การวางแผนเลือกพื้นที่ปฏิบัติงานในกรอบเวลาระยะสั้นและระยะยาวของนิสิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย, ชาญชัย ให้สงวน

วัตถุประสงค์ การวิจัยนี้มีวัตถุประสงค์เพื่อเปรียบเทียบแบบแผนการเลือกพื้นที่ปฏิบัติงานในกรอบเวลาระยะสั้น และระยะยาวของนิสิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย และระบุปัจจัยที่มีความสัมพันธ์กับแผนการเปลี่ยน พื้นที่ปฏิบัติงาน วัสดุและวิธีการ กลุ่มตัวอย่างเป็นนิสิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย ชั้นปีที่ 1 3 และ 6 จํานวน 258 คน ใช้แบบสอบถามชนิดตอบด้วยตนเองเก็บข้อมูลเกี่ยวกับแผนการเลือกพื้นที่ปฏิบัติงานในกรอบเวลาระยะสั้นและระยะยาว รวมทั้งข้อมูลที่เกี่ยวข้อง ใช้สถิติถดถอยโลจิสติกแบบขั้นตอนเพื่อระบุปัจจัยทํานายที่มีความสัมพันธ์กับ การเปลี่ยนพื้นที่ปฏิบัติงาน ผลการศึกษา นิสิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย ส่วนใหญ่ร้อยละ 94.8 คิดจะปฏิบัติงานใช้ทุน แต่มี เพียงร้อยละ 34.8 ที่คิดจะอยู่ต่อในระบบราชการหลังใช้ทุนครบ 3 ปี นิสิตร้อยละ 91.6 วางแผนปฏิบัติงานใน กรอบเวลาระยะสั้นอยู่นอกพื้นที่กรุงเทพมหานครและปริมณฑล แต่ในกรอบเวลาระยะยาว นิสิตส่วนใหญ่ร้อยละ 65.7 เลือกที่จะปฏิบัติงานในเขตกรุงเทพมหานครและปริมณฑล เมื่อเปรียบเทียบระหว่าง 2 กรอบเวลา มีนิสิตร้อยละ 57.0 ที่แสดงความคงตัวของพื้นที่ปฏิบัติงาน และร้อยละ 43.0 แสดงแผนการเปลี่ยนแปลงพื้นที่ปฏิบัติงาน ผลการวิเคราะห์ถดถอยโลจิสติก พบว่ามี 2 ปัจจัยทํานายที่มีความสัมพันธ์กับแผนการเปลี่ยนแปลงพื้นที่ปฏิบัติงาน คือ การมีภูมิลําเนาในกรุงเทพมหานครและปริมณฑล และการวางแผนปฏิบัติงานระยะยาวอยู่ในภาคเอกชน สรุป การเลือกพื้นที่ปฏิบัติงานในกรอบเวลาระยะยาวมีแบบกระส่วนต่างจากกรอบเวลาระยะสั้นโดยนิสิตทันตแพทย์ส่วนใหญ่แสดงถึงความตั้งใจที่จะโยกย้ายเข้ามาปฏิบัติงานในกรุงเทพมหานครและปริมณฑล อันอาจสะท้อนให้ เห็นถึงการปรับตัวในระระยาวเพื่อคลี่คลายสภาวะการพลัดถิ่นฐานทางสังคม (2 ทันต จุฬาฯ 2551;31:87-102)

Clinical data show a strong correlation between tobacco and alcohol use and the development of oral squamous cell carcinoma (SCC). While this association implies that the oral mucosa actively metabolizes carcinogens, there is little information which depicts the carcinogen metabolizing enzymes within the oral cavity. Glutathione S-transferases (GSTs) primary function is to detoxify carcinogens by increasing their water solubility, GSTs represent key carcinogen metabolizing enzymes. Notably, individuals with a null phenotype for certain GST isoforms are at an increased risk to develop cancer. This study investigated the function and distribution of GSTs in human oral tissues. Quantitative PCR (qPCR) has recently been used to quantify microorganisms in complex communities, including dental plaque biofilms. However, there is variability in the qPCR protocols being used. This study was designed to evaluate the validity of two of these variables with the intent of developing a more standardized qPCR protocol. The two variables evaluated were (1) the use of DNA content versus actual cell counts to estimate bacterial numbers in mixed plaque samples and (2) the effectiveness of three different universal primers versus species specific primers in amplifying specific target pathogens in these samples. Dental enamel forms through a protein-controlled mineralization and enzymatic degradation with a nanoscale precision that new engineering technologies may be able to mimic. Recombinant fulllength human amelogenin (rH174) and a matrix-metalloprotease (MMP-20) were employed in a pHstat titration system that enabled a continuous supply of calcium and phosphate ions over several days, mimicking the initial stages of matrix processing and crystallization in enamel in-vitro. Effects on the self-assembly and crystal growth from a saturated aqueous solution containing 0.4 mg/ml rH174 and MMP-20 with the weight ratio of 1:1000 with respect to rH174 were investigated. The purpose of this study was to compare the density of gutta percha following three different obturation techniques using two split-tooth models. One split-tooth model was constructed with a straight rooted maxillary incisor and the other with the curved palatal root of a maxillary molar. Each tooth was obturated using single cone, cold lateral, and warm vertical obturation techniques without sealer. Each obturation technique was performed 20 times for each of the root systems. Foi realizada uma pesquisa de campo em duas escolas da rede pública estadual de Porto Alegre, com a finalidade de verificar a presença de dentes permanentes irrompidos em crianças com faixa etária entre seis a dez anos. Foram examinadas 327 crianças, considerando as variáveis de idade, sexo e raça. Não foi observada diferença significativa para a época de erupção para as variáveis de raça e sexo. Foi observado que aos seis anos de idade uma porcentagem maior de incisivos inferiores (78,7%) irrompeu antes que os primeiros molares inferiores (75,9%), assim como a erupção na arcada inferior antecedeu a da arcada …

