This study evaluated reproducibility of periodontal probing by conventional manual and automated force-controlled electronic probes. A total of 1206 sites (6 sites per tooth including maxillary and mandibular incisor/canine, premolar, and molar sites) in 15 patients with history of moderate or advanced chronic periodontal disease were examined. Probings included probing pocket depths (PD), as well as probing attachment levels (PAL) from a reference stent. Measurements were recorded to the nearest 0.5 mm with both methods. การทดสอบความคงสภาพของตํารับ ยาอมบ้วนปากฟลูออไรด์, วนิดา แสงอลังการ, สุนีย์ ศรีอ่อนรอด

ยาอมบ้วนปากฟลูออไรด์จํานวน 12 สูตรที่ผ่านการทดสอบประสิทธิภาพของสารกันบูดแล้วถูกนําไป ทดสอบความคงสภาพทางด้านเคมี ฟิสิกส์ โดยวิธีการเร่งภายใต้อุณหภูมิ 30° ซ. 40° ซ. และ 50° ซ. เป็นเวลานาน 4 สัปดาห์ เมื่อตรวจวิเคราะห์ดูการเปลี่ยนแปลงทุก 1 สัปดาห์ผลการทดลองพบว่า ความร้อนไม่ทําให้ความเข้มข้นของฟลูออไรด์อิออนเปลี่ยนแปลงในทุกสูตรที่ใช้ทดลอง แต่ความร้อนที่อุณหภูมิ 50° ซ. ทําให้ค่าความหนืดและสภาพความเป็นกรด-ด่างของตํารับยาลดลงอย่างมีนัยสําคัญทางสถิติ (P < 0.05) แสดงว่าสูตรยาอมบ้วนปาก ฟลูออไรด์ทั้ง 12 สูตร เมื่อได้รับความร้อนที่อุณหภูมิ 50° ซ. ไม่มีความคงสภาพทางฟิสิกส์ หากต้องการให้ตํารับยาคงสภาพพบว่าควรปรับปรุงสูตรโดยเติมสารที่มีคุณสมบัติเป็นบัฟเฟอร์ เช่น ฟอสเฟตบัฟเฟอร์ และควรปรับสภาพความเป็นกรด-ด่างไว้ที่ประมาณ 6-7

การเปรียบเทียบปัจจัยซึ่งเกี่ยวข้องกับการเกิดโรคไลเคน-พลานัสในช่องปากชนิดอีโรซีฟและไม่อีโรซีฟ, กอบกาญจน์ ทองประสม, ดวงพร ศุภประดิษฐ์, นฤมล ภววรพันธุ์

ศึกษาผู้ป่วย OLP จํานวน 50 ราย โดยแบ่งออกเป็น 2 กลุ่ม คือ กลุ่มอีโรซีฟ และกลุ่ม ไม่อีโรซีฟ จากการตรวจทางคลินิก และการตรวจชิ้นเนื้อเพื่อยืนยันการวินิจฉัยโรค พบว่าผู้ป่วยส่วนใหญ่เป็นเพศหญิงมากกว่าเพศชาย ช่วงอายุที่พบบ่อยประมาณ 40-60 ปี ระยะเวลาของการเกิดโรคมีตั้งแต่ 4-168 เดือน บริเวณที่พบรอยโรคในช่องปากมากที่สุดคือ กระพุ้งแก้ม รองลงมาคือ เหงือก ลิ้น ริมฝีปาก เพดานปาก และพื้นช่องปาก ตามลําดับจากการเปรียบเทียบกลุ่มผู้ป่วยทั้ง 2 กลุ่ม พบว่ากลุ่มอีโรซีฟ และไม่อีโรซีฟ ไม่มีความแตกต่างกันในเรื่องของเพศจํานวนผู้ป่วย ตําแหน่งที่พบ ระยะเวลาของการเกิดโรค นอกจากนี้ยังได้ทําการเปรียบเทียบความสัมพันธ์ของปัจจัย ต่าง ๆ ที่อาจก่อให้เกิดโรคไลเคนพลานัส เช่น โรคของระบบ การแพ้ การใช้ยา วัสดุอุดฟัน ครอบฟัน และฟันปลอมชนิดถอดได้ พบว่าทั้งหมดนี้ไม่มีความสัมพันธ์กับการเกิดโรคไลเคนพลานัส ในช่องปากทั้งสองชนิดอย่างมีนัยสําคัญ ทางสถิติ (P>0.05)

