ช่องว่างของการบูรณะเรซิน คอมโพสิตชนิดบูรณะทั้งก้อนในโพรงฟันชนิดคลาสทู, ชินเดช จิระอานนท์

การศึกษาในครั้งนี้มีวัตถุประสงค์เพื่อประเมินถึงผลของวิธีการบูรณะวัสดุเรซิน คอมโพสิตชนิดบูรณะทั้งก้อนที่มีรูปแบบผลิตภัณฑ์และลักษณะการใช้งานที่แตกต่างกันในโพรงฟันชนิดคลาสทู ต่อช่องว่างที่เกิดขึ้นในการบูรณะ ด้วยเครื่องไมโครคอมพิวเตดโทโมกราฟฟี่ โดยทำการเตรียมโพรงฟันชนิดคลาสทูในฟันกรามน้อยซี่ที่หนึ่งทั้งหมด 40 ซี่ และแบ่งตามกลุ่มของการบูรณะเป็น 4 กลุ่ม ได้แก่ กลุ่มที่ 1 บูรณะด้วยฟิลเทค บัลค์ฟิลล์ โพสทีเรียแบบหลอด (Filtek Bulk Fill Posterior, Syringes) โดยตักวัสดุใส่โพรงฟันเพียงครั้งเดียว กลุ่มที่ 2 บูรณะด้วยฟิลเทค บัลค์ฟิลล์ โพสทีเรียแบบหลอด (Filtek Bulk Fill Posterior, Syringes) โดยตักวัสดุใส่โพรงฟันสองครั้ง กลุ่มที่ 3 บูรณะด้วยฟิลเทค บัลค์ฟิลล์ โพสทีเรียแบบแคปซูล (Filtek Bulk Fill Posterior, Capsule) โดยฉีดวัสดุเพียงครั้งเดียว และกลุ่มที่ 4 ทำการบูรณะด้วยโซนิคฟิลล์แบบแคปซูล (SonicFill 2, Capsule) โดยฉีดวัสดุเพียงครั้งเดียวจากเครื่องมือฉีดสำหรับวัสดุโซนิคฟิลล์ (SonicFill Handpiece) สารยึดติด OptiBond FL ถูกใช้เตรียมพื้นผิวโพรงฟันก่อนบูรณะ ภายหลังบูรณะเก็บชิ้นตัวอย่างไว้ในตู้ที่มีความชื้นสัมพัทธ์ร้อยละ 100 ที่ อุณหภูมิ 37 องศาเซลเซียส เป็นเวลา 24 ชั่วโมง จากนั้นประเมินร้อยละของช่องว่างที่เกิดขึ้นทั้งหมดภายในโพรงฟันที่บูรณะแล้วด้วยเครื่องไมโครคอมพิวเตดโทโมกราฟฟี่ วิเคราะห์ข้อมูลด้วย One-way ANOVA ร่วมกับ Tukey Post-hoc test ที่ระดับความเชื่อมั่นร้อยละ 95 ผลการศึกษาพบว่า ร้อยละของช่องว่างในกลุ่มที่ 2 ซึ่งบูรณะด้วยการตักวัสดุสองครั้ง (1.62 %) มากกว่ากลุ่มอื่น ๆ อย่างมีนัยสำคัญ ขณะที่ร้อยละของช่องว่างของกลุ่มที่ 1 ซึ่งตักวัสดุเพียงครั้งเดียวหรือกลุ่มที่ 3 และ 4 ซึ่งฉีดวัสดุใส่โพรงฟันเพียงครั้งเดียวไม่แตกต่างกัน (0.49 %, 0.33 % และ 0.21 % ตามลำดับ) …

