Background: Severe asthma affects 5% to 10% of asthma patients but constitutes close to one-half of the medical costs related to asthma due to higher morbidity and health care utilization. Biologic agents have become a standard of care in those unresponsive to standard treatments yet the choice of biologic agent is complex due to the varying mechanisms of action, efficacies, and lack of head-to-head comparisons. Therefore, clinicians need further clinical guidance to optimize their use.

There is a significant knowledge gap in how T cells promote emphysema in smokers with chronic obstructive pulmonary disease (COPD). Single-cell RNA sequencing (scRNA seq) analysis of human samples and relevant clinical data can provide new mechanistic insights into disease pathogenesis. We generated a human lung scRNA seq dataset with extensive disease characteristic annotation and analyzed a second independent scRNA seq dataset to examine the pathophysiological role of T cells in emphysema. Comparisons of pulmonary immune landscapes in emphysematous (E)-COPD, non-emphysematous (NE)-COPD, and control showed positive enrichment of T cells in E-COPD. This dissertation employs a three-study qualitative design to investigate the role of mentorship and social interaction in the identity development of racial minority undergraduate students in STEM disciplines. Grounded in Communities of Practice theory, this research amplifies student narratives to elucidate the relational and contextual factors that inform science identity, belonging, competence, and persistence in higher education. BACKGROUND: Identifying preventive measures for adolescent mental health is a global public health priority, as poor mental health heightens the risk of early death from related issues, including suicide. This study examined the associations between adherence to the 24-Hour Movement Guidelines and anxiety among a nationally representative sample of South Korean adolescents, using an intersectional identity framework.

METHODS: Data from the 2020-2022 Korea Youth Risk Behavior Surveys were analyzed. Adolescents aged 12-17 years were grouped based on adherence to guidelines: none, physical activity (PA), sedentary behavior (SB), sleep, two guideline combinations, and all three. To better characterize the potential biological mechanisms underlying insulin resistance (IR) and dementia, we derive cross-population and population specific polygenic scores [PSs] for fasting insulin and IR-related partitioned PSs [pPSs]. We conduct a cross-sectional study of the associations of these genetic scores with neurological outcomes in >17k participants (36% men, mean age 55 yrs) from the Trans-Omics for Precision Medicine (TOPMed) program (50% Non-Hispanic White, 23% Black/African American, 21% Hispanic/Latino American, and 4% Asian American). We report significant negative associations (P <  0.002) of the cross-population (P = 1.3 × 10-5) and European (PEA = 3.0 × 10-8) fasting insulin PSs with total cranial volume, and of a metabolic syndrome European PS with general cognitive function (BEA = -0.13, PEA = 0.0002) and lateral ventricular volume (BEA = 0.09, PEA = 0.002). We identify suggestive negative associations (P <  0.007) of metabolic syndrome and obesity pPSs with general cognitive function, and of lipodystrophy pPSs with total cranial volume. A higher genetic predisposition to IR is associated with lower brain size, and a genetic predisposition to specific IR-related type 2 diabetes subtypes, such as metabolic syndrome and mechanisms of IR mediated through obesity and lipodystrophy, is potentially involved in cognitive decline.

Grounded in Freeman’s (1984) stakeholder theory, the purpose of this qualitative pragmatic inquiry research project was to identify and explore practical strategies that small business leaders use to retain employees beyond 2 years. A stroke is categorized as quickly developing symptoms of cerebral function loss with no apparent cause other than that of vascular origin. Stroke detection in prehospital settings, such as an ambulance, is subjective and often requires additional imaging methods to obtain a definitive conclusion. Common non-invasive methods for stroke detection include Magnetic Resonance Imaging (MRI) and Computed Tomography (CT) scans. Still, these techniques can be expensive, time-consuming, and dangerous due to exposure to strong magnetic fields and radiation. This commentary explores how neuromorphic artificial intelligence, AI designed to mimic the causal and adaptive processing of the human brain, differs from traditional mechanistic systems and what this means for military applications. Traditional AI follows straightforward stimulus-response patterns that work well in predictable scenarios, but they struggle in complex, dynamic environments. Neuromorphic AI uses more human-like causal processing, integrating multiple streams of data and contextual cues to make decisions that better adapt to ambiguity and uncertainty. BACKGROUND: Cardiovascular diseases (CVDs) are the leading cause of mortality and are among the foremost causes of disability globally. CVD burden has continued to increase in most countries since 1990, with trends driven by changing exposures to harmful risk factors, population growth, and population aging.

OBJECTIVES: We report estimates of global, national, and subnational CVD burden, including 18 subdiseases and 12 associated modifiable risk factors. We analyzed change in CVD burden from 1990 to 2023 and identified drivers of change including population growth, population aging, and risk factor exposure.