การศึกษาฟันรูปโชเวลของชนกลุ่มน้อยเผ่าซาไก ซึ่งอาศัยอยู่ที่หมู่บ้านคลองตุง ตําบลปะเหลียน จังหวัดตรัง จํานวน 13 คน 45 ซีฟัน จากแบบพิมพ์ฟัน พบฟันรูปโชเวล จําแนกตามวิธี Hrdlicka's classification และ Dahlberg's Plaque P.1. ได้ 55.5 เปอร์เซนต์ จากการศึกษาพบว่าใกล้เคียงกับชาวเมลานีเซียน ฟิจิ นิวกินี อเมริกันนิโกร แสดงว่าชนชาติเหล่านี้มีสายพันธุ์เดียวกัน หรือมีบรรพบุรุษเดียวกัน

การกําจัดคราบจุลินทรีย์ แผลจากปลายขนแปรง และการกระแทกของแปรงสีฟัน 4 ชนิด ในกลุ่มวัยรุ่นและผู้ใหญ่, ชุติมา ไตรรัตน์วรกุล, ภารณี เดชจิตอนันต์, วริษฐา ลิ้มบุญภิวัฒน์

นิสิตทันตแพทย์อายุ 17-20 ปี จํานวน 28 คน มีฟันอย่างน้อย 24 ซี่ และสุขภาพช่องปากสมบูรณ์ ได้รับการสอนวิธีแปรงฟันแบบมอดิฟายด์แบส (modified Bass) วันละ 2 ครั้ง แต่ละคนจะใช้แปรงสีฟัน 4 ชนิด (Research-D, Premium, GUM #411, Dental-C) แต่ละชนิดเป็นเวลา 4 สัปดาห์ ผู้วิจัยวัดดัชนีคราบจุลินทรีย์ ปริมาณแผลที่ผิวเหงือกจากปลายขนแปรง และแผลจากการกระแทกก่อนและหลังการแปรงทุกสัปดาห์ และทําการวิเคราะห์โดยวิธี Kruskal-Wallis I way ANOVA ที่ระดับนัยสําคัญ 0.10 หลังเสร็จสิ้นการแปรง 4 เดือน กลุ่มทดลองตอบแบบสอบถามเกี่ยวกับความคิดเห็นจากการใช้แปรงทั้ง 4 ชนิด ผลของการศึกษาพบว่าแปรงสีฟัน Premium ซึ่งมีราคาต่ำสุด มีประสิทธิภาพในการกําจัดคราบจุลินทรีย์สูงสุด ในขณะที่ไม่มีความแตกต่างของปริมาณแผลที่เกิดจากปลายขนแปรง 4 ชนิด และแผลจากการกระแทกของหัวแปรงพบมากที่สุดจาก GUM #411 กลุ่มทดลองมีความคิดเห็นว่าแปรงสีฟันที่รู้สึกว่าใช้ถนัดมือดีที่สุด ขนแปรงนุ่มที่สุด และไม่รู้สึกเจ็บเหงือกจากขนแปรงคือ Research-D แปรงที่รู้สึกว่าเข้าได้ทั่วถึงดีที่สุดคือ Premium และแปรงที่รู้สึกว่าไม่กระแทกเหงือกและกระพุ้งแก้มคือ Premium และ GUM #411

การเปรียบเทียบการวัดความสูญเสียการยึดเกาะอวัยวะปริทันต์ด้วยเรดิโอวิสิโอกราฟฟี กับออโทเมตต์โพรบ นอกปาก, นวลฉวี หงษ์ประสงค์

