ความต้านทานแบบไดอะมีทรัลเทนไซล และ ความต้านทานต่อแรงอัดของวัสดุสร้างแกนฟันชนิดต่างๆ, แมนสรวง อักษรนุกิจ, พรพิมล ชรากร, เพ็ญพรรณ ตรงสวัสดิ์
Chulalongkorn University Dental Journal
วัตถุประสงค์ ศึกษาเปรียบเทียบความต้านทานแบบไดอะมีทรัลเทนไซล (Diametral tensile strength, DTS) และความต้านทานต่อแรงอัด (Compressive strength, CS) ของวัสดุสร้างแกนฟัน วัสดุและวิธีการ ชิ้นงานจํานวน 112 ชิ้น เตรียมจากวัสดุ 7 ชนิด ได้แก่ บิสคอร์ (Biscore), ไทคอร์ (Ti-core),ลักษ์อัลลอยด์ (Luxalloy), คอมโพกลาส (Compoglass), ไดแรค (Dyract), ไวทริเมอร์ (Vitremer) และดีแทคโมลาร์ (Ketac Molar) ในจํานวน 112 ชิ้น 56 ชิ้น (8 ชิ้นต่อวัสดุ 1 ชนิด) มีเส้นผ่าศูนย์กลาง 6 มิลลิเมตร หนา 3 มิลลิเมตร ใช้สําหรับทดสอบความต้านทานไดอะมีทรัลเทนไซล และอีก 56 ชิ้น (8 ชิ้นต่อวัสดุ 1 ชนิด) มีเส้นผ่าศูนย์ กลาง 3 มิลลิเมตร หนา 6 มิลลิเมตร ใช้สําหรับทดสอบความต้านทานต่อแรงอัด แซ่ชิ้นงานทั้งหมดในน้ํากลั่น และ เก็บไว้ที่อุณหภูมิ 37 องศาเซลเซียส ความชื้นสัมพัทธ์ 100% เป็นเวลา 24 ชั่วโมง ก่อนการทดลอง นําชิ้นงาน มาหาค่าความต้านทานไดอะมีทรัลเทนไซล และความต้านทานต่อแรงอัด ด้วยเครื่องลอยด์ยูนิเวอร์ซัลเทสติ้ง [Lloyd Instruments, LR 10K (Lloyd Instruments Ltd., Segensworth, Fareham)] ด้วยความเร็วหัวทดสอบ 5 มิลลิเมตร ต่อนาทีผลการศึกษาและสรุป จากการทดสอบทางสถิติโดยใช้ ANOVA พบความแตกต่างของความต้านทานไดอะ มีทรัลเทนไซล และ ความต้านทานต่อแรงอัดระหว่างวัสดุอย่างมีนัยสําคัญทางสถิติที่ p<0.001 จากการศึกษาพบว่า วัสดุจําพวก กลาสไอโอโนเมอร์ชนิดดัดแปลงด้วยเรซิน มีค่าความต้านทานไดอะมีทรัลเทนไซล และ ความต้านทานต่อ แรงอัดต่ํา ไม่เหมาะสมกับการนํามาใช้สร้างแกนฟัน

วัตถุประสงค์ การศึกษานี้มีวัตถุประสงค์เพื่อสํารวจความต้องการการบําบัดรักษาทางทันตกรรม และปริมาณ การได้รับการบําบัดรักษาทางทันตกรรมของผู้ป่วยสูงอายุที่เข้ามารับการรักษาในคลินิกของคณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย วิธีการศึกษา วิธีการสํารวจทําโดยการเก็บรวบรวมข้อมูลสภาวะทางทันตกรรมและความต้องการการบําบัด รักษาทางทันตกรรมของผู้ป่วยสูงอายุจากแฟ้มผู้ป่วยสูงอายุที่มารับการรักษาในคณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัยในช่วงปีพ.ศ. 2533-2537 เป็นจํานวนทั้งหมด 1,354 ราย การวิเคราะห์ข้อมูลได้จาก การแบ่งกลุ่มศึกษาออกเป็น 5 กลุ่มช่วงอายุ โดยนําข้อมูลที่บันทึกได้มาคํานวณเป็นร้อยละและเปรียบเทียบ ความสัมพันธ์ของข้อมูล ผลการทดลองและสรุป ผลการสํารวจพบว่าจํานวนผู้ป่วยสูงอายุมีแนวโน้มเพิ่มขึ้นอย่างรวดเร็ว ความ ต้องการบําบัดรักษาพิจารณาจากคําบอกกล่าวสําคัญ ซึ่งพบว่ามีปัญหาเกี่ยวกับฟันปลอมคิดเป็นร้อยละ 20.56 รองลงมาคือฟันหายไปคิดเป็นร้อยละ 18.35 และปวดฟันคิดเป็นร้อยละ 12.46 สําหรับงานบริการทาง ทันตกรรมที่ได้รับการรักษามากที่สุดคืองานทันตกรรมประดิษฐ์คิดเป็นร้อยละ 32.69 รองลงมาคือถอนฟันและ ศัลยกรรมในช่องปากคิดเป็นร้อยละ 14.08 ประเภทของงานทันตกรรมประดิษฐ์ชนิดฟันปลอมทั้งปากมีแนวโน้ม ลดลงจากร้อยละ 68.35 ในปีพ.ศ.2533 เป็นร้อยละ 48.88 ในปีพ.ศ.2537 ส่วนการใส่ฟันปลอมบางส่วนมีแนวโน้มเพิ่มขึ้นจากร้อยละ 31.01 ในปีพ.ศ. 2533 เป็นร้อยละ 50.00 ในปีพ.ศ.2537 แสดงให้เห็นว่าผู้ป่วย สูงอายุมีแนวโน้มของการมีสันเหงือกว่างลดลงแต่มีฟันธรรมชาติเหลืออยู่มากขึ้น

