การเปรียบเทียบการวินิจฉัยโรคสมองเสื่อมด้วยภาพซ้อนทับ Pet-T1 และ Pet-Dti, ปรเมษฐ์ วงษา

จุดประสงค์ของงานวิจัยนี้ เพื่อสร้างแม่แบบภาพเพทมาตรฐานบริเวณสมองขึ้นมาใหม่ และเพื่อเปรียบเทียบประสิทธิภาพในการกระบวนการซ้อนทับภาพเพทระหว่างแม่แบบภาพเพทบริเวณสมองที่สร้างขึ้นกับแม่ภาพเพทบริเวณสมองแบบเดิมที่มีการใช้งานอยู่ในปัจจุบัน แม่แบบภาพเพทมาตรฐานบริเวณสมองสร้างขึ้นมาจากภาพเพท กับภาพเอ็มอาร์ไอแบบ DTI จากข้อมูลภาพถ่ายทางรังสีจากกลุ่มอาสาสมัครที่มีภาวะความจำปกติ จำนวน 24 คน อายุระหว่าง 42-79 ปี ซึ่งกลุ่มอาสาสมัครดังกล่าวจะต้องทำการถ่ายเพทบริเวณสมองที่มีการฉีดสารเภสัชรังสี 11C-PIB เข้าไปในร่างกาย และทำการถ่ายภาพเอ็มอาร์ไอบริเวณสมองด้วยเทคนิค T1 และเทคนิค DTI จากนั้นทำการแก้ไขค่ากระแสไฟฟ้ารบกวน การกำจัดคลื่นสัญญาณรบกวน และการสร้างแผนที่โดยวิธี Whole-brain probabilistic tractography จากภาพถ่ายเอ็มอาร์ไอเทคนิค DTI โดยแผนที่ดังกล่าวที่ถูกสร้างขึ้นมาจะถูกนำไปใช้ในขั้นตอนการซ้อนทับ และขั้นตอนการ Normalize กับภาพเพทเพื่อช่วยในการกำหนดขอบเขตภายในบริเวณเนื้อสมอง หลังจากนั้นทำการประเมินความถูกต้องของการซ้อนทับของภาพแม่แบบภาพเพทมาตรฐาน โดยการประเมินการซ้อนทับกับภาพเพทในกลุ่มผู้ป่วยที่มีภาวะสมองเสื่อมในระยะเริ่มต้นที่เข้ารับการถ่ายภาพเพทบริเวณสมองด้วยการตรวจแบบเดียวกัน จากนั้นทำการประเมินและให้คะแนนความถูกต้องในการซ้อนทับของภาพเพทกับแม่แบบภาพเพทมาตรฐานบริเวณสมองโดยรังสีแพทย์ผู้เชี่ยวชาญทางด้านเวชศาสตร์นิวเคลียร์จำนวน 3 คน ด้วยวิธีการแบบปกปิดข้อมูล แล้วทำการเปรียบเทียบค่าคะแนนดังกล่าวโดยใช้การทดสอบทางสถิติแบบ Wilcoxon Signed Ranks test การประเมินความสอดคล้องภายในโดยใช้สถิติ Fleiss Kappa test และสถิติ Cohen’s weighted Kappa test ผ่านโปรแกรม SPSS ที่ระดับความเชื่อมั่น ร้อยละ 95 นอกจากนี้ในงานวิจัยได้ทำการหาค่า Dice similarly coefficient เพิ่มเติม เพื่อเปรียบเทียบความถูกต้องของการซ้อนทับดังกล่าว ผลการศึกษาวิจัยพบว่าคะแนนประเมินความถูกต้องในการซ้อนทับของภาพเพทกับแม่แบบมาตรฐานภาพเพทบริเวณสมองที่สร้างขึ้นด้วยวิธีการใหม่มีค่าสูงกว่าแม่แบบมาตรฐานภาพเพทบริเวณสมองที่สร้างขึ้นด้วยวิธีการเดิมอย่างมีนัยสำคัญทางสถิติ ซึ่งสอดคล้องกับค่า Dice similarly coefficient ที่พบว่ามีค่าสูงสุดในแม่แบบมาตรฐานภาพเพทบริเวณสมองที่สร้างขึ้นด้วยวิธีการใหม่ อย่างไรก็ตามพบว่าผลการประเมินค่าความสอดคล้องภายในอยู่ในระดับสอดคล้องกันแบบเล็กน้อย ซึ่งจากผลการศึกษาวิจัยดังกล่าวสรุปได้ว่า แม่แบบมาตรฐานภาพเพทบริเวณสมองที่สร้างขึ้นมาใหม่โดยใช้ภาพเอ็มอาร์ไอเทคนิค DTI มาร่วมด้วย มีการซ้อนทับของภาพที่ดีกว่าเดิมและเหมาะสมในการที่จะนำไปใช้ในการช่วยวินิจฉัยผู้ป่วยที่มีสภาวะโรคสมองเสื่อม

