The PLATO network, a collection of mainframe computers, terminals, and educational software, has received an increasing amount of scholarly attention in the last decade as a social precursor to the modern internet. Existing histories have neglected to investigate the ways in which PLATO and its related business enterprise worked to accelerate the shift in university governance away from classical liberal ideas centering the public good and towards a more profit-centered neoliberal rationality. By tracing the rise of PLATO in Florida universities, this paper argues that PLATO aided and was aided by the shifting priorities of American universities in the 1970s …

Data For: "Alongshore Setup Gradients Drive Reef Flat Currents On A Coral Atoll Island", Ashley Holsclaw, Mathilde Lindhart, Gerd Masselink

This study presents extensive in situ observations from a coral atoll island in the Maldives to investigate the alongshore variability of wave setup and reef flat currents driven by heterogeneity in morphology and wave forcing. The island’s diverse reef morphology provided an ideal natural laboratory for assessing its impact on spatial hydrodynamic variability. Results indicate that shoreline wave setup increased with incident wave height and low reef flat water levels, regardless of reef geometry. Despite the significant variations in topography, bathymetry, and wave exposure, theoretical predictions of wave setup using existing parameterizations demonstrated strong agreement with observations across three distinct …

Achieving sustained local release of small hydrophilic drugs is challenging and is particularly important when the drugs are toxic. To address these challenges, we developed a hybrid system comprising drug-containing microparticles embedded within a nanoliposomal hydrogel matrix. This system forms through salt-induced gelation using physiologically relevant sodium chloride concentrations (0.9%), allowing for microparticle encapsulation without harsh chemical processes. In vitro, the hybrid system exhibited a slower release of encapsulated cargo compared to microparticles or hydrogel alone. In vivo proof of principle was provided with tetrodotoxin (TTX), a small hydrophilic and ultrapotent local anesthetic, which can cause systemic toxicity if the …

A Quality Improvement Project To Standardize Sexual Dysfunction Screening In Primary Care, Ashley M. D’Anna

Sexual health is an essential aspect of overall well-being but is often overlooked in primary care due to provider discomfort, time constraints, and lack of standardized screening protocols. This quality improvement (QI) project aimed to implement a consistent approach to screening for sexual dysfunction using the Arizona Sexual Experiences Scale (ASEX), a validated five-item tool assessing sex drive, arousal, orgasm, and satisfaction. Guided by Swanson’s Theory of Caring and Pender’s Health Promotion Model, the project emphasized compassionate, patient-centered care and the promotion of health-enhancing behaviors.

Scant research is available on how university leaders make decisions and communicate those decisions to their stakeholders during times of crisis. The purpose of this case study was to better understand the decision-making process used by administrative leaders at a higher education institution during a time of organizational crisis. This study examined decision-making by higher education leaders within and across different functional areas at a single higher education institution between February 2020 and September 2020, following the onset of the COVID-19 pandemic. The diffusion model, a cutting-edge deep generative technique, is gaining traction in biomedical informatics, showcasing promising applications across various domains. This review presents an overview of the working principles, categories, and numerous applications of diffusion models in biomedical research. In medical imaging, these models, through frameworks like Denoising Diffusion Probabilistic Models (DDPMs) and Stochastic Differential Equations (SDE), offer advanced solutions for image generation, reconstruction, segmentation, and denoising. Objective: The objective of this work is to develop a standard-based taxonomy of features that might affect user response to alerts using evidence from literature and public alert logic repositories.
Methods: We developed a taxonomy of features using multiple sources: (1) the Agency for Healthcare Research and Quality (AHRQ) CDS Connect Repository, (2) alert logic from commercial electronic health record (EHR) customers, and (3) published literature. Three categories (patient, provider, environment/context) were used a priori to develop the taxonomy. The final taxonomy was mapped to the Fast Healthcare Interoperability Resources (FHIR) standard for development of standardized CDS services.

Developmental processes extend beyond embryogenesis to support lifelong tissue adaptations. Avian feather follicles, with their resident stem cells and capacity for cyclic regeneration, provide a dynamic model for postnatal tissue remodeling. Here, we propose the Mandarin duck (Aix galericulata) as an ideal model to study lifelong developmental modulation, focusing on the sexually dimorphic "sail feather"-a secondary flight feather in males that undergoes seasonal transformation into a strikingly asymmetric, ornamented phenotype during the breeding season. We identified asymmetric morphogen expression in regenerating male sail feathers and used transcriptome and H3K27ac ChIP-seq to uncover male and female signaling pathways and regulatory elements. …

Microplastics are a diffuse contaminant with various global sources, pathways, and sinks. This study aimed to backtrack microplastics across environments using metaproteomic and eDNA metabarcoding information stored within the ecocorona. Pristine polyamide (PA) fibers, polyethylene terephthalate (PET) fibers and fragments, and PET and PA preincubated in bovine serum albumin (BSA) were deployed into a tank housing Penaeus monodon to develop an ecocorona. Upon collection, BSA was detected within the ecocorona, along with P. monodon proteins, using mass spectrometry. BSA preincubation influenced the diversity and abundance of ecocorona proteins with pristine microplastics having more significantly enriched proteins. Most ecocorona proteins reflected …

