Background: Matrix metalloproteinase (MMPs) are proteins that have a unique role in immunity, tissue remodeling, and tumorigenesis. MMPs can break down extracellular components, release bioactive molecules, and act on pro-inflammatory cytokines, chemokines and other proteins to regulate varied aspects of inflammation and immunity. Nicotine (3-(1-methyl-2-pyrrolidinyl) pyridine), a key toxic component of tobacco, is thought to dysregulate matrix metalloproteinase secretion in immune cells. Aim: Examine the effect of nicotine on MMP9 secretion and activity in primary monocytes. P. gingivalis is a major periodontal disease causing pathogen. Gingival epithelial cells being the first line of defense, interact and internalize the bacteria evoking a cell signaling response that finally leads to the release of cytokines. IL-6 is a potent pro- inflammatory cytokine, which is one of the responsible factors for bone resorption, a clinical manifestation of periodontitis. The experimental model study hypothesizes that P. gingivalis infection increases the production of IL-6 in telomerase immortalized gingival keratinocytes (TIGKs) through up-regulation of c-Jun (AP-1 reporter). Chlorhexidine mouth rinses remove dental plaque from teeth, preventing dental caries, dental decay, and more serious systemic infections. Tooth discoloration due to extrinsic staining is the most prominent side effect of regular chlorhexidine use. Decreasing the concentration of chlorhexidine reduces staining area and severity. Staining can also be prevented and treated by tooth bleaching, a common cosmetic dental procedure that diminishes extrinsic staining by oxidizing chromagens adsorbed onto the tooth surface. In the proposed trial, common bleaching agents will be investigated for their efficacies in preventing chlorhexidine staining. Often oral care is overlooked during the daily care of adults with intellectual and developmental disabilities (IDD). Due to the integration of adults with IDD from institutions into group homes and private homes, caregivers need to be taught to implement daily oral hygiene protocols. Introduction: This study compared the clinical efficacy of panoramic imaging and Cone Beam Computed Tomography (CBCT) in the optimal placement of virtual dental implants in the posterior edentulous bounded regions of the jaws. Material and methods: From a retrospective audit of patient records, fifty-two subjects were recruited with sixty-one dental implant sites in the maxilla and mandible. Physical measurements of the residual alveolar ridge were performed and consensus decisions of optimal implant length and bone graft necessity were obtained using reformatted panoramic alone (RP) or cross-sectional imaging (XS). Artifacts due to high-density objects (HDO) such as metallic dental restorations on maxillofacial cone beam computed tomographic (CBCT) images can render certain areas unsuitable for diagnosis. It was hypothesized that image quality due to HDO artifacts was affected by CBCT acquisition parameters and the number and configuration of HDO. Simulated complete maxillary and mandibular dental arches were constructed using dental stone and extracted teeth. Conservative coronal dental amalgam (MOD) restorations on premolar and molar teeth were used as HDOs. A gingival crevice model (epithelial cell-Porphyromonas gingivalis-neutrophil) was established and used to profile gingipain, matrix metalloproteinase (MMP), MMP mediators [neutrophil gelatinase-associated lipocalin (NGAL) and tissue inhibitor of metalloproteinases 1 (TIMP-1)] and cytokine networks. Smoking is the primary environmental risk factor for periodontitis. Therefore, the influence of cigarette smoke extract (CSE) was also monitored in the same model. Porphyromonas gingivalis alone induced low levels of interleukin-1b and interleukin-8 from epithelial cells, but high levels of both cytokines were produced on the addition of neutrophils. Background: Multipotential precursor cell lines derived from human bone marrow, capable of differentiating into cartilage or bone, may provide a useful tissue development model for studying the regulation and metabolism of putative growth and differentiation factors necessary for tissue regeneration. In mammals, the process of bone development depends on the proliferation and differentiation of osteoblast lineage cells, and the subsequent synthesis and mineralization of bone extracellular matrix (ECM). Purpose: The purpose of the study was to compare the effectiveness of the VELscope® Vx, versus a visual and tactile intraoral examination in detecting oral lesions in an adult, high risk population. Methods: A convenience sample of 30 participants (17 cigarette smokers and 13 dual addiction smokers) was enrolled. For the purpose of this study, dual addition was defined as cigarettes plus hookah usage. Two trained and calibrated dental hygienists conducted all examinations. Visual and tactile intraoral examinations were conducted, followed by VELscope® Vx florescence examinations. Targeting RNAi pathways provides a powerful tool to change biological processes post-transcriptionally in various health conditions such as cancer or autoimmune diseases. Background: Adult oral health has been identified as a major public health problem by the United States Surgeon General. A substantial number of adults between the ages of 20 and 64 have untreated tooth decay. There are limited treatment options for adults without dental insurance. Low-income adults must turn to free/low cost clinics, or private dentists that accept Medicaid to receive dental care. However, these resources are often limited. When the resources are not available these adults are left to deal with untreated tooth decay which can lead to worsening conditions and other health issues.

