Titratable, Weight-Based Cisatracurium Infusions For Ventilation Synchrony In Ards, Nanako Miimi, Pharmd, Celine Munoz
Books, Presentations, Posters, Etc.
Abstract: Cisatracurium is a non-depolarizing neuromuscular blocking agent used for paralysis in paitents with acute respiratory distress syndrome on mechanical ventilation. Paralysis prevents ventilator dyssynchrony and reduces patient’s work of breathing, which decreases the risk of pneumothorax. On November 1, 2023, PSVMC critical care units transitioned from fixed dosing of cisatracurium for ARDS to a weight-based, titratable dosing protocol. A single-center retrospective cohort study demonstrated cisatracurium titrated using train-of-four was associated with similar ventilatory and clinical outcomes with three folds reduction in cisatracrurium dose in comparison to fixed dose strategy. The purpose of this project is to evaluate the drug …

Electronic Theses and Dissertations
Pancreatic cancer is one of the deadliest cancers to treat. The primary cause for the drug resistance is the dense extracellular matrix (ECM) that surrounds the tumor and restricts blood supply. Gemcitabine (GEM) and paclitaxel (PTX) are drugs that are clinically used in the treatment of pancreatic cancer. GEM, however, is rapidly metabolized by cytidine deaminase (CDA) and eliminated from the body, and paclitaxel is a substrate for P- glycoprotein (P-gp) efflux receptors. Drug delivery using nanoparticles has been reported to help improve drug half-lives and bypass efflux receptors. This thesis aims to encapsulate PTX in PLGA nanoparticles (PTX-NP) and …

Pharmacy
Introduction: Phosphodiesterase 7 (PDE7) is a high-affinity cyclic AMP (cAMP)- specific PDE that is expressed in immune and proinflammatory cells. In this work, we explore the possibility that selective small molecule inhibitors of this enzyme family could provide a novel approach to alleviate the inflammation that is associated with many inflammatory diseases. Methods: A series of novel substituted 4-hydrazinoquinazoline derivatives and fused triazoloquinazolines were designed, synthesized, and evaluated in vitro for their PDE7A inhibition activities, in comparison with Theophylline, a non-selective PDE inhibitor, and BRL50481, a selective PDE7A inhibitor. This series of novel quinazoline derivatives were synthesized via multi-step reactions. …

Neuropathic pain is a critical source of comorbidity following spinal cord injury (SCI) that can be exacerbated by immune-mediated pathologies in the central and peripheral nervous systems. In this article, we investigate whether drug-free, biodegradable, poly(lactide- co -glycolide) (PLG) nanoparticle treatment mitigates the development of post-SCI neuropathic pain in female mice. Our results show that acute treatment with PLG nanoparticles following thoracic SCI significantly reduces tactile and cold hypersensitivity scores in a durable fashion. Nanoparticles primarily reduce peripheral immune-mediated mechanisms of neuropathic pain, including neuropathic pain-associated gene transcript frequency, transient receptor potential ankyrin 1 nociceptor expression, and MCP-1 (CCL2) chemokine …

With growing resistance to vaccinations-premised, for instance, on misplaced fears of side effects or religious objections-it takes little effort to imagine that a state might act to prohibit some or all uses of a particular vaccine licensed by the federal government. Indeed, one year ago, legislators in a couple of states toyed with the idea of banning the primary vaccines against Covid-19, and, twenty years ago, half a dozen states effectively barred certain uses of one flu vaccine formulation. Although federal law should preempt at least some of these types of state restrictions, the Supremacy Clause of the U.S. Constitution …

Despite newfound attention to how artificial intelligence (AI) may accelerate pharmaceutical development, federal regulators may find that current statutes are ambiguous or silent about their applicability to AI-based drug design and manufacturing methods. This poses a serious problem in the era of Loper Bright and the Major Questions Doctrine. As federal agencies struggle to adjust to courts’ growing demand for Congress to craft clear, explicit, and express delegations of authority, this note develops a statutory framework in which the Food and Drug Administration (FDA) would have more flexibility to regulate the use of AI in advanced drug manufacturing. Guided by …

Conventional bottom-up synthesis of carbon quantum dots (CQDs) often requires extended reaction times, high energy input, and specialized equipment, limiting scalability and sustainability. In this study, we present an eco-friendly and energy-efficient method for synthesizing CQDs using H₂CO₃ as a carbon precursor at just 72 °C for 1 hour—representing one of the lowest reported synthesis temperatures and shortest reaction times using simple apparatus. Graphene oxide vacancies act as catalytic and nucleation sites, promoting the formation of CQDs under these mild conditions. The resulting CQD solution exhibits strong yellow photoluminescence, with a maximum emission at 533 nm and excitation-independence across the …

Cannabidiol (CBD), a major phytocannabinoid present in Cannabis sativa, possesses multiple effective pharmacological activities, such as neuroprotective, cardioprotective and anti-inflammatory properties. However, its hydrophobic nature and rapid degradation mechanisms limit its clinical usefulness. Encapsulation of lipophilic compounds inside modified polymers with hydrophilic properties can improve the aqueous solubility and stability of drugs in specific environments. Therefore, this study focused on self-assembled poly N-isopropyl acrylamide (pNIPAM) grafted hyaluronic acid (HA) nanogels for encapsulating CBD. At the highest concentration 0.25 % w/w of HA-g-pNIPAM, encapsulation efficiency (% EE) of CBD 87.57 % was achieved compared to other two formulations 0.15 % w/w …
