The integration of a navigation software development kit (SDK) into a third-party application can present challenges related to the coupling of user interface (UI) element generation and layout logic, which may result in spatial conflicts and layout inflexibility. Systems and techniques are described for a delegate framework that can decouple UI generation from its placement. Immunoglobulin A vasculitis (IgAV), formerly Henoch–Schönlein purpura, is a small-vessel vasculitis characterized by palpable purpura, arthralgias, gastrointestinal involvement, and renal disease. Although IgAV is common in children, adult-onset disease is rare and may be associated with more severe manifestations, particularly renal involvement. In adults, IgAV may be difficult to recognize because of overlap with more common gastrointestinal, infectious, or medication-related conditions. We present a case of adult-onset IgAV initially attributed to presumed diverticulitis and later to a drug eruption following antibiotic exposure. This case highlights diagnostic pitfalls and the importance of early recognition and renal surveillance.

Introduction: Occurrence of 2 (or more) Hodgkin's and non-Hodgkin's lymphoma concordantly or sequentially in a single patient is very rare.

Case: An 88-year-old Italian male with a history of mucosa-associated lymphoid tissue (MALT) lymphoma in remission and indolent follicular lymphoma under surveillance, presented with acute autoimmune hemolytic anemia (AIHA). While initially speculated to be secondary to follicular lymphoma relapse, stable mesenteric lymphadenopathy compared to prior CT abdomen/pelvis and smudge cells on peripheral smear prompted further evaluation. Deep neck abscesses have become less common in the antibiotic era; yet, they still pose major risks of morbidity and mortality if not addressed appropriately. Their proximity to the upper respiratory tract and mediastinal structures further exacerbates this risk. We present the case of a 56-year-old man with a medical history of type 2 diabetes mellitus, who exhibited a two-month history of recurrent left-sided facial edema, pain, and dysphagia. We report a rare complication of prophylactic pancreatic duct stenting during ERCP for suspected hilar cholangiocarcinoma. A 47-year-old woman presented with two weeks of painless jaundice, total bilirubin 18 mg/dL, and cholestatic enzymes. Imaging revealed intrahepatic biliary dilation without CBD dilation. ERCP with cholangioscopy identified a right hepatic duct stricture; biopsies were obtained. Drug-associated bullous pemphigoid (DABP) refers to cases of bullous pemphigoid caused by exposure to various drugs. We present a case of nivolumab (a programmed cell death-ligand 1 (PD-L1) inhibitor) induced bullous pemphigoid in a patient treated for classic Hodgkin’s lymphoma, presenting four months after cessation of therapy. The patient demonstrated minimal clinical response to steroids and methotrexate. Corynebacterium species are often regarded as skin contaminants when isolated from clinical cultures. However, in select high-risk patients, these organisms can lead to significant infections, including infective endocarditis. We present the case of a 76-year-old woman with multiple comorbidities, including end stage renal disease (ESRD) on hemodialysis and an exposed tunneled central catheter, who developed Corynebacterium-associated native mitral valve endocarditis requiring surgical intervention. Despite prompt antimicrobial therapy and bioprosthetic valve replacement, her hospital course was complicated by heart block, empyema, and eventual cardiac arrest. Left ventricular non-compaction cardiomyopathy (LVNC) is a rare genetic cardiomyopathy characterized by prominent trabeculations and deep recesses that predispose to arrhythmias and heart failure. We describe a 42-year-old man with persistent tachycardia and exertional fatigue found to have CTI-dependent atrial flutter with 2:1 conduction. Transesophageal echocardiography showed severe biventricular dysfunction with an ejection fraction of 15%, and cardiac MRI confirmed LVNC with a non-compacted to compacted ratio of 3.3. Rasmussen’s aneurysm is a rare but potentially fatal complication of pulmonary tuberculosis. It represents a pseudoaneurysm of a pulmonary arterial branch caused by erosion of the arterial wall secondary to chronic inflammation within a tuberculosis cavity. Although uncommon, especially in non-endemic regions, rupture of this aneurysm can result in massive hemoptysis and carries a high mortality if not promptly recognized and managed. Dieulafoy lesions (DLs) are rare vascular malformations in the gastrointestinal tract that often present with gastrointestinal bleeding, most commonly in the stomach. Rectal DLs are exceptionally rare, representing less than 2% of cases. Warfarin use increases the risk for bleeding and management represents a challenge. We present a case of a 60-year-old male on chronic warfarin therapy who presented with severe hematochezia due to a rectal DL, requiring more than one colonoscopy and prothrombin complex concentrate (PCC) administration. While guidelines recommend PCC over fresh frozen plasma (FFP), they acknowledge that this is based on very low certainty of evidence.