วัตถุประสงค์ การวิจัยนี้มีวัตถุประสงค์เพื่อหาค่าเฉลี่ยมาตรฐานขององค์ประกอบรอยยิ้ม อันได้แก่ รูปแบบ ของรอยยิ้ม ความหมายของโค้งปลายฟันหน้าบนกับริมฝีปากล่าง ตําแหน่งของโค้งปลายฟันหน้าบนที่สัมผัสกับ ริมฝีปากล่าง และบริเวณโปร่งแสงของฟันหน้าบน วิธีการศึกษา ทําการศึกษาในประชากรกลุ่มตัวอย่างที่คัดเลือกมาจากนิสิตคณะทันตแพทยศาสตร์ จุฬาลงกรณ์ มหาวิทยาลัย ชั้นปีที่ 3 ซึ่งมีอายุระหว่าง 19 ปี 22 ปี ทั้งเพศชายและเพศหญิง จํานวน 100 คน โดยเลือกผู้ที่มี ฟันหน้าบนครบทุกซี่ ไม่มีช่องว่างระหว่างฟัน และมีการเรียงตัวของฟันอยู่ในแนวปกติ โดยให้กลุ่มประชากร ตัวอย่างยิ้มอย่างเต็มที่ภายใต้แสงไฟนีออนธรรมดาทีละคน ในขณะที่มีผู้สังเกตดูองค์ประกอบต่าง ๆ ของรอยยิ้ม และบันทึกผลของนิสิตแต่ละคน จากนั้นจึงนําข้อมูลมาคํานวณเป็นร้อยละ และวิเคราะห์ข้อมูลโดยวิธีสถิติชนิดไคสแควร์ ผลการศึกษาและสรุป จากการศึกษาพบว่า ร้อยละ 49 ของกลุ่มตัวอย่างขณะยิ้มเต็มที่จะเห็นฟันหน้าบน เกือบทั้งซี่ และเห็นเหงือกสามเหลี่ยมระหว่างฟันด้วย ร้อยละ 59 มีส่วนโค้งปลายฟันหน้าบนขนานกับเส้นโค้ง ด้านในของริมฝีปากล่าง ร้อยละ 74 ปลายฟันหน้าบนจะไม่สัมผัสกับริมฝีปากล่าง และร้อยละ 77 มีบริเวณ โปร่งแสงของฟันหน้าบนอยู่ที่ปลายฟัน องค์ประกอบของรอยยิ้มที่ขึ้นกับเพศ ได้แก่ รูปแบบรอยยิ้ม และบริเวณ โปร่งแสงของฟันหน้าบน พบว่าเพศชายและหญิงมีความแตกต่างกันอย่างมีนัยสําคัญทางสถิติที่ระดับ α = 0.05 ส่วนองค์ประกอบของรอยยิ้มที่ไม่ขึ้นกับเพศ ได้แก่ ความขนานและลักษณะการสัมผัสระหว่างโค้งปลายฟันหน้าบน และริมฝีปากล่าง พบว่าเพศชายและหญิงไม่แตกต่างกันอย่างมีนัยสําคัญทางสถิติที่ระดับ α = 0.05

The crucial function of macrophages in a variety of biological processes and pathologies render these cells important targets for gene therapeutic interventions. Commonly used synthetic gene delivery vectors have not been successful in transfecting these non-dividing cells. A combination strategy involving cationic liposomes to condense and carry DNA, transferrin to facilitate cellular uptake, and the pH-sensitive peptide GALA to promote endosome destabilization, resulted in significant expression of a luciferase gene. Transfection of macrophages was dependent on the degree of differentiation of the cells. During the past ten years significant advances have been made in the development of glass-ionomer bonding adhesives. The beneficial effects of fluoride are well documented