Congenital complete heart block (CCHB) is a rare condition associated with significant fetal and neonatal mortality. It may present as an isolated conduction abnormality in association with autoimmune disease, or in conjunction with congenital heart defects including heterotaxy-polysplenia syndrome. Early prenatal diagnosis and coordinated perinatal care are critical for improving outcomes. We present 2 cases of prenatally diagnosed CCHB. The first occurred in a fetus with heterotaxy-polysplenia syndrome, and the second in the setting of maternal anti-SSA antibody exposure. Both fetuses developed progressive bradycardia, with heart rates declining to < 50 beats/min, necessitating close fetal surveillance and individualized perinatal management. Preterm delivery and prompt placement of temporary pacing wires allowed initial stabilization before eventual definitive management, resulting in survival. The successful outcomes in these neonates highlight the critical role of multidisciplinary prenatal care, timely delivery planning, and early postnatal intervention in improving survival in fetuses diagnosed with CCHB.

OBJECTIVE: Purulent pericarditis is a rare, lethal complication of a bacterial pericardial infection. This case report outlines the protocol for instilling intrapericardial antibiotics for methicillin-sensitive Staphylococcus aureus (MSSA) pericarditis without fibrinolytics in a 51-year-old man.

KEY STEPS: An appropriately sized percutaneous pericardial drain was placed. Then, a 3-day course of intrapericardial infusions of 50 mL vancomycin was begun. Drainage of all accessible fluid was performed before each infusion. The daily infusions each had an 8-hour dwell time. The prior drainage protocol continued after each infusion.

POTENTIAL PITFALLS: Strict dosage and infusion length management are required to prevent direct toxicity. Potential …

Background Requests for pacemaker deactivation at end of life raise unique ethical challenges. Physicians often experience moral distress when patient autonomy conflicts with perceived duties of beneficence and non-maleficence. Case A 74-year-old man with metastatic renal cell carcinoma receiving checkpoint inhibitor therapy presented with third-degree heart block. He underwent extensive evaluations, including cardiac catheterization, transvenous pacing, and steroids. After two cardiac arrests, a permanent pacemaker was implanted. Despite treatment for several weeks, he experienced progressive functional and nutritional decline. He requested pacemaker deactivation with goal of comfort focused care. Liquid storage tanks are critical infrastructure, serving as key storage facilities for various products. As the contents in these tanks are usually highly toxic and flammable, damage to these structures under earthquakes can cause severe consequences, including energy shortage, fire and explosion, and spilling and long-term environmental damages.

Although new liquid storage tanks are designed according to the updated seismic design codes, large numbers of existing storage tanks were built before modern seismic design codes and did not undergo necessary seismic retrofitting. This brief pres­ents a two-step strategy for ensuring that all eligible U.S. children will be able to participate in the program. Background: Kidney function declines with age, largely due to chronic low-grade inflammation. Mesenchymal stem cells (MSCs) have demonstrated immunomodulatory effects in certain immune-mediated kidney diseases, but their role in preserving renal function in aging individuals without chronic kidney disease (CKD) remains unclear. Aim: The aim of this systematic review was to synthesise findings from qualitative studies exploring patients' experiences in hospital or at home following open-heart surgery.Methods: Systematic searches were conducted in CINAHL, MEDLINE, PsycINFO, ProQuest Dissertation and Theses, and Web of Science, covering the period from 2006 to 2023. EGFRex20 insertions (EGFRex20ins) can be classified as near- and far-loop based on the insertion location, however, the impact of location on responses to various EGFR tyrosine kinase inhibitors (TKIs) is poorly understood. In vitro studies show that afatinib, poziotinib, and zipalertinib more potently inhibited near-loop than far-loop insertions, whereas mobocertinib has similar IC50 in both groups. Molecular dynamics simulations reveal that near-loop insertions have multiple conformational states and lower transitional energy than far-loop insertions. Time online involves the risk of direct and vicarious online racial/ethnic discrimination. This study examined the day-to-day associations between online racial/ethnic discrimination and positive and negative affect, somatic symptoms, and anxiety. Participants were 208 fourth-year college students (25% men, 72.1% women, 2.9% not reporting gender; 36.1% Asian, 30.3% White, 17.3% Latinx, 7.7% Multiethnic, 8.7% Other; M age = 22 years). The sample resided in the U.S. Data were collected in 2020. Longitudinal data were collected via online surveys using a daily report approach. This review investigated the effects of adjuvant vitamin D therapy on Total Nasal Symptoms Score (TNSS), IgE, and eosinophil counts in patients with allergic rhinitis (AR). The meta-analysis used databases, such as PubMed, Science Direct, Scopus, Web of Science, SAGE journals, and clinicaltrials.gov. Quality appraisal was performed using RoB 2 and NOS tools. The research was guided by the PRISMA 2020 chart and analyzed using RevMan 5.4. Sixteen articles, including 11 randomised controlled trials and five cohorts, met the inclusion criteria. This study investigates the dominant themes and discursive structures within global film scholarship through a bibliometric analysis of 404 Scopus-indexed articles published between 2019 and 2024. Using VOSviewer to map keyword co-occurrence, citation networks, and authorship patterns, the research identifies five major thematic clusters: Hollywood, feminism, representation, Bollywood, and popular culture. The findings reveal a persistent dominance of Western narratives positioning them at the centre of global film studies. In contrast, themes related to non-Western cinema, such as Bollywood, appear less central and more fragmented within the academic discourse.