The purpose of this study was to compare three direct digital sensors (Kodak 6100, Schick CDR, and Dexis PerfectSize), a phosphor plate system (OpTime), and F-speed film to standard D-speed film in the detection of artificial bone lesions prepared in mandible bone sections. Multiple artificial bone lesions were prepared at varying depths in the cortical bone. Specimens were imaged with six different radiographic systems. Radiographs were randomly presented to nine different observers. The purpose of this study was to compare the shear bond strength of orthodontic brackets to enamel in four bonding protocols: SEP without prior pumicing (None), Pre-etch and SEP without prior pumicing (Pre-etch), control: SEP with prior pumicing (Pumice), and Pre-etch and SEP with prior pumicing (Both). 80 extracted bovine incisors were randomly divided into 4 groups of 20, and brackets were bonded under the different experimental conditions. Debonding force was measured with an Instron universal testing machine. A two-way ANOVA comparing the four groups indicated that there was a significant difference in debonding force (P = 0.001). Purpose: To determine whether Bispectral Index Monitoring is an effective tool for quantifying sedation depth after the administration of oral drug regimens in children.Methods: This retrospective study reviewed the charts of 75 children who received oral conscious sedation for dental treatment. Data collected from the chart included; 1) BIS values at 5 minute intervals and at five critical events: pre-operative, local anesthesia delivery, rubber dam placement (if utilized), during operative treatment, and postoperatively, 2) behavior ratings at the five critical events and an overall behavior assessment, 3) treatment data, and 4) demographic data. Purpose: The purpose of this study was to evaluate esthetic posterior stainless steel crowns (EP-SSC) and compare their relative shear strengths.Methods: Sixty EP-SSC were compared. 15 crowns from NuSmile® Primary Crowns, Kinder Krowns, and Dental Innovators 1UP and EC Crowns were studied. The crowns were cemented to a typodont tooth then thermally cycled in water baths to simulate oral conditions. Shear strengths were evaluated by subjecting these crowns to simulated forces of occlusion.Results: The force required to cause shearing of the esthetic facings was statistically significant. BACKGROUND: Dental malocclusion is present in all societies but its prevalence varies. Identifying occlusal problems, their incidence and the need for treatment can help to determine the appropriate treatment plan and manpower needed in orthodontics. The aim of the study was to analyze the malocclusion pattern and to provide quantitative information on the pattern of dentofacial characteristics among orthodontic population.METHODS: Varying dentofacial characteristics of 156 patients from June 2002 to April 2004, at the orthodontic unit of the Aga Khan University Hospital were analyzed. Controlling friction is an important aspect of orthodontics because it allows more predictable tooth movement with less anchorage loss. Friction at the bracket/archwire/ligation interface can reduce the force available for desired tooth movement. Initial stages of orthodontic treatment depend on the ability of the archwire to slide freely through the bracket, making friction an important aspect of all orthodontic brackets in the early phases of orthodontic treatment. The purpose of the present study was to assess treatment efficiency of one selfligating bracket system (In-Ovation, GAC International, Bohemia, NY) compared to a conventional ligating bracket system (Ovation, GAC International, Bohemia, NY) in extraction cases used in the same practice. Treatment duration, number of appointments, and number of arch wires used per patient were compared within one private orthodontic practice. The total sample size was 155 patients, 83 consecutively finished with conventionally-ligated Ovation brackets and 72 consecutively finished with In-Ovation self-ligating brackets. Introduction: Dental caries remain the most common infectious disease in children, and is in fact, on the rise in the U.S. among 2 to 5 year olds. Among hundreds of oral bacteria, mutans streptococci (MS) are considered the major species associated with dental caries. Studies have shown that increased MS in saliva or plaque is an important indicator for caries. In recent years, the use of a variety of temporary placed implants to attain absolute anchorage has been described in the orthodontic literature. The use of temporary implants, fixed to bone, overcomes some of the limitations of traditional anchorage and their reliance on patient compliance. The purpose of this study was to evaluate factors relating to the successful placement of miniscrew temporary anchorage devices in an orthodontic clinic.