การศึกษานี้มีวัตถุประสงค์เพื่อพัฒนาอุปกรณ์ฝึกเดินที่มีการนำอุปกรณ์วัดระยะทางด้วยแสง หรือ อุปกรณ์ไลดาร์ มาประยุกต์ใช้ในการเก็บข้อมูลการเดิน และนำข้อมูลการเดินที่อุปกรณ์ไลดาร์เก็บได้ มาทำการประมวลผลเป็นตัวแปรการเดินต่าง ๆ ได้อย่างแม่นยำโดยไม่จำเป็นต้องทำการปรับเทียบทุกครั้งก่อนใช้งาน และ ไม่จำเป็นต้องติดวัตถุลงบนร่างกายของผู้ใช้งานก็สามารถเก็บข้อมูลได้อย่างแม่นยำ โดยในการศึกษาครั้งนี้ ได้ทำการทดสอบความถูกต้องและความแม่นยำในการเก็บข้อมูลโดยเปรียบเทียบกับอุปกรณ์ถ่ายภาพความเคลื่อนไหวทางคลินิก โดยจากการทดสอบพบว่า จากความคลาดเคลื่อนในการวิเคราะห์ตัวแปรเชิงระยะทาง และ ความคลาดเคลื่อนในการวิเคราะห์ตัวแปรเชิงเวลา ที่ต่ำกว่า 10% โดยสรุป จากการทดสอบกับระบบเก็บข้อมูลการเดินมาตรฐานทางการแพทย์ มีความเป็นไปได้ที่จะนำอุปกรณ์ฝึกเดินพร้อมระบบวัดระยะทางด้วยแสงที่พัฒนาขึ้นนี้ มาใช้งานในการเก็บข้อมูลการเดินจริงได้ ซึ่งระบบฝึกเดินที่พัฒนาขึ้นนี้ จะมีประโยชน์ในการกายภาพบำบัดและการฟื้นฟูร่างกายแบบทางไกล

งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาการประยุกต์ใช้ไบโอเซนเซอร์ชนิดควอตซ์คริสตัลไมโครบาลานซ์ (quartz crystal microbalance, QCM) ในการวิเคราะห์อันตรกิริยาระหว่างแบคทีเรียก่อโรคปริทันต์ชนิด Aggregatibacter actinomycetemcomitans กับโมโนโคลนอลแอนติบอดีที่จำเพาะเพื่อการตรวจวัดแบคทีเรียก่อโรคปริทันต์ ไบโอเซนเซอร์สำหรับการตรวจวัดถูกสร้างขึ้นโดยใช้คิวซีเอ็มเซนเซอร์ ชนิด 30 เมกะเฮิรตซ์ ที่ผ่านการปรับสภาพพื้นผิวอิเล็กโทรดทองด้วย 11-mercaptoundecanoic acid (11-MUA) และได้ประเมินความหนาแน่นการเรียงตัวของชั้น 11-MUA ด้วยเทคนิคไซคลิกโวลแทมเมตรี จากนั้นทำการตรึงโมโนโคลนอลแอนติบอดีชนิด Anti-A. actinomycetemcomitans กับ 11-MUA โดยใช้ 1-ethyl-3-(3-dimethylaminopropyl) carbodiimide/N-hydroxysuccinimide (EDC/NHS) เพื่อใช้แอนติบอดีทำหน้าที่เป็นสารรู้จำทางชีวภาพ (biorecognition element) สำหรับการตรวจวัด A. actinomycetemcomitans การวิเคราะห์อันตรกิริยาระหว่างแบคทีเรียกับแอนติบอดีชนิด anti-A. actinomycetemcomitans ทำด้วยกัน 4 รูปแบบ ได้แก่ รูปแบบที่ 1 ใช้ข้อมูลการเปลี่ยนแปลงความถี่ (∆F) ซึ่งผลการศึกษาบ่งชี้ว่าสามารถใช้ข้อมูล ∆F ในการจำแนกความสามารถในการจับกันระหว่างแอนติบอดีและเซลล์แบคทีเรียชนิดต่าง ๆ ได้ โดยคู่แอนติบอดีและเซลล์แบคทีเรียที่มีความจำเพาะต่อกันมีรูปแบบ ∆F ที่แสดงให้เห็นแนวโน้มการเปลี่ยนแปลงของ ∆F ที่ขึ้นกับความเข้มข้น รูปแบบที่ 2 ใช้ค่าอนุพันธ์อันดับหนึ่งของการเปลี่ยนแปลงความถี่ (dF/dT) ซึ่งผลการศึกษาบ่งชี้ว่าสามารถใช้วิธีนี้จำแนกความจำเพาะของแอนติบอดีชนิด Anti-A. actinomycetemcomitans กับแบคทีเรียต่างชนิดได้ โดยเมื่อเกิดการจับกันระหว่างแอนติบอดีและเซลล์ที่มีความจำเพาะกัน ค่า dF/dT จะเปลี่ยนแปลงเป็นลบ และค่า dF/dT แสดงให้เห็นแนวโน้มการเปลี่ยนแปลงเป็นรูปแบบที่ขึ้นกับความเข้มข้นเช่นเดียวกับข้อมูล ∆F รูปแบบที่ 3 ใช้ข้อมูลเวลาการตอบสนอง (τ) ซึ่งผลการศึกษาบ่งชี้ว่าการจับกันระหว่างแอนติบอดีกับเซลล์ A. actinomycetemcomitans ที่ความเข้มข้นสูงสุด (1.16 × 108 เซลล์/มิลลิลิตร) ให้ค่า τ เฉลี่ยเพียง 143 วินาที ทำให้ทราบผลลัพธ์ของการจับกันระหว่างแอนติบอดีกับเซลล์ A. actinomycetemcomitans รวดเร็วกว่าการติดตามจากข้อมูล ∆F ถึง 3 เท่า รูปแบบที่ 4 ใช้ข้อมูลความชันของการตอบสนอง …

Play is widely acknowledged to provide children with various physical, emotional, social, and intellectual benefits. However, playgrounds are reported to be the leading location of unintentional recreation injuries to children aged 1-10. Over 185,000 children annually are being treated in emergency departments for playground injuries. These play-related injuries have remained stagnant according to epidemiology studies of the past three decades. Past epidemiological studies for playground injuries have been based on emergency department data, resulting in an underrepresentation of the magnitude of injuries occurring. Neuromotor dysfunction after a concussion is common, but balance tests used to assess neuromotor dysfunction are typically subjective. Current objective balance tests are either cost- or space-prohibitive, or utilize a static balance protocol, which may mask neuromotor dysfunction due to the simplicity of the task. To address this gap, our team developed an Android-based smartphone app (portable and cost-effective) that uses the sensors in the device (objective) to record movement profiles during a stepping-in-place task (dynamic movement). Recently, our work has identified two thermodynamically distinct types (A and B) of energetic processes naturally occurring on Earth. Type-A energy processes such as the classical heat engines, ATP hydrolysis, and many of the known chemical, electrical, and mechanical processes apparently follow well the second law of thermodynamics; Type-B energy processes, for example, the newly discovered thermotropic function that isothermally utilizes environmental heat energy to do useful work in driving ATP synthesis, follow the first law of thermodynamics (conservation of mass and energy) but do not have to be constrained by the second law, owing to its special asymmetric functions. …

The HITRAN database is a compilation of molecular spectroscopic parameters. It was established in the early 1970s and is used by various computer codes to predict and simulate the transmission and emission of light in gaseous media (with an emphasis on terrestrial and planetary atmospheres). The HITRAN compilation is composed of five major components: the line-by-line spectroscopic parameters required for high-resolution radiative-transfer codes, experimental infrared absorption cross-sections (for molecules where it is not yet feasible for representation in a line-by-line form), collision-induced absorption data, aerosol indices of refraction, and general tables (including partition sums) that apply globally to the data. …

Plasma-activated water (PAW) is considered to be an effective anticancer agent due to the diverse aqueous reactive oxygen and nitrogen species (RONS: ROS and RNS), but the drawback of low dose and short duration of RONS in acidified PAW limits their clinical application. Herein, this Letter presents an innovative therapeutic avenue for cancer treatment with highly-effective alkaline PAW prepared by air surface plasma. The purpose of this project was to create a radial artery model that is usable under ultrasound that would reduce complications during catheterization procedures. Each year, over one million of these procedures are done and complications can affect the outcomes of the patient. Using ultrasound during these procedures is an upcoming way to reduce these complications and improve the outcome that a patient will have.

