Abstract
Local Problem: The Concord Hospital system, serving 30+ communities, is situated in the heart of New Hampshire surrounded by densely wooded forests, lakes, and mountains and is a mecca for camping, vacationing, and sightseeing, doubling its population of 1.6 million residents during spring, summer, and fall months. Despite the serenity, there is a predator that lingers among the hiking trails and lake shorelines. From January 2021 through August 2022 there were a total of 41 hospitalizations from tick-borne diseases, some with life threatening symptoms. Unfortunately, nearly half (43%) of patients presented to their primary care provider first where the …
Increasing Access And Adherence To Cognitive Behavioral Therapy For Insomnia (Cbti) For Adults: A Quality Improvement Project, Kristen Newell
Increasing Access And Adherence To Cognitive Behavioral Therapy For Insomnia (Cbti) For Adults: A Quality Improvement Project, Kristen Newell
DNP Scholarly Projects
BACKGROUND: Insomnia is a sleep disorder that is becoming more prevalent. It can significantly impact a person's quality of life and it contributes to a substantial economic financial burden. Cognitive behavioral therapy for insomnia (CBTi) is the primary treatment for insomnia, but patients are rarely referred for this treatment. This quality improvement project focused on implementing motivational interviewing to increase patient utilization of CBTi. It also encouraged utilization of CBTi Coach, a free phone application that has all of the CBTi components, a rigid sleep behavioral plan and frequent follow-up appointments to improve sleep quantity and quality.
INTERVENTION: Patients that …
Decreasing Post-Operative Ventilator Time In The Cardiovascular Surgical Patient: A Nursing Education Quality Initiative, Amanda J. Bruno Roberts
Decreasing Post-Operative Ventilator Time In The Cardiovascular Surgical Patient: A Nursing Education Quality Initiative, Amanda J. Bruno Roberts
DNP Scholarly Projects
Extended mechanical ventilation times after open heart surgery can contribute to complications prolonging recovery and length of stay, as well as increasing mortality. Standardized staff education of immediate post-open-heart recovery and ventilator weaning protocols, combined with the use of ABCDE and VAP bundles can reduce these complications. At Hospital X during summer of 2022, the average length of time patients remained mechanically ventilated after open heart surgery increased. After an initial needs survey was distributed to nursing staff, a set of novel QR codes was created to simplify and centralize training for staff in the ICU to facilitate early extubation …
Modifiable Health Disparities And Cardiovascular Disease Risk Factors In Latino Migrant Seasonal Farmworkers: A Meta-Analysis, Cindy Mendez
Modifiable Health Disparities And Cardiovascular Disease Risk Factors In Latino Migrant Seasonal Farmworkers: A Meta-Analysis, Cindy Mendez
Nursing Theses and Dissertations
BACKGROUND: Cardiovascular disease is the leading cause of death in the United States (U.S.). Existing literature concluded that Latino migrant seasonal farmworkers endure higher rates of adverse health disparities and chronic diseases than the general U.S. population. However, research on the Latino migrant seasonal farmworkers’ modifiable health disparities (i.e., alcohol use and smoking) and cardiovascular disease risk factors (i.e., high cholesterol, diabetes mellitus, hypertension, and overweight/obesity) is scarce, particularly studies comparing male and female Latino migrant seasonal farmworkers. PURPOSE: This meta-analysis had two aims. First, examine whether male or female Latino migrant seasonal farmworkers in the U.S. have …

Significance: Certified nursing assistants (CNAs) provide direct care to those who live in long-term care facilities. As the population ages, the need for CNAs has grown. Nurse researchers have studied turnover and occupational stress, but no studies have examined racial/ethnic differences in CNA stress in East Texas.
Design / Purpose Statement: A convergent parallel mixed methods study was planned to address occupational stress experienced by CNAs in East Texas long-term care facilities. The Work Stress Scale and demographic questions were used to compare levels of reported occupational stress between racially diverse CNAs. However, lack of response to interview invitations necessitated …
The Relationship Between Perceived Stress And Academic Self-Efficacy To The Academic Performance Of Undergraduate Nursing Students, Ashley J. Pierre
The Relationship Between Perceived Stress And Academic Self-Efficacy To The Academic Performance Of Undergraduate Nursing Students, Ashley J. Pierre
Nursing Theses and Dissertations
When nursing students fail and are required to repeat a course in a pre-licensure nursing program, they are at risk for attrition. While nursing student attrition is a problem with recognized consequences to the student, school, and profession, the incidence of students who fail a required course and need to repeat it has had little attention. Despite research addressing students' experiences with course failure, little describes strategies and interventions to support the retention and success of these students. The first manuscript, Student Repeaters in Baccalaureate Nursing Programs: A Concept Analysis, provided a foundation that enhanced the conceptual understanding of nursing …