Immune-mediated necrotizing myopathy (IMNM) is a rare subtype of idiopathic inflammatory myopathies that can present with rhabdomyolysis. We report a 43-year-old woman with rapidly progressive proximal muscle weakness following a flu-like illness. Laboratory evaluation revealed markedly elevated creatine kinase (22,207 U/L) and transaminases, with positive anti-signal recognition particle (SRP) and Coxsackie B4 antibodies. Muscle biopsy demonstrated extensive necrosis, regeneration, and endomysial lymphocytic infiltration, confirming IMNM. The patient was treated with intravenous methylprednisolone, IVIG, and rituximab, followed by oral steroid taper and mycophenolate. Background: Primary AL amyloidosis is a rare systemic disorder caused by deposition of misfolded immunoglobulin light chains, leading to multi-organ dysfunction. Renal cell carcinoma (RCC) is common, with an age-adjusted incidence of 16.4 per 100,000 persons per year in the US (2017–2021). Steroid-responsive encephalopathy associated with autoimmune thyroiditis (SREAT), also known as Hashimoto encephalopathy, is a rare immune-mediated neurological syndrome with an incidence of 2.1 per 100,000 and a 4:1 female predominance. Neurological manifestations are independent of thyroid status, occurring in 30–40% of euthyroid patients, and include cognitive impairment, confusion, seizures, and psychiatric symptoms. Background: Drug-induced immune hemolytic anemia (DIIHA) secondary to piperacillin has an estimated incidence of 0.01–0.10% [1,2]. In patients with intestinal failure-associated liver disease (IFALD), chronic conjugated hyperbilirubinemia obscures typical hemolytic markers [3,4].

Case Presentation: A 27-year-old woman with traumatic short bowel syndrome requiring total parenteral nutrition for 6 months, IFALD (baseline total bilirubin 2.5 mg/dL), and chronic kidney disease experienced three piperacillin-tazobactam exposures over two months. Primary breast lymphomas account for less than 1% of all non-Hodgkin lymphomas and 0.04–0.5% of all malignant breast neoplasms. We report a case of breast extranodal marginal zone lymphoma (EMZL) in a 60-year-old woman with chronic hepatitis C virus (HCV) infection. After comprehensive evaluation excluded other known etiologies, chronic HCV infection emerged as the most plausible contributing factor. Epidemiologic and biological evidence supports an association between HCV and B-cell non-Hodgkin lymphomas, particularly marginal zone lymphomas. Systemic Sclerosis (SSc) is an autoimmune connective tissue disease marked by multiorgan fibrosis, including cutaneous involvement, and vasculopathy. Primary heart involvement in systemic sclerosis (SSc-pHI) includes heart failure, arrhythmia, pericarditis, pericardial effusion, valve disorders, and myocarditis. SSc-pHI remains a leading cause of mortality in this population. Pericardial effusion in SSc is usually associated with heart failure, pulmonary hypertension, and end-stage kidney disease. Cardiac tamponade is a rare initial presentation of SSc-pHI but carries a worse prognosis. Background: N-methyl-D-aspartate (NMDA) autoantibody encephalitis is a rare autoimmune condition affecting approximately 1.5 per million people per year [1]. It predominantly occurs in young women and is often associated with tumors, particularly teratomas [3]. Immune checkpoint inhibitors (ICIs) have transformed cancer therapy by enhancing antitumor immunity. Despite their remarkable efficacy in malignancies, particularly metastatic melanomas, the use of ICIs have been linked to life-threatening immune-related adverse events (irAEs) affecting several organ systems. Combination therapy with ipilimumab and nivolumab increases both efficacy and the risk of severe irAEs. We report a rare case of metastatic melanoma treated with combination ipilimumab and nivolumab, complicated by multisystem irAEs. Echocardiography is a crucial diagnostic tool for identifying cardiac sources of embolism, particularly in cardioembolic and cryptogenic strokes, yet its routine use in all stroke patients remains controversial.