The ability to don and doff fitted shoes can be a challenge for many people including those diagnosed with diabetes, obesity, limb loss, pregnancy, Parkinson’s disease, arthritis, surgical complications, and aging. Though these actions may seem trivial, the ability to independently put on shoes is necessary because it can significantly impact one’s capacity to complete everyday tasks, hold relationships, and maintain mental health. Extracellular-regulated kinase 3 (ERK3) is overexpressed in a variety of cancers, including lung cancer. This atypical mitogen-activated protein kinase (MAPK) has a unique structure which includes a C34 domain and C-terminus and about which relatively little is known. ERK3 has been demonstrated to promote cell migration and metastasis in multiple cancer types. A yeast two-hybrid assay using ERK3 as bait indicated Diacylglycerol kinase ζ (DGKζ) is a binding partner. These users experience discomfort and problems with the standard nasal cannula that are compounded by issues with oxygen sources being heavy with limited supply and tubing getting tangled, kinked, or not providing enough mobility. This project will focus on the issues caused by a pandemic, particularly, sanitation issues and difficulties in a public area. After a considerable amount of research, it became evident that current sanitation methods are time inefficient, not always effective and wasteful. There are numerous misconceptions on how these methods work and skepticism of the products themselves. The pandemic caused by COVID-19 is an extremely relevant issue that has affected schools, hospitals and other high traffic areas. Currently available training methods for cochlear implant surgeries such as cadavers and imaging systems are expensive and available for a limited number of training sessions. With the goal of decreasing risk factors associated with cochlear implant surgery, our team developed a cochlear implant training model prototype that is designed to provide a trial-and-error, tactile training method for developing force perception levels required to avoid causing damage to the cochlea. Glycolipids with diacetylene functional groups are fascinating compounds with many practical uses. Among these, diacetylene-containing gelators are especially important because they can form photopolymerizable gels, which are useful stimuli-responsive materials. Inspired by the unique properties of diacetylene-containing gelators and to understand the structural influences especially the location of the diacetylene functional groups on the self-assembling properties, a series of 15 novel N-acetyl-d-glucosamine derivatives with the diacetylene functional group introduced at the anomeric position were designed and synthesized. Cancer is the second leading cause of death in women and late stage (metastatic) cancers have abysmal survival rates compared to early stage regional cases (27% vs 86%). As a tumor grows, the surrounding extracellular matrix (ECM) is reorganized into a dense, collagen rich matrix. The new matrix of aligned collagen fibers provides unique mechanical cues such as anisotropic stiffness and contact guidance. Matrix turnover also constricts local vasculature and restricts delivery of key nutrients and signaling molecules to malignant cells to outside the tumor creating a chemotactic gradient from outside to inside. Chronic pain (CP) is a significant contributor to disability and disease burden globally. In 2019, approximately 50.2 million adults (20.4% of the US population) experienced chronic pain, contributing to $560-635 billion in direct medical costs. In addition, the worldwide prevalence of diabetes mellitus has reached epidemic proportions and is set to increase to 629 million by 2045. Almost 50% of patients with diabetes present with diabetic neuropathy (DN), and one in five patients with diabetes presents with painful DN (pDN) which is the most common cause of neuropathic pain (NP) in the US. This study proposes a novel computational method to quantify guide stability for Patient Specific Instrumentation (PSI) guides. A finite element contact model was used to analyze the final position of PSI guides on a femur across a range of loading parameters representing forces applied by a surgeon during operative use. Separate segmentation methods were used for the guide and bone geometry to represent differences between segmentation and actual patient geometry. Gene expression in mammalian cells requires complex nuclear choreography, and there is increasing evidence that spatiotemporal organization of chromatin and nuclear compartments plays an important role in gene expression. In this dissertation, I examined the function of SON, a splicing factor with a known role in nuclear organization, in chromatin-mediated gene expression. SON association with a transcriptionally inactive U2OS 2-6-3 reporter gene array provided a useful model to study SON’s chromatin dynamics. I demonstrated that SON associates with the inactive but not the activated array, and that SON’s RNA binding domains are not necessary for that association. Second, I discovered …