Quality Improvement Project To Assist In Implementation Of Prediabetes Screen Protocol, Kyai Forney
Quality Improvement Project To Assist In Implementation Of Prediabetes Screen Protocol, Kyai Forney
Doctoral Projects
Prediabetes is a reversible condition that causes blood glucose levels to be higher than normal, but not high enough to be diagnosed as type 2 diabetes. When one has prediabetes, the cells in the body do not respond normally to insulin, and insulin acts like a key to letting blood sugar into cells to use energy. The pancreas produces insulin to try to get the cells to respond, but eventually, the pancreas has a challenging time keeping up, and the blood sugar level rises causing prediabetes (Lehrman, 2021). A person can have prediabetes for years with no obvious symptoms, so …

An Examination Of The Trends In Activities Of Daily Living In The 12 Months After Total Knee Arthroplasty: A Descriptive Study, Irene Van Staden
Theses
Background
The current trend to reduce the length of stay for patients undergoing total knee arthroplasty, together with the steadily increasing number of procedures being performed, has impacted on the functional independence goals expected by patients and the ability of nurses to deliver adequate education prior to discharge. Over the past 10 years length of stay has reduced from 7-10 days to 3-4 days and patients are expected to largely self-manage their post-operative rehabilitation and optimise their functional outcomes.
Patient feedback confirmed that patients do not feel ready for discharge and lack the ability to self-manage some activities of daily …

'Crying Wolf': Generating New Understandings Of Alarm Fatigue Using A Convergent Mixed Methods Approach, Meagan Rae Vidyaeva
Theses
Introduction: Alarm fatigue is a consequence of sensory overload due to frequent nuisance alarms from physiologic monitoring devices. This ‘cry wolf’ phenomenon occurs frequently in the context of continuous cardiac monitoring and ST segment monitoring.
Research significance: The effect of these alarm types on nurses’ perceived alarm fatigue in Australian settings is yet to be examined. Accordingly, this thesis explores registered nurses (RN’s) perception of alarm fatigue. The effect of ST segment monitoring on perceived alarm fatigue, as well as the necessary resources for RNs to reduce alarm fatigue is also a focus of this thesis.
Methodology: The research was …

Gender Differences In Administration Of Tpa In Treatment Of Ischemic Stroke, Christina Annerino
Gender Differences In Administration Of Tpa In Treatment Of Ischemic Stroke, Christina Annerino
CURE Proceedings
As medicine and pharmacology advance through the years, new life-saving treatments are studied or discovered every day, and a medical emergency is no longer a death sentence. Even with conditions as serious as ischemic stroke, there is hope for survival and rehabilitation with the ‘clot-busting’ drug, tissue plasminogen activator, colloquially known as ‘tPA’. tPA is a thrombolytic agent, a substance that acts on fibrin in clots to dissolve them so they can no longer cause ischemia in blood vessels that results in a stroke. (Vega, 2022). tPA is an extremely effective treatment for ischemic stroke, demonstrated in 2013 by a …

Chronic Anemia Revealing An Idiopathic Watermelon Stomach: Case Report, Hanane Delsa, Imane Rahmouni, Yassamin Benhayoun Sadafyine, Fatima Belabbes, Anass Nadi, Fedoua Rouibaa
Health Sciences
Watermelon stomach, also known as gastric antral vascular ectasia (GAVE) syndrome, is a rare entity. Patients often present with profound unexplained anemia with or without bleeding externalization. We made the diagnosis during a digestive endoscopy for an etiological assessment of this anemia. Endoscopically, antral erythematous lesions in stripes with a punctate appearance, sometimes hemorrhagic, may suggest a watermelon stomach. The pathophysiology of the watermelon stomach remains complex and unclear. Although several pathologies remain implicated, sometimes, it can be idiopathic. The effectiveness of numerous treatments has been evaluated with relatively satisfactory results. This is a rare case of an idiopathic watermelon …

Heparin Vs. Saline: Evaluating Evidence-Based Best Practice For Maintaining Patency Of Implanted Ports, Shannon D. Murdock
Heparin Vs. Saline: Evaluating Evidence-Based Best Practice For Maintaining Patency Of Implanted Ports, Shannon D. Murdock
Master's Theses and Capstones
Background Implanted ports are frequently used for central venous access in the adult oncology population. Maintaining patency of these ports is important for continuing uninterrupted treatment and supportive care for oncologic patients. Historically, heparin solution has been used as a locking solution for implanted ports to prevent occlusion. However, evidence shows that 0.9% sodium chloride (normal saline or NS) as a lock solution is equally effective as heparin solution at maintaining implanted port patency and poses less risk to patients.
Local Problem In an ambulatory adult oncology clinic serving Seacoast New Hampshire, a gap was identified between clinical practice where …
Incorporating Non-Pharmacological Labor Coping Methods To Improve Nursing Care And Reduce Primary Cesarean Section Rates: A Quality Improvement Project, Cait Maccabe
Master's Theses and Capstones
Abstract
BACKGROUND: Cesarean sections (c-sections), while life-saving in some circumstances, carry a significantly higher risk of morbidity and mortality than vaginal birth for birthing patients and neonates alike. Despite this, rates of c-section have continued to rise in the US and around the world. As primary cesarean births are highly likely to lead to repeat cesarean births, concerted efforts are being made to reduce them both broadly and within the setting of this quality improvement (QI) project: a labor and delivery unit located within a Level II trauma center in New Hampshire. A literature review provided evidence supporting the use …