ค่าความสูญเสียการยึดเกาะอวัยวะปริทันต์เป็นตัวบ่งชี้ที่ดีในการวิเคราะห์และทํานายโรคปริทันต์ เรดิโอวิสิโอกราฟฟี่ (อาร์วีจี) เป็นเครื่องมือถ่ายภาพรังสีในปาก ซึ่งให้ภาพโดยตรงบนจอสามารถขยายภาพให้ได้รายละเอียดมากขึ้น และสามารถต่อกับเครื่องคอมพิวเตอร์ เพื่อเก็บข้อมูลทางภาพรังสีไว้ใช้ในงานวิจัยต่อไปได้ด้วย ส่วนออโทเมตช์โพรบ เป็นเครื่องมือวัดร่องลึกปริทันต์ หรือค่าความสูญเสียการยึดเกาะอวัยวะปริทันต์ด้วยไฟฟ้า ซึ่งเป็นที่ยอมรับว่า เป็นเครื่องมือที่วัดได้แม่นยํามากกว่าโพรบที่วัดด้วยมือธรรมดา ดังนั้นการทดลองนี้จึงต้องการตรวจสอบความแม่นยําในการวัดความสูญเสียการยึดเกาะอวัยวะปริทันต์ด้วยภาพรังสีซึ่งถ่ายจากเครื่องอาร์วีจี เมื่อเปรียบเทียบกับการวัดด้วยออโทเมตด์โพรบ นอกปาก การทดลอง ทําโดยสร้างบล็อกฟันที่เรียงฟันกรามน้อยและกรามใหญ่เหมือนฟันในปาก 4 ซี่ บล็อกที่ใช้เป็นส่วนผสมของขี้เลื่อยไม้กับปูนปลาสเตอร์ซึ่งให้ความทึบและเงาเหมือนกระดูกเมื่อถ่ายภาพรังสี สร้างรอยโรคของกระดูกที่ถูกทําลายเป็นร่องลึกทางด้านข้างของฟันจํานวน 20 รอยโรคในฟัน 5 บล็อก ถ่ายภาพรังสีด้วยอาร์วีจี และเก็บภาพรังสีในเครื่องคอมพิวเตอร์เพื่ออ่านค่าความสูญเสียการยึดเกาะอวัยวะปริทันต์ ด้วยโปรแกรมสําเร็จรูป แล้ววัดค่าความสูญเสียการยึดเกาะอวัยวะปริทันต์ทั้งยี่สิบรอยโรคด้วยออโทเมตต์โพรบ นํามาวิเคราะห์หาค่าเฉลี่ย และเปรียบเทียบความแตกต่างของค่าความยึดเกาะอวัยวะปริทันต์ที่ได้จากเครื่องมือทั้งสองชนิดด้วย paired-t test. พบว่า ค่าที่วัดได้ด้วยอาร์วีจีจะค่อนข้างสูงกว่าค่าที่วัดด้วยออโทเมตต์โพรบ แต่ค่าที่วัดได้ทั้งสองวิธีไม่มีความแตกต่างอย่างมีนัยสําคัญ จึงเห็นได้ว่า การใช้อาร์วีจีเก็บภาพรังสี และนํามาวัดค่าความสูญเสียการยึดเกาะอวัยวะปริทันต์จะทําได้ผลแม่นยํา และสะดวกกว่าการใช้เครื่องมือชนิดอื่น นอกจากนั้นอาร์วีจียังมีประโยชน์เป็นสื่อความเข้าใจ และยอมรับของผู้ป่วยปริทันต์ได้อย่างมีประสิทธิภาพ และอาจนํามาใช้ในศัลยกรรมปริทันต์ หรือศัลยกรรมปลูกรากเทียมเพื่อดูรายละเอียดของกระดูกในบริเวณที่ทํางาน

การผิดนัดทําฟันของผู้ใช้บริการที่คลินิกทันตกรรมนอกเวลา คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย, สุนทร ระพิสุวรรณ, ศุลีพร ธีระเจตกูล, สิริวรรณ ตันจันทร์พงศ์

จากแบบสอบถามซึ่งจัดทําขึ้นเพื่อการศึกษาปัจจัยที่ทําให้ผู้ใช้บริการทันตกรรมที่คลินิกนอกเวลาของคณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย ผิดนัด พบว่า 76.1% เป็นคนโสด 39.5% มีอาชีพรับราชการ 78% มีรายได้ไม่เกิน 10,000 บาท ปัจจัยที่ทําให้ต้องผิดนัด ได้แก่ติดธุระอื่นที่จําเป็นกว่า 51.4% ปัญหาจราจรติดขัด 46.50% ช่วงการนัดห่างกันนานเกินไป 25% ลืมการนัดหมาย 22% การวิเคราะห์สหสัมพันธ์พบว่า อายุกับเหตุผลติดธุระอื่นที่จําเป็น รายได้กับช่วงการนัดหมายที่ห่างนานเกินไป ปัญหาการเงินกับระยะเวลาที่ใช้การรักษามีความสัมพันธ์กันอย่างมีนัยสําคัญทางสถิติ (P < .01)

