ซิลเวอร์ไดเอมีนฟลูออไรด์เพื่อการหยุดยั้งฟันผุในฟันน้ำนม, ประสิทธิ์ วงศ์สุภา, สุกัญญา เธียรวิวัฒน์, จันทร์พิมพ์ หินเทาว์

วัตถุประสงค์ของบทความนี้เพื่อทบทวนความรู้ ที่เกี่ยวข้องกับซิลเวอร์ไดเอมีนฟลูออไรด์ที่ใช้ในทาง ทันตกรรมเพื่อหยุดยั้งฟันผุในฟันน้ํานม โดยข้อดีของซิลเวอร์ไดเอมีนฟลูออไรด์คือ การใช้งานที่ง่าย ปลอดภัย ซิลเวอร์ไดเอมีนฟลูออไรด์ที่ใช้ในประเทศไทย เพื่อหวังผลในการหยุดยั้งฟันผุในฟันน้ํานมในเด็กวัยก่อนเรียน ความเข้มข้นร้อยละ 38 และมีฟลูออไรด์เป็นส่วนประกอบ 44,800 ส่วนในล้านส่วน จากการศึกษาพบว่า ประสิทธิผลของซิลเวอร์ไดเอมีนฟลูออไรด์ในการหยุดยั้งโรคฟันผุในฟันน้ํานมสูงถึงร้อยละ 70- 90 ความถี่ในการ ทาอยู่ที่ 1-2 ครั้งต่อปี และพบว่าความถี่ 2 ครั้งมีประสิทธิผลในการหยุดยั้งฟันผุดีกว่าความถี่ 1 ครั้ง ข้อที่ต้อง ระมัดระวังในการใช้ คือทาเฉพาะบริเวณที่เป็นรอยผุ ที่ไม่ทะลุโพรงประสาท หรือไม่ทาในพื้นที่ไม่มีชีวิต สําหรับ ข้อด้อยคือทําให้รอยผู้นั้นเปลี่ยนเป็นสีดํา และกรณีที่ทาไปติดผิวหนังหรือเนื้อเยื่อจะทําให้เนื้อเยื่อเปลี่ยนเป็นสีดําเช่นกัน อย่างไรก็ดีเนื่องจากมีความเข้มข้นของฟลูออไรด์ในปริมาณที่สูงจึงควรใช้ด้วยความระมัดระวัง ซึ่งจาก ประสิทธิผลในการหยุดยั้งฟันผุ รวมทั้งข้อดีต่าง ๆ การใช้ซิลเวอร์ไดเอมีนฟลูออไรด์น่าจะเป็นทางเลือกหนึ่งในการ หยุดยั้งฟันผุโดยเฉพาะอย่างยิ่งในฟันน้ํานม แต่ควรจะมีการศึกษาวิจัยในเรื่องด้านประสิทธิผลด้านความคุ้มทุน รวมทั้งความถี่ที่มีความเหมาะสมในการทา รวมถึงการศึกษาการนํามาใช้ในฟันแท้ต่อไป (ว ทันต จุฬาฯ 2557;37:371-80)

