelines has led to variability in dosing and schedules. Here we present our experience on the use of rEpo in a level 3 NICU. Our unit protocol uses 400 IU/kg three times a week for a total of 9 doses if hematocrit < 30%. Objectives: The purpose of this study was to evaluate the response to rEpo in infants with anemia of prematurity. Methods: Charts of infants born between 2018 and 2020 with gestation ≤34 weeks who received rEpo for anemia of prematurity were retrospectively reviewed. We defined response to therapy as an increase of hematocrit by 5% from pre-treatment baseline. Infants who did not achieve the therapeutic goal or who received blood transfusions were considered non-responders. Neutropenia, thrombocytopenia, and retinopathy of prematurity (ROP) requiring treatment were treated as adverse events. Results: A total of 44 infants were treated with rEpo during the study period. Clinical characteristics are described in Table I. Average hematocrit at birth was 44.8±7.5 (32.7 to 60.8). Infants were started on rEPO at a median corrected age of 33.3 ±3 weeks and a weight of 1674±622 g. Average DOL was 37±18 at initiation of therapy. Average baseline hematocrit at the start of therapy was 27.9±1.8%. 80% of patients (35/44) responded with increase of hematocrit by >5%, 20% (9/44) did not respond. 75% of infants (33/44) received all 9 doses. 11 patients received …

Introduction: Neonatal onset of cytopenia autoinflammation rash and hemophagocytic lymphohistiocytosis syndrome (NOCARH), a subset of hemophagocytic lymphohistiocytosis (HLH), is a rare and life-threatening condition characterized by uncontrolled immune response. It presents a diagnostic challenge in neonates due to the overlap of clinical features with other conditions, particularly congenital infections. We report a case of NOCARH in a late pre-term neonate, highlighting an atypical age group and the diagnostic difficulty of this syndrome. Purpose: Appalachian Kentucky (KY) residents experience the highest lung cancer rates in the US with declines lagging among women; we sought to uncover barriers and facilitators to lung cancer prevention for Appalachian KY women and to identify community-specific interventions. Approach: We utilized concept mapping, a participatory mixed method, to generate consensus on perceived barriers and facilitators. Setting/Participants: We recruited 71 adult women from Appalachian KY counties. Method: After collecting online concept mapping data, we used multidimensional scaling to generate a point map of perceived similarities and hierarchical cluster analysis to create a thematic cluster map. Objective: By leveraging real-world electronic health record (EHR) data, this study set out to estimate individualized treatment effects (ITE) in longitudinal observational settings to advance personalized medicine, addressing key challenges that are often observed in real-world clinical scenarios and pose statistical challenges, including hidden confounding and dynamic treatment regimens.

Methods: We propose the Variational Temporal Deconfounder Network (VTDNet), a novel framework designed to account for time-varying hidden confounding using a variational recurrent transformer-based autoencoder. Objective: Efficacious modalities are limited in chronic low back pain (cLBP). We determined the efficacy of auricular point acupressure (APA) in older adults with cLBP.

Design: 3-arm randomized controlled trial.

Setting: Baltimore, Maryland.

Participants: Participants, ≥60 years with cLBP, were randomized (1:1:1) to APA with ear points targeted to cLBP (T-APA, n = 92), points nontargeted to cLBP (NT-APA, n = 91), or waitlist education control (n = 89), and followed up to 6 months (6M). Participants in the APA groups received 4 weekly APA sessions; the education control group received 4 weekly educational sessions.

Buckwheat (Fagopyrum esculentum), a gluten-free pseudocereal, has gained popularity in global food markets for its nutritional value and functional properties. However, despite its health-promoting potential, buckwheat is vulnerable to microbial contamination and mycotoxin accumulation during cultivation, processing, and storage. These factors present significant food safety concerns that remain understudied, particularly in North America.

This study investigated the microbiological quality and mycotoxin contamination of buckwheat flour and groats collected from different suppliers via online sourcing. In contemporary digital environments, exponential cyber threat growth has made cryptographic key generation a critical security challenge. Traditional Pseudo-Random Number Generators (PRNGs) and existing chaos-based methods often exhibit insufficient entropy, limited randomness quality, and inadequate resistance to statistical attacks. Current implementations frequently produce suboptimal entropy values and fail to meet modern cryptographic security standards and rigorous randomness testing protocols. The Blockchain algorithm has advanced the accountability and transparency of modern digital infrastructures. Enforcing responsible behavior and data integrity across distributed environments involves several key components, such as smart contracts, access control models, cryptographic techniques, and a decentralized identity framework. Wireless Sensor Networks (WSNs) have emerged as a transformative technology with applications in critical and often inaccessible environments, including military and security domains. These networks comprise cooperative sensor nodes that gather and relay data to a base station. However, despite the increasing sophistication in methods and greater emphasis on theory in recent decades, we have not made commensurate strides in relevance and impact. Donald Trump’s election as president during November 2024 results in perhaps the most dramatic philosophical change in approach to governmental regulation in over ninety years. The Great Depression of the 1930s created a need for regulatory reform and resulted in the advent of such new regulatory frameworks as the U.S. Securities and Exchange Commission (SEC). Purpose: There is limited research on the use of community advisory boards (CABs) in health research with transgender and nonbinary communities. Transgender communities are navigating challenging histories of health researcher exploitation and research distrust alongside exponential research growth. We explored strategies for forming and managing CABs when conducting transgender community-informed health research. Methods: We used purposive and snowball sampling to identify key informants (KIs): research leaders, implementing staff, and community partners engaging in transgender health research. This paper analyzes the problems of non-disclosure in the context of an asset sale between a buyer and a seller. The seller can have important information about the asset and can opportunistically decide not to disclose that information to the buyer, and the buyer may sue the seller ex post for remedy. The buyer and the court have a choice over different types of remedy. The paper considers three different remedy regimes: (1) no liability; (2) voidance; and (3) damages. Tree canopy inequality in low-income marginalized communities is a direct result of decades of environmental injustice within urban communities in the United States, specifically the Home Owner’s Loan Corporation. This phenomenon underscores the disproportionate distribution of tree cover, green spaces, and their myriad benefits in neighborhoods marked by poverty and social vulnerability. Rooted in a history of discriminatory urban planning practices and systemic disparities, tree canopy inequality has profound consequences on residents' health and well-being. It leads to urban heat island effects, reduced air quality, and additional socio-economic disparities. On average, urban communities of color have 33% less tree canopy …