and an agent which reduces or prevents a white spot lesion that commonly occurs clinically, is desirable. There has been a notable lack of randomized clinical trials to determine the prevalence of white spot lesions after orthodontic treatment although it is often reported in the literature. วัตถุประสงค์ การผ่าตัดโดยใช้หลักการเหนี่ยวนําให้เกิดเนื้อเยื่อใหม่ (จีทีอาร์) จําเป็นต้องใช้แผ่นกั้นเพื่อป้องกันไม่ให้เซลล์ที่ไม่เกี่ยวข้องเข้าไปในรอยโรคและขณะเดียวกันช่วยให้เซลล์ที่มีหน้าที่ในการสร้างอวัยวะปริทันต์มีโอกาสทํางานได้เต็มที่มากขึ้น การศึกษาครั้งนี้ทําเพื่อต้องการดูผลการรักษาความวิการของกระดูกบริเวณง่ามรากฟันระดับ ที่สามด้วยวิธีจีทีอาร์ โดยใช้แผ่นยางกันน้ําลายเป็นแผ่นกั้นร่วมกับการใช้วัสดุปลูกกระดูกร่วมด้วย วัสดุและวิธีการ ผู้ป่วยโรคปริทันต์อักเสบในผู้ใหญ่ 6 ราย เป็นอาสาสมัครในการทําวิจัยครั้งนี้ ผู้ป่วยได้รับการ รักษาโรคปริทันต์เบื้องต้นด้วยการเกลารากฟันให้เรียบ และสามารถควบคุมอนามัยในช่องปากได้เป็นอย่างดีแล้ว และ 5 มม. ร่วมด้วย มีฟันกรามล่างซึ่งมีความวิการของกระดูกบริเวณช่องรากฟันระดับที่สามและมีร่องลึกปริทันต์อย่างน้อย โดยผู้ป่วยได้รับการผ่าตัดด้วยวิธีการ จีทีอาร์ โดยใช้แผ่นยางกันน้ําลายเป็นแผ่นกั้นร่วมกับการใส่วัสดุปลูกกระดูกร่วมด้วย วัดค่าทางคลินิก คือค่าร่องลึกปริทันต์ ระดับการยึดเกาะทางคลินิก และการรุ่นของเหงือก ก่อนและหลังการผ่าตัด 6 6 และ 12 เดือน รวมทั้งถ่ายภาพรังสีด้วย มีการเปิดเหงือกเข้าไปดูรอยโรคหนึ่งราย การวิเคราะห์การ เปลี่ยนแปลงของค่าทางคลินิกใช้ paired t-test ที่ p = 0.05 ผลการศึกษา พบว่าค่าของร่องลึกปริทันต์ และค่าของระดับการยึดเกาะอวัยวะปริทันต์ทางคลินิกลดลงอย่างมีนัย สําคัญทางสถิติเมื่อเทียบกับก่อนการผ่าตัด และมีระดับการยึดเกาะเพิ่มขึ้นเฉลี่ย 2.33 + 1.74 มม. มีระดับกระดูก เพิ่มขึ้นระดับหนึ่งแต่ไม่เต็มรอยวิการ ทางคลินิกบริเวณง่ามรากมีการปิดของเนื้อเหงือกแน่นและแข็งแรงทําให้ทําความ สะอาดได้ดีขึ้น และเมื่อเปิดเข้าไปดูพบว่า มีการสร้างของกระดูกเพิ่มขึ้นบ้าง แต่ไม่เต็มถึงยอดของง่ามรากฟัน สรุป สามารถใช้แผ่นยางกันน้ําลายเป็นแผ่นกั้นในกระบวนการจีทีอาร์ ใช้ง่าย และให้ผลดีถึงแม้มีการสร้างกระดูก เพียงบางส่วนไม่เต็มรอยวิการของกระดูกบริเวณง่ามรากฟันระดับสามก็ตาม

Pleomorphic adenoma is the most common benign salivary gland neoplasm of both the major and minor salivary glands. This review article describes the clinical features, the histopathology, the immunohistochemical markers of both the epithelial and myoepithelial components, the differential diagnosis and the treatment. The histopathology of this tumor can display diverse morphological patterns, but it traditionally presents with ducts or tubules lined by inner ductal cells and outer myoepithelial cells arranged in a variety of matrix.

