Skeletal Class III with anterior open bite is one of the most challenging problems that orthodontists may encounter. Treatment modalities for skeletal openbite malocclusion include growth modification and surgicalorthodontic therapy; which treatment is chosen depends on the patient’s age and growth potential. Treatment considerations should focus on the patient’s facial profile, skeletal pattern, growth potential, and severity of dental malocclusion. Introduction: The purpose of this study was to investigate the correlation between the actual linear measurements on human skull and the cephalometric measurements obtained from conventional digital cephalogram and cone-beam computed tomography (CBCT) 3-D images. Materials and Methods: Lateral and postero-anterior cephalograms of 3 skulls were assessed by measuring the commonly used 11 linear measurements. The CBCT volume images were obtained and measured by using 3-D image software (i-CAT/ Simplant Pro 12.02). In treating patients with facial asymmetry, orthognathic surgery is often needed in combination with orthodontic treatment. Conventional surgical treatment planning uses dental casts and two-dimensional radiography to predict the amount of movement needed. However, the real movement modeled on the mounted casts on the articulator cannot be simulated accurately based on the two-dimensional radiographs. In this case report, we presented a new method using three-dimensional casting model for surgical planning and design. The iatrogenic injuries, which often lead to medical tangles, have been widely discussed in recent years.The areas of intraoral iatrogenic injuries in orthodontics include both hard tissues and soft tissues: tooth, alveolar bone, gingiva, oral mucosa, nerve and so on. Almost every procedure during orthodontic therapy could result in unpredictable injuries. To avoid negative effects, the clinicians should understand all causes of various iatrogenic injuries, and take measures during orthodontic treatment. This article discusses the etiologies, prevention and management of intraoral injuries, and it may help for clinicians.

A case was presented with an adult male patient diagnosed as skeletal class III and dental class I malocclusion, and complaining of his dental crowding. Undergone orthodontic related examinations and periodontal evaluations, the treatment plan was to combine atypical extraction with insertion of mini-implants into mandible, in order to facilitate control of anchorage. After the treatment, lateral profile and occlusion were both apparently improved. Discussions were made about usage of mini-implants in this case and other possible ways to treat the patient.

Correction Of A Lingual Tipped Mandibular Second Molar With Miniscrew Orthodontics – Case Report, Chun-Hung Yu, Sam Sheng-Pin Hsu, Yang-Ming Chang

A case of a 14 y/o male patient with a lingually impacted mandibular right 2nd molar was presented. After the third molar was extracted, a closed coil spring was used to connect the impaction to a miniscrew inserted in the nearby buccal shelf. Throughout the treatment process, all orthodontic appliances were hidden underneath the flap. The advantage of this closing flap technique is that it improves patient comfort while minimizing irritation to the surround muscle and soft tissue. Treatment options for uprighting lingually impacted mandibular 2nd molar, clinical considerations of orthodontic miniscrew placement, and design of biomechanics were also discussed.

Maxillary canines are essential for function and esthetics. Patients with impacted maxillary canine are considered to be more difficult to treat than the average orthodontic patients because successful treatment requires proper surgical technique and orthodontic mechanisms. This case report describes the treatment of a 10-year-old boy with a palatally impacted canine and rotated premolar. Using closed exposure with placement of an attachment, following by distal orthodontic force to bring the canine crown away from the root of lateral incisor, then buccal traction into normal occlusion. Purpose: The purpose of this study was to evaluate the possible factors that affected the duration of forced eruption of impacted upper incisors. Methods: 24 patients (mean age eight years and nine months old ), 25 upper impacted incisors were included in this study. Factors analysed included the age, the distance from impacted incisor edge to the occlusal plane, presence of odontoma or supernumerary tooth, the crown orientation,eruption space loss and root dilaceration. Anterior crossbite is a common clinical problem that requires early diagnosis and proper treatment. Here we report on a pair of siblings with anterior crossbite. They have a similar dental and facial profile at the start of treatment, but developed different types of dental facial patterns on follow-up. Our report highlights that it is difficult to precisely predict the facial growth simply based on patient’s family history and the data of cephalometric analysis. When we notice missing maxillary incisors, how to restore the extraction site is a difficult decision. Usually, we restore this kind of tooth with conventional fixed partial denture or implant prosthesis. However, some literatures present another solution: tooth substitution with proximal tooth by orthodontic force. During orthodontic tooth movement, bone remodeling is induced by mechanical stimulation. The mechanical regulation to stimulate the activities of osteoblasts and osteoclasts is a focus of bone studies, which also got more attention in orthodontics recently. Members of matrix metalloproteinase family (MMP) were found to be closely related to the bone remodeling during orthodontic treatment. The etiology of skeletal Class II malocclusion may include excessive growth of the maxilla, deficient growth of the mandible, or the combination of both. Oral habits such as mouth breathing, tongue thrusting, and thumb sucking can cause anterior open bite. Meanwhile, the vertical overgrowth of posterior dentoalveoalr process may be another contributory factor. The treatment options for skeletal Class II malocclusion with anterior open bite should be planned according to the discrepancy of the inter-jaw relationship. A stable mini-implant provides a splendid orthodontic anchorage, which leads a profound influence to the treatment course. This study used finite element method, with a molar and premolar on the posterior mandible by computerized simulation, to investigate the biomechanical effects of the orthodontic mini-implant. Factorial analyses were performed including the cortical bone thickness, the diameter of the mini-implant and angle of the orthodontic force. The results showed that the diameter of the mini-implant affected the stress distribution on the cortical bone. All parameters had significantly effects on the cancellous bone stresses. The main characteristics of craniosynostosis were premature fusion of cranial sutures with specific skull shapes. There are divided into nonsyndromic and syndromic craniosynostosis. Syndromic craniosynostosis (SCS) was an inherited congenital craniofacial disorder and approximately 15% of craniosynostosis had associated anomalies in the face and limbs. Crouzon syndrome, Apert syndrome, Pfeiffer syndrome and Jackson-Weiss syndrome were the examples of SCS. A case of mandibular prognathism with facial asymmetry was presented. The missing left central incisor was previously restored with a compromising cantilever bridge. Interdisciplinary approach including orthodontics, orthognathic surgery, and prosthetic dentistry was performed and gave patient a better esthetic and functional result. Ways to achieve better esthetics, evaluation and possibility of surgery-first orthognathic approach, and other possible treatment options were discussed.

Facial asymmetry may originate from mandibular deviation combined with secondary maxillary compensation in vertical dimension, which can lead to occlusal plan canting. Two-jaw surgery would be an ideal choice for these cases in the past. Nowadays, using mini-implants to intrude elongated maxillary teeth to correct the occlusal plane canting could simplify the surgical plan into one-jaw surgery. The aims of this retrospective cephalometric study were to assess the results of presurgical treatment on the subjects presenting with Class III malocclusion and to evaluate the correlation between soft and hard tissues. Initial (T0) and presurgical (T1) lateral cephalograms of 17 patients were examined. Mean age was 20.4 years initially. Subjects were treated with presurgical orthodontic treatment. Initial and presurgical variables were compared using paired t-test, and the relationship of soft and hard tissue variables was studied using Pearson correlation coefficient and linear regression equation.