พรีซิชันแอตแทชเมนต์เริ่มนํามาใช้ในปี ค.ศ. 1841 โดยมีข้อดีในเรื่องของความสวยงาม เพราะไม่ปรากฏให้เห็นแขนตะขอ รวมทั้งมีการกระจายแรงไปตามแนวแกนฟัน และต้านแรงด้านข้างได้ดี มีการออกแบบพรีซิชันแอตแทชเมนต์ที่ใช้ร่วมกับฟันปลอมบางส่วนชนิดถอดได้หลายชนิด เพื่อให้เหมาะกับฟันที่เหลือในสภาวะต่าง ๆ กัน ซึ่งสามารถจําแนกชนิดของตัวยึดได้หลายวิธีด้วยกัน เช่นจําแนกตามการผลิต ตามความสัมพันธ์กับฟันหลัก หรือตามการเคลื่อนที่ขณะใช้งาน บทความนี้จําแนกชนิดของพรีซิชันแอตแทชเมนต์ ตามความสัมพันธ์กับฟันหลัก อนึ่งพรีซิชันแอตแทชเมนต์เป็นตัวยึดที่มีคุณสมบัติน่าสนใจชนิดหนึ่ง ถึงแม้ว่าในขณะนี้การใช้อาจจะยังไม่แพร่หลายในประเทศไทย

จากรายงานการศึกษาที่ผ่านมาแสดงให้เห็นถึงความแตกต่างของสภาวะโรคฟันผุในแต่ละพื้นที่ของประเทศ ไทย ดังนั้นวัตถุประสงค์ของการศึกษานี้จึงต้องการทราบสภาวะโรคฟันผุและการใช้ฟันปลอมของผู้ป่วยที่ใช้บริการของโรงพยาบาลสังกัดกระทรวงสาธารณสุขในภาคใต้ของประเทศไทย ประชากรกลุ่มตัวอย่างอายุ 12 ปี 18 ปี 35-54 ปีและ 55 ปีขึ้นไป จํานวน 800 คน ซึ่งสุ่มมาจากผู้ป่วยกลุ่มงานทันตกรรมในโรงพยาบาลหาดใหญ่ โรงพยาบาลสงขลา และโรงพยาบาลปัตตานี กลุ่มอายุละ 200 คน ผลการศึกษาพบว่า ค่าเฉลี่ยฟันผุ ถอน อุดของแต่ละกลุ่มเท่ากับ 5.67, 8.61, 12.00 และ 18.68 ต่อคน ตามลําดับ สําหรับค่าเฉลี่ยของฟันที่ถอนของแต่ละกลุ่มแม้จะสูงถึง 0.16, 0.94, 5.85 และ 14.38 ที่ต่อคน แต่ค่าเฉลี่ยของจํานวนฟันปลอมที่ใส่ทดแทนค่อนข้างต่ำมาเพียง 0, 0.11, 0.91 และ 1.18 ต่อคน ตามลําดับเท่านั้น จากการเปรียบเทียบการใส่ฟันปลอมระหว่างบริเวณฟันหน้า และฟันหลังโดยการทดสอบสัดส่วน พบว่าสัดส่วน ของจํานวนฟันปลอมที่ใส่ต่อฟันที่ถอนไปของฟันหน้าสูงกว่าฟันหลังอย่างมีนัยสําคัญทางสถิติ (p<0.05) ยกเว้นใน บริเวณขากรรไกรล่างของกลุ่มอายุ 55 ปีขึ้นไปเท่านั้น สรุปได้ว่าประชาชนกลุ่มนี้มีปัญหาด้านสุขภาพในช่องปากสูงมาก และมีความต้องการบริการทันตกรรมทั้งด้านการรักษา และฟื้นฟูสภาพโดยการใส่ฟันปลอมมากด้วย

Interleukin 1-Beta has been shown to be a potent mediator of bone resorption and has been implicated in resorption in periradicular lesions. In this study we compared the local concentrations of IL-1β in symptomatic and asymptomatic human periradicular lesions. Periradicular tissues from patients with symptomatic and asymptomatic lesions were obtained and stored at -70°C. Pulpal tissues from unerupted third molars and chronically inflamed gingival tissues were also obtained and used as negative and positive controls, respectively. Adequate studies have not been conducted to assess the attitudes of dental hygienists toward independent practice of dental hygiene. The purpose of this study was to investigate these attitudes. The method of investigation was a survey questionnaire designed by the principle investigator. The research results indicated weak support for the independent practice of dental hygiene. The support for general supervision of hygienists in Virginia and the desire to be allowed to practice in nontraditional settings was stronger.