Objective To evaluate the efficacy in assisting gutta-percha removal of an essential oil from Citrus maxima (pomelo oil) and its cytotoxicity on human gingival fibroblasts when compared to orange oil and xylene. Materials and methods Forty human single-rooted teeth were instrumented and filled with gutta-percha at their apical one third. The gutta-percha filled in root canal was softened with each solvent, pomelo oil, orange oil, xylene or distilled water (n=10 for each group). วัตถุประสงค์ เพื่อเปรียบเทียบกําลังแรงยึดแบบเฉือนของแบร์กเกตโลหะ และดัชนีส่วนเหลือสารยึดติดเมื่อฉายแสงบ่มด้วยเครื่องฉายแสงแอลอีดีที่มีความเข้มแสงแตกต่างกัน 2 ชนิด วัสดุและวิธีการ ฟันกรามน้อยซี่ที่หนึ่ง 60 ซี่ที่ถอนจากผู้ป่วยจัดฟันจํานวน 30 คน (ด้านซ้าย 1 และด้านขวา 1 ปี ต่อผู้ป่วย 1 คน) สุ่มฟันกรามน้อยซี่ที่หนึ่ง 1 จากผู้ป่วยแต่ละคนเป็นกลุ่มที่ 1 และฟันกรามน้อยซี่ที่หนึ่งที่ เหลือเป็นกลุ่มที่ 2 ยึดแบร็กเกตโลหะบนผิวเคลือบฟันที่ถูกฝังในท่อพีวีซี ด้วยวัสดุเรซินยึดติดที่บ่มตัวด้วยแสง ยี่ห้อทรานสบอนด์เอ็กซ์ที โดยฉายแสงตั้งฉากกับฐานแบร์กเกตโลหะด้วยเครื่องฉายแสงแอลอีดีรุ่นวัดเล็กเก้อ ที่มีความเข้มแสง 850 มิลลิวัตต์/ตารางเซนติเมตร ในกลุ่มที่ 1 และเครื่องฉายแสงแอลอีดีรุ่นมินิแอลอีดีทรี ที่มี ความเข้มแสง 2,200 มิลลิวัตต์/ตารางเซนติเมตร ในกลุ่มที่ 2 เป็นเวลา 10 และ 3 วินาที ตามลําดับ จากนั้น แช่ฟันในน้ํากลั่นอุณหภูมิ 37 องศาเซลเซียส เป็นเวลา 24 ชั่วโมง แล้วทดสอบกําลังแรงยึดแบบเฉือนด้วยเครื่อง ทดสอบอเนกประสงค์ ความเร็วหัวตัด 0.5 มิลลิเมตร/นาที จนกระทั่งแบร์กเกตหลุด และศึกษาดัชนีส่วนเหลือสาร ยึดติดด้วยกล้องจุลทรรศน์สเตอริโอที่กําลังขยาย 10 เท่า วิเคราะห์ข้อมูลด้วยสถิติทดสอบที่แบบอิสระและสถิติการ ทดสอบไคสแควร์ตามลําดับ ที่ระดับนัยสําคัญทางสถิติ 0.05 ผลการศึกษา ค่ากําลังแรงยึดแบบเฉือนมีค่า 21.34 + 2.45 และ 19.32 + 1.89 เมกะปาสคาล ในกลุ่มที่ 1 และ 2 ตามลําดับ ซึ่งมีความแตกต่างอย่างมีนัยสําคัญทางสถิติ ค่าดัชนีส่วนเหลือสารยึดติดระหว่าง 2 กลุ่ม มีการกระจายที่แตกต่างกันอย่างมีนัยสําคัญทางสถิติ สรุป เครื่องฉายแสงแอลอีดีที่มีความเข้มแสงแตกต่างกัน มีผลอย่างมีนัยสําคัญทางสถิติต่อกําลังแรงยึดแบบเฉือนของแบร๊กเกตโลหะและดัชนีส่วนเหลือสารยึดติด (ว ทันต จุฬาฯ 2557;37:259-66)

AIM: Variations in the expression of cytokines during the progression of periodontitis remain ill-defined. We evaluated the expression of 19 cytokine genes related to T-cell phenotype/function during initiation, progression and resolution of periodontitis, and related these to the expression of soft and bone tissue destruction genes (TDGs).

MATERIALS AND METHODS: A ligature-induced periodontitis model was used in rhesus monkeys (M. mulatta) (n = 18). Gingival tissues were taken at baseline pre-ligation, 2 weeks and 1 month (Initiation) and 3 months (progression) post ligation. Ligatures were removed and samples taken 2 months later (resolution). Introduction: The purpose of this study was to use cone-beam computed tomography to quantitatively evaluate the skeletal effects of rapid maxillary expansion and orthodontic treatment, with a specific interest in evaluating whether the skeletal changes achieved by RME will maintain throughout orthodontic treatment. Introduction: This study was designed to evaluate, via tensile and bend testing, the mechanical properties of a newly-developed monocrystalline orthodontic archwire comprised of a blend of copper, aluminum, and nickel (CuAlNi). Methods: The sample was comprised of three shape memory alloys; CuAlNi, copper nickel titanium (CuNiTi), and nickel titanium (NiTi); from various orthodontic manufacturers in both 0.018” round and 0.019” x 0.025” rectangular dimensions. Statement of problem

The effect of microwave brazing on the strength properties of dental casting alloys is not yet known.