วัตถุประสงค์ การศึกษานี้มีวัตถุประสงค์เพื่อดูผลการรักษารอยวิการของกระดูกเบ้าฟันหลาย ๆ รอยโรคด้วยวิธี จีทีอาร์ โดยใช้แผ่นยางกันน้ําลายเป็นแผ่นกั้น และเปรียบเทียบผลการใช้แผ่นยางกันน้ําลายเป็นแผ่นกั้นเพียงอย่างเดียว กับใช้วัสดุปลูกกระดูกร่วมด้วยในเวลา 6 เดือน วัสดุและวิธีการ ผู้ร่วมโครงการเป็นผู้ป่วยโรคปริทันต์อักเสบในผู้ใหญ่ หญิง 4 คน ซึ่งได้รับการรักษาเบื้องต้น แล้วยังมีรอยโรคปริทันต์ที่มีความลึกของร่องลึกปริทันต์ เท่ากับหรือมากกว่า 5 มม. ในทั้งสองข้างของขากรรไกร เดียวกัน 10 ตําแหน่ง เลือกการรักษาด้วยการใช้แผ่นยางกันน้ําลายอย่างเดียว 5 ตําแหน่ง จํานวน 19 รอยโรค และใช้แผ่นยางกันน้ําลายร่วมกับการปลูกกระดูก 5 ตําแหน่ง จํานวน 32 รอยโรค โดยการสุ่ม วัดค่าทางคลินิกคือ ความลึกของร่องลึกปริทันต์ ระดับการยึดเกาะของอวัยวะปริทันต์ ระดับการรุ่นของเหงือกและการถ่ายภาพรังสีโดย ทําการบันทึก ก่อนการรักษา และหลังการรักษา 3 เดือน และ 6 เดือน วิเคราะห์ข้อมูลโดยใช้ Paired t-test ใน กลุ่มเดียวกัน และ Unpaired t-test ในระหว่างกลุ่มที่ P = 0.05 ผลการศึกษา พบว่าค่าความลึกของร่องลึกปริทันต์และระดับการยึดเกาะของอวัยวะปริทันต์ ลดลงอย่างมีนัยสําคัญ ที่ 3 เดือน และ 6 เดือน ส่วนเหงือกรุ่นจะเพิ่มมากขึ้นเล็กน้อย และระดับความสูงของกระดูกเพิ่มขึ้นเมื่อตรวจดูทาง x-ray ซึ่งผลการรักษาของทั้งสองกลุ่มมีลักษณะเช่นเดียวกัน และเมื่อเปรียบเทียบผลการรักษาทั้งสองกลุ่มพบว่าไม่แตกต่างกันอย่างมีนัยสําคัญทางสถิติสรุป สรุปได้ว่าแผ่นยางกันน้ําลายสามารถใช้ เป็นแผ่นกั้นในขบวนการ จีทีอาร์ และให้ผลการรักษาไม่แตกต่างกันระหว่างใช้แผ่นยางกันน้ําลายเป็นแผ่นกั้นอย่างเดียวหรือใช้วัสดุปลูกกระดูกร่วมด้วย

วัตถุประสงค์ เพื่อสํารวจปริมาณการใช้ฟิล์มของนิสิตโดยเฉลี่ยในการรักษาคลองรากฟัน 1 ปี และในแต่ละขั้นตอน การทํางาน และวิเคราะห์ปัญหาต่างๆ ของนิสิตที่เกี่ยวข้องกับการถ่ายภาพรังสีในคลินิกวิทยาเอ็นโดดอนต์ วิธีการ สํารวจจากใบเบิกฟิล์มในแฟ้มประวัติการรักษาคลองรากฟันของผู้ป่วย ที่นิสิตเป็นผู้รักษาและจัดทําแบบสอบถาม ผลการศึกษา ปริมาณการใช้ฟิล์มในการรักษาคลองรากฟัน 1 ซี่ มีค่าเฉลี่ย 8.7 ฟิล์ม