แลคโตเฟอรินเป็นปัจจัยสำคัญในระบบภูมิคุ้มกันแต่กำเนิด ซึ่งมีฤทธิ์ต้านเชื้อจุลชีพหลายชนิด ทั้งเชื้อรา ไวรัส และแบคทีเรียขอบเขตกว้าง มีการศึกษาเกี่ยวกับกลไกและบทบาททางชีวภาพของแลคโตเฟอรินออกมาอย่างต่อเนื่อง รวมถึงการศึกษาที่ผ่านมา ที่พบความสัมพันธ์ของระดับแลคโตเฟอรินที่ลดลงและการติดเชื้อราในช่องปาก อย่างไรก็ดี ยังไม่เคยมีการศึกษาถึงความสัมพันธ์ของระดับแลคโตเฟอรินในน้ำลายของผู้ป่วยที่ได้รับการรักษาด้วยยาสเตียรอยด์เฉพาะที่ ซึ่งทราบกันดีว่าเป็นปัจจัยหนึ่งที่เป็นช่องทางให้เกิดการติดเชื้อราแคนดิดาในช่องปาก การวิจัยนี้จึงมีวัตถุประสงค์เพื่อเปรียบเทียบระดับของแลคโตเฟอรินในน้ำลายก่อนและหลังการรักษาด้วยยาฟลูโอซิโนโลน อะเซทโทไนด์ ความเข้มข้นร้อยละ 0.1 ชนิดขี้ผึ้งป้ายปาก ในผู้เข้าร่วมโครงการที่เป็นกลุ่มผู้ป่วยโรคไลเคนแพลนัสช่องปาก และกลุ่มผู้ป่วยที่เป็นรอยโรคไลเคนอยด์ช่องปากสาเหตุจากยา ผลการทดลองไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติของระดับแลคโตเฟอรินในน้ำลายเปรียบเทียบก่อนและหลังการรักษาด้วยยาสเตียรอยด์เฉพาะที่ทั้งในผู้เข้าร่วมโครงการที่เป็นกลุ่มผู้ป่วยโรคไลเคนแพลนัสช่องปาก และกลุ่มผู้ป่วยที่เป็นรอยโรคไลเคนอยด์ช่องปากสาเหตุจากยา (p = 0.100 และ 0.099 ตามลำดับ) นอกจากนี้ ผลการวิเคราะห์ทางสถิติยังไม่พบความแตกต่างของอัตราการไหลของน้ำลาย และอัตราการไหลของแลคโตเฟอรินเปรียบเทียบก่อนและหลังการรักษาด้วยยาสเตียรอยด์เฉพาะที่ทั้งในผู้เข้าร่วมโครงการที่เป็นกลุ่มผู้ป่วยโรคไลเคนแพลนัสช่องปาก และกลุ่มผู้ป่วยที่เป็นรอยโรคไลเคนอยด์ช่องปากสาเหตุจากยา และจำนวนเชื้อราแคนดิดาในผู้เข้าร่วมโครงการที่เป็นกลุ่มผู้ป่วยโรคไลเคนแพลนัสช่องปาก อย่างไรก็ดี ผลการนับจำนวนโคโลนีของราแคนดิดาก่อนและหลังการรักษาด้วยยาสเตียรอยด์เฉพาะที่ในกลุ่มผู้ป่วยที่เป็นรอยโรคไลเคนอยด์ช่องปากสาเหตุจากยามีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p = 0.028)

การแสดงออกของซีดี 146 ในรอยโรคไลเคน แพลนัสและมะเร็งช่องปากในผู้ป่วยไทย, สรินธร ปริยะวาที

การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษาการแสดงออกของซีดี 146 ในเยื่อเมือกช่องปากปกติ (NOM) ไลเคน แพลนัสช่องปาก (OLP) อีพิทีเลียล ดิสเพลเซียช่องปาก (OED) และมะเร็งเยื่อบุผิวเซลล์สควอมัสช่องปาก (OSCC) ในผู้ป่วยไทย โดยนำชิ้นเนื้อจากกลุ่มตัวอย่าง กลุ่มละ 20 ชิ้นมาทำการศึกษาด้วยวิธีทางอิมมูโนฮิสโตเคมี เพื่อประเมินจำนวนเคอราติโนไซต์ที่มีการแสดงออกของซีดี 146 และความเข้มในการติดสี พบว่ามีการเพิ่มขึ้นอย่างมีนัยสำคัญของค่าเฉลี่ยร้อยละของจำนวนเคอราติโนไซต์ที่มีการแสดงออกของซีดี 146 ในกลุ่ม OLP (P < 0.001) และกลุ่ม OED (P < 0.001) เมื่อเทียบกับกลุ่ม NOM โดยกลุ่ม NOM, OLP, OED มีค่าเฉลี่ยร้อยละของจำนวนเคอราติโนไซต์ที่มีการแสดงออกของซีดี 146 เท่ากับ 19.04±15.32, 59.40±24.48, 60.04±28.87 ตามลำดับ พบการลดลงอย่างมีนัยสำคัญของค่าเฉลี่ยร้อยละของจำนวนเคอราติโนไซต์ที่มีการแสดงออกของซีดี 146 ในกลุ่ม OSCC เมื่อเทียบกับกลุ่ม OED (P<0.001) โดยค่าเฉลี่ยร้อยละของจำนวนเคอราติโนไซต์ที่มีการแสดงออกของซีดี 146 ในกลุ่ม OSCC เท่ากับ 22.13±21.03 พบความแตกต่างอย่างมีนัยสำคัญของความเข้มในการติดสีระหว่างกลุ่ม NOM กับกลุ่ม OLP (P<0.05) กลุ่ม NOM กับกลุ่ม OED (P<0.05) กลุ่ม OLP กับกลุ่ม OED (P<0.05) กลุ่ม OLP กับกลุ่ม OSCC (P<0.05) และกลุ่ม OED กับกลุ่ม OSCC (P<0.05) ชิ้นเนื้อส่วนใหญ่ของกลุ่ม NOM และกลุ่ม OSCC ติดสีเข้มระดับจาง กลุ่ม OLP ติดสีเข้มปานกลาง และกลุ่ม OED ติดสีเข้ม ผลการศึกษานี้พบว่าซีดี 146 อาจมีบทบาทหลายด้านในรอยโรคข้างต้น โดยมีการแสดงออกที่เพิ่มขึ้นในไลเคน แพลนัสและอีพิทีเลียล ดิสเพลเซียช่องปาก แต่มีการแสดงออกลดลงในมะเร็งเยื่อบุผิวเซลล์สควอมัสช่องปาก ซีดี 146 จึงอาจเกี่ยวข้องกับพยาธิกำเนิดของโรคไลเคน แพลนัสช่องปาก และการสูญเสียการแสดงออกของซีดี 146 อาจเกี่ยวข้องกับการเปลี่ยนแปลงจากอีพิเลียล ดิสเพลเซียไปเป็นมะเร็งเยื่อบุผิวเซลล์สควอมัสช่องปากได้

