This poem explores the shifting rhythms of attention as both a challenge and a gift in the life of a student of medicine. It illuminates the path toward knowledge and healing. Woven into its lines is the tension between effort and rest, intellect and care, science and humanity. The imagery of fire and thread reflects how learning is not only about the consumption of information, but also the shaping of character. Ultimately, the poem affirms that rest, presence, and attention are not distractions from becoming a healer, but integral to the art of it.

Introduction: Migraines account for approximately 1.2 million U.S. emergency department (ED) visits annually, yet fewer than 25% of individuals achieve sustained relief, underscoring the need for more effective, standardized care. The American Headache Society recommends first-line ED therapy with intravenous fluids, nonopioid analgesics, and antidopaminergic agents; if inadequate, escalation to triptans or dihydroergotamine is appropriate, with adjuncts such as magnesium or valproic acid. Opioids are discouraged due to limited efficacy and safety concerns. We evaluated the impact of implementing an evidence-based ED migraine pathway following provider education.

Introduction: Baptist Hospital of Miami (BHM) is transitioning to an academic medical center by starting medical residency training in July 2025. As healthcare systems expand, pharmacists must ensure safe medication dispensing and provide real-time clinical decision support. To support this transition, a structured internal medicine pharmacotherapy curriculum was developed to refresh clinical knowledge and enhance confidence among experienced clinical pharmacists.

This study reports the fabrication and comprehensive evaluation of a novel multilayered, sandwich-structured nanofiber scaffold composed of Tranexamic acid (TXA), chitosan (CS), polyvinyl alcohol (PVA), L-arginine, and polylactic acid (PLA) for biodegradable wound dressing applications. The mechanisms of pluton emplacement into continental crust have long been debated in igneous geology, with early models emphasizing forceful processes such as ballooning, later contrasted by alternative emplacement mechanisms including diapirism, incremental assembly, stoping and, emplacement by sheets and dikes. These contrasting perspectives highlight the complexity of the construction of mid-crustal intrusions and the need for case studies that integrate multiple datasets. This study evaluated the impact of graded amounts of nicotinic acid (NA) with a negative DCAD diet on cow health, colostrum composition and calf growth performance of Jersey cattle. We hypothesized that NA supplementation would increase IgG concentration of colostrum, increase calf intestinal absorption and growth and improve cow metabolic health. Thirty multiparous Jersey cows were blocked by calving date and assigned to either 0, 8 or 16 g/d of NA supplementation on top of the -40 mEq/kg DCAD dry cow diet. Breast cancer is one of the most frequently diagnosed cancers worldwide, with approximately 15% classified as Triple-Negative Breast Cancer (TNBC). TNBC lacks expression of estrogen (ER) and progesterone (PR), and does not overexpress HER-2, leaving patients without effective targeted therapies. As a result, treatment primarily relies on chemotherapy, most commonly paclitaxel, a taxane that stabilize microtubules, disrupts chromosome separation and induces apoptosis. However, TNBCs are an aggressive cancer subtype, and patients often will develop resistance after a few rounds of chemotherapy. A subset of eukaryotic pre-tRNAs have introns that must be spliced out to become mature molecules capable of functioning in translation. This essential processing step is performed by tRNA splicing endonuclease (TSEN), a highly conserved heterotetramer with two catalytic and two structural subunits. In addition to this conserved function, archaeal TSEN also processes pre-rRNA and yeast TSEN cleaves some mRNAs that encode mitochondrial proteins to initiate mRNA degradation. Interestingly, single amino acid mutations in TSEN cause the human Mendelian disease pontocerebellar hypoplasia (PCH) by an unknown mechanism. Pancreatic Ductal Adenocarcinoma (PDAC) is the third leading cause of cancer-related deaths with a low 5-year relative survival rate of approximately 13.3% for all stages combined. We have previously shown that exercise can improve quality of life and enhance functional capacity among patients with PDAC. Exercise induces a variety of changes in the tumor microenvironment with beneficial effects in several tumor types. However, our understanding of the clinical effects of exercise and the mechanisms that mediate anti-tumor effects are limited in pancreatic cancer.

House screening (HS) of doors, eaves, and windows using wire-mesh has demonstrated potential in the integrated vector management of malaria. However, limited epidemiological data are available to guide its implementation across different ecological settings. In a 16-month randomized controlled trial (follow-up period) conducted across three agroecological areas (dry mountain, plateau highland, and semi-arid) in Jabi Tehnan district, northwestern Ethiopia, treatment houses were equipped with HS combined with insecticide-treated nets (ITNs), while control houses received ITNs only. The intervention led to a significant 2.3-fold reduction in indoor malaria vector density, the primary entomologic outcome, largely influenced by An. gambiae s.l. mosquitoes. …

Amidst an emerging bipartisan consensus on greater corporate social accountability, America First has joined longstanding solicitude about the racial wealth gap. This Article offers yet another take on corporate purpose. As a project of recovering legal memory like Adrian Vermeule’s common good constitutionalism, it examines scholarship on the history of corporate purpose toward answering whether stakeholder capitalism should inform corporate regulation and, if so, how. It concludes from this history that the Anglo-American legal tradition clearly justifies regulators to hold business corporations accountable to the common good. Taxpayers have six wins and zero losses in cases that grant nonrecognition to exchanges that occur in proximity to tax-free business transactions (contributions to and distributions from entities) of the exchanged property. Those six cases (the proximate-exchange cases) were decided between 1983 and 1989.