Introduction: Iatrogenic esophageal perforation (IEP) is a serious adverse event of transesophageal echocardiogram (TEE) requiring prompt identification for urgent antibiotic administration and potential surgical repair. However, the relative infrequency of this complication implores the consideration of alternative diagnoses. We present a unique case of Ludwig’s angina following TEE initially concerning for IEP. Case Presentation: A 66-year-old female with gastric arteriovenous malformations and paroxysmal atrial fibrillation with recent left atrial appendage occlusion (LAAO) device placement presented to the emergency department for bilateral submandibular pain and swelling with acute onset dysphagia. Federated Learning (FL) offers a decentralized approach to training large language models (LLMs), addressing critical concerns around data privacy and transmission costs. However, FL's inherent distributed training paradigm can lead to increased energy consumption and carbon emissions, especially with random or uninformed client selection. To tackle this issue, we propose SustainFed-LLM, a novel Q-learning-based client selection framework that integrates real-time renewable energy availability, carbon intensity data, and fairness considerations. By dynamically assessing client performance, spare capacity, and sustainability metrics, SustainFed-LLM optimizes client participation to minimize environmental impact while maintaining model accuracy. BACKGROUND: Excessive use of postoperative prophylactic antibiotics in children's hospitals is a significant public health concern, leading to increased risks of infections like Clostridioides difficile, multidrug-resistant organisms, and unnecessary healthcare costs. Antibiotic stewardship programs (ASPs) are designed to optimize antibiotic use, but ideal strategies for implementing evidence-based guidelines remain unclear. We tested facilitation, a dynamic process where trained individuals support healthcare personnel in bridging evidence-practice gaps, as a promising strategy for the de-implementation of unnecessary postoperative antibiotics in healthcare.

The lymphatic system maintains tissue fluid balance, and its dysfunction can result in lymphedema. Although cholesterol is essential for cellular function, its role in lymphatic development has remained unknown. Here, we identify APOA1 binding protein (AIBP) as a key regulator that promotes lymphatic endothelial cell fate specification and lymphangiogenesis. Mechanistically, AIBP reduces plasma membrane cholesterol content, thereby enhancing VEGFR3 signaling by disrupting caveolae—small plasma membrane invaginations formed by the scaffolding protein caveolin-1 (CAV-1)—and relieving CAV-1–mediated inhibition. Microsurgical testicular sperm extraction (microTESE) with intracytoplasmic sperm injection (ICSI) represents the current standard treatment for nonobstructive azoospermia (NOA). However, cures remain unavailable for NOA patients lacking retrievable haploid cells. mRNA supplementation could be a potential treatment for genetic defects leading to impaired spermatogenesis. Lipid nanoparticles (LNPs) have emerged as mRNA delivery vehicles with minimal risk of genome integration; however, their ability to selectively deliver mRNA to specific cell types remains limited. Importance: Studies suggest that everolimus may reduce the risk of rejection, cardiac allograft vasculopathy (CAV), chronic kidney disease (CKD), and cytomegalovirus (CMV) after heart transplant. Everolimus use is controversial because of data demonstrating higher infection deaths when everolimus is introduced de novo after transplant. It is unclear whether everolimus is safe and effective when initiated at 6 months posttransplant in children, a population in which median graft survival is limited to 15 years and randomized clinical trials are lacking.

The delivery of culturally relevant content in K-12 computing education is widely recognized as essential for engaging students and fostering meaningful learning experiences. However, in many African contexts, K-12 computing teachers feel unprepared to address this need. Course materials often fail to reflect the cultural and linguistic diversity of the students they serve. In this study, we conducted participatory design sessions with education stakeholders, including primary and secondary school teachers, to develop culturally relevant programming activities for a national programming outreach initiative in Botswana. Contextualizing computer science education has been recognized as a key factor in enhancing student engagement and learning outcomes. This study investigates the initial perceptions of university computer science faculty in Africa regarding the benefits, adoption challenges, and institutional support required for the successful integration of contextually relevant materials into introductory computer science (CS1) courses. Faculty then assessed a set of previously developed contextually tailored materials, grounded in Banks' Additive Approach to curriculum reform and aligned to the CS curricula 2023. Passive acoustic monitoring techniques are useful for studying vocally active marine species, particularly in remote and difficult to access areas. In this study, three months of acoustic recordings were collected off Egmont Atoll, Chagos Archipelago in the central Indian Ocean to detect, characterise, and investigate temporal variability of delphinid whistles. A subsample (3.6% of the total dataset) was used to manually annotate whistles and test automated whistle detectors. Higher frequency whistles (6-25 kHz, n = 126) were manually classified into six categories: upsweep (56%), convex (19%), downsweep (13%), concave (5%), sine (4%), and constant frequency (3%). Previous research has suggested that changing the percentage of the course grade associated with exam grades in science, technology, engineering, and mathematics courses can change the gender gap in the course. It has also been shown that high-stakes assessments yield the lowest (relative) scores for female students. Previous research by the authors has shown that implementing retake exams can eliminate the gender gap in introductory physics courses. This paper explores several different hypotheses for why retake exams are associated with a zeroed gender gap. Recent advancements in reinforcement learning (RL) have significantly enhanced the autonomous decision-making capabilities of energy systems. In the realm of electric vehicle (EV) charging stations, operators aim to autonomously learn optimal pricing strategies based on local observations and user behavior. Traditional multi-agent reinforcement learning (MARL) approaches often assume cooperative settings with reward sharing or communication, real-world EV charging infrastructures are typically decentralized and competitive. This paper investigates the potential for tacit collusion in such realistic, non-cooperative environments. BACKGROUND AND OBJECTIVES: Trichloroethylene (TCE) is an important environmental contaminant in the United States due to widespread use industrially. Epidemiologic studies suggest that occupational exposure to TCE and TCE-contaminated drinking water may increase the risk of Parkinson disease (PD). The aim of this study was to investigate the nationwide relationship between ambient TCE and PD risk.