ปริมาณการใช้ฟิล์มในแต่ ละขั้นตอนการรักษามีค่าเฉลี่ย 1.12 ฟิล์มในขั้นตอนการถ่ายภาพรังสีก่อนการรักษา 2.66 ฟิล์ม ในขั้นตอนการหา ความยาวฟัน 2.49 ฟิล์มในขั้นตอนการลองกัตตาเปอร์ชาแท่งเอก และ 2.33 ฟิล์มในขั้นตอนการอุดคลองรากฟัน จากแบบสอบถามพบว่าสถานที่ วัสดุอุปกรณ์ ทั้งในแง่ของปริมาณและคุณภาพเป็นปัญหาที่นิสิตพบมาก และต้องการให้แก้ไขมากที่สุด นิสิตมีปัญหาเกี่ยวกับขั้นตอนการรักษาต่างๆ ที่ทําให้ต้องถ่ายภาพรังสีใหม่ในระดับ ปานกลาง โดยปัญหาที่พบมากที่สุดสามอันดับแรก ได้แก่ การใส่กัตตาเปอร์ชาแท่งเอกได้สั้นกว่าความยาวทํางาน การถ่ายภาพรังสีเพื่อแยกคลองรากฟันที่บังซ้อนกันออก และการล้างฟิล์มที่เข้มหรือจางเกินไป นิสิตมีความเห็นว่า กระบวนการเรียนการสอนทั้งในระดับพรีคลินิกและคลินิก ช่วยทําให้นิสิตมีความรู้และทักษะในการถ่ายภาพรังสีในผู้ป่วยจริงได้ในระดับปานกลาง

วัตถุประสงค์ เพื่อเปรียบเทียบการยึดอยู่ (Retention) และความแนบบริเวณขอบของครอบฟัน (Marginal seating) เมื่อใช้ซีเมนต์ชนิดซิงก์ฟอสเฟต (Zinc phosphate cement) ซีเมนต์ชนิดกลาสไอโอโนเมอร์ (Glass ionomer cement) และซีเมนต์ชนิดเรซิน (Resin cement) เป็นตัวยึดครอบฟัน วัสดุและวิธีการ แบ่งฟันกรามน้อย 30 ซี่ ออกเป็น 3 กลุ่ม กลุ่มละ 10 ซี่ กรอแต่งฟันแต่ละซี่ให้มีลักษณะ เหมือนกันโดยควบคุม ความสูง พื้นที่หน้าตัด และความเอียงของผนังตามแกน (Axial walls) ด้วยหัวกรอและ เครื่องควบคุมความขนาน (Milling machine) พิมพ์ฟันซี่ที่กรอแต่งมาทําครอบฟันด้วยโลหะเงินผสมเพลลิเดียม วัด ความสูงของครอบฟันโดยใช้เครื่อง Digimatic Indicator ยึดครอบฟันโลหะด้วยซีเมนต์ชนิดต่างกัน โดยกลุ่มที่ 1 ใช้ซิงก์ฟอสเฟต กลุ่มที่ 2 ใช้ กลาสไอโอโนเมอร์ และกลุ่มที่ 3 ใช้เรซิน วัดความสูงของครอบฟันหลังยึดด้วยซีเมนต์ และวัดแรงดึงสูงสุดที่ทําให้ครอบฟันหลุดจากตัวฟันด้วยเครื่อง Lloyd universal testing machine ผลการทดลอง ค่าเฉลี่ยความแตกต่างความสูงของครอบฟันก่อนและหลังซีเมนต์ (ไมโครเมตร) กลุ่มที่ 1 = 36.0 = 16.6 กลุ่มที่ 2 = 14.7 + 7.4 กลุ่มที่ 3 = 45.5 + 21.3 ค่าเฉลี่ยแรงดึงสูงสุด (นิวตัน) กลุ่มที่ 1 = 343.98 + 46.21 กลุ่มที่ 2 = 482.0 + 33.08 กลุ่มที่ 3 = 522.88 …