Purpose

The purpose of this study was to compare the strength properties of preceramic brazed joints obtained by using a microwave oven and a conventional torch flame for a high noble alloy (Au-Pd).

Material and methods

A total of 18 tensile bars made of an Au-Pd ceramic alloy were fabricated. Six specimens were cut and joined with a high-fusing preceramic solder in a specially designed microwave oven, and 6 specimens were joined with a conventional natural gas/oxygen torch. The remaining …

Aim

To identify predictors of compliance during non-surgical and supportive periodontal therapy (SPT).

Materials and Methods

In this retrospective study, demographic, dental, medical data of 427 new patients in a private practice were collected. Data were analysed in statistical models with non-surgical therapy and SPT compliance used as dependent variables.

Results

Of the 427 patients, 17.3% never agreed to initial therapy, 10.7% never completed therapy and 20.8% completed treatment, but never entered SPT. Of the 218 SPT patients, 56% became non-attenders after a period of 20 months, 33% were erratic attenders and 10.5% were regular attenders until the end of …

Background: Matrix metalloproteinase (MMPs) are proteins that have a unique role in immunity, tissue remodeling, and tumorigenesis. MMPs can break down extracellular components, release bioactive molecules, and act on pro-inflammatory cytokines, chemokines and other proteins to regulate varied aspects of inflammation and immunity. Nicotine (3-(1-methyl-2-pyrrolidinyl) pyridine), a key toxic component of tobacco, is thought to dysregulate matrix metalloproteinase secretion in immune cells. Aim: Examine the effect of nicotine on MMP9 secretion and activity in primary monocytes. P. gingivalis is a major periodontal disease causing pathogen. Gingival epithelial cells being the first line of defense, interact and internalize the bacteria evoking a cell signaling response that finally leads to the release of cytokines. IL-6 is a potent pro- inflammatory cytokine, which is one of the responsible factors for bone resorption, a clinical manifestation of periodontitis. Stem cell-based regenerative therapy is a promising treatment for head and neck cancer patients that suffer from chronic dry mouth (xerostomia) due to salivary gland injury from radiation therapy. Current xerostomia therapies only provide temporary symptom relief, while permanent restoration of salivary function is not currently feasible. Here, we identified and characterized a stem cell population from adult murine submandibular glands. Of the different cells isolated from the submandibular gland, this specific population, Lin-CD24+c-Kit+Sca1+, possessed the highest capacity for proliferation, self renewal, and differentiation during serial passage in vitro. Background

In chronic kidney disease (CKD), inadequate nutritional intake, inflammation, and increased oxidative stress have been the major contributing factors in malnutrition pathogenesis. However, there is still a paucity of evidence assessing the magnitude of the effect of tooth loss on malnutrition in CKD populations. The authors hypothesize that among patients with CKD, tooth loss may affect nutritional status, using the National Health and Nutrition Examination Survey 1988 to 1994 (NHANES III).

Methods

Aim: This study was conducted to evaluate the efficacy of using conventional surgical stents guided by cone beam computed tomography in determining the position and buccolingual inclination of implants placed in posterior mandibular knife edge ridge in cases of ridge splitting with and without bone grafting material. Introduction: In orthodontics, contact dermatitis caused by nickel allergy should be of concern with the number of nickel-containing appliances and wires used. Stainless steel archwires are commonly used throughout orthodontic treatment. With the release of nickel from these wires, some manufacturers have turned to nickel-free stainless steel alternatives. The goal of this research was to compare nickel-free stainless steel with traditional stainless steel archwires with regard to mechanical properties and microstructure. Introduction: Orthodontic treatment demands excellent outcomes in both function and esthetics. Despite the popularity of rapid palatal expansion in orthodontic treatment, few studies have examined its consequences on facial soft tissue using direct anthropometric measurements. The primary goal of this study was to determine facial soft tissue changes immediately following rapid palatal expansion. Materials and Methods: Twenty-eight patients (age range, 8-17 years) attending Marquette Dental School Orthodontic Post-Graduate Clinic were enrolled in the study.