Purpose: To understand guardian and child preferences for the appearance of their pediatric dentist. This was a cross-sectional descriptive study using survey methodology with patients and parents that attended the VCU Pediatric Dental clinic.

Methods: A total sample of 100 guardians and 97 pediatric patient participants completed the computer-based questionnaire. Four subjects were asked to pose for photographs wearing various combinations of attire (professional, casual, white coat, scrubs).

Results: Among guardians, 56% reported preferring a provider in scrubs, with white coat the second most preferred attire (39%) for their children. For pediatric patients, scrubs were still most often selected, …

Purpose: This study investigates knowledge about HPV and examines if pediatric dental providers should include HPV education for guardians of patients 10-18 years.

Methods: Legal guardians of 10-18 year-old patients of the Virginia Commonwealth University Pediatric Dental Clinic were enrolled in this prospective cohort study. Participants completed a baseline survey, were provided HPV education, completed an initial follow-up survey, and then completed a 6-month follow-up survey.

Results: A total of 54 participants completed the baseline and initial follow-up surveys and 17 completed the 6-month follow-up survey. The average number of correct responses was 3.4 of 6 knowledge questions, which significantly …

The objective of this study was to evaluate the biofilm removal efficacy of three root canal treatment systems: ProUltra® PiezoFlow™, traditional needle irrigation, and the GentleWave® system in an ex-vivo benchtop study. Twenty-four extracted maxillary and mandibular molars were selected. Teeth were all instrumented to a master apical file size #25 with 4% taper. Teeth were then randomly divided into four experimental groups and two control groups. Purpose: To assess the treatment modalities of pediatric dentists for restoring Class II lesions in primary molars.

Methods: A survey of eight cases was emailed to AAPD members, who were asked to choose a stainless steel crown (SSC) or composite resin. Treatment choice was associated with provider type, years in practice, behavior management, lesion detection, and patient age.

Results: Clinically detectable caries restored under general anesthesia were treated more often with a SSC. Faculty and residents chose a SSC at a greater rate. Providers with 10 or more years experience chose a SSC more often.

Background: Current research has demonstrated that aspirin and fish oil (EFA) increase plasma levels of specialized pro-resolution molecules (SPMs). This study investigates their effects on SPM precursor pools in platelet rich fibrin (PRF). Methods: Twenty healthy volunteers were randomly assigned to take aspirin; EFA or aspirin and EFA. Four hours later, SPM precursor levels were quantified using combined Liquid Chromatography tandem mass spectrometry. Background: Unfavorable outcomes following periodontal surgeries can be attributed to impaired resolution mechanisms likely due to decreased levels of specialized pro-resolution molecules (SPM). The current study investigates if SPM substrate pools in platelet-rich plasma preparations (PRP) can be increased by essential fatty acid (EFA) and / or aspirin supplementation. Methods: Nineteen healthy volunteers were randomly assigned to take i) aspirin; ii) EFA; iii) aspirin and EFA. Introduction: Orthodontists increasingly rely on digital models in clinical practice. The ability of modern scanners to articulate digital models must be scientifically evaluated. Methods:Twenty five digital articulated models were produced from four digital scanners in five experimental groups. The resulting inter-arch measurements were compared to the gold standard. An acceptable range of 0.5mm more or less than the gold standard was used for evaluation. Results: iTero® and iTero® Element yielded all acceptable inter-arch measurements. Objectives