As the number of older adults in the United States continues to grow, this change in patient demographics will profoundly affect the practice of dental hygiene. Not only are there more older adults, but they are also retaining more of their natural teeth than ever before. The risk of alveolar bone loss; gingival recession, which can lead to exposed cervical and root areas of the teeth; and tooth wear all increase with age. The confluence of these factors raises the risk of noncarious cervical lesions (NCCLs).

This study examines the inter-relation between enamel morphology and crack resistance by sectioning extracted human molars after loading to fracture. Cracks appear to initiate from tufts, hypocalcified defects at the enamel–dentin junction, and grow longitudinally around the enamel coat to produce failure. Microindentation corner cracks placed next to the tufts in the sections deflect along the tuft interfaces and occasionally penetrate into the adjacent enamel. Although they constitute weak interfaces, the tufts are nevertheless filled with organic matter, and appear to be stabilized against easy extension by self-healing, as well as by mutual stress-shielding and decussation, accounting at least in …

Keratocystic odontogenic tumors (KCOTs, previously known as odontogenic keratocysts) are aggressive, noninflammatory jaw lesions with a putative high growth potential and a propensity for recurrence. This article puts together a summary of the serial studies related to KCOTs undertaken by the author's research group in recent years. Intraosseous jaw cysts with a solely orthokeratinized lining epithelium have been suggested to differ from the typical KCOTs. We report 20 cases of such cyst type under the term of 'orthokeratinized odontogenic cyst (OOC)'. Odontogenic keratocysts are relatively common lesions that may occur in isolation or in association with nevoid basal cell carcinoma syndrome (or Gorlin syndrome). The PTCH gene has been reported to be associated with Gorlin syndrome. We investigated 10 cases of non-syndromic keratocysts and two other cases associated with Gorlin syndrome, looking for PTCH mutations. Four novel and 1 known PTCH mutations were identified in five individual patients. Of the 5 mutations identified, 2 were germ-line mutations (2619C>A; 1338_1339insGCG) in 2 cysts associated with Gorlin syndrome, and 3 were somatic mutations (3124_3129dupGTGTGC; 1361_1364delGTCT; 3913G>T) in 3 non-syndromic cysts. Objectives: PTCH, the human homologue of the Drosophila segment polarity gene, patched, has been identified as the gene responsible for nevoid basal cell carcinoma syndrome. The aim of this study was to investigate PTCH gene mutation in Chinese patients with nevoid basal cell carcinoma syndrome. Materials and methods:  DNA was isolated from both odontogenic keratocyst tissue and peripheral blood of five patients with syndrome and one patient with only multiple odontogenic keratocysts, and mutational analysis of the PTCH gene performed by direct sequencing after amplification of all 23 exons by polymerase chain reaction (PCR). OBJECTIVE: To investigate PTCH gene mutations in odontogenic keratocysts (OKC). METHODS: PCR-SSCP and DNA sequencing were used to analyze the PTCH gene mutations in 12 OKCs, including 10 sporadic and 2 nevoid basal cell carcinoma syndrome (NBCCS) associated OKC. RESULTS: Four mutations were identified in 4 cysts, among which two germline mutations were associated with NBCCS and 2 somatic mutations were in 2 unrelated sporadic cases. In addition, eight previously reported polymorphisms in the PTCH gene were also found in 10 cases. CONCLUSION: The present study indicated that both sporadic and NBCCS-related OKCs could carry PTCH gene mutation. Thus, mutational …

Apical periodontitis is the most common pathological process in the periapical region. Disciplined clinical and radiographic evaluations and appropriate diagnostic tests can detect lesions related to apical periodontitis. Aside from this, lesions mimicking pulp-related pathology but unrelated to pulpal infection and necrosis are occasionally discovered in the periapical region.