The duration and the frequency of follow-up after treatment of oral squamous cell carcinoma are not standardized in the current literature. The purpose of this study was to evaluate our local standard post-treatment and follow-up protocol.

Materials and methods

Overall, 228 patients treated curatively from 01/2006 to 07/2013 were reviewed. To evaluate the follow-up program, data on the secondary event were used. To determine risk groups, all patients with tumor recurrence were specifically analyzed. Relapse-free rate were estimated by the Kaplan-Meier product limit method. The chi-square test was used to identify independent risk factors for tumor relapse.

Objective: MMP-13 performs digestion of collagen, which is a primary component of the temporomandibular joint (TMJ) articular disc. This study evaluated the expression of MMP-13 in patients with anterior disc displacement with (ADDwR) and without reduction (ADDwoR), and in the presence of TMJ osteoarthrosis.

Methods: Thirty-nine human temporomandibular joint disc samples were collected and divided in two ways: ADDwR (21 samples), ADDwoR (10 samples), and a control group (8 samples); and with osteoarthrosis (10 samples) and without osteoarthrosis (29 samples). Lateral in-homogeneities in lipid compositions cause microdomains formation and change in the physical properties of biological membranes. With the presence of cholesterol and mixed species of lipids, phospholipid membranes segregate into lateral domains of liquid-ordered and liquid-disordered phases. Coupling of two-dimensional intralayer phase separations and interlayer liquid-crystalline ordering in multicomponent membranes has been previously demonstrated. By the use of digital holographic microscopy (DHMicroscopy), we quantitatively analyzed the volumetric dynamical behavior of such membranes. The specimens are lipid mixtures composed of sphingomyelin, cholesterol, and unsaturated phospholipid, 1,2-dioleoyl-sn-glycero-3-phosphocholine. A side effect of orthodontic treatment is the development of white spot lesions (WSLs). This thesis attempted to evaluate the effects of the use of APC™ Flash-Free (FF) and conventional adhesives in bonding of orthodontic brackets on plaque formation and retention, WSL development, and bracket failure in-vivo. This was accomplished by collecting plaque samples from around orthodontic appliances and analyzing them using ATP-driven bioluminescence; scanning enamel surfaces using the Canary System™ to evaluate WSL development, and counting the number of spontaneous debonding of brackets of the 4 maxillary incisors in the first 6 months of treatment. In September 2006, Wisconsin Medicaid changed its policy to allow nondentists to become certified Medicaid providers and to bill for sealants in public health settings.

Objective

This study examined changes in patterns of dental sealant utilization in first molars of Wisconsin Medicaid enrollees associated with a policy change.

The Electronic Data Systems of Medicaid Evaluation and Decision Support for Wisconsin from 2001 to 2009.

Study Design

Retrospective claims data analysis of Wisconsin Dental Medicaid for children aged 6-16 years.

Background

Few studies have directly compared dental procedures provided in public and private insurance plans for enrollees living in dental health professional shortage areas (DHPSAs). We examined the rates for the different types of dental procedures received by 0–18-year-old children living in DHPSAs and non-DHPSAs who were enrolled in Medicaid and those enrolled under Delta Dental of Wisconsin (DDW) for years 2002 to 2008.

Methods

Studies suggest that P. gingivalis functions as a keystone pathogen and interacts with primary colonizers in the supragingival biofilm such as S. gordonii. This interaction contributes to the initial colonization of the oral cavity by P. gingivalis and thus represents a potential target for therapeutic intervention. We have identified a peptide (BAR) derived from the streptococcal SspB protein that functions to inhibit P. gingivalis adherence to S. gordonii. Bone tissue engineering is sought to apply strategies for bone defects healing without limitations and short-comings of using either bone autografts or allografts and xenografts. The aim of this study was to fabricate a thin layer poly(lactic-co-glycolic) acid (PLGA) coated beta-tricalcium phosphate (β-TCP) scaffold with sustained release of vascular endothelial growth factor (VEGF). PLGA coating increased compressive strength of the β-TCP scaffolds significantly.
